Adherence to a lifestyle programme in overweight/obese pregnant women and effect on gestational diabetes mellitus: a randomized controlled trial
This study aims to determine whether the prescription of a detailed lifestyle programme in overweight/obese pregnant women influences the occurrence of gestational diabetes (GDM), and if this kind of prescription increases the adherence to a healthier lifestyle in comparison to standard care. The study was designed as a randomized controlled trial, with open allocation, enrolling women at 9-12 weeks of pregnancy with a BMI ≥ 25 kg/m(2) . The women assigned to the Intervention group (I = 96) received a hypocaloric, low-glycaemic, low-saturated fat diet and physical activity recommendations. Those assigned to the Standard Care group (SC = 95) received lifestyle advices regarding healthy nutrition and exercise. Follow-up was planned at the 16(th) , 20(th) , 28(th) and 36(th) weeks. A total of 131 women completed the study (I = 69, SC = 62). The diet adherence was higher in the I (57.9%) than in the SC (38.7%) group. GDM occurred less frequently in the I (18.8%) than in the SC (37.1%, P = 0.019) group. The adherent women from either groups showed a lower GDM rate (12.5% vs. 41.8%, P < 0.001). After correcting for confounders, the GDM rate was explained by allocation into the I group (P = 0.034) and a lower BMI category (P = 0.039). The rates of hypertension, preterm birth, induction of labour, large for gestational age babies and birthweight > 4000 g were significantly lower in I group. The incidence of small for gestational age babies was not different. These findings demonstrate that the adherence to a personalized, hypocaloric, low-glycaemic, low-saturated fat diet started early in pregnancy prevents GDM occurrence, in women with BMI ≥ 25 kg/m(2)
The Swiss and European Legislations on Business Human Rights : Gaps, extra-territorial application and reform needed towards harmonization
This article examines the disparities between the (existing and planned) business and human rights (BHR) legislations in Switzerland and the European Union (EU). More specifically, (i) the key differences that exist between the Swiss and EU BHR legislations are identified, and (ii) the proposed amendments to the Swiss BHR legislation to reach the desired harmonization with the EU framework are put forward. The Swiss BHR legislation, implemented in stages on 1 January 2021 and 1 January 2023, encompasses provisions related to ‘transparency on non-financial matters’, ‘transparency in raw material companies’, and ‘due diligence and transparency in relation to minerals and metals from conflict-affected areas and child labor’. The EU BHR legal framework, however, has undergone a major overhaul, with the recent adoption of the Corporate Sustainability Reporting Directive (CSRD) and the Directive
on Corporate Sustainability Due Diligence (CSDDD) that introduce additional obligations. These EU regulations have (or will have) important extraterritorial effects, including on Swiss undertakings operating—directly or indirectly—in the EU. To prevent Swiss companies from being subject to a diverse array of regulations (with an increased risk of sanctions in case of non-compliance), a reform of the Swiss BHR legislation is needed to ensure harmonization with EU BHR legislation. Joint watermarking and encryption of color images in the Fibonacci-Haar domain
A novel method for watermarking and ciphering color images, based on the joint use of a key-dependent wavelet transform with a secure cryptographic scheme, is presented. The system allows to watermark encrypted data without requiring the knowledge of the original data and also to cipher watermarked data without damaging the embedded signal. Since different areas of the proposed transform domain are used for encryption and watermarking, the extraction of the hidden information can be performed without deciphering the cover data and it is also possible to decipher watermarked data without removing the watermark. Experimental results show the effectiveness of the proposed scheme
Years of life that could be saved from prevention of hepatocellular carcinoma
BACKGROUND:
Hepatocellular carcinoma (HCC) causes premature death and loss of life expectancy worldwide. Its primary and secondary prevention can result in a significant number of years of life saved.
AIM:
To assess how many years of life are lost after HCC diagnosis.
METHODS:
Data from 5346 patients with first HCC diagnosis were used to estimate lifespan and number of years of life lost after tumour onset, using a semi-parametric extrapolation having as reference an age-, sex- and year-of-onset-matched population derived from national life tables.
RESULTS:
Between 1986 and 2014, HCC lead to an average of 11.5 years-of-life lost for each patient. The youngest age-quartile group (18-61 years) had the highest number of years-of-life lost, representing approximately 41% of the overall benefit obtainable from prevention. Advancements in HCC management have progressively reduced the number of years-of-life lost from 12.6 years in 1986-1999, to 10.7 in 2000-2006 and 7.4 years in 2007-2014. Currently, an HCC diagnosis when a single tumour <2 cm results in 3.7 years-of-life lost while the diagnosis when a single tumour 65 2 cm or 2/3 nodules still within the Milan criteria, results in 5.0 years-of-life lost, representing the loss of only approximately 5.5% and 7.2%, respectively, of the entire lifespan from birth.
