    Age and Body Satisfaction Predict Diet Adherence in Adolescents with Inflammatory Bowel Disease

    The aim of the current study was to determine whether age and body satisfaction predict dietary adherence in adolescents with Inflammatory Bowel Disease (IBD), and whether older females are less adherent than younger males and females. Forty-four participants aged 10-21 with IBD were recruited. Participants provided informed consent and demographics. Body satisfaction was measured by a questionnaire and a task in which participants selected their current and ideal body image out of silhouettes depicting bodies ranging from underweight to obese. Adherence was measured by marking a 100mm visual analog scale, the 1-week completion of a dietary log, and a questionnaire evaluating coping strategies used for overcoming obstacles to dietary adherence. Age was related to dietary adherence, with younger children being more likely to adhere. Participants more satisfied with their body reported better dietary adherence. Findings remained consistent across multiple measures of body satisfaction and adherence. Healthcare providers and parents should be informed of these findings in order to improve adherence

    RORα Coordinates Reciprocal Signaling in Cerebellar Development through Sonic hedgehog and Calcium-Dependent Pathways

    AbstractThe cerebellum provides an excellent system for understanding how afferent and target neurons coordinate sequential intercellular signals and cell-autonomous genetic programs in development. Mutations in the orphan nuclear receptor RORα block Purkinje cell differentiation with a secondary loss of afferent granule cells. We show that early transcriptional targets of RORα include both mitogenic signals for afferent progenitors and signal transduction genes required to process their subsequent synaptic input. RORα acts through recruitment of gene-specific sets of transcriptional cofactors, including β-catenin, p300, and Tip60, but appears independent of CBP. One target promoter is Sonic hedgehog, and recombinant Sonic hedgehog restores granule precursor proliferation in RORα-deficient cerebellum. Our results suggest a link between RORα and β-catenin pathways, confirm that a nuclear receptor employs distinct coactivator complexes at different target genes, and provide a logic for early RORα expression in coordinating expression of genes required for reciprocal signals in cerebellar development

    Helicobacter pylori and cancer among adults in Uganda

    Data from Africa on infection with Helicobacter pylori (H. pylori) are sparse. Therefore, as part of an epidemiological study of cancer in Uganda, we investigated the prevalence and determinants of antibodies against H. pylori among 854 people with different cancer types and benign tumours. Patients were recruited from hospitals in Kampala, Uganda, interviewed about various demographic and lifestyle factors and tested for antibodies against H. pylori. In all patients combined, excluding those with stomach cancer (which has been associated with H. pylori infection), the prevalence of antibodies was 87% (723/833) overall, but declined with increasing age (p = 0.02) and was lower among people who were HIV seropositive compared to seronegative (p <0.001). Otherwise, there were few consistent epidemiological associations. Among those with stomach cancer, 18/21 (86%) had anti-H. pylori antibodies (odds ratio 0.8, 95% confidence intervals 0.2–2.9, p = 0.7; estimated using all other patients as controls, with adjustment for age, sex and HIV serostatus). No other cancer site or type was significantly associated with anti-H. pylori antibodies. The prevalence of H. pylori reported here is broadly in accord with results from other developing countries, although the determinants of infection and its' role in the aetiology of gastric cancer in Uganda remain unclear

    Many Roads Lead to Lithium: Formation Pathways For Lithium-Rich Red Giants

    Stellar models predict that lithium (Li) inside a star is destroyed during the first dredge-up phase, yet 1.2% of red giant stars are Li-rich. We aim to uncover possible origins of this population, by analysing 1155 Li-rich giants (A(Li) \geq 1.5) in GALAH DR3. To expose peculiar traits of Li-rich stars, we construct a reference sample of Li-normal (doppelg\"anger) stars with matched evolutionary state and fiducial supernova abundances. Comparing Li-rich and doppelg\"anger spectra reveals systematic differences in the H-α\alpha and Ca-triplet line profiles associated with the velocity broadening measurement. We also find twice as many Li-rich stars appear to be fast rotators (2% with vbroad20v_\textrm{broad} \gtrsim 20 km s1^{-1}) compared to doppelg\"angers. On average, Li-rich stars have higher abundances than their doppelg\"angers, for a subset of elements, and Li-rich stars at the base of RGB have higher mean ss-process abundances (0.05\geq 0.05 dex for Ba, Y, Zr), relative to their doppelg\"angers. External mass-transfer from intermediate-mass AGB companions could explain this signature. Additional companion analysis excludes binaries with mass ratios \gtrsim 0.5 at \gtrsim 7 AU. We also discover that highly Ba-enriched stars are missing from the Li-rich population, possibly due to low-mass AGB companions which preclude Li-enrichment. Finally, we confirm a prevalence of Li-rich stars on the red clump that increases with lithium, which supports an evolutionary state mechanism for Li-enhancement.     Effects of esomeprazole treatment for gastroesophageal reflux disease on quality of life in 12- to 17-year-old adolescents: an international health outcomes study

