    Effect of physical training on urinary incontinence: a randomized parallel group trial in nursing homes

Urinary incontinence (UI) is a common complaint for residents in NHs. Reduced functional ability and residence in NHs are documented to be risk factors for UI.Objective: To investigate if an individualized training program designed to improve activity of daily living (ADL) and physical capacity among residents in nursing homes has any impact on UI.Materials and methods: This randomized controlled trial was a substudy of a Nordic multicenter study. Participants had to be >65 years, have stayed in the NH for more than 3 months and in need of assistance in at least one ADL. A total of 98 residents were randomly allocated to either a training group (n = 48) or a control group (n = 50) after baseline registrations. The training program lasted for 3 months and included accommodated physical activity and ADL training. Personal treatment goals were elicited for each subject. The control group received their usual care. The main outcome measure was UI as measured by a 24-hour pad-weighing test. There was no statistically significant difference between the groups on this measure at baseline (P = 0.15). Changes were calculated from baseline to 3 months after the end of the intervention.Results: Altogether, 68 participants were included in the analysis, 35 in the intervention group and 33 in the control group. The average age was 84.3 years. The 3 months' postintervention adjusted mean difference between groups according to amount of leakage was 191 g (P = 0.03). This result was statistically significant after adjusting for baseline level, age, sex, and functional status. The leakage increased in residents not receiving the experimental intervention, while UI in the training group showed improvement.Conclusion: The intervention group had significant better results compared with the control group after an individualized training program designed to improve ADL and physical capacity. Further studies are needed to evaluate the effect of a goal-oriented physical training program toward NH residents UI complaints.Keywords: effect study, nursing homes, residents, physical training program, urinary incontinenc

    Isometric force pillow: using air pressure to quantify involuntary finger flexion in the presence of hypertonia

    Survivors of central nervous system injury commonly present with spastic hypertonia. The affected muscles are hyperexcitable and can display involuntary static muscle tone and an exaggerated stretch reflex. These symptoms affect posture and disrupt activities of daily living. Symptoms are typically measured using subjective manual tests such as the Modified Ashworth Scale; however, more quantitative measures are necessary to evaluate potential treatments. The hands are one of the most common targets for intervention, but few investigators attempt to quantify symptoms of spastic hypertonia affecting the fingers. We present the isometric force pillow (IFP) to quantify involuntary grip force. This lightweight, computerized tool provides a holistic measure of finger flexion force and can be used in various orientations for clinical testing and to measure the impact of assistive devices

    Holz-Beton-Verbünde mit konduktiv erwärmter Schnellklebetechnik

    Innovationen im Bauwesen zielen häufig auf Kostenreduktion durch Materialeinspa-rung mit neuen Materialkombinationen und schnellerem Baufortschritt durch hohen Vorfertigungsgrad. Weiterhin kommen dem zunehmenden Wunsch nach Nutzung nach-wachsender Rohstoffe im Bauwesen insbesondere Holz-basierte Bauweisen entgegen, die schon seit langem im Fertighausbau für kleinere Gebäude wie 1- und 2-Familien-häuser am Markt verfügbar sind. Da reine Holzbauweisen jedoch für größere Gebäude auf verschiedene Hindernisse stoßen, sind sinnvolle Materialkombinationen mit etab-lierten Baustoffen des Hochbaus eine Lösung. Holz-Beton-Verbünde können in häufig verwendeten Bauelement-Bereichen wie Zwischendecken oder Dächern ihre Eigen-schaften günstig kombinieren und werden bisher üblicherweise entweder mit zahlrei-chen mechanischen Verbindungsmitteln (lange Schrauben) oder durch Verguss von Frischbeton auf Holz realisiert. Eine neuartige Schnellklebetechnik (erstmals untersucht für den Fertighausbau) vermeidet verschiedene Nachteile und ermöglicht Kleben im Hochbau mit beheizter Klebefuge. Strukturelle Klebstoffe können so schneller aushär-ten und ermöglichen einen beschleunigten Baufortschritt im Vergleich zu konventionell kalthärtenden Klebungen. Die Beheizung der Klebefuge wird dabei durch einen dünnen elektrisch leitfähigen Klebebandträger realisiert, der als Strom durchflossener linearer Widerstand kontrolliert erwärmt werden kann. In einem laufenden Forschungsprojekt der Industriellen Gemeinschafts-Forschung (IGF) wird diese Schnellbautechnik für den Einsatz auf der Baustelle von Hochbauten untersucht. Der Beitrag stellt die klebtechni-schen Herausforderungen an verschiedene Konstruktionsklebstoffe speziell an den Grenzflächen der sehr unterschiedlichen Werkstoffe wie Beton, Nadel- oder Laubholz sowie metallischen Klebebandträgern dar

Oceanography, (2022), https://doi.org/10.5670/oceanog.2022.205.The Processes driving Exchange At Cape Hatteras (PEACH) program seeks to better understand seawater exchanges between the continental shelf and the open ocean near Cape Hatteras, North Carolina. This location is where the Gulf Stream transitions from a boundary-trapped current to a free jet, and where robust along-shelf convergence brings cool, relatively fresh Middle Atlantic Bight and warm, salty South Atlantic Bight shelf waters together, forming an important and dynamic biogeographic boundary. The magnitude of this convergence implies large export of shelf water to the open ocean here. Background on the oceanography of the region provides motivation for the study and gives context for the measurements that were made. Science questions focus on the roles that wind forcing, Gulf Stream forcing, and lateral density gradients play in driving exchange. PEACH observational efforts include a variety of fixed and mobile observing platforms, and PEACH modeling included two different resolutions and data assimilation schemes.     Calcification of intervertebral discs in the dachshund: a radiographic and histopathologic study of 20 dogs

