Protocol for a longitudinal qualitative interview study: maintaining psychological well-being in advanced cancer - what can we learn from patients' and carers' own coping strategies?
IntroductionPeople with advanced cancer and their carers experience stress and uncertainty which affects the quality of life and physical and mental health. This study aims to understand how patients and carers recover or maintain psychological well-being by exploring the strategies employed to self-manage stress and uncertainty.Methods and analysisA longitudinal qualitative interview approach with 30 patients with advanced cancer and 30 associated family or informal carers allows the exploration of contexts, mechanisms and outcomes at an individual level. Two interviews, 4–12?weeks apart, will not only enable the exploration of individuals’ evolving coping strategies in response to changing contexts but also how patients’ and carers’ strategies inter-relate. Patient and Carer focus groups will then consider how the findings may be used in developing an intervention. Recruiting through two major tertiary cancer centres in the North West and using deliberately broad and inclusive criteria will enable the sample to capture demographic and experiential breadth.Ethics and disseminationThe research team will draw on their considerable experience to ensure that the study is sensitive to a patient and carer group, which may be considered vulnerable but still values being able to contribute its views. Public and patient involvement (PPI) is integral to the design and is evidenced by: a research advisory group incorporating patient and carers, prestudy consultations with the PPI group at one of the study sites and a user as the named applicant. The study team will use multiple methods to disseminate the findings to clinical, policy and academic audiences. A key element will be engaging health professionals in patient and carer ideas for promoting self-management of psychological well-being. Kronos: a workflow assembler for genome analytics and informatics.
BackgroundThe field of next-generation sequencing informatics has matured to a point where algorithmic advances in sequence alignment and individual feature detection methods have stabilized. Practical and robust implementation of complex analytical workflows (where such tools are structured into "best practices" for automated analysis of next-generation sequencing datasets) still requires significant programming investment and expertise.ResultsWe present Kronos, a software platform for facilitating the development and execution of modular, auditable, and distributable bioinformatics workflows. Kronos obviates the need for explicit coding of workflows by compiling a text configuration file into executable Python applications. Making analysis modules would still require programming. The framework of each workflow includes a run manager to execute the encoded workflows locally (or on a cluster or cloud), parallelize tasks, and log all runtime events. The resulting workflows are highly modular and configurable by construction, facilitating flexible and extensible meta-applications that can be modified easily through configuration file editing. The workflows are fully encoded for ease of distribution and can be instantiated on external systems, a step toward reproducible research and comparative analyses. We introduce a framework for building Kronos components that function as shareable, modular nodes in Kronos workflows.ConclusionsThe Kronos platform provides a standard framework for developers to implement custom tools, reuse existing tools, and contribute to the community at large. Kronos is shipped with both Docker and Amazon Web Services Machine Images. It is free, open source, and available through the Python Package Index and at https://github.com/jtaghiyar/kronos
Perturbations in the relaxation mechanism for a large cosmological constant
Recently, a mechanism for relaxing a large cosmological constant (CC) has
been proposed [arxiv:0902.2215], which permits solutions with low Hubble rates
at late times without fine-tuning. The setup is implemented in the LXCDM
framework, and we found a reasonable cosmological background evolution similar
to the LCDM model with a fine-tuned CC. In this work we analyse analytically
the perturbations in this relaxation model, and we show that their evolution is
also similar to the LCDM model, especially in the matter era. Some tracking

The operationalized psychodynamic diagnostics system. Clinical relevance, reliability and validity
In this paper, we present a multiaxial system for psychodynamic diagnosis, which has attained wide usage in Germany in the last 10 years. First we will discuss the 4 operationalized psychodynamic diagnostics (OPD) axes: illness experience and treatment assumptions, relationships, mental conflicts, and structure, then clinical applications will be outlined. Focus psychodynamic formulations can be employed both with inpatients and with outpatients. Studies show good reliability in a research context and acceptable reliability for clinical purposes. Validity will be separately summarized as content, criterion, and construct validity. Validity studies indicate good validity for the individual axes. Numerous studies on the OPD indicate areas of possible improvement, for example for clinical purposes the OPD should be more practically formulated

