    Occupational Nursing Intervention in cardiovascular assessment in health professionals in the Spanish Mediterranean area

    Introducción: Las enfermedades cardiovasculares son la primera causa de muerte en el mundo y guardan una estrecha relación con diferentes factores de riesgo modificables a través de un cambio en el estilo de vida de las personas. Los profesionales sanitarios presentan riesgo para su salud por ser una profesión ejercida bajo estrés y desgaste profesional. Objetivo: Identificar el nivel de riesgo cardiovascular según el grupo de profesional sanitario. Materiales y métodos: Estudio retrospectivo y transversal en 8475 profesionales sanitarios (6645 mujeres y 1830 varones) del área mediterránea española. Se valoran variables antropométricas relacionadas con riesgo cardiovascular, índices aterogénicos, síndrome metabólico, escalas de riesgo cardiovascular y escalas de riesgo de diabetes tipo 2. Se realiza análisis multivariante mediante regresión logística y se determinan las odss ratio. Resultados: La mayor prevalencia de riesgo cardiovascular se obtiene en auxiliares y la menor en Enfermería. Los varones presentan mayor riesgo que las mujeres en todos los parámetros analizados. El hábito tabáquico, consumo de alcohol y obesidad afectan negativamente al riesgo cardiovascular. La realización de ejercicio y una alimentación saludable afectan de manera positiva. Discusión: La existencia de diferentes factores de riesgo cardiovasculares en personas aparentemente sanas muestra la necesidad de más estudios para proporcionar las medidas preventivas más adecuadas. Es importante la figura del especialista en Enfermería del Trabajo, crucial para realizar una adecuada vigilancia de la salud de los trabajadores y detectar posibles patologías y alteraciones desconocidas en ese momento.Introduction: Cardiovascular diseases are the leading cause of death in the world and are closely related to different modifiable risk factors through a change in people’s lifestyle. Discussion: The existence of different cardiovascular risk factors in apparently healthy individuals shows the need for more studies to provide the most appropriate preventive measures. It is important the figure of the specialist in Occupational Nursing, essential to develop an adequate monitoring of the health of the workers and to detect possible pathologies unknown at that moment

    Vineyard Management and Physicochemical Parameters of Soil Affect Native Trichoderma Populations, Sources of Biocontrol Agents against Phaeoacremonium minimum

    [EN] Native strains of Trichoderma in vineyard soil represent an opportunity for reducing the incidence of grapevine trunk diseases (GTDs) in vineyards. Moreover, its relationship with the environment (physicochemical soil characteristics and farming management practices) remains unclear. In the current study, a survey was carried out on farming management used by viticulturists, and soil samples were studied to analyze their physicochemical properties and to isolate Trichoderma strains. Later, statistical analyses were performed to identify possible correlations between Trichoderma populations, soil management and soil characteristics. In addition, in vitro tests, including antibiosis and mycoparasitism, were performed to select those Trichoderma strains able to antagonize Phaeoacremonium minimum. In this study a positive correlation was found between the iron content and pH in the soil, and a lower pH increases Trichoderma populations in soils. Vineyard management also affects Trichoderma populations in the soil, negatively in the case of fertilization and tillage and positively in the case of herbicide spraying. Two Trichoderma native strains were selected as potential biocontrol agents (Trichoderma gamsii T065 and Trichoderma harzianum T087) using antibiosis and mycoparasitism as mechanisms of action.     Effect of Farnesol in Trichoderma Physiology and in Fungal–Plant Interaction

    [EN] Farnesol is an isoprenoid intermediate in the mevalonate (MVA) pathway and is produced by the dephosphorylation of farnesyl diphosphate. Farnesol plays a central role in cell growth and differentiation, controls production of ubiquinone and ergosterol, and participates in the regulation of filamentation and biofilm formation. Despite these important functions, studies of farnesol in filamentous fungi are limited, and information on its effects on antifungal and/or biocontrol activity is scarce. In the present article, we identified the Trichoderma harzianum gene dpp1, encoding a diacylglycerol pyrophosphatase that catalyzes production of farnesol from farnesol diphosphate. We analyzed the function of dpp1 to address the importance of farnesol in Trichoderma physiology and ecology. Overexpression of dpp1 in T. harzianum caused an expected increase in farnesol production as well as a marked change in squalene and ergosterol levels, but overexpression did not affect antifungal activity. In interaction with plants, a dpp1-overexpressing transformant acted as a sensitizing agent in that it up-regulated expression of plant defense salicylate-related genes in the presence of a fungal plant pathogen. In addition, toxicity of farnesol on Trichoderma and plants was examined. Finally, a phylogenetic study of dpp1 was performed to understand its evolutionary history as a primary metabolite gene.     Effects of trichothecene production by Trichoderma arundinaceum isolates from bean-field soils on the defense response, growth and development of bean plants (Phaseolus vulgaris)

