    Oral plus vaginal alpha-lipoic acid in women at risk for preterm delivery

    Objective: The etiology of preterm labor is multifactorial. An inflammatory response is always involved with the activation of NF-kB that determines synthesis and release of inflammatory molecules, implicated in fetal membrane activation, cervical modifications, abdominal pain and spontaneous uterine contractions. There is a close relationship between preterm birth and cervical shortening in the second quarter of pregnancy. We evaluated the benefits of alpha-lipoic acid administration on women considered at risk of preterm delivery due to the presence of symptoms (pelvic pain and uterine contractions) or reduced cervical length. Patients and Methods: This prospective observational study was carried out at the Gynecology and Obstetrics Unit of Palermo University Hospital (Palermo, Italy), from October 2015 to April 2016. The inclusion criteria were: women aged 18-35, with gestational age between 24 and 33 weeks of amenorrhea, pregnancy at risk of preterm delivery due to cervical length between 35-25 mm (in presence of symptoms) or < 30 and > 15 mm (if asymptomatic), intact membranes and negative for vaginosis. Patients were treated daily with alpha lipoic acid orally (300 mg, twice a day for 30 days) and vaginally (10 mg, once a day for 10 days), or untreated (controls). Patients were evaluated at the baseline (T 0), after 7 days, after 30 days, and at 34 weeks of gestation considering: maternal characteristics, symptomology and cervical length. Results: Among 60 analyzed women, 50 were treated orally and vaginally with alpha-lipoic acid, whereas 10 did not undergo any therapy. In the treated group, 10 patients were asymptomatic and 40 symptomatic. The symptoms disappeared in 37 patients. In the untreated group, 4 women were symptomatic and 6 asymptomatic. At the end all women were symptomatic. Mean cervical length showed a reduction in the untreated group compared to the treated group. Conclusions: The vaginal/oral-combined administration with alpha-lipoic acid showed effectiveness in reducing symptoms and preventing cervical shortening in our set of patients. No adverse effects were detected during the treatment

    Prostate cancer is the second most diagnosed cancer in males in the world. Plasma quantification of prostate-specific antigen substantially improved the early detection of prostate cancer, but still lacks the required specificity. Clinical management of prostate cancer needs advances in the development of new non-invasive biomarkers, ameliorating current diagnosis and prognosis and guiding therapeutic decisions. microRNAs (miRNAs) are a class of small non-coding RNAs that regulate gene expression at the post-transcriptional level. These miRNAs are expressed in the cells and are also present in cell-derived extracellular vesicles such as exosomes. Exosomes have been shown to act as mediators for cell to cell communication because of the regulatory functions of their content. High levels of exosomes are found in several body fluids from cancer patients and could be a potential source of non-invasive biomarkers.     Amphiregulin contained in NSCLC-exosomes induces osteoclast differentiation through the activation of EGFR pathway

    Non-small cell lung cancer (NSCLC) remains the leading cause of cancer-related deaths worldwide. The majority of patients are diagnosed in advanced disease stage. Bone metastasis is the most frequent complication in NSCLC resulting in osteolytic lesions. The perfect balance between bone-resorbing osteoclasts and bone-forming osteoblasts activity is lost in bone metastasis, inducing osteoclastogenesis. In NSCLC, the epidermal growth factor receptor (EGFR) pathway is constitutively activated. EGFR binds Amphiregulin (AREG) that is overexpressed in several cancers such as colon, breast and lung. Its levels in plasma of NSCLC patients correlate with poor prognosis and AREG was recently found as a signaling molecule in exosomes derived from cancer cell lines. Exosomes have a key role in the cell-cell communication and they were recently indicated as important actors in metastatic niche preparation. In the present work, we hypothesize a role of AREG carried by exosomes derived from NSCLC in bone metastasis induction. We observed that NSCLC-exosomes, containing AREG, induce EGFR pathway activation in pre-osteoclasts that in turn causes an increased expression of RANKL. RANKL is able to induce the expression of proteolytic enzymes, well-known markers of osteoclastogenesis, triggering a vicious cycle in osteolytic bone metastasis

In this work, we focus our attention on scheduling a number of incoming vessels by assigning to each a berthing position, a mooring time and a number of Quay Cranes. This problem is known as the Berthing Allocation and Quay Crane Assignment problem. To formulate the problem, we first propose a mixed integer linear programming model to minimize the total weighted service time of the incoming vessels. Then, a meta-heuristic algorithm (Genetic Algorithm (GA)) is presented for solving the proposed problem.     Exosomes are nano-sized vesicles of endolysosomal origin, released by several cytotypes in physiological and pathological conditions. Tumor derived exosomes, interacting with other cells of the tumor microenvironment, modulate tumor progression, angiogenic switch, metastasis, and immune escape. Recently, extracellular vesicles were proposed as excellent biomarkers for disease monitoring and prognosis in cancer patients. Non-small cell lung cancer (NSCLC) has a poor 5-year survival rate due to the delay in the detection of the disease. The majority of patients are diagnosed in an advanced disease stage. Exosomes might be promising beneficial tools as biomarker candidates in the scenario of NSCLC, since they contain both, proteins and miRNAs. The clinical case reported in this manuscript is a proof of concept revealing that NSCLC exosomes and sorted miRNAs might constitute, in a near future, novel biomarkers. Besides having informed a specific reflexive agenda of representation, the angle of gender has presided to most research issues as an analytical overall parti pris: from the gendered nature of prison regimes to the gendered character of prison cultures, socialities and ‘pains of imprisonment’. This more ‘gendercentric’ agenda is however becoming more diversified for theoretical and empirical reasons alike. These involve a recognition of the diversity of women prisoners’ experiences and identities, and an attention to a wider variety of aspects of carceral life.     Closed circuits : kinship, neighborhood and incarceration in urban Portugal

