    Enrique Rambal y el melodrama de la primera mitad del siglo XX

    RESUMEN Mi estudio se ha centrado en el análisis del teatro que realizó el actor Enrique Rambal durante la primera mitad del siglo XX. Con esta investigación sobre un personaje concreto de la escena se ha intentado recuperar algunos aspectos del teatro español contemporáneo hasta ahora no analizados centrados en la escena valenciana. En primer lugar, se ha intentado recuperar los aspectos personales de Enrique Rambal. Una breve biografía junto con un amplio capítulo sobre las formaciones que lideró en sus distintas composiciones en nuestro primer capítulo que se ha elaborado. A continuación se ha intentado completar dicha información con algunos de sus principales colaboradores literarios, actores, actrices junto a otros miembros de su compañía que destacaron en sus puestas en escena. En el segundo apartado del capítulo dedicado a la compañía de Rambal y su puesta en escena se ha querido detallar los elementos básicos para la práctica escénica. Telones, vestuario, maquillaje e iluminación entre otros, son los elementos tradicionales que adquieren peculiaridad en el caso de Rambal. Dada la importancia que tenían para Rambal los trucos escénicos en su puesta en escena, se ha analizado dentro de un apartado especial. Junto a la figura de Rambal se encuentra ligado un género, el melodrama. En nuestro análisis se ha verificado el inicio del esplendor del melodrama en los primeros años del siglo. El melodrama es un género que evoluciona a medida que avanza el siglo y a veces la misma obra se adaptaba a las nuevas exigencias del momento. Rambal popularizó el teatro hasta el punto de poder competir con otros espectáculos como el cine y el fútbol que en esas primeras décadas del siglo ganaban más adeptos. La personalidad de Rambal influyó a varias generaciones de público, no sólo valenciano sino de toda España, como ya se ha dicho, por su impulso por la renovación del arte escénico. __________________________________________________________________________________________________My research has focused on the analysis of the theatre carried out by the actor Enrique Rambal during the first half of the twentieth century. With this research about a particular actor, I have tried to retrieve some aspects of contemporary Spanish theatre not discussed so far focusing on the Valencian scene. Firstly, I have tried to retrieve the personal aspects of Enrique Rambal. A brief biography along with a comprehensive chapter about the formations which he led in his various compositions developed in the first chapter. Next I have completed this information with some of his senior literary aides, actors and actresses along with other members of his company that stood out in its mise en scène. In the second paragraph of the chapter about Rambals company and his staging I wanted to detail the basic elements for the practice stage. Curtains, costumes, make-up and lighting among others, are the traditional elements that acquire peculiarity in the case of Rambal. A particular genre is linked to Rambals figure: the melodrama. In my analysis it can verified the beginning of the splendour of the melodrama in the early years of the century. The melodrama is a genre that evolves as the century progresses, and sometimes the same book was adapted to the new demands of the moment. Rambal popularized the theatre to the point of being able to compete with other means of entertainment such as cinema and football in the early decades of the century. The personality of Rambal influenced several generations of audiences, not only in Spain but in the American continent, as I have previously said, since he promoted the renewal of the performing arts

    Effectiveness of the MULTIPAP Plus intervention in youngest-old patients with multimorbidity and polypharmacy aimed at improving prescribing practices in primary care: study protocol of a cluster randomized trial

    Background: The progressive ageing of the population is leading to an increase in multimorbidity and polypharmacy, which in turn may increase the risk of hospitalization and mortality. The enhancement of care with information and communications technology (ICT) can facilitate the use of prescription evaluation tools and support system for decision-making (DSS) with the potential of optimizing the healthcare delivery process. Objective: To assess the effectiveness and cost-effectiveness of the complex intervention MULTIPAP Plus, compared to usual care, in improving prescriptions for young-old patients (65-74 years old) with multimorbidity and polypharmacy in primary care.     Health of Spanish centenarians: A cross-sectional study based on electronic health records

    Background: With the number of centenarians increasing exponentially in Spain, a deeper knowledge of their socio-demographic, clinical, and healthcare use characteristics is important to better understand the health profile of the very elderly. Methods: We conducted a retrospective, cross-sectional observational study in the EpiChron Cohort (Aragon, Spain) aimed at analyzing the socio-demographic, clinical, drug use and healthcare use characteristics of 1680 centenarians during 2011-2015, using data from electronic health records and clinical-administrative databases. Results: Spanish centenarians (79.1% women) had 101.6 years on average. Approximately 80% of centenarians suffered from multimorbidity, with an average of 4.0 chronic conditions; 50% were exposed to polypharmacy, with an average of 4.8 medications; only 6% of centenarians were free of chronic diseases and only 7% were not on medication. Centenarians presented a cardio-cerebrovascular pattern in which hypertension, heart failure, cerebrovascular disease and dementia were the most frequent conditions. Primary care was the most frequently visited healthcare level (79% of them), followed by medical specialist consultations (23%), hospitalizations (13%), and emergency service use (9%). Conclusions: Multimorbidity is the rule rather than the exception in Spanish centenarians. Addressing medical care in the very elderly from a holistic geriatric view is critical in order to preserve their health, and avoid the negative effects of polypharmacy

