    Factores domiciliarios asociados con la presencia de hidatidosis humana en tres comunidades rurales de Junín, Perú

    Introduction. Hydatid disease, a zoonosis caused by the larval stage of the parasite Echinococcus granulosus, is a public health problem at national level, especially in those regions dedicated to raising livestock. By now, there are many factors, at individual level, that have been associated to the infection by E. granulosus; nevertheless there is not any previous report that explore the association between household characteristics and the presence of the disease among household members. Objective. To explore the association between household characteristics and the presence of hydatid disease among household members. Material and methods. We performed a cross-sectional study in the households of 3 rural communities located in Junín, we evaluated the association between household characteristics and the presence of hydatid disease by multiple logistic regression (MLR). Results. from 417 evaluated households, 56 (13%) of them had at least one positive case among its members. Multivariate analysis showed that households with three or more members, located in the community with the lowest quintile of poverty, that reported raising livestock, and with evaluation coverage greater than 25% were more likely to have at least one positive case among its members. Conclusion. The observed characteristics be taken into account in the preliminary definition of high-risk subgroups, optimizing the use of resources and improving the effectiveness of screening programs. La hidatidosis, zoonosis producida por el estadio larvario de la taenia Echinococcus granulosus, es un problema de salud pública a nivel nacional, en especial en aquellas regiones dedicadas a la crianza de ganado. A la fecha, se han descrito factores, a nivel individual, asociados con la infección por E. granulosus; sin embargo, no se encontró reporte previo que explore la asociación entre características de la vivienda y la presencia de esta enfermedad en alguno de sus miembros. Objetivos. Explorar la asociación entre las características de la vivienda y la presencia de hidatidosis entre las personas que la habitan. Materiales y métodos. Se realizó un estudio transversal en viviendas de tres comunidades rurales de Junín. Se evaluó el grado de asociación entre las características de la vivienda y la presencia de hidatidosis en el hogar mediante un análisis de regresión logística múltiple (RLM). Resultados. De un total de 417 viviendas evaluadas, 56 (13%) de ellas tenían al menos un caso positivo entre sus miembros; luego del análisis de RLM se observó que aquellas viviendas con más de tres miembros, localizadas en la comunidad con quintil de pobreza más bajo, que refirieron crianza de animales, y con una cobertura de evaluación mayor al 25% presentaron una mayor probabilidad de tener al menos un caso de positivo entre sus miembros. Conclusión. Las características observadas deben ser tomadas en cuenta para la determinación preliminar de subgrupos de alto riesgo, optimizando así el uso de los recursos y mejorando la eficacia de los programas de despistaje

    Taenia solium Cysticercosis Hotspots Surrounding Tapeworm Carriers: Clustering on Human Seroprevalence but Not on Seizures

    Cysticercosis is a parasitic disease caused by the tapeworm Taenia solium, common in areas with limited sanitation or with migration from these populations. The adult parasite is hosted in the human intestine and releases large numbers of eggs with the feces. Human beings sometimes ingest eggs due to poor hygiene, and then eggs sometimes lodge on the brain and after a few years can cause intense headaches and seizures. During a study in seven rural communities in Peru, individuals exposed to T. solium eggs were often tightly clustered at the homes or immediate surrounding of the carriers of the adult parasite. However, no aggregation of cases of seizures was found near carriers. It appears that seizures do not cluster around carriers because several years pass between exposure to T. solium eggs and the onset of seizures. During these years the adult parasite has probably died or people had moved within or even outside their communities. Therefore, only a partial understanding of the epidemiology of cysticercosis is gained by studying seizures cases

    Human and Canine Echinococcosis Infection in Informal, Unlicensed Abattoirs in Lima, Peru

    Echinococcus granulosus infections are a major public health problem in livestock-raising regions around the world. The life cycle of this tapeworm is sustained between dogs (definitive host, canine echinococcosis), and herbivores (intermediary host, cystic hydatid disease). Humans may also develop cystic hydatid disease. Echinococcosis is endemic in rural areas of Peru; nevertheless, its presence or the extension of the problem in urban areas is basically unknown. Migration into Lima, an 8-million habitant's metropolis, creates peripheral areas where animals brought from endemic areas are slaughtered without veterinary supervision. We identified eight informal, unlicensed abattoirs in a peripheral district of Lima and performed a cross-sectional study in to assess the prevalence of canine echinococcosis, evaluated by coproELISA followed by PCR evaluation and arecoline purge. Eight of 22 dogs (36%) were positive to coproELISA, and four (18%) were confirmed to be infected with E. granulosus tapeworms either by PCR or direct observation (purge). Later evaluation of the human population living in these abattoirs using abdominal ultrasound, chest X-rays and serology, found 3 out of 32 (9.3%) subjects with echinococcal cysts in the liver (two viable, one calcified), one of whom had also lung involvement and a strongly positive antibody response. Autochthonous transmission of E. granulosus is present in Lima. Informal, unlicensed abattoirs may be sources of infection to neighbouring people in this urban environment

    Evaluation of Oxfendazole, Praziquantel and Albendazole against Cystic Echinococcosis: A Randomized Clinical Trial in Naturally Infected Sheep

