Effects of TNFα receptor TNF-Rp55- or TNF-Rp75- deficiency on corneal neovascularization and lymphangiogenesis in the mouse
Tumor necrosis factor (TNF)α is an inflammatory cytokine likely to be involved in the process of corneal inflammation and neovascularization. In the present study we evaluate the role of the two receptors, TNF-receptor (TNF-R)p55 and TNF-Rp75, in the mouse model of suture-induced corneal neovascularization and lymphangiogenesis. Corneal neovascularization and lymphangiogenesis were induced by three 11–0 intrastromal corneal sutures in wild-type (WT) C57BL/6J mice and TNF-Rp55-deficient (TNF-Rp55d) and TNF-Rp75-deficient (TNF-Rp75d) mice. The mRNA expression of VEGF-A, VEGF-C, Lyve-1 and TNFα and its receptors was quantified by qPCR. The area covered with blood- or lymphatic vessels, respectively, was analyzed by immunohistochemistry of corneal flatmounts. Expression and localization of TNFα and its receptors was assessed by immunohistochemistry of sagittal sections and Western Blot. Both receptors are expressed in the murine cornea and are not differentially regulated by the genetic alteration. Both TNF-Rp55d and TNF-Rp75d mice showed a decrease in vascularized area compared to wild-type mice 14 days after suture treatment. After 21 days there were no differences detectable between the groups. The number of VEGF-A-expressing macrophages did not differ when comparing WT to TNF-Rp55d and TNF-Rp75d. The mRNA expression of lymphangiogenic markers VEGF-C or LYVE-1 does not increase after suture in all 3 groups and lymphangiogenesis showed a delayed effect only for TNF-Rp75d. TNFα mRNA and protein expression increased after suture treatment but showed no difference between the three groups. In the suture-induced mouse model, TNFα and its ligands TNF-Rp55 and TNF-Rp75 do not play a significant role in the pathogenesis of neovascularisation and lymphangiogenesis
Visual outcomes and complications following posterior iris-claw aphakic intraocular lens implantation combined with penetrating keratoplasty
Abstract Background To evaluate the indication, visual outcome, and complication rate after implantation of a posterior iris-claw aphakic intraocular lens (IOL) during penetrating keratoplasty. Methods This retrospective study comprised 23 eyes (23 patients) without adequate capsule support undergoing posterior iris-claw aphakic IOL implantation (Verisyse™/ Artisan®) during penetrating keratoplasty between 2005 and 2010. Mean follow-up was 18 months (range from 12 to 37 months). Results The IOLs were inserted during an IOL exchange in 17 eyes and as a secondary procedure in six aphakic eyes. Pseudophakic bullous keratopathy with corneal scar after anterior chamber intraocular lens (ACIOL) was the main indication for penetrating keratoplasty in 16 eyes (69.6 %). The final corrected distance visual acuity (CDVA) in logMAR (mean 1.0 ± 0.46) improved significantly (p < 0.05) compared to the preoperative CDVA (mean 1.8± 0.73). Twenty eyes (86.9 %) had a final visual acuity in logMAR better than the pre-operative CDVA. The mean postoperative IOP 16.3 mmHg±4.0 was not significantly (p > 0.05) higher compared to the preoperative IOP 15.6 mmHg±5.1. Complications included slight temporary pupil ovalization in three eyes (13.0 %) and iris-claw IOL sublocation in three eyes (13.0 %); all IOLs could be easily repositioned. Cystoid macular edema occured in one eye (4.3 %) 8 weeks after primary surgery. All grafts remained clear without any sign of graft rejection. Conclusions Retropupillar iris-claw IOL during penetrating keratoplasty provides good visual outcomes with a favorable complication rate, and can be used for a wide range of indications in eyes without adequate capsule support
A new approach of the sterilization of allogenic soft tissue grafts in anterior cruciate ligament reconstruction
Einleitung: Allogene Sehnentransplantate haben in den letzten 20 Jahren auf