    [EN] The trichothecene toxin-producing fungus Trichoderma arundinaceum has potential as a biological control agent. However, most biocontrol studies have focused only on one strain, IBT 40837. In the current study, three Trichoderma isolates recovered from bean-field soils produced the trichothecene harzianum A (HA) and trichodermol, the latter being an intermediate in the HA biosynthesis. Based on phylogenetic analysis, the three isolates were assigned to the species T. arundinaceum. Their genome sequences had a high degree of similarity to the reference IBT 40837 strain, in terms of total genome size, number of predicted genes, and diversity of putative secondary metabolite biosynthetic gene clusters. HA production by these bean-field isolates conferred significant in vitro antifungal activity against Rhizoctonia solani and Sclerotinia sclerotiorum, which are some of the most important bean pathogens. Furthermore, the bean-field isolates stimulated germination of bean seeds and subsequent growth of above ground parts of the bean plant. Transcriptomic analysis of bean plants inoculated with these T. arundinaceum bean-field soil isolates indicated that HA production significantly affected expression of plant defense-related genes; this effect was particularly significant in the expression of chitinase-encoding genes.     Mortality from gastrointestinal congenital anomalies at 264 hospitals in 74 low-income, middle-income, and high-income countries: a multicentre, international, prospective cohort study

    Summary Background Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. Methods We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung’s disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. Findings We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung’s disease) from 264 hospitals (89 in high-income countries, 166 in middleincome countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income countries, and 50 (5·6%) of 896 in high-income countries (p<0·0001 between all country income groups). Gastroschisis had the greatest difference in mortality between country income strata (nine [90·0%] of ten in lowincome countries, 97 [31·9%] of 304 in middle-income countries, and two [1·4%] of 139 in high-income countries; p≤0·0001 between all country income groups). Factors significantly associated with higher mortality for all patients combined included country income status (low-income vs high-income countries, risk ratio 2·78 [95% CI 1·88–4·11], p<0·0001; middle-income vs high-income countries, 2·11 [1·59–2·79], p<0·0001), sepsis at presentation (1·20 [1·04–1·40], p=0·016), higher American Society of Anesthesiologists (ASA) score at primary intervention (ASA 4–5 vs ASA 1–2, 1·82 [1·40–2·35], p<0·0001; ASA 3 vs ASA 1–2, 1·58, [1·30–1·92], p<0·0001]), surgical safety checklist not used (1·39 [1·02–1·90], p=0·035), and ventilation or parenteral nutrition unavailable when needed (ventilation 1·96, [1·41–2·71], p=0·0001; parenteral nutrition 1·35, [1·05–1·74], p=0·018). Administration of parenteral nutrition (0·61, [0·47–0·79], p=0·0002) and use of a peripherally inserted central catheter (0·65 [0·50–0·86], p=0·0024) or percutaneous central line (0·69 [0·48–1·00], p=0·049) were associated with lower mortality. Interpretation Unacceptable differences in mortality exist for gastrointestinal congenital anomalies between lowincome, middle-income, and high-income countries. Improving access to quality neonatal surgical care in LMICs will be vital to achieve Sustainable Development Goal 3.2 of ending preventable deaths in neonates and children younger than 5 years by 2030

