    Comparing men's and womens' experiences of multiple exclusion homelessness

    This article explores gender as a variable in multiple exclusion homelessness in England. Much past research has taken insufficient account of the gender of homeless people, especially the predominance of men in the single homeless population and of women heading homeless households with dependent children. Drawing on qualitative data generated in a study of multiple exclusion homelessness in London and Nottingham, the article considers three ways in which gender may act as a homelessness variable: in people's susceptibility to homelessness, in their experiences of homelessness and in their encounters with accommodation services. By comparing the accounts of homeless men and women with complex support needs with evidence from staff working for support agencies, the overall aim of the article is to offer a critical examination of the gendered assumptions of homelessness policy and practice

    Lessons learned from the development and implementation of a patient-reported outcome and experience measure (POEM) in an Australian glaucoma practice.

    BACKGROUND: A patient's perception of how their glaucoma is managed will influence both adherence to their medication and outcome measures such as quality of life. METHODS: Prospective consecutive study using a Glaucoma Patient-reported Outcome and Experience Measure (POEM) modified for an Australian ophthalmic private clinical practice setting. The Australian Glaucoma POEM consists of eight items related to the patient's understanding of the diagnosis and management, acceptability of the treatment, whether they feel their glaucoma is getting worse, interfering with their daily life and concerns regarding loss of vision as well as addressing whether they feel safe under the care of their glaucoma team and how well their care is organised. RESULTS: Two hundred and two patients (M:F 91:111) participated in the study. Mean ± standard deviation for subject age was 69 ± 13 years. Patient's overall perception of their treatment and outcome was favourable. Younger patients felt their glaucoma interfered more with their daily lives and were more worried about losing vision from glaucoma. The greater the number of medications in use, the more they felt their glaucoma was getting worse and that glaucoma interfered with their daily lives. With all other variables accounted for by the multivariate linear model, female patients more strongly agreed that they understood their glaucoma diagnosis and glaucoma management. The patients with a severe visual defect in their worse eye, reported a greater perceived understanding of their glaucoma diagnosis and management and that they felt that glaucoma had a greater interference on their daily life. They were also more concerned about losing vision from glaucoma than their fellow glaucoma patients with less severe or no visual field deficit in the worse eye. CONCLUSIONS: The modified POEM demonstrates potential to capture the concerns of a practice's glaucoma cohort with a view to enhancing the quality of glaucoma care delivered

    A logical framework for forensic DNA interpretation

    The forensic community has devoted much effort over the last decades to the development of a logical framework for forensic interpretation, which is essential for the safe administration of justice. We review the research and guidelines that have been published and provide examples of how to implement them in casework. After a discussion on uncertainty in the criminal trial and the roles that the DNA scientist may take, we present the principles of interpretation for evaluative reporting. We show how their application helps to avoid a common fallacy and present strategies that DNA scientists can apply so that they do not transpose the conditional. We then discuss the hierarchy of propositions and explain why it is considered a fundamental concept for the evaluation of biological results and the differences between assessing results given propositions that are at the source level or the activity level. We show the importance of pre-assessment, especially when the questions relate to the alleged activities, and when transfer and persistence need to be considered by the scientists to guide the court. We conclude with a discussion on statement writing and testimony. This provides guidance on how DNA scientists can report in a balanced, transparent, and logical way

    A three-dimensional multidimensional gas-kinetic scheme for the Navier-Stokes equations under gravitational fields

    This paper extends the gas-kinetic scheme for one-dimensional inviscid shallow water equations (J. Comput. Phys. 178 (2002), pp. 533-562) to multidimensional gas dynamic equations under gravitational fields. Four important issues in the construction of a well-balanced scheme for gas dynamic equations are addressed. First, the inclusion of the gravitational source term into the flux function is necessary. Second, to achieve second-order accuracy of a well-balanced scheme, the Chapman-Enskog expansion of the Boltzmann equation with the inclusion of the external force term is used. Third, to avoid artificial heating in an isolated system under a gravitational field, the source term treatment inside each cell has to be evaluated consistently with the flux evaluation at the cell interface. Fourth, the multidimensional approach with the inclusion of tangential gradients in two-dimensional and three-dimensional cases becomes important in order to maintain the accuracy of the scheme. Many numerical examples are used to validate the above issues, which include the comparison between the solutions from the current scheme and the Strang splitting method.     Rasch analysis of the Patient Rated Elbow Evaluation questionnaire

Background: The Patient Rated Elbow Evaluation (PREE) was developed as an elbow joint specific measure of pain and disability and validated with classical psychometric methods. More recently, Rasch analysis has contributed new methods for analyzing the clinical measurement properties of self-report outcome measures. The objective of the study was to determine aspects of validity of the PREE using the Rasch model to assess the overall fit of the PREE data, the response scaling, individual item fit, differential item functioning (DIF), local dependency, unidimensionality and person separation index (PSI). Methods: A convenience sample of 236 patients (Age range 21-79 years; M: F- 97:139) with elbow disorders were recruited from the Roth|McFarlane Hand and Upper Limb Centre, London, Ontario, Canada. The baseline scores of the PREE were used. Rasch analysis was conducted using RUMM 2030 software on the 3 sub scales of the PREE separately. Results: The 3 sub scales showed misfit initially with disordered thresholds on17 out of 20 items), uniform DIF was observed for two items ( Carrying a 10lbs object from specific activities subscale for age group; and household work from the usual activities subscale for gender); multidimensionality and local dependency. The Pain subscale satisfied Rasch expectations when item 2 Pain - At rest was split for age group, while the usual activities subscale readily stood up to Rasch requirements when the item 2 household work was split for gender. The specific activities subscale demonstrated fit to the Rasch model when sub test analysis accounted for local dependency. All three subscales of the PREE were well targeted and had high reliability (PSI \u3e0.80). Conclusion: The three subscales of the PREE appear to be robust when tested against the Rasch model when subject to a few alterations. The value of changing the 0-10 format is questionable given its widespread use; further Rasch-based analysis of whether these findings are stable in other samples is warranted

    Local Leaders in Random Networks

    We consider local leaders in random uncorrelated networks, i.e. nodes whose degree is higher or equal than the degree of all of their neighbors. An analytical expression is found for the probability of a node of degree kk to be a local leader. This quantity is shown to exhibit a transition from a situation where high degree nodes are local leaders to a situation where they are not when the tail of the degree distribution behaves like the power-law kγc\sim k^{-\gamma_c} with γc=3\gamma_c=3. Theoretical results are verified by computer simulations and the importance of finite-size effects is discussed.Comment: 4 pages, 2 figure
