Major Modes of Variability
This chapter focused on major modes of variability which serve the key role in controlling the regional climate. In terms of tropospheric variability, it defined and discussed ENSO (El Niño Southern Oscillation), NAO (North Atlantic Oscillation), AO and AAO (Arctic Oscillation, Antarctic Oscillation), Indian Monsoon, Indian Ocean Dipole (IOD), PDO (Pacific Decadal Oscillation) and AMO (Atlantic Multidecadal Oscillation). Later it attended stratosphere variability; this constitutes QBO (quasi-biennial oscillation) and SSW (stratospheric sudden warming). Main characteristic features of each of these modes were elaborately discussed
On the Expansion for Surface Displacement in the Neighborhood of a Crack Tip
It is shown that in the expansion of the crack opening displacement vs distance from the tip, there is no linear term present. This should lead to improved accuracy of the near tip fields and improved stress intensity factor results. The two-dimensional discussion should be able to be carried over to three dimensions
Mapping the current psychology provision for children and young people with juvenile dermatomyositis
Objectives: Juvenile Dermatomyositis (JDM) is a rare, chronic autoimmune condition of childhood, with known psychosocial implications. In this study, we sought to establish current psychological support for children and young people across the UK with rheumatic conditions, with a specific focus on those with JDM. Methods: Electronic surveys were distributed to the 15 centres that belong to the JDM Research Group in the UK, collecting responses from health-care professionals in the fields of medicine, nursing and psychology. Results: One hundred per cent of professionals from medicine and nursing replied from all 15 centres. Of these, 7 (47%) did not have a named psychologist as part of their rheumatology team, despite the majority [13 (87%)] having >200 paediatric rheumatology patients. Of the remaining centres, hospital psychology provision varied considerably. When rating their service, only 3 (8%) of 40 professionals scored their service as five (where one is poor and five is excellent); there were wide discrepancies in these scores. Many challenges were discussed, including limited psychology provision, lack of time and difficulties in offering support across large geographical areas. Conclusion: Many of the challenges discussed are applicable to other centres worldwide. Suggestions have been proposed that might help to improve the situation for children and young people with rheumatic conditions, including JDM. Based on these findings, we suggest that rheumatology teams maximize use of these data to advocate and work toward more comprehensive psychology provision and support in their individual centres

Atlantic multidecadal variability and the U.K. acsis program
Abstract
Atlantic multidecadal variability (AMV) is the term used to describe the pattern of variability in North Atlantic sea surface temperatures (SSTs) that is characterized by decades of basinwide warm or cool anomalies, relative to the global mean. AMV has been associated with numerous climate impacts in many regions of the world including decadal variations in temperature and rainfall patterns, hurricane activity, and sea level changes. Given its importance, understanding the physical processes that drive AMV and the extent to which its evolution is predictable is a key challenge in climate science. A leading hypothesis is that natural variations in ocean circulation control changes in ocean heat content and consequently AMV phases. However, this view has been challenged recently by claims that changing natural and anthropogenic radiative forcings are critical drivers of AMV. Others have argued that changes in ocean circulation are not required. Here, we review the leading hypotheses and mechanisms for AMV and discuss the key debates. In particular, we highlight the need for a holistic understanding of AMV. This perspective is a key motivation for a major new U.K. research program: the North Atlantic Climate System Integrated Study (ACSIS), which brings together seven of the United Kingdom’s leading environmental research institutes to enable a broad spectrum approach to the challenges of AMV. ACSIS will deliver the first fully integrated assessment of recent decadal changes in the North Atlantic, will investigate the attribution of these changes to their proximal and ultimate causes, and will assess the potential to predict future changes.</jats:p

On the role of Rossby wave breaking in quasi‐biennial odulation of the stratospheric polar vortex
The boreal‐winter stratospheric polar vortex is more disturbed when the quasi‐biennial oscillation (QBO) in the lower stratosphere is in its easterly phase (eQBO), and more stable during the westerly phase (wQBO). This so‐called “Holton‐Tan effect” (HTE) is known to involve Rossby waves (RWs) but the details remain obscure.
