    Esta tesis de grado da cuenta del proceso de diagnóstico, planificación y gestión de una herramienta estratégica de comunicación: el sitio web, en este caso, implementado con la empresa Gustavo Salva Gestión de Negocios. Pero no sólo lo presentamos como un producto final, resultado de un proceso, sino que también presentamos aquí una guía práctica para los profesionales del ámbito comunicacional de distintas organizaciones, ya que da cuenta de los pasos para llevar a cabo un producto de estas características, los tiempos, las estrategias de comunicación con el cliente y con el equipo interdisciplinario con el que trabajamos. A su vez, podrá servir a todos los interesados en sitios Webs y medios tecnológicos.     Voxel model in BNCT treatment planning: Performance analysis and improvements

    In recent years, many efforts have been made to study the performance of treatment planning systems in deriving an accurate dosimetry of the complex radiation fields involved in boron neutron capture therapy (BNCT). The computational model of the patient's anatomy is one of the main factors involved in this subject. This work presents a detailed analysis of the performance of the 1 cm based voxel reconstruction approach. First, a new and improved material assignment algorithm implemented in NCTPlan treatment planning system for BNCT is described. Based on previous works, the performances of the 1 cm based voxel methods used in the MacNCTPlan and NCTPlan treatment planning systems are compared by standard simulation tests. In addition, the NCTPlan voxel model is benchmarked against in-phantom physical dosimetry of the RA-6 reactor of Argentina. This investigation shows the 1 cm resolution to be accurate enough for all reported tests, even in the extreme cases such as a parallelepiped phantom irradiated through one of its sharp edges. This accuracy can be degraded at very shallow depths in which, to improve the estimates, the anatomy images need to be positioned in a suitable way. Rules for this positioning are presented. The skin is considered one of the organs at risk in all BNCT treatments and, in the particular case of cutaneous melanoma of extremities, limits the delivered dose to the patient. Therefore, the performance of the voxel technique is deeply analysed in these shallow regions. A theoretical analysis is carried out to assess the distortion caused by homogenization and material percentage rounding processes. Then, a new strategy for the treatment of surface voxels is proposed and tested using two different irradiation problems. For a parallelepiped phantom perpendicularly irradiated with a 5 keV neutron source, the large thermal neutron fluence deviation present at shallow depths (from 54% at 0 mm depth to 5% at 4 mm depth) is reduced to 2% on average. Reassigning fluence values in the case of this phantom in angular position produced the maximum deviation in the thermal fluence to decrease from 140% to 23% at the surface of the phantom. Thus, even for the largest deviations, obtained by intentionally placing the phantom in the most disadvantageous position with respect to the voxel grid, the reassignment shows very good performance. Since these results substantially improve the performance of the 1 cm based voxel model in surface boundary regions, the proposed strategy will be implemented in future versions of the NCTPlan code.Fil: González, Sara Josefina.     Rhodium self-powered neutron detector as a suitable on-line thermal neutron flux monitor in BNCT treatments

