    Popüler Kültür’ün Sınıf İçi Öğretim Uygulamalarına Adaptasyonuna Yönelik Niteliksel Bir Araştırma

    The aim of this study was to define the impact of infusing popular culture’s elements such as comics, TV series, popular music etc. on teaching practice in classroom in Turkey. Popular culture plays an important role in education. This study reveals that teachers often use popular culture that plays an important role in the lives of children in the classroom. Teachers attempt to integrate popular culture into the lessons in many different ways consciously or spontanesly. Popular culture is reflected in the, methods, and educational activities and drills in daily lesson plans. Popular culture is included sometimes as a reinforcing unit for some other units in lesson plans. Teachers noticed that they had to integrate popular culture into the teaching context because it allowed them to make connections between real life and school thus making daily classroom implementations better. Although teachers are against popular culture, they all agree that it is a reality for schooling and sometimes they use it reluctantly.Bu araştırmanın amacı Türkiye’de sınıf içi öğretim uygulamalarına çizgi film, TV dizileri vb.den oluşan popular kültürün öğelerinin etkisini belirlemektir. Popüler kültür eğitimde önemli bir rol oynamaktadır. Bu çalışma da çocukların yaşamında önemli rol oynayan popular kültürün öğelerinin öğretmenler tarafından öğretimde ne düzeyde kullanıldığını ortaya koymaktadır. Öğretmenler popular kültürün öğelerini bilinçli ya da bilinçsizce öğretime adapte etmektedirler. Popüler kültür öğretmenlerin günlük ders planlarındaki alıştırmalar, eğitimsel etkinlikler ve yöntemlerine de etki etmektedir. Popüler kültür bazen ders planlarında güdüleyici ve motivasyon öğesi olarak da yer almaktadır. Öğretmenler de buna karşı olsalar bile bazen gerçek yaşamla bağlantıyı kolaylaştırdığı için günlük sınıf içi etkinliklerde popüler kültürün etkisi olduğunu fark etmişlerdir. Karşı olsalar bile popüler kültürün eğitimin bir gerçeği olduğunu kabul etmişlerdir

    Severe Immune Thrombocytopenia in Pregnancy Treated with Eltrombopag. A Case Report.

    Immune Thrombocytopenia (ITP) which is a common, acquired and autoimmune disease is defined as low platelet count secondary to increased platelet destruction or impaired thrombopoiesis by anti-thromboycte antibodies. Pregnancy-associated thrombocytopenia accounts for 5% of all the cases. Thrombopoietin (TPO) mimetic drugs, such as Eltrombopag, have been used successfully in many patients with ITP; however data on its use in pregnancy is limited. In this report, a case who was followed up with ITP and given Eltrombopag during her pregnancy, cause it could not have been controlled by any other treatment is presented. Enoxaparin therapy was iniatiated for thromboprophylaxis after the patient’s platelet count responded to Eltrombopag treatment. Delivery was carried out by cesarean section. Baby was born with low birth weight and there was not any malformation. Nevertheless, further research is needed to find out whether there is a relationship between Eltrombopag use in pregnancy and low birth weight

    THE EVALUTION OF STRESS DISTRIBUTIONS IN 3 AND 5 UNIT DENTAL AND IMPLANT SUPPORTED FIXED ZIRCONIA RESTORATIONS: FINITE ELEMENT ANALYSIS

    Purpose: In this study, it is aimed to compare the distribution of stress on anchorage and implants in 3-and-5-member-dental and implant supported zirconia restorations by using finite element analysis.Material and Method: Stress distribution formed in anchorage and implants as a result of chewing forces was analyzed in dental and implant (Astra Tech Microthread Osseo Speed, Sweeden) supported models of zirconia restoration with 5-member placed on the numbers of 43, 44, 45, 46 and 47 and with 3-member placed on the number of 45, 46 and 47.  The study was performed through static nonlinear analysis with the three-dimensional finite element analysis method.  Results: The highest and the lowest stress were respectively found on the number of 45 and 47 in 3-member dental supported model. The highest and the lowest stress in 5-membered dental-supported model were respectively found on the tooth of number 45 and on the root apex of the implant of number 43. Stress accumulation was observed in the cervical portion of the implant in implant-supported models. Stress accumulation in the tooth-supported model was found less than in implant-supported modelsConclusion: The extreme forces on the dental and implant-supported restorations with increased members can reduce survival rate of restorations in mouth. To prefer dental implants with larger diameter and longer length along with infrastructure like zirconia to design posterior implant-supported restorations can keep restorations in mouth for longer period

    Distance Education in Medical Schools: the Experience and Opinions of Academicians and Students

