A multimodális posztoperatív fájdalomcsillapítás szerepe primer csípőízületi arthroplasticában
Absztrakt:
Bevezetés: Az egyre nagyobb számban végzett nagyízületi
arthroplasticák miatt előtérbe került a posztoperatív kórházi napok számának
csökkentése, valamint a rehabilitáció gyorsítása. Ehhez elengedhetetlen a
megfelelő, hatékony, biztonságos posztoperatív fájdalomcsillapítás.
Célkitűzés: Vizsgálatunk célja az volt, hogy
összehasonlítsuk a hagyományos és az újfajta multimodális fájdalomcsillapító
kezelésben részesült betegek posztoperatív funkcionális eredményeit, a műtétet
követő fájdalomszintjüket és opioidfájdalomcsillapító-igényeiket.
Módszer: Prospektív vizsgálatunkba 81 beteget vontunk be,
akiknél 2017. február és 2018. január között intézetünkben primer
csípőprotézis-implantációt végeztünk. A randomizált módon csoportosított betegek
közül a kontrollcsoportban 38 fő, a multimodális fájdalomcsillapításban
részesült betegek csoportjában 43 fő szerepelt. Vizuális analóg skála (VAS)
segítségével ellenőriztük a betegek szubjektív fájdalomszintjét, valamint
rögzítettük fájdalomcsillapító-igényüket. Eredmények:
Multimodális fájdalomcsillapításban részesült betegeinknél az opioidtartalmú
fájdalomcsillapítók iránti szignifikánsan alacsonyabb igényt, valamint
szignifikánsan alacsonyabb VAS-értéket tapasztaltunk a posztoperatív 6. órában,
3. és 4. napon. Továbbá e csoport betegei szignifikánsan rövidebb ideig
tartózkodtak a kórházban a műtétet követően. Következtetések:
Tapasztalataink alapján a multimodális fájdalomcsillapítás egyoldali primer
csípőízületi arthroplastica esetén szignifikánsan csökkentette a betegek
posztoperatív szubjektív fájdalomérzetét és fájdalomcsillapító-igényét. Orv

Abstract:
Introduction: Due to the increasing number of arthroplasties,
the number of post-operative hospital days was reduced and the rate of
rehabilitation accelerated. For this, proper postoperative analgesia is
essential and the multimodal pain relief is an excellent tool.
Aim: The aim of our study was to compare postoperative
functional results, postoperative pain levels, and opioid analgesic needs of
patients who received conventional and novel analgesic treatments.
Method: In our prospective study, 81 patients were enrolled
who underwent primary hip prosthesis surgery in our institution between February
2017 and January 2018. Of the randomized patients, 38 were in the control group
and 43 in the multimodal pain group. We have tested their analgesic needs and
the subjective pain levels of patients with the help of the visual analog scale
(VAS). Results: Patients receiving multimodal pain relief had
significantly lower opioid analgesic requirements and significantly lower VAS
values assessed at 6 hours, and 3 and 4 days postoperatively. In addition, the
patients in this group stayed in the hospital for a significantly shorter time
after surgery. Conclusions: Based on our experience, multimodal
pain relief in one-sided primary hip joint arthroplasty significantly reduced
the patients’ postoperative subjective pain sensation and pain killer

A vértranszfúzió gyakorisága primer csípőprotézis-beültetés után
Absztrakt:
Bevezetés: Csípőprotézis-beültetés után gyakran van szükség
vértranszfúzióra, mely esetenként komoly mellékhatásokkal járhat, nehezen
hozzáférhető és költséges. Célkitűzés: Munkánk célja az volt,
hogy megvizsgáljuk, saját gyakorlatunkban milyen gyakran van szükség primer
csípőprotézis-beültetés során vérátömlesztésre, és ennek gyakoriságát mely
tényezők befolyásolják. Módszer: Vizsgálatunkban 210,
csípőprotézis-beültetésen átesett beteg anyagát dolgoztuk fel. Feljegyeztük a
műtét előtti hemoglobin- és hematokritértékeket, a betegek életkorát, nemét,
testtömegindexét, a beültetett protézis típusát, a drénhasználatot, valamint a
perioperatív időszakban használt véralvadásgátló és a műtét során alkalmazott
vérzéscsökkentő szereket. Az adatokat összevetettük az alkalmazott transzfúzió
mennyiségével és típusával. Eredmények: Összesen a betegek
41%-a kapott vérkészítményt, az allogén transzfúzió aránya 8,6% volt. Az
autotranszfúziós betegek nem igényeltek allogénvér-átömlesztést. A transzfúziós
igény függött a preoperatív hemoglobin- és hematokritértékektől, a protézis
típusától, a drénhasználattól és a vérzéscsillapító szerek használatától. A
vérátömlesztés mennyisége nem mutatott összefüggést a betegek nemével és
testtömegindexével. Következtetés: Vizsgálatunk alapján allogén
vér transzfúziójára az esetek kevesebb mint 10%-ában van szükség primer
csípőprotézis-beültetés során. A vérátömlesztést a leginkább a preoperatív
hemoglobin- és hematokritértékek, valamint az alkalmazott, fibrinolízist gátló
szerek befolyásolják. Az autotranszfúzió alkalmas módszer az