Grund ihrer großen Vorteile wie dem Entfall der Entnahmemorbidität und der
beliebigen Transplantatdimensionierung immer mehr an Bedeutung in der
Kreuzbandchirurgie gewonnen. Jedoch existieren bis dato keine
Sterilisationsverfahren, die einen kompletten Schutz vor Infektion und
Kontamination ohne Beeinträchtigung der initialen biomechanischen und
strukturellen Eigenschaften der Allografts gewährleisten können. Das
Peressigsäure-Ethanol-Unterdruck-Verfahren ist ein etabliertes und
kostengünstiges Sterilisationsverfahren bei allogenen Knochentransplantaten
und bietet auch bei allogenen Weichteiltransplantaten einen adäquaten
antibakteriellen, antimykotischen und antiviralen Schutz. Nach hoffnungsvollen
in-vitro Studien ist dies die erste in-vivo Studie am Schafsmodell zur
Untersuchung biomechanischer Eigenschaften und der Revaskularisierung in der
frühen Einheilungsphase des Remodelings von allogenen Sehnentransplantaten
nach Sterilisation mit dem Peressigsäure-Ethanol-Unterdruck-Verfahren zum
vorderen Kreuzbandersatz. Methoden: 18 ausgewachsene weibliche Merino-Schafe
erhielten einen Ersatz des vorderen Kreuzbandes mittels PES-sterilisierten
allogenen Flexorsehnentransplantaten. Nach 6 und 12 Wochen erfolgte die Tötung
sowie die biomechanische Testung mittels standardisierter Schubladen- und
Versagenstests von jeweils 9 Tieren. Nach histologischer Aufarbeitung erfolgte
neben konventionellen (HE, Masson Goldner) Färbungen zur deskriptiven
Auswertung der zellulären und strukturellen Umbauvorgänge eine
immunhistochemische (von-Willebrand-Faktor/Faktor VIII) Färbung von
Endothelzellen der Gefäßwand mit anschließender Auszählung der Gefäßanschnitte
in Querschnittspräparaten in je drei Zonen (subsynovial, intermediär und
zentral) zur quantitativen Bestimmung der Revaskularisierung des Allografts.
Eine Vorstudie lieferte Kontrollgruppen, die zu gleichen Standzeiten mit dem
selben Operateur am Schafsmodell identische histologische und biomechanische
Untersuchungen zum Vergleich tiefgefrorener, unsterilisierter Allografts und
Autografts nutzten. Ergebnis: Das PES-sterilisierte Allograft war in allen
biomechanischen Testungen dem tiefgefrorenen, unsterilisierten Allograft und
dem Autograft unterlegen, wobei die Unterschiede im Verlauf nach 12 Wochen
immer deutlicher wurden. In Korrelation zeigte sich eine verlangsamte
Revaskularisierung. Die Gefäßdichte war nach 6 und 12 Wochen in allen Regionen
im Vergleich zu den Kontrollgruppen verringert. Auch deskriptiv war die
Revaskularisierung und zelluläre Repopulation der PES-behandelten Allografts
weniger weit nach zentral fortgeschritten als in den Kontrollgruppen. Die
Gewebestruktur erschien in den PES-Allografts im Vergleich stets ungeordneter.
Bei 66,6% der 12-Wochentiere bildete sich zudem eine klinisch asymptomatische,
aber histologisch gesicherte fibrotische Hypertrophie am Eingang zum tibialen
Knochentunnel aus. Intraligamentär zeigte sich jedoch nur eine leicht
chronisch granulierende Entzündung zum Zeitpunkt von 6 und 12 Wochen.
Diskussion/Fazit: Entgegen hoffnungsvoller in-vitro Studien sind die
verlangsamte bzw. gestörte Revaskularisierung sowie die im Vergleich zu
Autografts und tiefgefrorenen, unsterilisierten Allografts signifikant
schlechteren biomechanischen Eigenschaften ein Indiz für den negativen
Einfluss des Peressigsäure-Ethanol-Unterdruck-Verfahrens auf den
Remodelingprozess knochenblockfreier Flexorsehnentransplantate im
Schafsmodell. Auf diesen Ergebnissen basierend können wir die Sterilisation
knochenblockfreier Sehnentransplantate mit dem Peressigsäure-Ethanol-
Unterdruck-Verfahren zum vorderen Kreuzbandersatz vorerst nicht
weiterempfehlen.Introduction: Allografts have gained significant popularity in revision and
primary reconstructive procedures of the anterior cruciate ligament during the
last 20 years. Their great advantage is the avoidance of harvest morbidity.