An assessment of risk, in terms of delivery delays, cost overrun, and performance achievement, associated with the V/STOL technology airplane is presented. The risk is discussed in terms of weight, structure, aerodynamics, propulsion, mechanical drive, and flight controls. The analysis ensures that risks associated with the design and development of the airplane will be eliminated in the course of the program and a useful technology airplane that meets the predicted cost, schedule, and performance can be produced
Ileal neuroendocrine tumor in a patient with sclerosing mesenteritis: Which came first?
Objective: Unusual clinical courseBackground: Jejunoileal neuroendocrine tumors (JI-NETs) are rare tumors that can be associated with mesenteric fibrosis. This case report is of an incidental finding of a JI-NET in a patient who was previously misdiagnosed with sclerosing mesenteritis.Case Report: A 42-year-old man was admitted to our institution with diffuse abdominal pain and clinical and radiographic signs of bowel obstruction. He had a previous diagnosis of sclerosing mesenteritis, which had been histologically diagnosed after an exploratory laparoscopy performed in 2009 for recurrent acute abdominal pain. He was also annually monitored through computed tomography scans for an incidentally discovered, gradually enlarging mesenteric mass for which a "wait and watch" management approach was adopted. After a period of fasting and observation, the patient underwent an urgent exploratory laparotomy because of his worsening condition. Intraoperatively, an ileocecal resection was performed, along with excision of the known mesenteric mass. The pathology report revealed an ileal NET with nodal metastases within the mesentery and mesenteric tumor deposits (pT3N1).Conclusions: JI-NETs are rare entities, which are usually encountered as incidental findings or in patients with unspecific abdominal pain. Our case represents a probable delayed diagnosis of JI-NET in the context of sclerosing mesenteritis; therefore, a possible association between these 2 conditions should be investigated
Effective growth of matter density fluctuations in the running LCDM and LXCDM models
We investigate the matter density fluctuations \delta\rho/\rho for two dark
energy (DE) models in the literature in which the cosmological term \Lambda is
a running parameter. In the first model, the running LCDM model, matter and DE
exchange energy, whereas in the second model, the LXCDM model, the total DE and
matter components are conserved separately. The LXCDM model was proposed as an
interesting solution to the cosmic coincidence problem. It includes an extra
dynamical component, the "cosmon" X, which interacts with the running \Lambda,
but not with matter. In our analysis we make use of the current value of the
linear bias parameter, b^2(0)= P_{GG}/P_{MM}, where P_{MM} ~
(\delta\rho/\rho)^2 is the present matter power spectrum and P_{GG} is the
galaxy fluctuation power spectrum. The former can be computed within a given
model, and the latter is found from the observed LSS data (at small z) obtained
by the 2dF galaxy redshift survey. It is found that b^2(0)=1 within a 10%
accuracy for the standard LCDM model. Adopting this limit for any DE model and
using a method based on the effective equation of state for the DE, we can set
a limit on the growth of matter density perturbations for the running LCDM
model, the solution of which is known. This provides a good test of the
procedure, which we then apply to the LXCDM model in order to determine the
physical region of parameter space, compatible with the LSS data. In this
region, the LXCDM model is consistent with known observations and provides at

Dilution versus pollution in watercourses affected by acid mine drainage: a graphic model for the Iberian Pyrite Belt (SW Spain)
The aim of this study was to chemically characterize
the water quality impacts of the 88 acid mine drainage
(AMD) generating mines in the Spanish sector of the Iberian
Pyrite Belt (IPB). This was necessary because the Water
Framework Directive of the European Union and the hydrological
plans of the Tinto, Odiel, and Piedras river basins
require that water quality be improved enough to allow at
least some of the rivers in the IPB to sustain healthy fish
populations by 2027. The results indicate a clear decrease in
metals, arsenic, and sulfate concentrations and increased pH

The intense exploration of sulphide ore deposit in São Domingos mine, produced considerable amount of sulphide material that when are exposed to weathering generates acid mine drainage (AMD). The main goal of this paper is to study a specific occurrence regarding composition and morphology of the salts that arise at the end of the dry season. Soluble salts appear at the banks of the drainage channels and over the waste materials, exhibiting different colours and aspects. The obtained results showed by X-Ray diffraction and scanning electronic microscopy, that the mineralogy of salt efflorescences is dominated by sulphates, displaying different occurrence modes. Melanterite and copiapite are the main iron sulphates while minerals from the series of pickeringite-halotrichite dominate the aluminium sulphates. Hubble expansion and structure formation in the "running FLRW model" of the cosmic evolution
A new class of FLRW cosmological models with time-evolving fundamental
parameters should emerge naturally from a description of the expansion of the
universe based on the first principles of quantum field theory and string
theory. Within this general paradigm, one expects that both the gravitational
Newton's coupling, G, and the cosmological term, Lambda, should not be strictly
constant but appear rather as smooth functions of the Hubble rate. This
scenario ("running FLRW model") predicts, in a natural way, the existence of
dynamical dark energy without invoking the participation of extraneous scalar
fields. In this paper, we perform a detailed study of these models in the light
of the latest cosmological data, which serves to illustrate the
phenomenological viability of the new dark energy paradigm as a serious
alternative to the traditional scalar field approaches. By performing a joint
likelihood analysis of the recent SNIa data, the CMB shift parameter, and the
BAOs traced by the Sloan Digital Sky Survey, we put tight constraints on the
main cosmological parameters. Furthermore, we derive the theoretically
predicted dark-matter halo mass function and the corresponding redshift
distribution of cluster-size halos for the "running" models studied. Despite
the fact that these models closely reproduce the standard LCDM Hubble
expansion, their normalization of the perturbation's power-spectrum varies,
imposing, in many cases, a significantly different cluster-size halo redshift
distribution. This fact indicates that it should be relatively easy to
distinguish between the "running" models and the LCDM cosmology using realistic