    <p>Abstract</p> <p>Background</p> <p>The purpose of the study was to compare radiographic and histopathologic findings with regard to number and extent of calcified discs in the dachshund.</p> <p>Methods</p> <p>The intervertebral discs of 20 dachshunds were subjected to a radiographic and histopathologic examination. The dogs were selected randomly from clinical cases euthanased for reasons unrelated to research at the Norwegian School of Veterinary Science. Lateral radiographs were taken of the vertebral columns after removing them from the carcasses. The histopathologic examination included 5 μm thick sections in the transverse plane, stained with hematoxylin-eosin and von Kossa. Radiographs and histological sections were evaluated independently.</p> <p>Results</p> <p>A total of 148 (28.5%) calcified discs were identified at the radiographic and 230 (45.7%) at the histopathologic examination. Of 92 discs found to be calcified by histopathology, but not by radiography, the degree of calcification was evaluated as 'slight' in 84 (91.3%). All the intervertebral discs (n = 138) that were found to be calcified by radiography were also found to be calcified by histopathology.</p> <p>Conclusion</p> <p>A sensitivity of 0.6 and specificity of 1.0 for radiography was calculated when using histopathology as the gold standard.</p

    Barotropic tides in the South Atlantic Bight

    The characteristics of the principal barotropic diurnal and semidiurnal tides are examined for the South Atlantic Bight (SAB) of the eastern United States coast. We combine recent observations from pressure gauges and ADCPs on fixed platforms and additional short-term deployments off the Georgia and South Carolina coasts together with National Ocean Service coastal tidal elevation harmonics. These data have shed light on the regional tidal propagation, particularly off the Georgia/South Carolina coast, which is perforated by a dense estuary/tidal inlet complex (ETIC). We have computed tidal solutions for the western North Atlantic Ocean on two model domains. One includes a first-order representation of the ETIC in the SAB, and the other does not include the ETIC. We find that the ETIC is highly dissipative and affects the regional energy balance of the semidiurnal tides. Nearshore, inner, and midshelf model skill at semidiurnal frequencies is sensitive to the inclusion of the ETIC. The numerical solution that includes the ETIC shows significantly improved skill compared to the solution that does not include the ETIC. For the M2 constituent, the largest tidal frequency in the SAB, overall amplitude and phase error is reduced from 0.25 m to 0.03 m and 13.8° to 2.8° for coastal observation stations. Similar improvement is shown for midshelf stations. Diurnal tides are relatively unaffected by the ETIC

    Gytekartleggingstokt for nordøstarktisk hyse ble gjennomført 8. -19. april 2022 med FF Kristine Bonnevie. Toktet startet i Bodø og ble avsluttet i Tromsø. Toktet dekket det antatte gyteområdet langs Eggakanten mellom Malangsgrunnen og Bjørnøyrenna. Kontinuerlige akustiske registringer ble gjort langs 3194 nautiske mil kurslinjer. Det ble tatt 38 stasjoner med egghåv og CTD, og 29 bunntrål. Gytende hyse ble funnet i de fleste trålhalene. De innsamlete eggene er sendt til genetisk analyse for å kunne skille torsk- og hyseegg.publishedVersio

    Involvement of extracellular vesicles in the progression, diagnosis, treatment, and prevention of whole-body ionizing radiation-induced immune dysfunction

    Acute radiation syndrome (ARS) develops after exposure to high doses of ionizing radiation and features immune suppression and organ failure. Currently, there are no diagnostics to identify the occurrence or severity of exposure and there are limited treatments and preventative strategies to mitigate ARS. Extracellular vesicles (EVs) are mediators of intercellular communication that contribute to immune dysfunction across many diseases. We investigated if EV cargo can identify whole body irradiation (WBIR) exposure and if EVs promote ARS immune dysfunction. We hypothesized that beneficial EVs derived from mesenchymal stem cells (MSC-EVs) would blunt ARS immune dysfunction and might serve as prophylactic radioprotectants. Mice received WBIR (2 or 9 Gy) with assessment of EVs at 3 and 7 days after exposure. LC-MS/MS proteomic analysis of WBIR-EVs found dose-related changes as well as candidate proteins that were increased with both doses and timepoints (34 total) such as Thromboxane-A Synthase and lymphocyte cytosolic protein 2. Suprabasin and Sarcalumenin were increased only after 9 Gy suggesting these proteins may indicate high dose/lethal exposure. Analysis of EV miRNAs identified miR-376 and miR-136, which were increased up to 200- and 60-fold respectively by both doses of WBIR and select miRNAs such as miR-1839 and miR-664 were increased only with 9 Gy. WBIR-EVs (9 Gy) were biologically active and blunted immune responses to LPS in RAW264.7 macrophages, inhibiting canonical signaling pathways associated with wound healing and phagosome formation. When given 3 days after exposure, MSC-EVs slightly modified immune gene expression changes in the spleens of mice in response to WBIR and in a combined radiation plus burn injury exposure (RCI). MSC-EVs normalized the expression of certain key immune genes such as NFκBia and Cxcr4 (WBIR), Map4k1, Ccr9 and Cxcl12 (RCI) and lowered plasma TNFα cytokine levels after RCI. When given prophylactically (24 and 3 hours before exposure), MSC-EVs prolonged survival to the 9 Gy lethal exposure. Thus, EVs are important participants in ARS. EV cargo might be used to diagnose WBIR exposure, and MSC-EVs might serve as radioprotectants to blunt the impact of toxic radiation exposure