However, until today most of the current sterilization procedures have
inherent disadvantages for the graft´s biological properties and mechanical
function. Peracetic acid-ethanol (PAA) sterilization of soft-tissue allografts
has gained increasing attention due to its successful application in the
sterilization of bone allografts. It also showed adequate antibiotic,
antifungal and antiviral protection for soft tissue grafts. In-vitro studies
showed no adverse effect on the mechanical properties of such sterilized soft-
tissue grafts. However, no in-vivo studies exist that analyzed the
revascularization of the short-term healing of soft tissue grafts and their
biomechanic proporties after this sterilization procedure for anterior
cruciate ligament (ACL) reconstruction. Methods: 18 mature female merino sheep
received open ACL reconstruction with a PAA sterilized soft-tissue allograft.
Nine sheep were sacrificed at 6 and 12 weeks, when they underwent an anterior-
posterior (ap) drawer test and a load-to-failure test to obtain the structural
properties. Conventional histological analysis (H&E;, Masson Goldner) was
used for descriptive analysis of cell distribution, cell morphology,
appearance of foreign-body giant and inflammatory cells. The endothelial
surface cells of blood vessels were immunostained with antihuman von-
Willebrand-factor/factor VIII in transverse sections to obtain the
quantification of the vascular density. Therefore the transverse sections were
subdivided into a subsynovial (SUB), an intermediate (MED) and a central (CNT)
region. Fresh-frozen allograft (ALLO) and autologous (AUTO) ACL
reconstructions of a previous study with an identical animal model and
histological and biomechanical evaluations at corresponding time points served
as controls. Results: Nearly all biomechanical properties of the PAA
sterilized allograft were reduced in correlation to the control groups and the
differences became more and more significant after 12 weeks. At the same time
a reduced revaskularization was observed. The vascular density after 6 and 12
weeks was reduced in correlation to the control groups. Further showed the
descriptive analysis that the revascularisation and the cellular repopulation
were less expanded to the center of the grafts. Also the structural properties
seemed less remodelled. In 67% percent of all PAA animals a fibrotic tissue
was found at the tibial tunnel entrance, which was not observed in any of the
control groups. Althoug intraligamental could only a mild chronical
granulocytic inflammation detected. Discussion: Even though in-vitro
mechanical data were promising, PAA sterilization lead to an inhibition or
rather reduction of the remodeling processes in the healing ACL soft-tissue
graft. These alterations of graft ligamentization and especially
revascularization resulted in substantially impaired structural properties and
show the adverse effect of PAA sterilization on the graft healing of soft
tissue grafts in the sheep model. Therefore, based on these findings we do not
recommend sterilization of soft-tissue allografts with peracetic acid-ethanol
claw intraocular lens
Bei insuffizientem Kapselsackhalteapparat gibt es eine große Auswahl an
alternativen Fixationstechniken einer IOL, wie z.B. kammerwinkel- oder
irisgestützte Vorderkammerlinsen sowie intra-/transskleral-fixierte (Naht,
Fibrinkleber, etc.) und irisgestützte Hinterkammerlinsen. Alle Verfahren haben
je nach anatomischer Ausgangssituation und Integrität des Zonula-Kapselsack-
Komplexes sowie der benachbarten Strukturen wie Iris, Sklera und
Hornhautendothel unter Berücksichtigung der Lebenserwartung und okularen
Komorbiditäten des Patienten ihre Berechtigung und sind mit unterschiedlichen
Vor- und Nachteilen behaftet. Wir konnten im bisher größten publizierten
Patientenkollektiv an phaken, aphaken und pseudophaken Augen ohne adäquaten
Zonula-Kapsel-Komplex die sehr guten Ergebnisse der inversen, retropupillaren
Irisklauenlinse bei geringer Komplikationsrate bekräftigen. Ferner berichteten
wir erstmals über mittelfristige Ergebnisse der retropupillaren
Irisklauenlinse bei einem adäquaten Endothelzellverlust von 5,5% über eine
mittlere Nachbeobachtungszeit von 34 Monaten. Anhand lichtmikroskopischer
Untersuchungen konnten wir morphologische Charakteristika nach traumatischer
und spontaner Desenklavation von Irisklauenlinsen aufweisen. Basierend auf den
Ergebnissen dieser Untersuchungen sehen wir die kontrovers diskutierte
Reenklavation einer traumatisch dislozierten Irisklauenlinse gegenüber einem
Linsenaustausch nicht mehr als gerechtfertigt. In der klinischen Anwendung
