    Amoxicillin induced toxic epidermal necrolysis

    Toxic epidermal necrolysis (TEN) is a rare life-threatening adverse drug reaction associated with mucocutaneous eruptions and peeling of skin (sloughing) mostly caused by drugs like sulphonamides, beta lactams, carbamazepine and non-steroidal anti-inflammatory drugs (NSAIDs). Amoxicillin is a broad spectrum, bactericidal, Beta-lactam antibiotic used in treatment of various infections. Here by we have reported the case of amoxicillin induced severe toxic epidermal necrolysis. A Patient admitted in the hospital with the symptoms of epidermal sloughing that resulted in bare dermis as he received Amoxicillin drug for his diagnosis of fever. After clear examination TEN was confirmed and suspected with the cause due to Amoxicillin. The drug was stopped and patient was treated with other drugs for symptomatic cure. The patient was recovered from his condition and improved significantly

    Squeezing in Multivariate Spin Systems

    In contrast to the canonically conjugate variates qq,pp representing the position and momentum of a particle in the phase space distributions, the three Cartesian components, JxJ_{x},JyJ_{y}, JzJ_{z} of a spin-jj system constitute the mutually non-commuting variates in the quasi-probabilistic spin distributions. It can be shown that a univariate spin distribution is never squeezed and one needs to look into either bivariate or trivariate distributions for signatures of squeezing. Several such distributions result if one considers different characteristic functions or moments based on various correspondence rules. As an example, discrete probability distribution for an arbitrary spin-1 assembly is constructed using Wigner-Weyl and Margenau-Hill correspondence rules. It is also shown that a trivariate spin-1 assembly resulting from the exposure of nucleus with non-zero quadrupole moment to combined electric quadrupole field and dipole magnetic field exhibits squeezing in cerain cases.Comment: 13 pages, 1 Table, Presented at ICSSUR-05, Franc

    Classification of stillbirth by relative condition at death (Re Co De) at various trimesters of pregnancy: a rural tertiary teaching hospital based study

    Background: New global health figures show India to have the highest rates of stillbirth in the world. While maternal and under 5 child mortality rates have halved, stillbirth remains a neglected global endemic. To reduce stillbirths, the prevalence, risk factors and causes must be known. The aim of the present study is to know the prevalence and classify stillbirths by ReCoDe classification system at different trimesters of pregnancy.Methods: This was a retrospective study done between January 2013 to March 2017 at MediCiti Institute of Medical Sciences, a rural tertiary teaching hospital, Telangana, India. A total of 112 cases of stillbirths were included. Data was obtained on demographic variables, risk factors such as preeclampsia, etc. Data regarding mode of delivery, fetal asphyxia, were recorded.Results: Stillbirth rate was 12.1/1000 births. Fifty four percent of the women were unbooked. Preterm stillbirths were a majority (67%). The intra-partum still birth rate was low (15.1%) contrary to what is seen in low middle-income countries. Gestational hypertension/Pre-eclampsia, abruptio placenta, fetal growth restriction and oligohydramnios were the leading causes of stillbirths.Conclusions: Pregnant women from rural background with low socio-economic status are prone for stillbirths. As stillbirths were more among unbooked cases, the study highlights the importance of counselling, creating awareness in the rural areas regarding the importance of regular antenatal checkups. Identifying risk factors like pre-eclampsia, anemia etc., at early weeks will enable us to initiate appropriate strategies to improve pregnancy outcome

    Vulval carcinoma at the site of recent Bartholin gland excision: a rare case report

    Around 2-7% of all vulvar cancers are represented by primary carcinomas of Bartholin gland. Here we are presenting such a rare case of carcinoma of Bartholin gland. 45/F P2L2 presented in our OPD with non-healing lesion in vulva for 2 months. She had history of Bartholin’s cyst excision (elsewhere), 2 months back (the exact time since when she is having the non-healing lesion in vulva). Diagnostic biopsy revealed it to be vulval squamous cell carcinoma. She was given two cycles of Neo adjuvant chemotherapy (Inj. Paclitaxel and Carboplatin). In view of better response to the treatment patient was planned for surgery. Patient underwent wide local excision with bilateral inguinofemoral lymphadenectomy. Although the majority of vulvar lesions are benign, especially in women younger than 50 years of age, any solid mass should be carefully evaluated for malignancy. Preferably biopsy of Bartholin gland should be considered if the patient is more than 40 years of age and should be mandatory in a menopausal woman