Abstract:
Introduction: Total hip arthroplasty is one of the most common
surgical procedures that requires blood transfusion, with the possible risk of
significant complications. Aim: A retrospective study was
performed to analyze the predictors of blood transfusion after primary total hip
arthroplasty. Method: We collected the data of 210 consecutive
patients undergoing total hip arthroplasty. Patient’s data, preoperative
hemoglobin and hematocrit level, the type of the prosthesis, the use of a
suction drain, tranexamic acid or anticoagulants and the type and amount of
blood transfusion were recorded. Results: A total of 41% of our
patients required transfusion, 8.6% receiving allogenic blood. Significant
predictors of allogenic blood transfusion were preoperative hemoglobin and
hematocrit levels, the type of prosthesis, the use of suction drainage and the
use of tranexamic acid. Patients with pre-donated autologous blood did not
require allogenic blood transfusion. Conclusion: Based on our
study, total hip arthroplasty necessitates allogenic blood transfusion in 8.6%.
Major predictors associated with the need for transfusion are preoperative
hemoglobin and hematocrit levels, the type of prosthesis, the use of suction
drainage and the use of tranexamic acid. Pre-donated autologous blood helps to

Investigation of the effect of rifampicin resistance and risk factors on recovery rates after DAIR procedure in patients with prosthetic joint infection
Abstract Background Rifampicin plays a key role in the management of prosthetic joint infections (PJIs), however, the emergence of rifampicin resistance is associated with less favourable clinical outcomes. The purpose of this study was to investigate the impact of rifampicin resistance and other patient-related factors on recovery rates among patients with PJI undergoing debridement, antibiotics and implant retention (DAIR). Methods We reviewed medical records and microbiology reports of 67 patients (37 males and 30 females) undergoing DAIR due to PJI between 2014 and 2021. Patient-related factors, co-morbidities and microbiological reports were collected and reviewed. Forty-four patients had hip, 21 had knee, 1 had shoulder and 1 had elbow joint infection. Obtained data were statistically analysed with a logistic regression model. Results Rifampicin-sensitive organism was isolated in 47 cases. Recovery rate was 72.3% in the sensitive and 76.9% in the resistant group. We found no significant effect of rifampicin resistance on the probability of recovery. Age and diabetes mellitus showed negative clinical impact on recovery. Staphylococcus aureus and coagulase-negative Staphylococci were predominant in the rifampicin-sensitive (66.6% of the isolates) and Gram-negative rods in the resistant group (65.2%). Conclusions Based on our results, higher age and diabetes mellitus may have a clinically relevant negative impact on clinical outcome, however, this effect was not statistically significant. This may be due to the limited number of patients included in this study. Web- And cloud-based software infrastructure for materials design
AbstractMolecular dynamics (MD) simulations play an important role in materials design. However, the effective use of the most widely used MD simulators require significant expertise of the scientific domain and deep knowledge of the given software tool itself. In this paper, we present a tool that offers an intuitive, component-oriented approach to design complex molecular systems and set up initial conditions of the simulations. We integrate this tool into a web- and cloud-based software infrastructure, called MetaMDS, that lowers the barrier of entry into MD simulations for practitioners. The web interface makes it possible for experts to build a rich library of simulation components and for ordinary users to create full simulations by parameterizing and composing the components. A visual programming interface makes it possible to create optimization workflows where the simulators are invoked multiple times with various parameter configurations based on results of earlier simulation runs. Simulation configurations including the various parameters, the version of tools utilized and the results are stored in a database to support searching and browsing of existing simulation outputs and facilitating the reproducibility of scientific results
Genetic Structure and Diversity of Native Tench (Tinca tinca L. 1758) Populations in Hungary—Establishment of Basic Knowledge Base for a Breeding Program
Tench is a cyprinid fish that has undergone human-induced translocations. The natural populations of the species are on the decline due to habitat loss and spawning grounds degradation. The genetic diversity of seven natural populations was investigated to establish the genetic knowledge base for successful conservation efforts and for selective breeding. Twelve microsatellite markers, the sequencing of a 615 bp section of mtDNA (Cytb) and PCR-RFLP analysis of two nuclear markers (Act) and (RpS7) were used to analyze the genetic variation and structure among 175 individuals. All microsatellite loci were found to have moderate levels of polymorphism. The pairwise Fst values between population pairings were moderate; the populations were aligned to four clusters. The Cytb gene showed 20 haplotypes; 67.1% of individuals were categorized as Eastern, while 32.9% to the Western haplogroup. Analysis of the Act and RpS7 genes showed that the level of hybridization among the two haplogroups is high within the sampled populations. Hungarian Tench populations are genetically less diverse compared to natural populations in Western-Europe, but they still represent valuable genetic resources and Lake Fertő, Lake Kolon and Csörnöc-Herpenyő populations can be optimal candidates for future selective breeding programs
