    Extending General Equilibrium to the Tariff Line: U.S. Dairy in the DOHA Development Agenda

    Market access has been at the core of eight negotiating rounds of the General Agreement on Tariffs and Trade (GATT). Yet, agricultural trade remains a heavily protected sector, characterized by higher tariffs relative to industrial goods, large tariff dispersions, numerous specific tariffs and systems of tariff-rate-quotas. This has made the analysis of trade liberalization a formidable task among policy analysts. Previous studies of agricultural trade liberalization have used partial or general equilibrium models of trade. However, each of these modeling strategies has their drawbacks. General equilibrium (GE) models have been criticized because they face serious aggregation issues and miss much of the policy detail that occurs at the tariff line. Partial equilibrium (PE) models on the other hand are often more disaggregated but lack internal consistency and have nothing to say about the economy-wide effects from trade reform. The purpose of this paper is threefold. One, we develop a methodology that combines PE and GE modeling techniques permitting us to extend GE to the tariff line. Two, we introduce a fully disaggregated U.S. dairy sector and compare PE and GE liberalization results from global dairy reform, thereby offering some insight into the potential errors implicit in current GE studies.     Effects of Cowichan Lake water management on Vancouver Lamprey in British Columbia and identification of possible enhancement opportunities

    Vancouver Lamprey (also known as Cowichan Lake Lamprey; Entosphenus macrostomus) is listed as a threatened species under the Species at Risk Act (SARA). This species is endemic to three interconnected lakes on Vancouver Island, British Columbia. Cowichan Lake, one of the lakes in which this species resides, is a water reservoir managed to meet the downstream conservation and anthropogenic needs of the watershed. Currently Cowichan Lake water levels and outflows to the Cowichan River are controlled in part by the water control weir at the outlet of Cowichan Lake. Recent droughts in the Cowichan River watershed have resulted in lower than normal summer water levels in the lake which has resulted in reduced outflows to the river as well as dewatering of alluvial fans and lake margins. These dewatered areas include Vancouver Lamprey critical habitat, which is protected under SARA. Protection of Vancouver Lamprey’s critical habitat from destruction was accomplished in February 2020 through a SARA critical habitat order made under subsections 58(4) and (5), which invoked the prohibition in subsection 58(1) against the destruction of the identified critical habitat. It is worth noting that protections under SARA apply to all aquatic species listed as extirpated, endangered or threatened regardless of whether the species is on federal or provincial lands. Decreased lake water levels potentially destroy critical habitat and negatively impact spawning and rearing for this species. Fisheries and Oceans Canada’s (DFO) Fish and Fish Habitat Protection Program (FFHPP) has requested that Science Branch provide science advice on the effect water management has on Vancouver Lamprey spawning and rearing habitat, as well as identification of other conservation issues and enhancement opportunities for critical habitat. The assessment and advice arising from this Canadian Science Advisory Secretariat (CSAS) Science Response (SR) process will be used to inform resource managers on the water management of Cowichan Lake and support the development of current and new water use policy. It may also inform future water management decisions related to the design/operation of the new weir. The specific objectives of this review are to: 1. Evaluate what range of water levels are required to provide maximum protection to Vancouver Lamprey and its critical habitat by life stage for adults (spawning), eggs (incubation), and ammocoetes (feeding and rearing), and when these water levels are required by the species for these life stages (i.e. biologically significant periods). 2. Evaluate how much critical habitat (in square meters or hectares) is being affected by lake draw downs, and at what lake elevations critical habitat starts being affected within the normal range of current water management (full supply level and zero storage with minimum release of 7.08 m3/s into the Cowichan River). 3. Identify conservation issues for Vancouver Lamprey and the types of potential enhancement or rehabilitation opportunities that may exist for this species in the Cowichan Lake watershed. 4. Examine and identify uncertainties in the data and methods.     Teleology and Realism in Leibniz's Philosophy of Science

    This paper argues for an interpretation of Leibniz’s claim that physics requires both mechanical and teleological principles as a view regarding the interpretation of physical theories. Granting that Leibniz’s fundamental ontology remains non-physical, or mentalistic, it argues that teleological principles nevertheless ground a realist commitment about mechanical descriptions of phenomena. The empirical results of the new sciences, according to Leibniz, have genuine truth conditions: there is a fact of the matter about the regularities observed in experience. Taking this stance, however, requires bringing non-empirical reasons to bear upon mechanical causal claims. This paper first evaluates extant interpretations of Leibniz’s thesis that there are two realms in physics as describing parallel, self-sufficient sets of laws. It then examines Leibniz’s use of teleological principles to interpret scientific results in the context of his interventions in debates in seventeenth-century kinematic theory, and in the teaching of Copernicanism. Leibniz’s use of the principle of continuity and the principle of simplicity, for instance, reveal an underlying commitment to the truth-aptness, or approximate truth-aptness, of the new natural sciences. The paper concludes with a brief remark on the relation between metaphysics, theology, and physics in Leibniz

