    A System “PUMP”ed to Its Limits: A Case of Proton Pump Inhibitor-Induced Acute Interstitial Nephritis

    Proton pump inhibitors (PPIs) are some of the most commonly prescribed medications. Patients are given PPIs in the hospital as GI prophylaxis or sometimes as long-term therapies to reduce the symptoms of gastroesophageal reflux disease and gastrointestinal ulcers1. There has been increasing data, including case reports implicating PPIs with acute interstitial nephritis and acute renal failure3. A 26-year-old female with no past medical history presented to the emergency department (ED) complaining of the inability to voiding urine, facial swelling, and non-radiating, bilateral flank pain for three days. The patient was seen in the ED 10-days prior due to gastritis and was subsequently discharged with pantoprazole. Initial labs revealed eosinophilia, creatinine:15.20 and BUN:57. Labs acquired 10-days prior showed a creatinine:1.05 and BUN:8. Urinalysis showed 200mg/dL(proteins), 13(WBCs/hpf) and 14(RBCs/hpf). Renal ultrasound revealed increased echotexture consistent with medical renal disease and without obstructions. Catheterization liberated minimal urine thus prompting emergent hemodialysis. Urine microscopy revealed WBCs casts. She underwent four hemodialysis sessions showing significant clinical improvement and was discharged on day 12 with a Cr:2.4 and producing \u3e2L/day of urine. Although, the effectiveness of PPI treatment is undisputable, there is a growing body of evidence documenting their association with adverse effects4. This clinical vignette showcases the rare but potential, rapid onset acute interstitial nephritis associated with the use of PPIs. Therefore, prompt recognition of this adverse effect is imperative to improve patients’ outcomes. We recommend exercising caution when prescribing PPIs by performing close monitoring of renal function and stay vigilant for other side effects

    Paclitaxel, better known as the anticancer drug Taxol (R), has been isolated from several plant species and has been shown to be produced by fungi, actinomycetes, and even bacteria isolated from marine macroalgae. Given its cytostatic effect, studies conducted in the 1990's showed that paclitaxel was toxic to many pathogenic fungi and oomycetes. Further studies led to the idea that the differences in paclitaxel sensitivity exhibited by different fungi were due to differences in the p-tubulin protein sequence. With the recent isolation of endophytic fungi from the leaves and bark of the Himalayan Yew, Taxus wallichiana Zucc., and the availability of genomes from paclitaxel-producing fungi, we decided to further explore the idea that endophytic fungi isolated from Yews should be well-adapted to their environment by encoding13-tubulin proteins that are insensitive to paclitaxel. Our results found evidence of episodic positive/diversifying selection at 10 sites (default p-value threshold of 0.1) in the 1-tubulin sequences, corresponding to codon positions 33, 55, 172, 218, 279, 335, 359, 362, 379, and 406. Four of these positions (i.e., 172, 279, 359, and 362) have been implicated in the binding of paclitaxel by beta-tubulin or formed part of the binding pocket. As expected, all the fungal endophytes grew in different media regardless of the paclitaxel concentration tested. Furthermore, our results also showed that Taxomyces andreanae CBS 279.92, the first fungus shown to produce paclitaxel, is a Basidiomycete fungus as the two beta tubulins encoded by the fungus clustered together with other Basidiomycete fungi

    Mi tesis tuvo como finalidad especial establecer claramente la incidencia de la posesión pacífica de los terrenos agrícolas en el derecho a la prescripción adquisitiva de dominio de los pobladores de la Provincia de Puerto Inca 2017. Fue plantear la incidencia de la posesión pacífica de los terrenos agrícolas en el derecho a la prescripción adquisitiva de dominio de los pobladores de la Provincia de Puerto Inca 2017, El artículo 950° del Código Civil prescribe que “La propiedad inmueble se adquiere por prescripción mediante la posesión continua, pacífica y pública como propietario durante diez años. Se adquiere a los cinco años cuando median justo título y buena fe.” Debemos indicar que tenemos tres tipos de prescripción adquisitiva de dominio, Vía Administrativa Predios Rurales según Ley N° 27867 Art. 51°; Vía Notarial Predios Urbanos con o sin edificación Ley N° 27157 Art. 21° y Vía Judicial Predios Rústicos y Urbanos Art. 486° y 504° del C.P.C.. En tal sentido, se puede afirmar que la posesión pacífica debe ser entendida como aquella que se ejerce sin perturbación y aceptación de los demás integrantes de la sociedad. Simplemente he querido plantear el tema del fundamento de la incidencia de la posesión pacifica porque creo que ofrece un enorme espacio para la reflexión. Lo que está expresado aquí, está sujeto a crítica y si eso ocurre se habrá cumplido mi propósito, porque nadie es absoluto, lo cual permitirá a los pobladores, más aún a todos los agricultores de la Provincia de Puerto Inca plantear su demanda para la Prescripción Adquisitiva de dominio de sus Inmuebles donde se encuentran posesionados.Tesi