    Surgical management of a case of recurrent carcinoma cervix: a case report

    Worldwide cervical cancer is the fourth most common cancers among females. Pelvic recurrence, distant metastases, or a combination of both can occur in patients of carcinoma cervix. A 42-year-old P2L2 with a history of carcinoma cervix stage 3-b came to our OPD for routine follow up. Two year back she was managed with chemoradiation for her primary carcinoma which had parametrium extension. There was a 1 × 1 cm lesion on the anterior vaginal wall which was diagnosed to be cervical recurrence. Patient underwent total abdominal hysterectomy with bilateral salpingo-oophorectomy with pelvic lymphadenectomy. Patient is disease free till now. Treatment decisions should be individualised based on the performance status of the patient, the site of recurrence and/or metastases, the extent of metastatic disease, and prior treatment

    VALIDATED STABILITY INDICATING HIGH PERFORMANCE LIQUID CHROMATOGRAPHIC METHOD FOR THE QUANTIFICATION OF ASENAPINE MALEATE

    Objective: A stability indicating high performance liquid chromatography method using the photodiode array detector is developed and validated for the analysis of asenapine in bulk and in its tablet formulation. Methods: The method utilized a BDS Y Persil C18 (250 x 4.6 mm, 5 µm) column with 0.1% orthophosphoric acid and methanol (65:35 v/v) as mobile phase. The analysis was performed at 30 oC column temperature with a flow rate of 1 ml/min and detection at 227 nm. The method was validated for specificity, sensitivity, precision, linearity, accuracy and robustness. Asenapine was subjected to stress degradation studies under acidic, basic, oxidative, thermal and photolytic conditions. Results: Under optimized experimental conditions, asenapine was eluted from the column at a retention time of 6.781 min. The method was linear in the range of 100-300 µg/ml. The linear regression data showed good relationship (correlation coefficient (R2) = 0.9999). The relative standard deviation and mean recovery values were within limits. The analytical performance of the method was not affected when small variations in the experimental parameters were made. Degradation products resulting from stress degradation studies did not interfere with the detection of asenapine. Conclusion: The proposed stability indicting high performance liquid chromatography method is sensitive, precise, accurate, robust and specific. This method can be used for quantification of asenapine in bulk drug and in tablet dosage form in the presence of its stress degradation products

    Dose response effects of carbohydrate nature of lyophilized leaf powder of Azadirachta indica on testis of albino rats

    The purpose investigation is designed to investigate the effect of carbohydrate nature of lyophilized Azadirachta indica leaf powder on the histology of testis, since experimental studies shown that the chemical nature of leaf powder was studied and observed that the leaf powder was carbohydrate-rich in nature. In the present study of histomorphometric and histopathological dose dependent assessments of carbohydrate nature of lyophilized A.indica leaf powder on the testis of male wistar rats given 25, 50 and 75 mg in suspension of 1 mL Propylene Glycol, respectively/kg body weight, once daily, orally, for 24 days showed varying degrees of desquamation of seminiferous epithelial cells, degeneration of seminiferous tubules and presence of large-sized multinucleated cells as well as the spermatogenesis was arrested either at the primary spermatocytes or the spermatogonial stages. Damaged seminiferous tubules contained the abundance of vacuoles with varying size Total count, cell and nuclear diameter of spermatogonia, spermatocytes, spermatids and Leydig cells were reduced in a dose dependent manner (P ≤ 0.05). From the results obtained, it can be observed such effects on the testis are possibly due to curtailing of androgen supply within the testis or it may be a direct effect of this carbohydrate-rich nature of lyophilized A. indica leaf powder on the tissue probably due to direct or indirect gonadotrophic hormone deficiency
