    We investigate both theoretically and experimentally the stochastic switching between two counter-propagating lasing modes of a semiconductor ring laser. Experimentally, the residence time distribution cannot be described by a simple one parameter Arrhenius exponential law and reveals the presence of two different mode-hop scenarios with distinct time scales.     Exploring multi-stability in semiconductor ring lasers: theory and experiment

    We report the first experimental observation of multi-stable states in a single-longitudinal mode semiconductor ring laser. We show how the operation of the device can be steered to either monostable, bistable or multi-stable dynamical regimes in a controlled way. We observe that the dynamical regimes are organized in well reproducible sequences that match the bifurcation diagrams of a two-dimensional model. By analyzing the phase space in this model, we predict how the stochastic transitions between multi-stable states take place and confirm it experimentally.Comment: 4 pages, 5 figure

    Impact of nonlocal interactions in dissipative systems: towards minimal-sized localized structures

    In order to investigate the size limit on spatial localized structures in a nonlinear system, we explore the impact of linear nonlocality on their domains of existence and stability. Our system of choice is an optical microresonator containing an additional metamaterial layer in the cavity, allowing the nonlocal response of the material to become the dominating spatial process. In that case, our bifurcation analysis shows that this nonlocality imposes a new limit on the width of localized structures going beyond the traditional diffraction limit.Comment: 4 pages, 4 figure

    Effects of inhomogeneities and drift on the dynamics of temporal solitons in fiber cavities and microresonators

    In Ref. [Parra-Rivas at al., 2013], using the Swift-Hohenberg equation, we introduced a mechanism that allows to generate oscillatory and excitable soliton dynamics. This mechanism was based on a competition between a pinning force at inhomogeneities and a pulling force due to drift. Here, we study the effect of such inhomogeneities and drift on temporal solitons and Kerr frequency combs in fiber cavities and microresonators, described by the Lugiato-Lefever equation with periodic boundary conditions. We demonstrate that for low values of the frequency detuning the competition between inhomogeneities and drift leads to similar dynamics at the defect location, confirming the generality of the mechanism. The intrinsic periodic nature of ring cavities and microresonators introduces, however, some interesting differences in the final global states. For higher values of the detuning we observe that the dynamics is no longer described by the same mechanism and it is considerably more complex.Comment: 11 pages, 9 figure

    Background: The aim of this study was to develop, together with the Lung Foundation Netherlands and Dutch Kidney Patients Association, patients and clinicians, a measure to evaluate patient experiences with the orphan drugs pirfenidone (for idiopathic pulmonary fibrosis [IPF]) and eculizumab (for atypical haemolytic uraemic syndrome [aHUS]), as well as a generic measure of patient experiences and satisfaction with medications. Methods: Development of the Patient Experiences and Satisfaction with Medications (PESaM) questionnaire consisted of four phases: literature review (phase I); focus groups and individual patient interviews (phase II); item generation (phase III); and face and content validity testing (phase IV). Literature review aimed to identify existing disease-specific and generic patient experience measures to provide guidance on the domains of medication use relevant to patients, the number of items and type of response categories, and to generate an initial pool of items.     Kidney transplantation is the best treatment option for patients with end-stage renal failure. At present, approximately 800 Dutch patients are registered on the active waiting list of Eurotransplant. The waiting time in the Netherlands for a kidney from a deceased donor is on average between 3 and 4years. During this period, patients are fully dependent on dialysis, which replaces only partly the renal function, whereas the quality of life is limited. Mortality among patients on the waiting list is high. In order to increase the number of kidney donors, several initiatives have been undertaken by the Dutch Kidney Foundation including national calls for donor registration and providing information on organ donation and kidney transplantation. The aim of the national PROCARE consortium is to develop improved matching algorithms that will lead to a prolonged survival of transplanted donor kidneys and a reduced HLA immunization. The latter will positively affect the waiting time for a retransplantation. The present algorithm for allocation is among others based on matching for HLA antigens, which were originally defined by antibodies using serological typing techniques. However, several studies suggest that this algorithm needs adaptation and that other immune parameters which are currently not included may assist in improving graft survival rates. We will employ a multicenter-based evaluation on 5429 patients transplanted between 1995 and 2005 in the Netherlands. The association between key clinical endpoints and selected laboratory defined parameters will be examined, including Luminex-defined HLA antibody specificities, T and B cell epitopes recognized on the mismatched HLA antigens, non-HLA antibodies, and also polymorphisms in complement and Fc receptors functionally associated with effector functions of anti-graft antibodies. From these data, key parameters determining the success of kidney transplantation will be identified which will lead to the identification of additional parameters to be included in future matching algorithms aiming to extend survival of transplanted kidneys and to diminish HLA immunization. Computer simulation studies will reveal the number of patients having a direct benefit from improved matching, the effect on shortening of the waiting list, and the decrease in waiting time

    Allocation to highly sensitized patients based on acceptable mismatches results in low rejection rates comparable to non-sensitized patients

    Whereas regular allocation avoids unacceptable mismatches on the donor organ, allocation to highly sensitized patients within the Eurotransplant Acceptable Mismatch (AM) program is based on the patient's HLA phenotype plus acceptable antigens. These are HLA antigens to which the patient never made antibodies, determined by extensive laboratory testing. AM patients have superior long-term graft survival compared to highly sensitized patients in regular allocation. Here, we questioned whether the AM program also results in lower rejection rates. From the PROCARE cohort, consisting of all Dutch kidney transplants 1995-2005, we selected deceased donor single transplants with minimum one HLA mismatch and determined the cumulative 6-month rejection incidence for patients in AM or regular allocation. Additionally, we determined the effect of minimal matching criteria of one HLA-B plus one HLA-DR, or two HLA-DR antigens on rejection incidence. AM patients showed significantly lower rejection rates than highly immunized patients in regular allocation, comparable to non-sensitized patients, independent of other risk factors for rejection. Contrasting to highly sensitized patients in regular allocation, minimal matching criteria did not affect rejection rates in AM patients. Allocation based on acceptable antigens leads to relatively low risk transplants for highly sensitized patients with rejection rates similar to non-immunized individuals.