    A rhodium self-powered neutron detector (Rh SPND) has been specifically developed by the Comisión Nacional de Energía Atómica (CNEA) of Argentina to measure locally and in real time thermal neutron fluxes in patients treated with boron neutron capture therapy (BNCT). In this work, the thermal and epithermal neutron response of the Rh SPND was evaluated by studying the detector response to two different reactor spectra. In addition, during clinical trials of the BNCT Project of the CNEA, on-line neutron flux measurements using the specially designed detector were assessed. The first calibration of the detector was done with the well-thermalized neutron spectrum of the CNEA RA-3 reactor thermal column. For this purpose, the reactor spectrum was approximated by a Maxwell-Boltzmann distribution in the thermal energy range. The second calibration was done at different positions along the central axis of a water-filled cylindrical phantom, placed in the mixed thermal-epithermal neutron beam of CNEA RA-6 reactor. In this latter case, the RA-6 neutron spectrum had been well characterized by both calculation and measurement, and it presented some marked differences with the ideal spectrum considered for SPND calibrations at RA-3. In addition, the RA-6 neutron spectrum varied with depth in the water phantom and thus the percentage of the epithermal contribution to the total neutron flux changed at each measurement location. Local (one point-position) and global (several points-positions) and thermal and mixed-field thermal neutron sensitivities were determined from these measurements. Thermal neutron flux was also measured during BNCT clinical trials within the irradiation fields incident on the patients. In order to achieve this, the detector was placed on patient´s skin at dosimetric reference points for each one of the fields. System stability was adequate for this kind of measurement. Local mixed-field thermal neutron sensitivities and global thermal and mixed-field thermal neutron sensitivities derived from measurements performed at the RA-6 were compared and no significant differences were found. Global RA-6-based thermal neutron sensitivity showed agreement with pure thermal neutron sensitivity measurements performed in the RA-3 spectrum. Additionally, the detector response proved nearly unchanged by differences in neutron spectra from real (RA-6 BNCT beam) and ideal (considered for calibration calculations at RA-3) neutron source descriptions. The results confirm that the special design of the Rh SPND can be considered as having a pure thermal response for neutron spectra with epithermal-to-thermal flux ratios up to 12%. In addition, the linear response of the detector to thermal flux allows the use of a mixed-field thermal neutron sensitivity of 1.95 ± 0.05 × 10(-21) A n(-1)[middle dot]cm² [middle dot]s. This sensitivity can be used in spectra with up to 21% epithermal-to-thermal flux ratio without significant error due to epithermal neutron and gamma induced effects. The values of the measured fluxes in clinical applications had discrepancies with calculated results that were in the range of -25% to +30%, which shows the importance of a local on-line independent measurement as part of a treatment planning quality control system. The usefulness of the CNEA Rh SPND for the on-line local measurement of thermal neutron flux on BNCT patients has been demonstrated based on an appropriate neutron spectra calibration and clinical applications.Fil: Miller, Marcelo Eduardo.     Quality and shelf life of two cultivars of sapota mamey (Pouteria sapota) harvested in Yucatán, Mexico