    DergiPark: 819194tmsjAims: This study aims to identify the thoughts and opinions of medical schools’ students and academicians about the distance medical education and examination system that has been rapidly applied during the COVID-19 pandemic Methods: A ques- tionnaire was prepared via Google Forms which consisted of a total of 3 sections. Informed consent was obtained in the first section and participants were directed to “student” or “academician” section. There were 28 questions for the students and 24 questions for the academicians. Categorical variables were demonstrated as numbers and percentages, whereas continuous variables were presented as minimum, maximum, and mean values. Chi-squared test was used to compare preclinic and clinic year students, and the academicians in preclinic, medical, and surgical fields. Results: A total of 321 participants completed the questionnaire. The mean participant ages were 21.4 years and 41.68 years for the students and the academicians, respectively. Only 30% of the students thought the distance education lessons were beneficial while it was 35.5% for the academicians. 25.8% of the academicians and 29.6% of the students were indecisive on the matter. When the examination process was taken into account 67.7% of the academicians and 56.9% of the students thought the online examinations were not reliable. Conclusion: It is predicted that the pandemic process will continue in the next academic years. Considering the current situation, distance education seems to be the best option to ensure that the learning process can continue while protecting the health of students and academicians. Although distance education is not sufficient by itself in medical education during and after the COVID-19 pandemic, it is a method that should be used in almost every field of medical education, especially in the preclinical phase. Both the distance education, and online examination process require improvements and they are needed to be supported with face to face lectures and practices

    Assesment of patient's perspective on treatment free remission in CML: A Turkish multicenter cohort

    Treatment free remission (TFR) is one of the main goals of therapy in Chronic Myeloid Leukemia (CML). Patient perspective and expectations have an indicative effect on discontinuation decisions. We analyzed 116 CML patients' perspective on TFR and factors that associated with willingness for tyrosine kinaz inhibitor (TKI) discontinuation from four different center in Turkey based on a national survey. Patients in our cohort had 7.8% awareness of TFR and after brief information willingness to attempt TFR has been reached to 58.9% in all population. Shorter disease duration was associated with enhanced willingness to attempt discontinuation in multivariate analyses (p=0.031). Most common patient related limitation for TKI discontinuation decision was anxiety about disease relaps and TKI resistance. Even disease relaps in TFR means molecular and detectable with close monitoring, patients still had considerable concerns about TKI discontinuation

    Determination of Yield and Quality Characteristics of Lavandula Cultivars in the Kahramanmaras Region

    In recent years, there has been a growing interest in herbal remedies for therapeutic purposes. Aromatic plants have become a significant source of raw materials for the fragrance, food, and cosmetics industries. Additionally, the emergence of new applications and the trend towards natural nutrition and alternative healing methods, often referred to as "going natural," have increased the interest in medicinal and aromatic plants in our country, as well as in other countries around the world. In this study were investigated to determine yield and quality characteristics of seven different lavender (Lavandula intermedia Emeric ex Loisel. = L. hybrida L.) (Grasso, Süper-A, Seguret, Dutch, Abrial, Akmeşe, English) cultivars under dry and irrigated cultivation in Kahramanmaraş conditions between 2019 and 2020. The research was carried out in a randomized block design with 4 replications. Fresh stem flower yield was the highest variety Grasso(672.81kg/da). The lowest fresh stem flower yield was found in Seguret variety with 611.33kg/da The highest dry stemless flower yield was determined in Grasso variety with 59,66 kg/da. There were significant differences determined between cultivars the end of 2 years. The highest average fresh stem flower yield (693.067 kg/da) and the highest average dry branched flower yield (252.588 kg/ha) were measured in GRASSO variety

    Detection of Novel NF1 Variants with Next-Generation DNA Sequencing Technology and Genotype-Phenotype Characteristics of Neurofibromatosis

    MakaleWOS:000925992500001Objective: Neurofibromatosis type 1 (NF1, #162200) is a common neurological disorder with de novo or inherited germline mutations of the Neurofibromin (NF1, *613113). The purpose of this study is to increase the limited knowledge of NF1 in a small population-based dataset. Materials and Methods: This study enrolled patients with clinically suspected NF1 referred to the Kayseri Training and Research Hospital, Medical Genetics Department, between 2015 and 2017. The local ethics committee approved this study. Next-generation sequencing was performed for the genetic analysis. The genetic, demographic, and clinical features of the participants were characterized. Results: A total of 79 cases of NF1 were included. Of these cases, 40 were male, and 39 were female. The mean age was 11.9 years, and most were younger than 18 years. The most common complaint was cafe au lait macules. The 61 (77.3%) patients had pathogenic variants, and 16 (26.2%) were novel. Mostly affected mutation sites were exonic regions (n=54, 88.5%). The most common mutated exon was exon 38 (n=7, 11.5%), and most of the detected mutations were nonsense mutations (31%). Conclusion: It is one of Turkiye's largest NF1 study groups, where all exons of the NF1 gene were analyzed. This study contributes novel variants to the literature. There was no mutational hotspot region, and no significant relationship between genotype and phenotype was observed. Further studies and large sample sizes are required to better understand the relationship between NF and genetic changes