    Cognitive functioning in dyskinetic cerebral palsy: Its relation to motor function, communication and epilepsy

    Background: Cerebral palsy (CP) is a disorder of motor function often accompanied by cognitive impairment. There is a paucity of research focused on cognition in dyskinetic CP and on the potential effect of related factors. Aim: To describe the cognitive profile in dyskinetic CP and to assess its relationship with motor function and associated impairments. Method: Fifty-two subjects with dyskinetic CP (28 males, mean age 24 y 10 mo, SD 13 y) and 52 typically-developing controls (age- and gender-matched) completed a comprehensive neuropsychological assessment. Gross Motor Function Classification System (GMFCS), Communication Function Classification System (CFCS) and epilepsy were recorded. Cognitive performance was compared between control and CP groups, also according different levels of GMFCS. The relationship between cognition, CFCS and epilepsy was examined through partial correlation coefficients, controlling for GMFCS. Results: Dyskinetic CP participants performed worse than controls on all cognitive functions except for verbal memory. Milder cases (GMFCS I) only showed impairment in attention, visuoperception and visual memory. Participants with GMFCS II-III also showed impairment in language-related functions. Severe cases (GMFCS IV-V) showed impairment in intelligence and all specific cognitive functions but verbal memory. CFCS was associated with performance in receptive language functions. Epilepsy was related to performance in intelligence, visuospatial abilities, visual memory, grammar comprehension and learning. Conclusion: Cognitive performance in dyskinetic CP varies with the different levels of motor impairment, with more cognitive functions impaired as motor severity increases. This study also demonstrates the relationship between communication and epilepsy and cognitive functioning, even controlling for the effect of motor severity

    Drug repurposing in oncology: a systematic review of randomized controlled clinical trials

    Quality pharmacological treatment can improve survival in many types of cancer. Drug repurposing offers advantages in comparison with traditional drug development procedures, reducing time and risk. This systematic review identified the most recent randomized controlled clinical trials that focus on drug repurposing in oncology. We found that only a few clinical trials were placebo-controlled or standard-of-care-alone-controlled. Metformin has been studied for potential use in various types of cancer, including prostate, lung, and pancreatic cancer. Other studies assessed the possible use of the antiparasitic agent mebendazole in colorectal cancer and of propranolol in multiple myeloma or, when combined with etodolac, in breast cancer. We were able to identify trials that study the potential use of known antineoplastics in other non-oncological conditions, such as imatinib for severe coronavirus disease in 2019 or a study protocol aiming to assess the possible repurposing of leuprolide for Alzheimer’s disease. Major limitations of these clinical trials were the small sample size, the high clinical heterogeneity of the participants regarding the stage of the neoplastic disease, and the lack of accounting for multimorbidity and other baseline clinical characteristics. Drug repurposing possibilities in oncology must be carefully examined with well-designed trials, considering factors that could influence prognosis

    Executive function and general intellectual functioning in dyskinetic cerebral palsy: comparison with spastic cerebral palsy and typically developing controls

    Aim: To comprehensively describe intellectual and executive functioning (EF) in people with dyskinetic cerebral palsy (DCP), by comparing their performance with that of: 1) age and sex-matched typically developing controls (TDC); and 2) participants with spastic cerebral palsy (SCP) matched for age, sex, term/preterm and gross motor function classification system (GMFCS). Method: This cross sectional study was conducted by the University of Barcelona in collaboration with five institutions. Participants were people with DCP (n = 52; 24 females, median age 20.5 y: 5mo, interquartile range [IQR] = 13.75 y: 7mo; GMFCS I-V). As comparison groups, participants with SCP (n = 20; 10 females, median age = 20.5 y: 5.5mo, IQR = 13.75 y 9mo; GMFCS I-V) and TDC (n = 52; 24 females, median age = 20 y: 4mo, IQR = 12 y 7mo) were included. Intelligence and EF were assessed using common tests in all participants. Results: Both CP groups had lower intelligence than TDC and performed poorer in almost all EF tasks. Intelligence was higher in DCP than SCP (z = -2.51, p = 0.01). Participants with DCP also performed significantly better in goal-setting tasks (z = 2.27, p = 0.03) and information processing (z =-2.54, p = 0.01) than those with SCP. Conclusion: People with DCP present lower general intellectual functioning and poorer EF across multiple domains than typically developing controls. People with DCP have higher general intellectual functioning and better EF than people with SCP when levels of motor severity are similar