    [SPA] En Yucatán, México, existe una gran diversidad de frutales tropicales y subtropicales poco conocidos en los mercados internacionales, pero que poseen adecuadas características de calidad para su comercialización. El mamey (Pouteria sapota) es una de estas plantas cuyo cultivo se está incrementando. Se han seleccionado algunos cultivares con características sobresalientes, como los cultivares Magaña-1 (M1) y Magaña-2 (M2). Hoy día, para que frutos tropicales exóticos, como el mamey, puedan ser competitivos en los mercados de frutas frescas, es necesario que se mantengan con buena calidad por un tiempo adecuado para facilitar su transporte y comercialización. En este trabajo, frutos de mamey de los cultivares M1 y M2 cultivados en Huertas Magaña en el estado de Yucatán, México, fueron evaluados para conocer su calidad y vida útil con refrigeración. Se cosecharon 160 frutos por cultivar, 40 de cada selección se dejaron madurar a temperatura ambiente (28 ± 2 ºC) y los restantes se refrigeraron a 14 ± 1 º C. Al madurar se evaluaron las principales características físicas, químicas y sensoriales. Los frutos almacenados a temperatura ambiente se evaluaron cada día hasta que maduraron y de los frutos refrigerados, cada 5 días se fueron retirando 30 frutos, hasta completar 20 días, y a la salida se dejaron madurar a temperatura ambiente. Ya maduros, se les evaluaron sus características de calidad y se determinó su vida útil. Se encontró que los frutos de M1 y M2 almacenados a temperatura ambiente maduraron 6 y 7 días después de su cosecha, respectivamente, manteniéndose con buena calidad 1 o 2 días adicionales. Los frutos de ambos cultivares refrigerados a 14 ºC durante 15 días, maduraron satisfactoriamente cuando se regresaron a la temperatura ambiente, alcanzando sus características propias de calidad, como son sabor dulce, pulpa de color rojo escarlata, buen aroma, poca fibra, cáscara dura, características de calidad adecuadas para su comercialización. Se concluye que los frutos de mamey de los cultivares M1 y M2 evaluados, independientemente del tiempo que tardaron en madurar, desarrollaron sus características de calidad a los 18 y 20 días, por lo que su vida útil se incrementó 11 y 13 días respecto a los frutos madurados a temperatura ambiente. [ENG] In Yucatán, Mexico exists a great diversity of tropical and subtropical fruits with appropriated characteristics of quality for their marketing, but they are not so much known in the international markets. The production of mamey (Pouteria sapota), one of these fruits, has been increasing. Some cultivars of this fruit with excellent characteristics of quality have been selected, like Magaña-1 (M1) and Magaña-2 (M2) cultivars. Nowadays, the tropical fruits like mamey could be competitive in the fresh fruits international markets; however it is necessary that they maintain their quality for an adequate time in order to facilitate their transport and commercialization. In this work, fruits of two cultivars of mamey, M1 and M2, harvested in Huertas Magaña in Yucatán, Mexico, were evaluated in order to know their quality and shelf life at 14ºC after harvesting. 160 fruits of each cultivar were harvested, 40 fruits of each cultivar were allowed ripening at room temperature (28 ± 2 ºC) and the others were stored at 14 ± 1 º C. When these fruits were maturated their main physical, chemical and general acceptance characteristics of quality were evaluated. During the ripening, the fruits stored at room temperature were evaluated every day, until they reach their characteristics for consuming. In the case of the refrigerated fruits, 30 mamey fruits were taken from storage every 5 days, until 20 days were completed of storage and were allowed to ripening at room temperature (28 ± 2 °C). The principal characteristics of quality of these matured fruits were analyzed and their shelf lives were determined too. It was found that mamey fruits of M1 and M2 cultivars stored at room temperature matured 6 and 7 days later of their harvest, respectively, conserving their characteristics of quality 1 or 2 days more at the same temperature. The refrigerated fruits of both mamey cultivars after 15 days matured satisfactorily when they were changed to room temperature, their normal characteristics of quality like sweet flavor, red scarlet colour pulp, characteristic aroma, a little quantity of fiber and hard skin were reached. We conclude that the mamey fruits of M1 and M2 cultivars storage at 14 ± 1 º C, developed their normal characteristics of quality at 18 and 20 days after they were harvested, which indicates than their shelf life were increased 11 and 13 days more than matured fruits at room temperature.A la plantación de mamey “Huertas Magaña” por la materia prima otorgada para este estudio.     Identification of polysaccharide capsules among extensively drug-resistant genitourinary Haemophilus parainfluenzae isolates

    The human commensal Haemophilus parainfluenzae is emerging as an opportunistic multidrug-resistant pathogen. The objectives of this work were to characterise a new capsular operon of extensively drug-resistant (XDR) H. parainfluenzae clinical isolates and study their resistance mechanisms using whole-genome sequencing. All strains were resistant to: ß-lactams, via amino acid changes in PBP3 (S385T, I442F, V511A, N526K and V562I); quinolones, by alterations in GyrA (S84F and D88Y) and ParC (S84F and S138T); chloramphenicol, through the presence of catS; macrolides, via the presence of mel and mef(E)-carrying MEGA element; and tetracycline, through the presence of tet(M) and/or tet(B). Phylogenetic analysis revealed high genomic diversity when compared to the H. parainfluenzae genomes available on the NCBI, the isolates from this study being closely related to the Swiss XDR AE-2096513. A full capsular operon showing homology to that of H. influenzae was identified, in accordance with the observation of a capsular structure by TEM. This study describes for the first time a capsular operon in H. parainfluenzae, a major determinant of pathogenicity that may contribute to increased virulence in XDR clinical isolates. Moreover, phylogenetic analysis suggests the possible spread of an XDR-encapsulated strain in Europe