    Clinical outcomes and treatment patterns of primary central nervous system lymphoma: Multicenter retrospective analysis

    Objectives: Primary central nervous system lymphoma (PCNSL) is a rare malignant disease with poor prognosis. Its low incidence leads to challenges in decision-making for treatment. As a matter of fact, there is still no consensus on the appropriate treatment modalities. In this context, the objective of this study is to investigate and comparatively assess the efficacies of several treatment modalities in the treatment of PCNSL. Methods: Thirty-four patients diagnosed with PCNSL at 5 different hematology centers between 2007 and 2021 were included in the study. Patients’ data from all five centers were collected retrospectively. Since ibrutinib is not approved for this indication in Turkey, consent for off-label use of ibrutinib is obtained from each patient. Ethics committee ap-proval was obtained on June 9, 2021 with decision number 2021/18-05. Results: The median age of the patients was 59 (min.: 22, max.: 78) years. The male-to-female ratio was 1.26/1. Nineteen (55.9%) patients had Eastern Cooperative Oncology Group (ECOG) performance score of ≥2. Fifteen (44.1%) patients had normal lactate dehydrogenase (LDH) levels and only 14.7% of the patients had B symptoms at the time of diagnosis. Magnetic resonance imaging (MRI) revealed a single mass lesion in 14 (41.2%) patients. As an induction therapy, meth-otrexate-based regimen was administered in 29 (85.3%) patients. Only 14 of the 34 patients received 4 or more cycles of high-dose methotrexate (MTX). About 32.4% of the patients received radiation therapy (RT) during follow-up as a part of induction therapy. Five patients received only RT due to poor performance status. Ibrutinib was administered in 5 patients for refractory disease. It was determined that four or more cycles of MTX treatment increased progression-free survival (PFS) (p=0.031) and overall survival (OS) (p=0.012). Moreover, RT improved PFS (p=0.023). Considering that the complete response achieved by induction therapy influences long-term survival, achievement of the best response to the treatment regimens administered in combination with new agents may prolong survival (PFS: p=0.01, OS: p=0.023). Conclusion: The findings of this study indicate that the initial response to treatment is crucial. Additionally, it was found that high-dose MTX treatment should be administered for 4 cycles or more in order to achieve the best results. Furthermore, it was determined that ibrutinib monotherapy was well-tolerated in our patients with relapsed/refractory disease, with excellent clinical benefits. In conclusion, a combination therapy consisting of high-dose MTX, ibrutinib, and rituximab appears to be a promising initial treatment approach in appropriate patients

    Clinical Characteristics and Visual Outcomes of Pediatric Optic Neuritis: A Single Center Experience

    INTRODUCTION: The aim of this study was to describe the clinical characteristics, visual outcomes of pediatric patients presenting with first-episode of optic neuritis. METHODS: We reviewed medical records of the patients newly diagnosed with optic neuritis younger than 18 years between January 2014 and December 2018 retrospectively. RESULTS: Twenty-eight patients were included to this study. The mean age at first onset of optic neuritis was 13.2+-3.1 years (range 6.2-17.3 years). The mean follow-up period was 4.2+-3.2 (range 0.6-13.08) years. 7 of 28 (25%) patients had recurrent optic neuritis. Optic neuritis involvement was unilateral in 17 of 28 (60%) patients. Forty percent of the patients had idiopathic optic neuritis. Of the six patients with demyelinating lesions in cranial magnetic resonance imaging (MRI) at the first admission, three were diagnosed with multiple sclerosis (MS) at the time of first optic neuritis attack, and three were diagnosed within 13.4+-4.8 months after the first episode. Eight of 21 optic neuritis patients (38%) had oligoclonal band positivity and the incidence of MS was significantly higher in these patients (p=0.014). The mean visual acuity at nadir was 0.48+-0.27 at admission. Whereas it was 0.74+-0.31 and 0.76+-0.33 at 1 and 6 months respectively. There was a strong correlation between first and sixth-month visual acuity (r=0.98, p=0.00). DISCUSSION AND CONCLUSION: Our study demonstrated that poor visual acuity (worse than 0.5) at 1 month can predict poor vision at 6 months.
