    Respiratory and haemodynamic changes during decremental open lung positive end-expiratory pressure titration in patients with acute respiratory distress syndrome

    INTRODUCTION: To investigate haemodynamic and respiratory changes during lung recruitment and decremental positive end-expiratory pressure (PEEP) titration for open lung ventilation in patients with acute respiratory distress syndrome (ARDS) a prospective, clinical trial was performed involving 12 adult patients with ARDS treated in the surgical intensive care unit in a university hospital. METHODS: A software programme (Open Lung Tool) incorporated into a standard ventilator controlled the recruitment (pressure-controlled ventilation with fixed PEEP at 20 cmH2O and increased driving pressures at 20, 25 and 30 cmH2O for two minutes each) and PEEP titration (PEEP lowered by 2 cmH2O every two minutes, with tidal volume set at 6 ml/kg). The open lung PEEP (OL-PEEP) was defined as the PEEP level yielding maximum dynamic respiratory compliance plus 2 cmH2O. Gas exchange, respiratory mechanics and central haemodynamics using the Pulse Contour Cardiac Output Monitor (PiCCO), as well as transoesophageal echocardiography were measured at the following steps: at baseline (T0); during the final recruitment step with PEEP at 20 cmH2O and driving pressure at 30 cmH2O, (T20/30); at OL-PEEP, following another recruitment manoeuvre (TOLP). RESULTS: The ratio of partial pressure of arterial oxygen (PaO2) to fraction of inspired oxygen (FiO2) increased from T0 to TOLP (120 +/- 59 versus 146 +/- 64 mmHg, P < 0.005), as did dynamic respiratory compliance (23 +/- 5 versus 27 +/- 6 ml/cmH2O, P < 0.005). At constant PEEP (14 +/- 3 cmH2O) and tidal volumes, peak inspiratory pressure decreased (32 +/- 3 versus 29 +/- 3 cmH2O, P < 0.005), although partial pressure of arterial carbon dioxide (PaCO2) was unchanged (58 +/- 22 versus 53 +/- 18 mmHg). No significant decrease in mean arterial pressure, stroke volume or cardiac output occurred during the recruitment (T20/30). However, left ventricular end-diastolic area decreased at T20/30 due to a decrease in the left ventricular end-diastolic septal-lateral diameter, while right ventricular end-diastolic area increased. Right ventricular function, estimated by the right ventricular Tei-index, deteriorated during the recruitment manoeuvre, but improved at TOLP. CONCLUSIONS: A standardised open lung strategy increased oxygenation and improved respiratory system compliance. No major haemodynamic compromise was observed, although the increase in right ventricular Tei-index and right ventricular end-diastolic area and the decrease in left ventricular end-diastolic septal-lateral diameter during the recruitment suggested an increased right ventricular stress and strain. Right ventricular function was significantly improved at TOLP compared with T0, although left ventricular function was unchanged, indicating effective lung volume optimisation

    Parallel hardware architectures for the cryptographic Tate pairing

    Identity-based cryptography uses pairing functions, which are sophisticated bilinear maps defined on elliptic curves. Computing pairings efficiently in software is presently a relevant research topic. Since such functions are very complex and slow in software, dedicated hard- ware (HW) implementations are worthy of being stud- ied, but presently only very preliminary research is avail- able. This work affords the problem of designing paral- lel dedicated HW architectures, i.e.,co-processors, for the Tate pairing, in the case of the Duursma-Lee algorithm in characteristic 3. Formal scheduling methodologies are applied to carry out an extensive exploration of the archi- tectural solution space, evaluating the obtained structures by means of different figures of merit such as computation time, circuit area and combinations thereof.Comparisons with the (few) existing proposals are carried out, show- ing that a large space exists for the efficient parallelHW computation of pairings

    First record of terrestrial Enchytraeidae (Annelida: Clitellata) in Versailles palace's park, France

    France can be qualified as terra incognita regarding terrestrial enchytraeids because very little data has been recorded so far in this country. In spring and autumn 2016, enchytraeid communities were investigated in a loamy soil in a meadow located in the park of Versailles palace, France. In total, twenty four enchytraeid species were identified, belonging to six different genera i.e. eleven Fridericia species, four Enchytraeus species, four Achaeta species, two Buchholzia species, two Marionina species and one Enchytronia species. According to the published data, this was one of the highest diversity found in a meadow in Europe

    In Vitro Evaluation of an Active Heat-and-Moisture Exchanger: The Hygrovent Gold

    BACKGROUND: To improve the heat and humidification that can be achieved with aheat-and-moisture exchanger (HME),a hybrid active (ie,adds heat and water) HME,the Hygrovent Gold,was developed. We evaluated in vitro the performance of theHygrovent Gold. METHODS: We tested the Hygrovent Gold (with and without its supplemental heat andmoisture options activated),the Hygrobac,and the Hygrovent S. We measured theabsolute humidity,using a test lung ventilated at minute volumes of 5,10,and15 L/min,in normothermic (expired temperature 34 degrees C) and hypothermic(expired temperature 28 degrees C) conditions. We also measured the HMEs' flowresistance and weight after 24 h and 48 h. RESULTS: In its active mode the Hygrovent Gold provided the highest absolutehumidity,independent of minute volume,in both normothermia and hypothermia. Therespective normothermia and hypothermia absolute humidity values at 10 L/min were36.3 + 1.3 mg/L and 27.1 + 1.0 mg/L with the active Hygrovent Gold,33.9 + 0.5mg/L and 24.2 + 0.8 mg/L with the passive Hygrovent Gold,33.8 + 0.56 mg/L and24.4 + 0.4 mg/L with the Hygrobac,and 33.9 + 0.8 mg/L and 24.6 + 0.6 mg/L withthe Hygrovent S. The efficiency of the tested HMEs did not change over time. At24 h and 48 h the increase in weight and flow resistance was highest in theactive Hygrovent Gold. CONCLUSIONS: The passive Hygrovent Gold provided adequate heat and moisture innormothermia,but the active Hygrovent Gold provided the highest humidity,inboth normothermia and hypothermia

    A 1 m3^3 Gas Time Projection Chamber with Optical Readout for Directional Dark Matter Searches: the CYGNO Experiment

    The aim of the CYGNO project is the construction and operation of a 1~m3^3 gas TPC for directional dark matter searches and coherent neutrino scattering measurements, as a prototype toward the 100-1000~m3^3 (0.15-1.5 tons) CYGNUS network of underground experiments. In such a TPC, electrons produced by dark-matter- or neutrino-induced nuclear recoils will drift toward and will be multiplied by a three-layer GEM structure, and the light produced in the avalanche processes will be readout by a sCMOS camera, providing a 2D image of the event with a resolution of a few hundred micrometers. Photomultipliers will also provide a simultaneous fast readout of the time profile of the light production, giving information about the third coordinate and hence allowing a 3D reconstruction of the event, from which the direction of the nuclear recoil and consequently the direction of the incoming particle can be inferred. Such a detailed reconstruction of the event topology will also allow a pure and efficient signal to background discrimination. These two features are the key to reach and overcome the solar neutrino background that will ultimately limit non-directional dark matter searches.Comment: 5 page, 7 figures, contribution to the Conference Records of 2018 IEEE NSS/MI