    Understanding the potentiality of accelerator based-boron neutron capture therapy for osteosarcoma: Dosimetry assessment based on the reported clinical experience

    Background: Osteosarcoma is the most frequent primary malignant bone tumour, and its incidence is higher in children and adolescents, for whom it represents more than 10% of solid cancers. Despite the introduction of adjuvant and neo-adjuvant chemotherapy that markedly increased the success rate in the treatment, aggressive surgery is still needed and a considerable percentage of patients do not survive due to recurrences or early metastases. Boron Neutron Capture Therapy (BNCT), an experimental radiotherapy, was investigated as a treatment that could allow a less aggressive surgery by killing infiltrated tumour cells in the surrounding healthy tissues. BNCT requires an intense neutron beam to ensure irradiation times of the order of 1h. In Italy, a Radio Frequency Quadrupole (RFQ) proton accelerator has been designed and constructed for BNCT, and a suitable neutron spectrum was tailored by means of Monte Carlo calculations. This paper explores the feasibility of BNCT to treat osteosarcoma using this neutron source based on accelerator. Methods: The therapeutic efficacy of BNCT was analysed evaluating the dose distribution obtained in a clinical case of femur osteosarcoma. Mixed field dosimetry was assessed with two different formalisms whose parameters were specifically derived from radiobiological experiments involving in vitro UMR-106 osteosarcoma cell survival assays and boron concentration assessments in an animal model of osteosarcoma. A clinical case of skull osteosarcoma treated with BNCT in Japan was re-evaluated from the point of view of dose calculation and used as a reference for comparison. Results: The results in the case of femur osteosarcoma show that the RFQ beam would ensure a suitable tumour dose painting in a total irradiation time of less than an hour. Comparing the dosimetry between the analysed case and the treated patient in Japan it turns out that doses obtained in the femur tumour are at least as good as the ones delivered in the skull osteosarcoma. The same is concluded when the comparison is carried out taking into account osteosarcoma irradiations with photon radiation therapy. Conclusions: The possibility to apply BNCT to osteosarcoma would allow a multimodal treatment consisting in neo-adjuvant chemotherapy, high-LET selective radiation treatment and a more conservative surgery.Fil: Bortolussi, Silva.     Genome-wide analysis of urogenital and respiratory multidrug-resistant Haemophilus parainfluenzae

    Objectives: To characterize the mechanisms of antimicrobial resistance and the prevalence of the polysaccharide capsule among urogenital and respiratory Haemophilus parainfluenzae isolates. Methods: Antimicrobial susceptibility was tested by microdilution. Fifty-five MDR strains were subjected to WGS and were phylogenetically compared with all the available H. parainfluenzae genomes from the NCBI database. The identification of the capsular bexA gene was performed by PCR in 266 non-MDR strains. Results: In 31 of the 42 ampicillin-resistant strains, blaTEM-1 located within Tn3 was identified. β-Lactamase-negative cefuroxime-resistant strains (n=12) presented PBP3 substitutions. The catS gene (n=14), the tet(M)-MEGA element (n=18) and FolA substitutions (I95L and F154V/S) (n=41) were associated with resistance to chloramphenicol, tetracycline plus macrolides, and co-trimoxazole, respectively. Thirty-seven isolates had a Tn10 harbouring tet(B)/(C)/(D)/(R) genes with (n=15) or without (n=22) catA2. Putative transposons (Tn7076-Tn7079), including aminoglycoside and co-trimoxazole resistance genes, were identified in 10 strains (18.2%). These transposons were integrated into three new integrative and conjugative elements (ICEs), which also included the resistance-associated transposons Tn3 and Tn10. The capsular operon was found only in the urogenital isolates (18/154, 11.7%), but no phylogenetic clustering was observed. The capsular operons identified were similar to those of Haemophilus influenzae serotype c and Haemophilus sputorum type 2. Conclusions: The identification of ICEs with up to three resistance-associated transposons suggests that these transferable elements play an important role in the acquisition of multidrug resistance in H. parainfluenzae.     Overview of pneumococcal serotypes and genotypes causing diseases in patients with chronic obstructive pulmonary disease in a Spanish hospital between 2013 and 2016

