    Relation between liver lipid content and plasma biochemical indicators in dairy cows

    Background: The incidence of metabolic disorders in dairy cows has increased as a result of intensive production practices and genetic selection for milk yield. Among such disorders, liver lipidosis is very frequently observed and has significant negative effects on cow health and milk yield. Severe energy supply deficit causes high fat mobilization, resulting in rapid body condition loss and liver lipid infiltration, which cannot always be detected by known blood biochemical indicators. The main objective of the study was to determine possible associations of blood biochemical parameters with liver lipid infiltration severity in high-yield multiparous Holstein cows along prepartum and postpartum periods. Materials, Methods & Results: Fifty-four high-yield Holstein cows from a herd managed in a free-stall system in the northwest region of the state of Rio Grande do Sul, Brazil were evaluated. Cows were divided in two groups according to their physiological status. Group 1 included 19 prepartum cows, with an average milk yield of 44.7 L/d in the previous lactation, and 2.8 average parity, and Group 2 included 35 postpartum cows, with an average milk yield of 46.8 L/d in the previous lactation, and 2.5 average parity. The farm produced 10,000 kg milk per year, containing 3.5% of fat, 3.2% of protein and 4.7% of lactose. Total cholesterol (CH), plasma triglycerides (TGp), beta-hydroxybutyrate (BHB), albumin (ALB), non-esterified free fatty acids (NEFA) levels, and aspartate aminotransferase (AST) and creatine kinase (CK) ac-tivities were analyzed in the plasma and liver triglycerides (TGl) levels were determined in liver tissue samples. The SPSS statistical software was used for the statistical analyses. The obtained data were analyzed for normality by the Shapiro-Wilk test and for homoscedasticity by Bartlett test. Data were submitted to analysis of variance, and means were compared by the t-test (PROC TTEST). Pearson correlation test (PROC CORR) was used to compare the results between prepartum and postpartum cows, and between those with and without lipidosis. An overall incidence of 20% severe hepatic lipidosis was determined both in prepartum and postpartum cows. Positive significant correlations were obtained between days in milk (DIM) and AST (r= 0.43; P < 0.01), CH and TGp (r= 0.44; P < 0.01), TGl and AST (r=0.32; P < 0.05), NEFA and AST (r= 0.45; P < 0.01) and CK and AST (r= 0.447; P < 0.01). Negative significant correlations were detected between TGp and DIM (r= -0.45; P < 0.01), and between TGl and TGp (r= -0.30; P < 0.05). Discussion: Prepartum cows showed moderate fat infiltration in the liver, while 25.7% of postpartum cows presented severe lipidosis. AST values increased as lactation progressed, suggesting a greater tendency of liver damage, in early postpartum. The obtained CK values were within the reference range, except for the cows with hepatic lipidosis, indicat¬ing that the measured AST activity was originated from the liver and not from the muscle tissues. TGp, NEFA and AST values indicate the degree of fat mobilization and, possibly, liver infiltration, while TGl indicates fat infiltration in the liver. Although negative energy balance (NEB) may trigger lipolysis and some degree of hepatic lipidosis, plasma BHB values did not significantly increase. The obtained results show that the evaluated plasma parameters may be useful to establish guidelines for the identification of changes in the metabolic profile of dairy cows during different production stages, as well as measures for the prevention and control of metabolic disorders

    El presente artículo tiene como propósito conocer y aplicar la instalación y configuración de los servicios de NethServer, para poder administrar una plataforma TI de una forma segura y acorde a las necesidades de una empresa. Dentro de los servicios que se configuraron están el DHCP, DNS, Directorio Activo, File Server, Print Server y temas de seguridad como lo son VPN, Cortafuegos y Proxy; los cuales en conjunto prestan una correcta administración de la red a nivel de intranet e internet, junto con los componentes de una infraestructura de TI brindando una solución integral a la problemática presentada. La metodología se aplicará sobre entornos de pruebas aislados, detallando paso a paso la configuración en interfaz web de NethServer con capturas ilustrativas y resultados de validación funcional desde clientes GNU/Linux. Esto puede replicarse en cualquier ambiente empresarial que requiera gestionar su infraestructura TI de forma unificada con soluciones de código abierto.The purpose of this article is know and apply the installation and configuration of the NethServer services, to be able to manage a IT platform in a secure way and according to the needs of a company. Within the services that were configured are DHCP, DNS, Active Directory, File Server, Print Server and security issues such as VPN, Firewall and Proxy; which together provide a correct network administration at the intranet and internet level, together with the components of an IT infrastructure providing a comprehensive solution to the problem presented. The methodology will be applied on isolated test environments, detailing step by step the configuration in the web interface of NethServer with illustrative screenshots and results Functional validation from GNU/Linux clients. This can be replicated in any business environment that requires manage your IT infrastructure in a unified way with open source solutions