empfiehlt es sich daher, traumatisch dislozierte Irisklauenlinsen
auszutauschen und nicht zu reenklavieren. Aufgrund der guten Ergebnisse der
retropupillar fixierten Irisklauenlinse bei Erwachsenen führten wir zum ersten
Mal diese alternative Fixationstechnik bei kindlichen Augen ohne adäquaten
Kapselsackhalteapparat mit guten Ergebnissen bei geringer Komplikationsrate
durch. Nach eigenen positiven Ergebnissen in Kombination mit einer
perforierenden Keratoplastik kombinierten wir die retropupillare
Irisklauenlinse erstmals mit einer hinteren lamellierenden DMEK (Descemet
membrane endothelial keratoplasty) und konnten somit das Indikationsspektrum
dieser alternativen Fixationstechnik weiter verbreitern. Zusammenfassend
stellt die Implantation einer retropupillaren Irisklauenlinse eine schnelle,
sichere, einfache und effektive chirurgische Korrektur einer Aphakie oder
Linsen(sub-)luxation in einem breiten Spektrum an Augen ohne adäquaten
Kapselsackhalteapparat dar. Die tiefere postoperative Vorderkammer und der
größere Abstand vom cornealen Endothel resultieren in einem geringeren
Endothelzellverlust im Vergleich zur originalen, präpupillaren
Fixationsvariante.There is a wide range of alternative fixation techniques of intraocular lenses
in the absence of adequate capsular support, such as angle- or iris-supported
anterior chamber lenses and intra- / transsclerally-fixated (suture, fibrin
glue, etc.) and iris-supported posterior chamber lenses. All procedures have
different advantages and disadvantages depending on the anatomical situation
and integrity of the capsular bag complex and the neighboring structures such
as the iris, sclera and corneal endothelium. Furthermore, life expectancy and
ocular comorbidities of the patient should be considered. In the so far
largest published (to the best of our knowledge) study on posterior iris-claw
aphakic lenses we were able to confirm the very good results of the inverse,
retropupillar implantation technique in phakic, aphakic and pseudophakic eyes
without adequate capsular support with a low complication rate. In addition,
we first reported mid-term results of retropupillary fixated iris claw lenses
with an adequate endothelial cell loss of 5.5% over a mean follow-up of 34
months. Using light microscopy we were able to analyze morphological
characteristics after traumatic and spontaneous disenclavation of iris claw
lenses. Based on these results, we do not recommend the controversially
discussed reenclavation procedure of traumatically dislocated iris claw
lenses. We therefore advise to perform an IOL exchange. Due to the good
results of retropupillary fixated iris claw lenses in adults, we introduced
this alternative fixation technique in children with good results and a low
complication rate. According to our good results in combination with a
penetrating keratoplasty, we first combined the retropupillary fixated iris
claw lens with a DMEK (Descemet Membrane Endothelial Keratoplasty) procedure.
In summary, the implantation of a retropupillary iris claw lens is a fast,
safe, simple and effective surgical correction of aphakia or lens
(sub-)luxations in eyes without adequate capsular support. The deeper anterior
chamber and the greater distance from the corneal endothelium result in a
lower endothelial cell loss compared to the original, prepupillar fixation
technique
Comparison of ICare and IOPen vs Goldmann applanation tonometry according to international standards 8612 in glaucoma patients
AIM: To compare IOPen and ICare rebound tonometry to Goldmann applanation tonometry (GAT) according to International Standards Organization (ISO) 8612 criteria.
METHODS: Totally 191 eyes (n=107 individuals) were included. Criteria of ISO 8612 were fulfilled: 3 clusters of IOP, measured by GAT, were formed. The GAT results were given as mean±standard deviation.
RESULTS: GAT (19.7±0.5 mm Hg) showed a significant correlation to ICare (19.8±0.5 mm Hg) (r=0.547, P<0.001) and IOPen (19.5±0.5 mm Hg) (r=0.526, P<0.001). According to ISO 8612 criteria in all 3 IOP groups the number of outliers (of the 95% limits of agreement) exceeded 5% for ICare and IOPen vs GAT: No.1 (n=68) 29.4% and 22.1%, No.2 (n=62) 35.5% and 37.1%, No.3 (n=61) 26.2% and 42.6%, respectively.
CONCLUSION: The strict requirements of the ISO 8612 are not fulfilled in a glaucoma collective by ICare and IOPen at present. As long as the Goldmann tonometry is applicable it should be used first of all for reproducible IOP readings. ICare and IOPen tonometry should be considered as an alternative tool, if application of Goldmann tonometry is not possible