    Background: Streptococcus pneumoniae is an important pathogen in chronic obstructive pulmonary disease (COPD). We aimed at showing the recent changes in the epidemiology of pneumococcal diseases in patients with COPD, especially after the introduction of the 13-valent pneumococcal conjugate vaccine (PCV13). Methods: From 2013 to 2016, strains causing invasive pneumococcal disease (IPD), non-bacteremic pneumococcal pneumonia (non-BPP), and acute exacerbation of COPD (AE-COPD) were prospectively included. Antimicrobial susceptibility testing, serotyping, and genotyping were analyzed. Results: We collected 345 pneumococci from 286 COPD patients (57 IPD, 78 non-BPP, and 210 AE-COPD). The most frequent serotypes were serotypes 3 (14.0%), 8 (14.0%), and 12F (8.8%) in IPD; serotypes 3 (16.7%), 11A (9%), 9L/N (7.7%), and 23A (7.7%) in non-BPP; and serotypes 11A (11%), nontypeable (11%), and 6C (10%) in AE-COPD. Resistance rates were highest among AE-COPD strains. Penicillin resistance was associated with serotypes 11A (CC156) and 19A (CC320 and CC230). Compared with previous studies, fluoroquinolone resistance in AE-COPD increased (9.5%), PCV13 serotypes decreased (31.6%, 26.9%, and 16.7% for IPD, non-BPP, and AE-COPD, respectively), and serotype 11A-CC156 in AE-COPD and serotype 8 in IPD increased. Conclusion: The epidemiology of pneumococcal disease in COPD changed after the introduction of PCV13 in children. Increases in the highly invasive serotype 8 among patients with IPD and in serotype 11A-CC156 among patients with AE-COPD could compromise the ability of current PCVs to prevent diseases. Vaccines with a greater coverage could improve the benefits of adult vaccination

    Extending neutron autoradiography technique for boron concentration measurements in hard tissues

    The neutron autoradiography technique using polycarbonate nuclear track detectors (NTD) has been extended to quantify the boron concentration in hard tissues, an application of special interest in Boron Neutron Capture Therapy (BNCT). Chemical and mechanical processing methods to prepare thin tissue sections as required by this technique have been explored. Four different decalcification methods governed by slow and fast kinetics were tested in boron-loaded bones. Due to the significant loss of the boron content, this technique was discarded. On the contrary, mechanical manipulation to obtain bone powder and tissue sections of tens of microns thick proved reproducible and suitable, ensuring a proper conservation of the boron content in the samples. A calibration curve that relates the 10B concentration of a bone sample and the track density in a Lexan NTD is presented. Bone powder embedded in boric acid solution with known boron concentrations between 0 and 100 ppm was used as a standard material. The samples, contained in slim Lexan cases, were exposed to a neutron fluence of 1012 cm−2 at the thermal column central facility of the RA-3 reactor (Argentina). The revealed tracks in the NTD were counted with an image processing software. The effect of track overlapping was studied and corresponding corrections were implemented in the presented calibration curve. Stochastic simulations of the track densities produced by the products of the 10B thermal neutron capture reaction for different boron concentrations in bone were performed and compared with the experimental results. The remarkable agreement between the two curves suggested the suitability of the obtained experimental calibration curve. This neutron autoradiography technique was finally applied to determine the boron concentration in pulverized and compact bone samples coming from a sheep experimental model. The obtained results for both type of samples agreed with boron measurements carried out by ICP-OES within experimental uncertainties. The fact that the histological structure of bone sections remains preserved allows for future boron microdistribution analysis.Fil: Provenzano, Lucas.