    <p>Abstract</p> <p>Background</p> <p>Although gastroesophageal reflux disease (GERD) is common in adolescents, the burden of GERD on health-related quality of life (HRQOL) in adolescents has not been previously evaluated. Therefore, the objective of the study was to examine the effect of GERD on HRQOL in adolescents.</p> <p>Methods</p> <p>This international, 31-site, 8-week safety study randomized adolescents, aged 12 to 17 years inclusive, with GERD to receive esomeprazole 20 or 40 mg once daily. The Quality of Life in Reflux and Dyspepsia questionnaire (QOLRAD), previously validated in adults, consists of 25 questions grouped into 5 domains: emotional distress, sleep disturbance, food/drink problems, physical/social functioning, and vitality. The QOLRAD was administered at the baseline and week-8 (final) visits.</p> <p>Results</p> <p>Of the 149 patients randomized, 134 completed the QOLRAD at baseline and final visits and were eligible for analysis of their HRQOL data. Baseline QOLRAD scores indicated GERD had a negative effect on the HRQOL of these adolescents, especially in the domains of vitality and emotional distress, and problems with food/drink. At the final visit, mean scores for all 5 QOLRAD domains improved significantly (<it>P </it>< .0001); change of scores (ie, delta) for all domains met or exceeded the adult QOLRAD minimal clinically significant difference standard of 0.5 units.</p> <p>Conclusion</p> <p>GERD had a negative effect on QOL in adolescents.     Joint ESPGHAN/NASPGHAN Guidelines for the Management of Helicobacter pylori in Children and Adolescents ( Update 2016)

    Background: Because of the changing epidemiology of Helicobacter pylori infection and low efficacy of currently recommended therapies, an update of the European Society for Paediatric Gastroenterology Hepatology and Nutrition/North American Society for Pediatric Gastroenterology, Hepatology and Nutrition recommendations for the diagnosis and management of H pylori infection in children and adolescents is required. Methods: Asystematic review of the literature (time period: 2009-2014) was performed. Representatives of both societies evaluated the quality of evidence using GRADE (Grading of Recommendation Assessment, Development, and Evaluation) to formulate recommendations, which were voted upon and finalized using a Delphi process and face-to-face meeting. Results: The consensus group recommended that invasive diagnostic testing for Hpylori be performed only when treatment will be offered if tests are positive. To reach the aim of a 90% eradication rate with initial therapy, antibiotics should be tailored according to susceptibility testing. Therapy should be administered for 14 days, emphasizing strict adherence. Clarithromycin-containing regimens should be restricted to children infected with susceptible strains. When antibiotic susceptibility profiles are not known, high-dose triple therapy with proton pump inhibitor, amoxicillin, and metronidazole for 14 days or bismuth-based quadruple therapy is recommended. Success of therapy should be monitored after 4 to 8 weeks by reliable noninvasive tests. Conclusions: The primary goal of clinical investigation is to identify the cause of upper gastrointestinal symptoms rather than H pylori infection. Therefore, we recommend against a test and treat strategy. Decreasing eradication rates with previously recommended treatments call for changes to first-line therapies and broader availability of culture or molecular-based testing to tailor treatment to the individual child

    The primordial non-Gaussianity of local type (f_NL) in the WMAP 5-year data: the length distribution of CMB skeleton