    Telepsychiatry During the COVID-19 Pandemic: Development of a Protocol for Telemental Health Care

    Background The rapid spread of the Coronavirus disease 2019 (COVID-19) has forced most countries to take drastic public health measures, including the closure of most mental health outpatient services and some inpatient units. This has suddenly created the need to adapt and expand telepsychiatry care across the world. However, not all health care services might be ready to cope with this public health demand. The present study was set to create a practical and clinically useful protocol for telemental health care to be applied in the context of the current COVID-19 pandemic. Methods A panel of psychiatrists from 15 different countries [covering all World Health Organization (WHO) regions] was convened. The panel used a combination of reactive Delphi technique and consensus development conference strategies to develop a protocol for the provision of telemental health care during the COVID-19 pandemic. Results The proposed protocol describes a semi-structured initial assessment and a series of potential interventions matching mild, moderate, or high-intensity needs of target populations. Conclusions Telemedicine has become a pivotal tool in the task of ensuring the continuous provision of mental health care for the population, and the outlined protocol can assist with this task. The strength of this protocol lies in its practicality, clinical usefulness, and wide transferability, resulting from the diversity of the consensus group that developed it. Developed by psychiatrists from around the globe, the proposed protocol may prove helpful for many clinical and cultural contexts, assisting mental health care providers worldwide

    COVID-19 mental health care toolkit: an international collaborative effort by Early Career Psychiatrists section

    The collaborative effort of an international research team from the Early Career Psychiatrists section of the World Psychiatry Association has brought about an easy-to-use, quick and stepwise mental health care toolkit for the identification and appropriate referral of those in need of mental health care during the pandemic. This simple guide can be applied in the general outpatient setting and is catered for all healthcare professionals, regardless of their expertise within the mental health field with minimal training. It is our hope that by incorporating this toolkit into our daily clinical care during the pandemic for high-risk patients and patients with non-specific complaints, we will be able to bridge the mental health gap present in our society

    Influence of clinical and neurocognitive factors in psychosocial functioning after a first episode non-affective psychosis: differences between males and females

    BackgroundDeficits in psychosocial functioning are present in the early stages of psychosis. Several factors, such as premorbid adjustment, neurocognitive performance, and cognitive reserve (CR), potentially influence functionality. Sex differences are observed in individuals with psychosis in multiple domains. Nonetheless, few studies have explored the predictive factors of poor functioning according to sex in first-episode psychosis (FEP). This study aimed to explore sex differences, examine changes, and identify predictors of functioning according to sex after onset.Materials and methodsThe initial sample comprised 588 individuals. However, only adults with non-affective FEP (n = 247, 161 males and 86 females) and healthy controls (n = 224, 142 males and 82 females) were included. A comprehensive assessment including functional, neuropsychological, and clinical scales was performed at baseline and at 2-year follow-up. A linear regression model was used to determine the predictors of functioning at 2-year follow-up.ResultsFEP improved their functionality at follow-up (67.4% of both males and females). In males, longer duration of untreated psychosis (β = 0.328, p = 0.003) and worse premorbid adjustment (β = 0.256, p = 0.023) were associated with impaired functioning at 2-year follow-up, while in females processing speed (β = 0.403, p = 0.003), executive function (β = 0.299, p = 0.020) and CR (β = −0.307, p = 0.012) were significantly associated with functioning.ConclusionOur data indicate that predictors of functioning at 2-year follow-up in the FEP group differ according to sex. Therefore, treatment and preventative efforts may be adjusted taking sex into account. Males may benefit from functional remediation at early stages. Conversely, in females, early interventions centered on CR enhancement and cognitive rehabilitation may be recommended