    The notion that prisons are a ‘world apart’, with their walls severing prisoners from their external relationships, and incarceration an interruption, ‘time away’ spent in a separate social universe, has provided an adequate framework for understanding the social realities of imprisonment in the past. But it has also created an analytical dead angle that prevents us from identifying the ramifying social effects of concentrated incarceration upon both the prison and heavily penalized lower-class neighborhoods. This article addresses these effects with data from an ethnographic revisit of a major women’s prison in Portugal, where the recomposition of the inmate population that has accompanied the rapid inflation of the country’s carceral population is especially pronounced and entails the activation of wide-ranging carceralized networks bringing kinship and neighborhood into the prison as well as the prison into the domestic world. The analysis focuses on the ways whereby these constellations have transformed the experience of confinement and the texture of correctional life, calling for a reconsideration of the theoretical status of the prison as a ‘total institution’ and for exploring anew the boundary that separates it (or not) from outside worlds.Wenner-Gren Foundation for Anthropological Research

    Servicio Social ComunitarioInvestigación realizada tuvo como objetivo la realización de una animación sociocultural en el establecimiento carcelario la Modelo ubicado en la ciudad de Bogotá- Colombia, específicamente en la unidad de salud mental, a través de metodologías participativas y alternativas de intervención como el Aprendizaje Servicio Solidario (ASS); de esta manera fue posible identificar en un primer momento algunas de las necesidades sentidas, percibidas e inferidas, tanto de la institución, como de las personas privadas de la libertad (PPL), a través del arte y el deporte. En un segundo momento se desarrolló una intervención a través de las metodologías ya mencionadas, utilizando el deporte como herramienta principal, buscando potencializar en los internos diferentes alternativas para la solución de problemas y la transformación de las necesidades evaluadas. En cuanto a este último objetivo de la intervención no todas las necesidades pudieron tener alguna modificación, puesto que algunas de ellas se encuentran vinculadas con lineamientos institucionales fuera del alcance de esta investigación. Para este estudio no se contó con una muestra estable, ya que para cada sesión los PPL podían elegir su participación de manera voluntaria y en casos particulares se evidenciaban traslados de patio o de establecimiento carcelario y, además, se presentaron situaciones de inseguridad en el patio, que no permitieron el desarrollo adecuado de las últimas sesiones de intervención.140 p.1.     Baseline factors associated with early and late death in intracerebral haemorrhage survivors

    Background and purpose: The aim of this study was to determine whether early and late death are associated with different baseline factors in intracerebral haemorrhage (ICH) survivors. Methods: This was a secondary analysis of the multicentre prospective observational CROMIS‐2 ICH study. Death was defined as ‘early’ if occurring within 6 months of study entry and ‘late’ if occurring after this time point. Results: In our cohort (n = 1094), there were 306 deaths (per 100 patient‐years: absolute event rate, 11.7; 95% confidence intervals, 10.5–13.1); 156 were ‘early’ and 150 ‘late’. In multivariable analyses, early death was independently associated with age [per year increase; hazard ratio (HR), 1.05, P = 0.003], history of hypertension (HR, 1.89, P = 0.038), pre‐event modified Rankin scale score (per point increase; HR, 1.41, P < 0.0001), admission National Institutes of Health Stroke Scale score (per point increase; HR, 1.11, P < 0.0001) and haemorrhage volume >60 mL (HR, 4.08, P < 0.0001). Late death showed independent associations with age (per year increase; HR, 1.04, P = 0.003), pre‐event modified Rankin scale score (per point increase; HR, 1.42, P = 0.001), prior anticoagulant use (HR, 2.13, P = 0.028) and the presence of intraventricular extension (HR, 1.73, P = 0.033) in multivariable analyses. In further analyses where time was treated as continuous (rather than dichotomized), the HR of previous cerebral ischaemic events increased with time, whereas HRs for Glasgow Coma Scale score, National Institutes of Health Stroke Scale score and ICH volume decreased over time. Conclusions: We provide new evidence that not all baseline factors associated with early mortality after ICH are associated with mortality after 6 months and that the effects of baseline variables change over time. Our findings could help design better prognostic scores for later death after ICH
