Nowadays, concerns about the consequences of events caused by hazardous substances have become a global issue, therefore, the purpose of this study is to analyze gas leak events in gas refinery by the combination of Tripod Beta and RCA methods. The case study is (phases of 9 and 10)the fifth refinery of South Pars Gas Complex. In order to gather information, library and field methods were used. The information was gathered by visiting the site and interviewing with operators and safety personnel. The questionnaire was used in this study. The statistical population included 15HSE and professional health care experts working at safety and operation units in the refinery. The gathered information was analyzed by a combination of Tripod Beta and RCA methods and library studies were used to make a conceptual research model and a theoretical framework. Based on the obtained results, the average age of participants in this study was 4.35 years. The results show that the dangers to human health with the average of 78.3 are the most important danger compared to equipment and environmental dangers. The most important threat to the equipment is the detectors’ breakdown and massive explosion, and the most important environmental danger is the increase of hydro-sulfur in the atmosphere. The death and injury to vital and sensitive parts of body have the highest frequency among other dangers. In fact, experts believed that death or serious injury to personnel should take the first priority. Corrective actions can include current processes and procedures, use of equipment, rules and regulations and the training of human resources

Existence of advanced financial institutes and markets along with application of new approaches is directly related to any country's development level. Under this condition, more absorption of financial resources which influences the absorption of these resources by different banking groups is an issue that attracted the attention of financial and credential institutes. This research analyzes the performance of Mellat and Parsian banks about the absorption of banking resources and factors affecting it. The survey research method was used and the data collection tool was a questionnaire. The statistical population in terms of the spatial and temporal domains, were all customers of Mellat and Parsian banks of Bushehr Province during 2013 in which based on Cochran modified formula with 95% confidence, a sample size of 384 were specified. Of course, in order to prevent predicted errors and limitations, the ample population included 500 people of the active population. They were studied using descriptive statistics and inferential statistics methods. The research results show that, from customer's point of view, there is a significant difference between interest on deposits, loans, facilities, and physical capabilities of banks, allocating bank employees and a variety of banking services in any of the branches of Mellat Bank and branches of Parsian Bank. No significance difference was found in employees' attitudes with customers and the quality of banking services

Nowadays, it is vital to pay attention more seriously to the urban environmental risks because of its effects on citizens' lives. One of these problems is groundwater pollution which threatens much valuable water resources. Drinking water in Qom City is provided from different resources, such as watersheds and water wells which are located within the city. Although there is a shortage of water supply, according to the extreme water demand, these wells are used to provide city drinking water. Unfortunately, Sewage collection network in this city has not been completed yet and only a small amount of municipal wastewater is collected by the sewer network. As a result the city aquifer has been contaminated by the wastewater infiltration through the septic tanks scattered across the city. In order to assess the amount of contamination in water wells in urban areas of Qom City, sampling and testing of 24 deep wells which are scattered in the city was performed on December 2013. The meseared qualitative parameters consist of color, T, TU, TSS, pH, EC, TDS, DO, BOD5, COD, Total Alkalinity, CO3, HCO3, Total Hardness, Ca, Mg, SO4, Cl, Na, K, NH4, NO2, NO3, PO4, Br, I, TC, FC, FS, HPC, Fe, Hg, Pb, Co, Cd, Ni, As, Zn, B, Cu, Mn, Sn, Ba, and Sr. In this study, the drinking water physical, chemical and microbiological specifications guidelines of Iranian Standards and Industrial Research and the World Health Organization standards were used to evaluate the amount of the aquifer pollution. The results indicate that the aquifer in Qom is polluted widely by B, NO3, TC, FC and HPC