    Developing Supply Chain Competencies Through Experiential Learning and Games

    [EN] The Supply Chain Management field has a shortage of talent to deal with the complex problems organizations face nowadays. Higher education institutions have worked to close the gap in knowledge and competencies with various results. This paper presents the results of implementing a Supply Chain Management course, which includes experiential learning, gaming, and industry-academia collaboration to develop four competencies in this field.     In vitro inhibitory activity of Acca sellowiana fruit extract on end products of advanced glycation

    "Introduction Hyperglycemia plays an important role in the pathogenesis of diabetic complications, as it increases protein glycation, as well as the progressive accumulation of advanced glycation end products (AGEs), which are complex structures that produce fluorescence. The glycation reaction raises the levels of protein carbonyl, N ?-(carboxymethyl)lysine (CML), and fructosamine and decreases the level of thiol groups. Methods In the present study, the antiglycation activity was determined by fluorescence intensity using the bovine serum albumin (BSA)/glucose, CML method, and the level of fructosamine. The oxidation of proteins was determined by the carbonyl protein content and thiol groups. Results The results show that the hexane extract of Acca sellowiana (FOH) at different concentrations (0.30-5 mg/ml) significantly inhibited the formation of AGEs in the BSA/glucose model during the 4 weeks of the study. FOH reduced the levels of fructosamine and CML. Our results showed a significant effect of FOH in the prevention of oxidative damage of proteins, as well as an effect on the oxidation of thiol groups and carbonyl proteins. Conclusion The present study indicates that FOH is effective in inhibiting the glycation of proteins in vitro, so it can prevent or ameliorate the chronic conditions of diabetes associated with the formation of AGEs.

    Protection of silver nanoparticles using Eysenhardtia polystachya in peroxide-induced pancreatic beta-Cell damage and their antidiabetic properties in zebrafish

    "Background: The aim was to explore the efficacy of extract of Eysenhardtia polystachya-loaded silver nanoparticles (EP/AgNPs) on pancreatic β cells, INS-1 cells, and zebrafish as a valuable model for the study of diabetes mellitus. Materials and methods: EP/AgNPs were synthesized using methanol/water bark extract of E. polystachya and characterized using various physicochemical techniques. Results: Immersion of adult zebrafish in 111 mM glucose solution resulted in a sustained hyperglycemic, hyperlipidemic state, and serum insulin levels decreased. The synthesized EP/AgNPs showed an absorption peak at 413 nm on ultraviolet–visible spectroscopy, revealing the surface plasmon resonance of the nanoparticles. Transmission electron microscopy indicated that most of the particles were spherical, with a diameter of 10–12 nm, a polydispersity index of 0.197, and a zeta potential of -32.25 mV, suggesting high stability of the nanoparticles. EP/AgNPs promote pancreatic β-cell survival, insulin secretion, enhanced hyperglycemia, and hyperlipidemia in glucose-induced diabetic zebrafish. EP/AgNPs also showed protection of the pancreatic β-cell line INS-1 against hydrogen peroxide-induced oxidative injury. Conclusion: The results indicate that EP/AgNPs have good antidiabetic activity and therefore could be used to prevent the development of diabetes.

    Intensity of Oestrus Signalling Is the Most Relevant Indicator for Animal Well-Being in High-Producing Dairy Cows

    Full signalling of oestrous behaviour is vital for proper timing of AI and good reproductive performance, currently jeopardized by shorter observations of oestrus behaviour. Alternative indicators including progesterone (P4) recordings on-farm are tested. Oestrous intensity of 37 heifers (H) and 30 1st-parity dairy cows (C1) either Swedish Red (32) or Swedish Holstein (35) with high genetic potential for milk production, was studied in relation to AI. P4-levels in blood or milk were monitored on-farm at 0, 7, and 20 d post-AI with a portable ELISA reader (eProCheck800). Avoidance distance and body condition were scored at day 7, and pregnancy diagnosed by P4 (day 20) and trans-rectal palpation (day 50). More heifers (46%) than C1-cows (10%) showed standing oestrus (strongest intensity, P < 0.05), leading to higher pregnancy rate at d50 (72% versus 37% for C1, P < 0.01) and calving rate (H: 64%, C1: 33%, P < 0.05). Avoidance distances were short (<1 m), reflecting good human-animal interaction. Visually-recorded standing oestrus yielded 4.8 fold higher odds of pregnancy, respectively 4.6-fold higher odds of calving. On-farm P4-recordings had complementary value yet less accuracy. Intensity of oestrus signalling relates to animal well-being, reflected in pregnancy-to-term being a good indicator for optimal welfare in high-producing dairy cattle

    The parthenogenetic marbled crayfish, or Marmorkrebs (Procambarus virginalis Lyko 2017), is an emerging model organism. We describe a method to isolate cells from early-stage embryos and culture them in vitro. The identity of the cells was confirmed by sequencing the cytochrome c oxidase subunit I (COI) gene.