CONCLUSIONS:
Hepatocellular carcinoma occurrence results in the loss of a considerable number of years-of-life, especially for younger patients. In recent years, the increased possibility of effectively treating this tumour has improved life expectancy, thus reducing years-of-life lost

Rural villages and centres are a fundamental source of traditional and local knowledge connected to social, environmental and economic needs over the centuries. In order to effectively preserve this cultural heritage, as well as prevent abandonment and degradation, it is crucial to undertake a knowledge project that involves comprehensive information gathering and site-specific understanding. The growing complexity of urban systems necessitates a holistic approach to knowledge management, which can mitigate risks while enhancing operational effectiveness. This study aims to systematise knowledge acquisition process in an organic manner for defining recovery, enhancement and protection strategies for small centres. A methodological framework is then proposed, which can serve as a reference for the definition of a specific action plan replicable in similar territorial contexts, accounting local peculiarities and specificities. The methodological approach is applied to the case study of Castelnuovo Cilento, Southern Italy, with many vernacular heritage examples to maintain, conserve and pass on to future generations
Integrated Approach of Life Cycle Assessment and Experimental Design in the Study of A Model Organic Reaction: New Perspectives in Renewable Vanillin-Derived Chemicals
This work is focused on performing a quantitative assessment of the environmental impacts associated with an organic synthesis reaction, optimized using an experimental design approach. A nucleophilic substitution reaction was selected, employing vanillin as the substrate, a phenolic compound widely used in the food industry and of pharmaceutical interest, considering its antioxidant and antitumoral potential. To carry out the reaction, three different solvents have been chosen, namely acetonitrile (ACN), acetone (Ace), and dimethylformamide (DMF). The syntheses were planned with the aid of a multivariate experimental design to estimate the best reaction conditions, which simultaneously allow a high product yield and a reduced environmental impact as computed by Life Cycle Assessment (LCA) methodology. The experimental results highlighted that the reactions carried out in DMF resulted in higher yields with respect to ACN and Ace; these reactions were also the ones with lower environmental impacts. The multilinear regression models allowed us to identify the optimal experimental conditions able to guarantee the highest reaction yields and lowest environmental impacts for the studied reaction. The identified optimal experimental conditions were also validated by experimentally conducting the reaction in those conditions, which indeed led to the highest yield (i.e., 93%) and the lowest environmental impacts among the performed experiments. This work proposes, for the first time, an integrated approach of DoE and LCA applied to an organic reaction with the aim of considering both conventional metrics, such as reaction yield, and unconventional ones, such as environmental impacts, during its lab-scale optimization

Dietary interventions, lifestyle changes, and dietary supplements in preventing gestational diabetes mellitus: a literature review
Gestational diabetes mellitus (GDM) is associated with increased rates of fetal morbidity and mortality, both during the pregnancy and in the postnatal life. Current treatment of GDM includes diet with or without medications, but this management is expensive and poorly cost-effective for the health care systems. Strategies to prevent such condition would be preferable with respect to its treatment. The aim of this literature review was to evaluate studies reporting the efficacy of the most used approaches to prevent GDM as well as evidences of efficacy and safety of dietary supplementations. Systematic literature searches were performed in electronic databases, covering the period January 1983 to April 2014. Randomized controlled clinical trials were included. Quality of the articles was evaluated with the Jadad scale. We did not evaluate those articles that were already entered in the most recent systematic reviews, and we completed the research with the trials published thereafter. Of 55 articles identified, 15 randomized controlled trials were eligible. Quality and heterogeneity of the studies cannot allow firm conclusions. Anyway, trials in which only intake or expenditure has been targeted mostly reported negative results. On the contrary, combined lifestyle programs including diet control (orienting food intake, restricting energy intake) associated with moderate but continuous physical activity exhibit better efficacy in reducing GDM prevalence. The results from dietary supplements with myoinositol or probiotics are promising. The actual evidences provide enough arguments for implementing large-scale, high-quality randomized controlled trials looking at the possible benefits of these new approaches for preventing GDM
Semirigid Ligands Enhance Different Coordination Behavior of Nd and Dy Relevant to Their Separation and Recovery in a Non-aqueous Environment
Rare-earth elements are widely used in high-end technologies, the production of permanent magnets (PMs) being one of the sectors with the greatest current demand and likely greater future demand. The combination of Nd and Dy in NdFeB PMs enhances their magnetic properties but makes their recycling more challenging. Due to the similar chemical properties of Nd and Dy, their separation is expensive and currently limited to the small scale. It is therefore crucially important to devise efficient and selective methods that can recover and then reuse those critical metals. To address these issues, a series of heptadentate Trensal-based ligands were used for the complexation of Dy3+ and Nd3+ ions, with the goal of indicating the role of coordination and solubility equilibria in the selective precipitation of Ln3+-metal complexes from multimetal non-water solutions. Specifically, for a 1:1 Nd/Dy mixture, a selective and fast precipitation of the Dy complex occurred in acetone with the Trensalp-OMe ligand at room temperature, with a concomitant enrichment of Nd in the solution phase. In acetone, complexes of Nd and Dy with Trensalp-OMe were characterized by very similar formation constants of 7.0(2) and 7.3(2), respectively. From the structural analysis of an array of Dy and Nd complexes with TrensalR ligands, we showed that Dy invariably provided complexes with coordination number (cn) of 7, whereas the larger Nd experienced an expansion of the coordination sphere by recruiting additional solvent molecules and giving a cn of >7. The significant structural differences have been identified as the main premises upon which a suitable separation strategy can be devised with these kind of ligands, as well as other preorganized polydentate ligands that can exploit the small differences in Ln3+ coordination requirements