This tropical‐extratropical connection is re‐examined in an attempt to explain its intra‐seasonal variation and its relation to Rossby wave breaking (RWB). Reanalyses in isentropic coordinates from the National Center for Environmental Prediction Climate Forecast System for the 1979 – 2017 period are used to evaluate the relevant features of RWB in the context of waveguide, wave mean‐flow interaction, and the QBO‐induced meridional circulation. During eQBO, the net extratropical wave forcing is enhanced in early winter with ~25% increase in upward propagating PRWs of zonal wavenumber 1 (wave‐1). RWB is also enhanced in the lower stratosphere, characterized by convergent anomalies in the subtropics and at high‐latitudes and strengthened waveguide in between at 20‐40°N, 350‐650 K. In late winter, RWB leads to finite amplitude growth, which hinders upward propagating PRWs of zonal wavenumber 2 and 3 (wave‐2‐3). During wQBO, RWB in association with wave‐2‐3 is enhanced in the upper stratosphere. Wave absorption/mixing in the surf zone reinforces a stable polar vortex in early to middle winter. A poleward confinement of extratropical waveguide in the upper stratosphere forces RWB to extend downward around January. A strengthening of upward propagating wave‐2‐3 follows and the polar‐vortex response switches from reinforcement to disturbance around February, thus a sign reversal of the HTE in late winter
Rationale and design of the ADDITION-Leicester study, a systematic screening programme and randomised controlled trial of multi-factorial cardiovascular risk intervention in people with type 2 diabetes mellitus detected by screening.
BACKGROUND: Earlier diagnosis followed by multi-factorial cardiovascular risk intervention may improve outcomes in type 2 diabetes mellitus (T2DM). Latent phase identification through screening requires structured, appropriately targeted population-based approaches. Providers responsible for implementing screening policy await evidence of clinical and cost effectiveness from randomised intervention trials in screen-detected T2DM cases. UK South Asians are at particularly high risk of abnormal glucose tolerance and T2DM. To be effective national screening programmes must achieve good coverage across the population by identifying barriers to the detection of disease and adapting to the delivery of earlier care. Here we describe the rationale and methods of a systematic community screening programme and randomised controlled trial of cardiovascular risk management within a UK multiethnic setting (ADDITION-Leicester). DESIGN: A single-blind cluster randomised, parallel group trial among people with screen-detected T2DM comparing a protocol driven intensive multi-factorial treatment with conventional care. METHODS: ADDITION-Leicester consists of community-based screening and intervention phases within 20 general practices coordinated from a single academic research centre. Screening adopts a universal diagnostic approach via repeated 75g-oral glucose tolerance tests within an eligible non-diabetic population of 66,320 individuals aged 40-75 years (25-75 years South Asian). Volunteers also provide detailed medical and family histories; complete health questionnaires, undergo anthropometric measures, lipid profiling and a proteinuria assessment. Primary outcome is reduction in modelled Coronary Heart Disease (UKPDS CHD) risk at five years. Seven thousand (30% of South Asian ethnic origin) volunteers over three years will be recruited to identify a screen-detected T2DM cohort (n = 285) powered to detected a 6% relative difference (80% power, alpha 0.05) between treatment groups at one year. Randomisation will occur at practice-level with newly diagnosed T2DM cases receiving either conventional (according to current national guidelines) or intensive (algorithmic target-driven multi-factorial cardiovascular risk intervention) treatments. DISCUSSION: ADDITION-Leicester is the largest multiethnic (targeting >30% South Asian recruitment) community T2DM and vascular risk screening programme in the UK. By assessing feasibility and efficacy of T2DM screening, it will inform national disease prevention policy and contribute significantly to our understanding of the health care needs of UK South Asians. Recommendations for designing genetic test reports to be understood by patients and non-specialists.
Funder: David and Claudia Harding FoundationPatients and non-specialist healthcare professionals are increasingly expected to understand and interpret the results of genetic or genomic testing. These results are currently reported using a variety of templates, containing different amounts, levels, and layouts of information. We set out to establish a set of recommendations for communicating genetic test results to non-expert readers. We employed a qualitative-descriptive study design with user-centred design principles, including a mixture of in-person semi-structured interviews and online questionnaires with patients, healthcare professionals and the general public. The resulting recommendations and example template include providing at-a-glance comprehension of what the test results mean for the patient; suggested next steps; and details of further information and support. Separation and inclusion of technical methodological details enhances non-specialists' understanding, while retaining important information for specialists and the patients' records. The recommendations address the high-level needs of patients and their non-specialist clinicians when receiving genetic test results. These recommendations provide a solid foundation for the major content and structure of reports, and we recommend further engagement with patients and clinicians to tailor reports to specific types of test and results
