Utilisation of an operative difficulty grading scale for laparoscopic cholecystectomy
Background
A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets.
Methods
Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall’s tau for dichotomous variables, or Jonckheere–Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis.
Results
A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p < 0.001). In the CholeS cohort, a higher difficulty grade was found to be most strongly associated with conversion to open and 30-day mortality (AUROC = 0.903, 0.822, respectively). On multivariable analysis, the Nassar operative difficultly scale was found to be a significant independent predictor of operative duration, conversion to open surgery, 30-day complications and 30-day reintervention (all p < 0.001).
Conclusion
We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty
Plant species composition alters the sign and strength of an emergent multi-predator effect by modifying predator foraging behaviour
The prediction of pest-control functioning by multi-predator communities is hindered by the non-additive nature of species functioning. Such non-additivity, commonly termed an emergent multi-predator effect, is known to be affected by elements of the ecological context, such as the structure and composition of vegetation, in addition to the traits of the predators themselves. Here we report mesocosm experiments designed to test the influence of plant density and species composition (wheat monoculture or wheat and faba bean polyculture) on the emergence of multi-predator effects between Adalia bipunctata and Chrysoperla carnea, in their suppression of populations of the aphid Metopolophium dirhodum. The mesocosm experiments were followed by a series of behavioural observations designed to identify how interactions among predators are modified by plant species composition and whether these effects are consistent with the observed influence of plant species composition on aphid population suppression. Although plant density was shown to have no influence on the multi-predator effect on aphid population growth, plant composition had a marked effect. In wheat monoculture, Adalia and Chrysoperla mixed treatments caused greater suppression of M. dirhodum populations than expected. However this positive emergent effect was reversed to a negative multi-predator effect in wheat and faba bean polyculture. The behavioural observations revealed that although dominant individuals did not respond to the presence of faba bean plants, the behaviour of sub-dominants was affected markedly, consistent with their foraging for extra-floral nectar produced by the faba bean. This interaction between plant composition and predator community composition on the foraging behaviour of sub-dominants is thought to underlie the observed effect of plant composition on the multi-predator effect. Thus, the emergence of multi-predator effects is shown to be strongly influenced by plant species composition, mediated, in this case, by the provision of extra-floral nectar by one of the plant species
The spatial distribution of canopy- resident and ground-resident cereal aphids (Sitobion avenae and Metopolophium dirhodum) in winter wheat
We investigated, within two cereal fields in Southern England, the within-canopy spatial distribution of the aphids Sitobion avenae and Metopolophium dirhodum in relation to crop yield and plant nitrogen. We extended the study to investigate the spatial distribution of aphids that fell to, or returned from, the ground in order to estimate availability of the within-canopy aphid population to ground-active predators. We revealed that crop canopy aphid spatial pattern was associated with nitrogen or yield.
Differences were evident between species: S. avenae was generally negatively associated with yield or plant nitrogen, whilst M. dirhodum exhibited positive association. For both aphid species, we observed strong spatial pattern for aphids falling to the ground and conclude that this could, in part, mediate the effectiveness of ground-active predators as pest control agents
No correlation between the diversity and productivity of assemblages: evidence from the phytophage and predator assemblages in various cotton agroecosystems
Scope for non-crop plants to promote conservation biological control of crop pests and serve as sources of botanical insecticides
Besides providing food and shelter to natural enemies of crop pests, plants used in conservation biological control interventions potentially provide additional ecosystem services including providing botanical insecticides. Here we concurrently tested the strength of these two services from six non-crop plants in managing cabbage pests in Ghana over three successive field seasons. Crop margin plantings of Ageratum conyzoides, Tridax procumbens, Crotalaria juncea, Cymbopogon citratus, Lantana camara and Talinum triangulare were compared with a bare earth control in a three-way split plot design such that the crop in each plot was sprayed with either a 10% (w/v) aqueous extract from the border plant species, a negative control (water) and a positive control (emamectin benzoate ‘Attack’ insecticide). Pests were significantly less numerous in all unsprayed treatments with non-crop plant margins and in corresponding sprayed treatments (with botanical or synthetic insecticide positive control) while treatments with bare earth margin or sprayed with water (negative controls) had the highest pest densities. Numbers of predators were significantly depressed by synthetic insecticide but higher in other treatments whether unsprayed or sprayed with botanical insecticide. We conclude that some plant species have utility in both conservation biological control and as source of botanical insecticides that are relatively benign to natural enemies. In this crop system, however, the additional cost associated with using botanical insecticides was not justified by greater levels of pest suppression than achieved from border plants alone
Binocular rivalry in children with schizophrenia: the conscious and unconscious cognitive processing of interpersonal information
背景: 儿童期精神分裂是一种严重的精神障碍,有理论认为其认知功能无论是意识水平还是潜意识水平都存在异常。但目前尚无针对儿童期精神分裂的潜意识认知功能研究。
目的: 开发新的双眼竞争测验版本,用于评估个体在意识和潜意识状态下对人际交往信息的认知加工过程,并依此判断儿童期精神分裂症患者的社会认知功能是否受损。
方法: 选取3种不同类型的图片(图片中无人物、有2~3个人物、有4个及以上的人物)共30张,这些图片在双眼竞争测验不可见模式(存在双眼竞争性抑制,反映潜意识状态下认知加工)和可见模式(无双眼竞争性抑制,反映意识状态下)中展示。对15名年龄≤16岁、以妄想为主要症状的精神分裂症患者及15名健康儿童进行双眼竞争测验,通过对目标图片出现后立即在其左侧或右侧出现光栅的方向的判别来比较两组儿童的正确率,并比较两组间的反应时间。
结果: 患者组对所有类型图片中光栅方向判断的正确率均低于对照组,但是12对比较中仅2对的差异有统计学意义。与对照组相比,无论是在可见模式还是非可见模式下,患者对人物图片的注意要比对无人图片的注意有所增加,但无显著性意义。我们并未发现精神病性症状的严重程度与对图像认知加工的受损程度之间存在任何关联。当要求被试对三组图片进行评分时,患者组对存在2~3个人物的图片报告的高兴程度明显高于对照组的评分,差异有统计学意义。
结论: 儿童期精神分裂症患者对描绘人际关系的图片注意一定程度的增加,提示该病与社会信息的认知处理过程受损相关,但目前结果尚不能证实这一关系。我们将双眼竞争范式应用于认知功能差异的研究,只是取得部分的成功,其主要原因是该测验中对不同类型图片的反应注意量的关键指标存在较大的变异。</p