    Introduction: Metabolic syndrome (MS) is a set of metabolic disorders related to lifestyle, genetic and environmental factors that increase the risk of cardiovascular disease. Objective: Determine the usefulness of different indices related to cardiovascular risk in predicting MS. Materials and methods: Cross-sectional study in 61283 workers in the Mediterranean area. The indices studied are: body mass index, ponderal index, waist to height ratio, waist to hip ratio, abdominal volume index, body adiposity index, visceral adiposity index, body shape index, body roundness index, conicity index, cholindex, lipid accumulation product and atherogenic indices. The MS is determined by the NCEP ATP III and IDF criteria. ROC curves and cutoff points of each of the indices that best predict SM are determined. Results: The indices that best predict the MS with the NCEP ATP III criteria are the atherogenic index triglycerides / HDL, visceral adiposity index and lipid accumulation product, whereas if we use the IDF criteria the best results are obtained with body roundness index, waist to height ratio, abdominal volume index and lipid accumulation product. The worst results are obtained with the body shape index. Conclusions: The analyzed indices are useful in predicting the onset of MS and could be an alternative to calculate MS. Being easy to get and cheap they are especially useful in the primary care levels.Introducción: El síndrome metabólico (SM) es un conjunto de alteraciones metabólicas relacionadas con el estilo de vida, factores genéticos y ambientales que aumentan el riesgo padecer una enfermedad cardiovascular. Objetivos: Determinar la utilidad de diferentes índices relacionados con el riesgo cardiovascular en la predicción de SM. Material y métodos: Estudio descriptivo transversal en 61283 trabajadores del área mediterránea. Los índices estudiados son: índice de masa corporal, índice ponderal, índice cintura/altura, índice cintura/cadera, índice de volumen abdominal, índice de adiposidad corporal, índice de adiposidad visceral, índice de forma del cuerpo, índice de redondez del cuerpo, índice de conicidad, cholindex, producto de acumulación de lípidos e índices aterogénicos. El SM se determina con los criterios NCEP ATP III e IDF. Se determinan las curvas ROC y los puntos de corte de cada uno de los índices que mejor predicen el SM. Resultados: Los índices que mejor predicen el SM con los criterios NCEP ATPIII son el índice aterogénico triglicéridos/HDL, índice de adiposidad visceral y producto de acumulación de lípidos, mientras que si empleamos los criterios IDF los mejores resultados se obtienen con índice de redondez del cuerpo, índice cintura altura, índice de volumen abdominal y producto de acumulación de lípidos. Los peores resultados se obtienen con el índice de forma del cuerpo. Conclusiones: Los índices analizados son útiles para predecir la aparición de SM y podrían ser una alternativa para calcular SM. Al ser fáciles de obtener y baratos son especialmente útiles en los niveles asistenciales primarios

    Effect of Fruit Weight and Fruit Locule Number in Bell Pepper on Industrial Waste and Quality of Roasted Pepper

    Bell pepper (Capsicum annuum L.), one of the most consumed vegetables worldwide, shows great differences between its diverse varieties. These differences affect the fruit type, size and shape. Food preservation techniques prolong the availability of sweet pepper. Roasted pepper is a product marketed with the European recognition of Protected Geographical Indication ‘Pimiento Asado del Bierzo’. The objective of this work was to analyse the effect of the fruit weight and fruit locule number of the industrial fresh pepper on quality and roasted pepper yield. Large trilocular fruits and large tetralocular fruits reached higher roast yield and uniformity than small trilocular fruits. Regardless of fruit locule number and fruit weight, the overall quality of all the samples of roasted pepper was categorised as very good. Large tetralocular and large trilocular fruits are the most appropriate peppers for industrial purposes, whereas small trilocular fruits should be intended for the fresh product market. This easy method of sorting bell pepper fruit attending to fruit weight will decrease the amount of pepper waste in the industrial roasting process (around 18%), while maintaining the high overall quality of the final product. Moreover, the faster peeling of large peppers will also contribute to increasing the productivity of the industrial processing of roasted pepper

    Xylotrechus arvicola (Coleoptera: Cerambycidae) capture in vineyards in relation to climatic factors

    BACKGROUND: Captures and seasonal abundance of Xylotrechus arvicola (Coleoptera: Cerambycidae) in relation to climatic factors were studied in vineyards between the years 2013 and 2020. Insects captures from vine wood in two Vitis vinifera varieties were evaluated every year by counting the number of insects captured with CROSSTRAP®. The captured insects were grouped (by sex and total) into ranges of 10 days and compared to climatic data (daily average, temperature and rainfall) for each cultivar and year. RESULTS: The capture periods spanned from 1 June and 31 July, with the period from 1 to 30 June having the greatest number of insect captures, as long as weather conditions were favourable, i.e. temperature above 20.00 °C and accumulated rainfall in 10 days lower than 0.40 mm, verified through the analysis of parameter estimates, in which, only the temperature parameter was significantly. CONCLUSIONS: The study provided useful information for the integrated pest management of X. arvicola through mass trapping in vineyards when temperature exceeds 20.00 °C and the accumulated rainfall is less than 0.40 mm in 10 days to obtain peak captures.