    We present skeleton studies of non-Gaussianity in the CMB temperature anisotropy observed in the WMAP5 data. The local skeleton is traced on the 2D sphere by cubic spline interpolation which leads to more accurate estimation of the intersection positions between the skeleton and the secondary pixels than conventional linear interpolation. We demonstrate that the skeleton-based estimator of non-Gaussianity of the local type (f_NL) - the departure of the length distribution from the corresponding Gaussian expectation - yields an unbiased and sufficiently converged f_NL-likelihood. We analyse the skeleton statistics in the WMAP5 combined V- and W-band data outside the Galactic base-mask determined from the KQ75 sky-coverage. The results are consistent with Gaussian simulations of the the best-fitting cosmological model, but deviate from the previous results determined using the WMAP1 data. We show that it is unlikely that the improved skeleton tracing method, the omission of Q-band data, the modification of the foreground-template fitting method or the absence of 6 extended regions in the new mask contribute to such a deviation. However, the application of the Kp0 base-mask in data processing does improve the consistency with the WMAP1 results. The f_NL-likelihoods of the data are estimated at 9 different smoothing levels. It is unexpected that the best-fit values show positive correlation with the smoothing scales. Further investigation argues against a point-source or goodness-of-fit explanation but finds that about 30% of either Gaussian or f_NL samples having better goodness-of-fit than the WMAP5 show a similar correlation. We present the estimate f_NL=47.3+/-34.9 (1sigma error) determined from the first four smoothing angles and f_NL=76.8+/-43.1 for the combination of all nine.     H. pylori-infection and antibody immune response in a rural Tanzanian population

    BACKGROUND: Helicobacter pylori (H. pylori) infection is ubiquitous in sub-Saharan Africa, but paradoxically gastric cancer is rare. METHODS: Sera collected during a household-based survey in rural Tanzania in 1985 were tested for anti-H. pylori IgG and IgG subclass antibodies by enzyme immunoassay. Odds ratios (OR) and confidence intervals (CI) of association of seropositivity with demographic variables were computed by logistic regression models. RESULTS: Of 788 participants, 513 were aged ≤17 years. H. pylori seropositivity increased from 76% at 0–4 years to 99% by ≥18 years of age. Seropositivity was associated with age (OR 11.5, 95% CI 4.2–31.4 for 10–17 vs. 0–4 years), higher birth-order (11.1; 3.6–34.1 for ≥3(rd )vs. 1(st )born), and having a seropositive next-older sibling (2.7; 0.9–8.3). Median values of IgG subclass were 7.2 for IgG1 and 2.0 for IgG2. The median IgG1/IgG2 ratio was 3.1 (IQR: 1.7–5.6), consistent with a Th2-dominant immune profile. Th2-dominant response was more frequent in children than adults (OR 2.4, 95% CI 1.3–4.4). CONCLUSION: H. pylori seropositivity was highly prevalent in Tanzania and the immunological response was Th2-dominant. Th2-dominant immune response, possibly caused by concurrent bacterial or parasitic infections, could explain, in part, the lower risk of H. pylori-associated gastric cancer in Africa

    Measurement of the Strong Coupling Constant from Inclusive Jet Production at the Tevatron pˉp\bar pp Collider

    We report a measurement of the strong coupling constant, αs(MZ)\alpha_s(M_Z), extracted from inclusive jet production in ppˉp\bar{p} collisions at s=\sqrt{s}=1800 GeV. The QCD prediction for the evolution of αs\alpha_s with jet transverse energy ETE_T is tested over the range 40<ETE_T<450 GeV using ETE_T for the renormalization scale. The data show good agreement with QCD in the region below 250 GeV. In the text we discuss the data-theory comparison in the region from 250 to 450 GeV. The value of αs\alpha_s at the mass of the Z0Z^0 boson averaged over the range 40<ETE_T<250 GeV is found to be αs(MZ)=0.1178±0.0001(stat)0.0095+0.0081(exp.syst)\alpha_s(M_{Z})= 0.1178 \pm 0.0001{(\rm stat)}^{+0.0081}_{-0.0095}{\rm (exp. syst)}. The associated theoretical uncertainties are mainly due to the choice of renormalization scale (^{+6%}_{-4%}) and input parton distribution functions (5%).Comment: 7 pages, 3 figures, using RevTeX.     Measurement of the lepton charge asymmetry in W-boson decays produced in p-pbar collisions

    We describe a measurement of the charge asymmetry of leptons from W boson decays in the rapidity range 0 enu, munu events from 110+/-7 pb^{-1}of data collected by the CDF detector during 1992-95. The asymmetry data constrain the ratio of d and u quark momentum distributions in the proton over the x range of 0.006 to 0.34 at Q2 \approx M_W^2. The asymmetry predictions that use parton distribution functions obtained from previously published CDF data in the central rapidity region (0.0<|y_l|<1.1) do not agree with the new data in the large rapidity region (|y_l|>1.1).Comment: 13 pages, 3 tables, 1 figur
