Beta-adrenoblockers in clinical practice: is there any difference?

    Get PDF
Modern views on various β-adrenoblockers (BB) place in clinical practice are presented. BB class is a clinically heterogeneous group. Recent criticism of BB should not disorient practitioners, who must understand which BB are mentioned. Some BB, mostly non-selective, demonstrate adverse effects and cannot be used in certain clinical situations: metabolic disturbances, chronic obstructive pulmonary disease (COPD), peripheral artery atherosclerosis. At the same time, modern super-selective BB are safe and can be administered even in individuals with metabolic syndrome, diabetes mellitus, COPD, or peripheral artery atherosclerosis

Aim. To assess microcirculation (MC) dynamics during rosuvastatin therapy in patients with dyslipidemia (DL) and mild to moderate arterial hypertension (AH). Material and methods. The study included 25 patients with total cholesterol (TCH) level >5,0 mmol/l, low-density lipoprotein CH (LDL-CH) level > 3,0 mmol/l, systolic blood pressure (SBP) 140-179 mm Hg, and diastolic BP (DBP) 90-109 mm Hg. For 12 weeks, all participants received rosuvastatin, in constant dose of 10 mg/d. MC was assessed by laser Doppler flowmetry. Results. Rosuvastatin beneficially influenced MC, in various pathological MC types – spastic and hyperemic, according to main MC parameters – MC and capillary blood flow reserve. Regulatory mechanisms’ changes (myogenic amplitude increase) pointed to a decrease in peripheral vascular resistance. Another positive vascular effect of rosuvastatin manifested in significant SBP and DBP reduction, by 8 and 6 mm Hg, respectively. Rosuvastain also demonstrated a substantial lipid-lowering effect. Conclusion. A new statin, rosuvastatin, demonstrated not only substantial lipid-lowering effect, but also beneficial vascular action: it improved MC and reduced BP in patients with DL and mild to moderate AH

The Role of Modern Pedagogical Technologies in the Formation of Students' Communicative Competence

The article deals with the issues related to developing communicative competence of future English teachers. It considers the notion that developing communicative competence can benefit not only to the interactive competences of the learner from the educational point of view, but also psycho-emotional characteristics and socio-cultural development of a student as a person.  As we know, communicative competence is the ability that means interacting effectively with others and competence is understood as a combination of language skills an individual has for learning a foreign language. Such potential contributes to his/her attaining high levels of performance. This paper reflects the following: the theory of communicative competence and some of its models; the importance of developing communicative competence of students who are future English teachers and the implications of communicative competence in English language teaching and learning. Furthermore, the work suggests the basic methodic principles of developing communicative competence of future English teachers

Aim. Study of the effect of modified twin block activator used for distoclusion treatment on stomatognathic system and its improvement. Methods. 55 patients with skeletal distoclusion were examined. Out of them 28 patients treated orthodontically comprised the study group and 27 non-treated patients were included into the control group. The patients' age was 10-14 years (mean age 11.78±0.91), the duration of treatment was 1.5-2 years (mean duration 1.82±0.43). To avaluate the changes in stomatognathic system, cephalometric analysis was performed before and after treatment, and in the control group cephalometric analysis was performed with a 2-year interval. Results. During treatment with modified twin block appliance, SNB angle reached norm (80°). This is an important positive result in the treatment of distoclusion. ANB angle reflecting sagittal relationship between the upper and lower jaws and WITS decreased and reached normal size. Overjet normalized due to upper incisors retrusion and repositioning of the mandible forward in patients of the study group. In this group due to changes in soft tissues, in-profile esthetic and harmonious appearance of lips and chin was restored. In case of relative physiological rest the opened lips became closer and provided esthetic optimum of the face. Conclusion. Use of modified twin block appliance for the treatment of patients with distoclusion due to repositioning of the mandible forward allows improving sagittal relationship between the upper and lower jaws as well as overjet correction without increasing face height; use of modified twin block appliance allows normalizing sagittal occlusion and protrusion of upper incisors and upper lip