    Cystic Echinococcosis (CE) is a near-cosmopolitan parasitic zoonosis that causes economic losses in many regions of the world. This parasitic infection can be regarded as an emerging or re-emerging disease causing considerable losses in livestock production. CE is produced by the larval cystic stage (hydatid) of the dog parasite Echinococcus granulosus. After infective eggs are ingested, cysts develop mainly in lungs and liver of humans and animals (sheep, cattle, pigs, horses, etc). Infected people may require surgery and/or Albendazole-based chemotherapy. In this study, we evaluated the effects of Oxfendazole alone (an antiparasitic drug used in animals), Oxfendazole plus Praziquantel, and Albendazole plus Praziquantel against hydatid cysts in sheep over 4 to 6 weeks of treatment. All of the treatments in this study were efficacious in killing the larval stages and, therefore, in minimizing the risk of a dog acquiring new infections (taenias). These treatment schemes can be added to control measures in animals and eventually could be used for the treatment of human infection. Further investigations on different schedules of monotherapy or combined chemotherapy are needed, as well as studies to evaluate the safety and efficacy of Oxfendazole in humans

    Water Quality and Microbiological Contamination across the Fish Marketing Chain: A Case Study in the Peruvian Amazon (Lagoon Yarinacocha)

    The contamination of the surface water of lagoons is a common problem in developing countries, and can affect fishing activities. A case study was conducted on water quality and microbiological contamination of the fishing marketing chain in the Peruvian Amazon (Laguna de Yarinacocha). The microbiological, physical–chemical and parasitological parameters of the surface water were evaluated in three points of the lagoon near the landing stage; and microbiological parameters of facilities, handlers and three species of fish (Prochilodus nigricans, Mylossoma duriventre and Siluriforme spp.). In the water, there were coliform counts ≥ 23 (Most probable number—MPN)/100 mL, Escherichia coli ≥ 3.6 MPN/100 mL, and Pseudomona spp. up to 2.2 MPN/100 mL; high turbidity and variable amounts of parasites. In facilities and handlers, high levels of coliforms, mainly Escherichia coli, and Staphylococcus aureus and Escherichia coli, were found in M. duriventre meat. A poor quality of the surface water of the lagoon is concluded that compromises part of the fishing marketing chain, mainly facilities and manipulators. Furthermore, the levels of Staphylococcus aureus and Escherichia coli in fish meat show poor handling practices and possible risk of contamination by water sources

    Development and Validation of a Copro-Enzyme–Linked Immunosorbent Assay Sandwich for Detection of Echinococcus granulosus–Soluble Membrane Antigens in Dogs

    Cystic echinococcosis (CE) is a parasitic zoonosis caused by the larval stage of the tapeworm Echinococcus granulosus. Detection of the adult stage in the canine definitive host is essential for estimating infection rates, surveillance and monitoring of CE control programs. This study sought to develop and validate a coproantigen sandwich enzyme–linked immunosorbent assay (copro-ELISA), based on antibodies against E. granulosus–soluble membrane antigens (EGMA), that is capable of distinguishing infected and noninfected dogs. Anti-E. granulosus polyclonal immunoglobulin G antibodies were obtained from rabbit antiserum against EGMA. Optimization of the test was performed with 51 positive and 56 negative stool samples of canine echinococcosis. Specificity, sensitivity, cross-reactivity, intra- and inter-assay precision, and over time detection were evaluated. According to the receiver operating characteristic analysis, the diagnostic sensitivity and specificity were 96.1% (CI: 85.9–99.6) and 98.2% (CI: 89.5–100), respectively. Negative and positive predictive values were 96.5% (CI: 91.7–100) and98%(CI: 94.1–100), respectively.Nocross-reactivity with Taenia hydatigena, Dipylidium caninum, or Toxocara canis was observed. Intra- and inter-assay repeatability showed values of less than15%of the variation coefficient. The over time detection was from 20 to 27 days postinfection with E. granulosus. The copro-ELISA based on EGMA detection offers a simplified in-house development of diagnostic testing. This assay showed high specificity and sensitivity and had no cross-reactivity with other parasites.     Scoping review protocol on control strategies for Echinococcus granulosus

    Introduction: Cystic echinococcosis (CE), previously known as hydatidosis, is one of the most important zoonotic parasitic diseases of humans and livestock caused by the larval stage of the canine tapeworm Echinococcus granulosus sensu lato. Cystic echinococcosis has great economic and public health significance with a worldwide distribution. It is listed among the most severe parasitic diseases in humans, representing one of the 17 neglected diseases prioritised by the World Health Organisation. Methods and analyses: This study will review the literature on Echinococcus granulosus control strategies until 2022. This will be achieved by searching for English, Spanish, and Portuguese peer-reviewed articles in the following electronic databases MEDLINE (via PubMed), EMBASE, SCOPUS, Web of Science, and Global Index Medicus to locate the relevant articles. We will perform a grey literature search including the American Society of Tropical Medicine Annual Conference Abstract Book from 2017 to 2022, the World Association of Echinococcosis Congress Abstract Book from 2017 to 2022, Google Scholar (first 200 hits), and review the references of the selected articles. No restrictions on study design will be applied. We will develop a systematic search strategy using a combination of keywords and Boolean operators AND/OR. Eligibility screening and data extraction will be conducted by two independent reviewers, and disagreements resolved by a third independent reviewer. Ethics and dissemination: Ethical review is not required as scoping reviews are a form of secondary data analysis that synthesizes data from public available sources. Review findings will be shared with researchers, public health authorities, and other relevant stakeholders and disseminated through a peer-reviewed publication and conference presentations.