    Effects of gestational age at birth on perinatal structural brain development in healthy term-born babies

    Infants born in early term (37-38 weeks gestation) experience slower neurodevelopment than those born at full term (40-41 weeks gestation). While this could be due to higher perinatal morbidity, gestational age at birth may also have a direct effect on the brain. Here we characterise brain volume and white matter correlates of gestational age at birth in healthy term-born neonates and their relationship to later neurodevelopmental outcome using T2 and diffusion weighted MRI acquired in the neonatal period from a cohort (n = 454) of healthy babies born at term age (>37 weeks gestation) and scanned between 1 and 41 days after birth. Images were analysed using tensor-based morphometry and tract-based spatial statistics. Neurodevelopment was assessed at age 18 months using the Bayley Scales of Infant and Toddler Development, Third Edition (Bayley-III). Infants born earlier had higher relative ventricular volume and lower relative brain volume in the deep grey matter, cerebellum and brainstem. Earlier birth was also associated with lower fractional anisotropy, higher mean, axial, and radial diffusivity in major white matter tracts. Gestational age at birth was positively associated with all Bayley-III subscales at age 18 months. Regression models predicting outcome from gestational age at birth were significantly improved after adding neuroimaging features associated with gestational age at birth. This work adds to the body of evidence of the impact of early term birth and highlights the importance of considering the effect of gestational age at birth in future neuroimaging studies including term-born babies

    Disaster management in low- and middle-income countries: scoping review of the evidence base

    Introduction: Globally, there has been an increase in the prevalence and scale of disasters with low- and middle-income countries (LMICs) tending to be more affected. Consequently, disaster risk reduction has been advocated as a global priority. However, the evidence base for disaster management in these settings is unclear. Methods: This study is a scoping review of the evidence base for disaster management in LMIC. Potentially relevant articles between 1990 and 2011 were searched for, assessed for relevance and subsequently categorised using a thematic coding framework based on the US Integrated Emergency Management System model. Results: Out of 1545 articles identified, only 178 were from LMIC settings. Most were of less robust design such as event reports and commentaries, and 66% pertained to natural disasters. There was a paucity of articles on disaster mitigation or recovery, and more were written on disaster response and preparedness issues. Discussion: Considerably more articles were published from high-income country settings that may reflect a publication bias. Current grey literature on disaster management tends not to be peer reviewed, is not well organised and not easy to access. The paucity of peer-reviewed publications compromises evidence review initiatives that seek to provide an evidence-base for disaster management in LMIC. As such, there is an urgent need for greater research and publication of findings on disaster management issues from these settings

    Utilisation of an operative difficulty grading scale for laparoscopic cholecystectomy

    Background A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets. Methods Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall’s tau for dichotomous variables, or Jonckheere–Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis. Results A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p < 0.001). In the CholeS cohort, a higher difficulty grade was found to be most strongly associated with conversion to open and 30-day mortality (AUROC = 0.903, 0.822, respectively). On multivariable analysis, the Nassar operative difficultly scale was found to be a significant independent predictor of operative duration, conversion to open surgery, 30-day complications and 30-day reintervention (all p < 0.001). Conclusion We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty

    Analysis of early mesothelial cell responses to Staphylococcus epidermidis isolated from patients with peritoneal dialysis-associated peritonitis

    The major complication of peritoneal dialysis (PD) is the development of peritonitis, an infection within the abdominal cavity, primarily caused by bacteria. PD peritonitis is associated with significant morbidity, mortality and health care costs. Staphylococcus epidermidis is the most frequently isolated cause of PD-associated peritonitis. Mesothelial cells are integral to the host response to peritonitis, and subsequent clinical outcomes, yet the effects of infection on mesothelial cells are not well characterised. We systematically investigated the early mesothelial cell response to clinical and reference isolates of S. epidermidis using primary mesothelial cells and the mesothelial cell line Met-5A. Using an unbiased whole genome microarray, followed by a targeted panel of genes known to be involved in the human antibacterial response, we identified 38 differentially regulated genes (adj. p-value &lt; 0.05) representing 35 canonical pathways after 1 hour exposure to S. epidermidis. The top 3 canonical pathways were TNFR2 signaling, IL-17A signaling, and TNFR1 signaling (adj. pvalues of 0.0012, 0.0012 and 0.0019, respectively). Subsequent qPCR validation confirmed significant differences in gene expression in a number of genes not previously described in mesothelial cell responses to infection, with heterogeneity observed between clinical isolates of S. epidermidis, and between Met-5A and primary mesothelial cells. Heterogeneity between different S. epidermidis isolates suggests that specific virulence factors may play critical roles in influencing outcomes from peritonitis. This study provides new insights into early mesothelial cell responses to infection with S. epidermidis, and confirms the importance of validating findings in primary mesothelial cells
