Electrical stimulation treatment for facial palsy after revision pleomorphic adenoma surgery.
Surgery for pleomorphic adenoma recurrence presents a significant risk of facial nerve damage that can result in facial weakness effecting patients' ability to communicate, mental health and self-image. We report two case studies that had marked facial weakness after resection of recurrent pleomorphic adenoma and their progress with electrical stimulation. Subjects received electrical stimulation twice daily for 24 weeks during which photographs of expressions, facial measurements and Sunnybrook scores were recorded. Both subjects recovered good facial function demonstrating Sunnybrook scores of 54 and 64 that improved to 88 and 96, respectively. Neither subjects demonstrated adverse effects of treatment. We conclude that electrical stimulation is a safe treatment and may improve facial palsy in patients after resection of recurrent pleomorphic adenoma. Larger studies would be difficult to pursue due to the low incidence of cases
Application of Confocal Laser Scanning Microscopy to Cytocompatibility Testing of Potential Orthopaedic Materials in Immortalised Osteoblast-Like Cell Lines
Confocal laser scanning microscopy (CLSM) was used in conjunction with in vitro cell culture to investigate cellular interactions with orthopaedic biomaterials. Transfected rat and human osteoblasts were seeded on two potential isoelastic hip prosthesis materials, carbon fibre reinforced polyetheretherketone (PEEK) and epoxy. Titanium 318 alloy was employed as a control. Determination of the material surface contour, an important factor influencing cellular adhesion, proliferation and function, was performed using the industry standard Talysurf® and compared to analogous results obtained using the CLSM. The latter technique consistently gave higher values of material roughness but offers the advantage that it can be used to correlate roughness with cell distribution on the same samples, whereas Talysurf® measurement of roughness requires clean rigid samples. Image analysis and processing, performed on cells after attachment and culture on the materials for 48 hours, provided cell morphology data. Cells cultured on titanium were larger, with a higher percentage of cytoplasm, than those grown on either of the other materials. The macroscopic surface of epoxy resulted in smaller cells with altered morphology, which orientated themselves along carbon fibres. In conclusion, we believe CLSM offers great potential for investigating the cellular interactions of biomaterials involving minimal sample preparation, non-invasive optical sectioning of samples and minimal opportunity for generation of cellular deformation and sample preparation artefacts
Potential impact of reactive vaccination in controlling cholera outbreaks: An exploratory analysis using a Zimbabwean experience
Background. To contain ongoing cholera outbreaks, the World Health Organization has suggested that reactive vaccination should be considered in addition to its previous control measures.
Objectives. To explore the potential impact of a hypothetical reactive oral cholera vaccination using the example of the recent large-scale cholera outbreak in Zimbabwe.
Methods. This was a retrospective cost-effectiveness analysis calculating the health and economic burden of the cholera outbreak in Zimbabwe with and without reactive vaccination. The primary outcome measure was incremental cost per disability-adjusted life year (DALY) averted.
Results. Under the base-case assumptions (assuming 50% coverage among individuals aged ≥2 years), reactive vaccination could have averted 1 320 deaths and 23 650 DALYs. Considering herd immunity, the corresponding values would have been 2 920 deaths and 52 360 DALYs averted. The total vaccination costs would have been ~21 million, respectively, with per-dose vaccine price of US1. The incremental costs per DALY averted of reactive vaccination were 370, respectively, for vaccine price set at 1. Assuming herd immunity, the corresponding cost was 5, and the programme was cost-saving with a vaccine price of $1. Results were most sensitive to case-fatality rate, per-dose vaccine price, and the size of the outbreak.
Conclusions. Reactive vaccination has the potential to be a cost-effective measure to contain cholera outbreaks in countries at high risk. However, the feasibility of implementation should be further evaluated, and caution is warranted in extrapolating the findings to different settings in the absence of other in-depth studies
The Causal Structure of Emotions in Aristotle: Hylomorphism, Causal Interaction between Mind and Body, and Intentionality
Recently, a strong hylomorphic reading of Aristotelian emotions has been put forward, one that allegedly eliminates the problem of causal interaction between soul and body. Taking the presentation of emotions in de An. I 1 as a starting point and basic thread, but relying also on the discussion of Rh. II, I will argue that this reading only takes into account two of the four causes of emotions, and that, if all four of them
are included into the picture, then a causal interaction of mind and body remains within Aristotelian emotions, independent of how strongly their hylomorphism is understood. Beyond the discussion with this recent reading, the analysis proposed of the fourfold causal structure of emotions is also intended as a hermeneutical starting point for a comprehensive analysis of particular emotions in Aristotle. Through the different causes Aristotle seems to account for many aspects of the complex phenomenon of emotion, including its physiological causes, its mental causes, and its intentional object
Optical Characterisation of a Camera module Developed for Ultra-low NEP TES Detector Arrays at FIR Wavelengths
Here we report on the optical design and on the spectral-spatial characterisation of a small 16 pixel camera. The prototype uses TES detectors with NEPs ~10-16 W/Hz0.5 which have been fabricated with near identical optical coupling structures to mimic their much lower NEP counterparts (~10-19 W/Hz0.5). This modification, which is achieved through changing only the pixel thermal conductance, G, has allowed us to perform spectral/spatial cryogenic testing using a 100mK ADR to view room temperature thermal sources. The measurements show a flat spectral response across the waveband and minimal side lobe structure in the antenna patterns down to 30dB
First characterization of a superconducting filter-bank spectrometer for hyper-spectral microwave atmospheric sounding with transition edge sensor readout
We describe the design, fabrication, integration and characterization of a
prototype superconducting filter bank with transition edge sensor readout
designed to explore millimetre-wave detection at frequencies in the range 40 to
65 GHz. Results indicate highly uniform filter channel placement in frequency
and high overall detection efficiency. The route to a full atmospheric sounding

Dynamics of nonequilibrium quasiparticles in a double superconducting tunnel junction detector
We study a class of superconductive radiation detectors in which the
absorption of energy occurs in a long superconductive strip while the redout
stage is provided by superconductive tunnel junctions positioned at the two
ends of the strip. Such a device is capable both of imaging and energy
resolution. In the established current scheme, well studied from the
theoretical and experimental point of view, a fundamental ingredient is
considered the presence of traps, or regions adjacent to the junctions made of
a superconducting material of lower gap. We reconsider the problem by
investigating the dynamics of the radiation induced excess quasiparticles in a
simpler device, i.e. one without traps. The nonequilibrium excess
quasiparticles can be seen to obey a diffusion equation whose coefficients are
discontinuous functions of the position. Based on the analytical solution to
this equation, we follow the dynamics of the quasiparticles in the device,
predict the signal formation of the detector and discuss the potentiality
