    Strategic plan for the travel mobile app: The NoQ

    This Master thesis pretends to develop a detailed strategic plan for the innovative idea of the NoQ mobile travel application. The main purpose of this plan is to examine the feasibility and viability of the project. In order to achieve this, several fundamental steps have been taken. First, the key concepts for this thesis, such as tourism, visitor attractions and smart tourism, were identified and explained for their better understanding and unification. After that, a micro and macro analysis of the market in which the project will be implemented was conducted. To deepen the investigation, an internal evaluation of the company was carried out and the level of its competitiveness was examined. The knowledge and data obtained in the first part of the thesis were used in the following functional part. This section defines the future objectives of the strategic plan, as well as, the development strategy that includes the mission, vision, values and critical success factor of the NoQ. The final chapter contains the project implementation plan, including marketing activities shown by means of the Marketing Mix, along with the company organizational structure and schedule of operations for setting up the business. Furthermore, this section specifies the NoQ’s key resources, which have been broken down into human, physical, intellectual and financial resources. Finally, the implementation requirements for establishing a company and additional legal requirements have been defined.A presente dissertação de mestrado pretende desenvolver um plano estratégico detalhado para a aplicação móvel de viagens NoQ. O principal objetivo deste plano é estudar a viabilidade deste projeto. Para isso, várias etapas fundamentais foram tomadas. Primeiramente, os conceitos-chave para elaborar esta tese, como o turismo, as atrações turísticas e o turismo inteligente, foram identificados e explicados em detalhe para uma melhor compreensão. Seguidamente, foram realizadas uma micro e macro análise do mercado em que o projeto estará integrado. Para aprofundar a análise, foi realizada uma avaliação interna da empresa e foi também analisado o seu nível de competitividade. Os conhecimentos e dados obtidos na primeira parte da tese foram utilizados na parte subsequente da tese. Esta secção define os objetivos futuros do plano estratégico, bem como a estratégia de desenvolvimento que inclui a missão, visão, valores e fator crítico de sucesso do NoQ. O capítulo final contém o plano de implementação do projeto, incluindo as ações de marketing apresentadas por meio do Marketing Mix, juntamente com a estrutura organizacional da empresa e o cronograma de operações para a implantação do negócio. Além disso, esta seção especifica os recursos-chave do NoQ, que foram divididos em recursos humanos, físicos, intelectuais e financeiros. It is based on an attempt to minimize human impact on the environment while maintaining the natural functionality and productivity of the agricultural system. At the same time, every human activity in the natural environment results in greater or lesser changes in the soil ecosystem. Organic farming also has an impact on physical and chemical parameters and soil biological activity. These changes should be monitored and considered in the context of long-term land management.     Soil is an ecosystem capable of producing the resources necessary for the development of the living organisms. Soil microorganisms (bacteria and fungi) are responsible for biomass decomposition, biogenic element circulation, which makes nutrients available to plants, biodegradation of impurities, and maintenance of soil structure. The presence of microorganisms in soil depends on their chemical composition, moisture, pH, and structure. Human activity has an indispensable influence on the formation of ecosystems. Soil tillage has an impact on the chemical and physical parameters of the soil, and thus on its biological properties. The use of inappropriate agro-technology can lead to degradation of the soil environment. Changes in soil properties may cause changes in soil abundance, activity, and diversity. Cultivation can affect microorganisms, causing their mortality and reducing the availability of nourishment in the soil. Therefore, it is extremely important to assess the diversity and microbiological activity of soil in relation to soil-tillage technology

    Activity of Dehydrogenases as an Indicator of Soil Environment Quality

    Activity of dehydrogenases (DHs) indicates the presence of viable and physiologically active (physiologically active or physiological activity) microorganisms. Their presence (activity) in soil is correlated with the content of organic carbon, microorganisms, nitrifying activity and microbial respiration. Determination of DHs activity allows to control changes in soil microbial population and is considered as an important parameter of soil quality. The aim of the study was to determine the effects of different farming systems on the enzymatic activity in soil under winter wheat. The research was conducted in the years 2014–2016 on long-term field experiment under two different farming systems (ecological and conventional) at the IUNG (PIB) Experimental Station located in Osiny (Lublin Voivodeship), Poland. Each farming system differs in crop rotation system and whole agrotechnics, which have been adapted to its specificity. Determination of DHs activity was performed using Casida et al.’s (1964) method with modifications. Measured DHs activity was expressed in milligrams of triphenyl formazan (TPF) per 100 g of soil within 24 hours. The results showed that ecological farming system beneficially influenced soil environment.

    Definition of medical error and physicians’ interest in changes in the law

    The main purpose of the paper is to discuss the definition of medical error. Moreover, the statistical analysis was aimed at demonstrating at what level is the legal knowledge of the professional group of doctors, in relation to the performed profession. The research group consisted of doctors of different specializations, of different ages, with diverse work experiences, performing their profession in the Lubelskie voivodeship. These were people working on the basis of contract of employment, civil law contracts or individual medical practice, employed in provincial hospitals, clinics, district hospitals, outpatient’s clinics, ambulances or medical centers. The author's questionnaire survey consisted of questions and answers for 298 doctors. Damage resulting from a widely understood medical error can be caused not only by the physician but by all medical staff or due to the organizational failure of the medical establishment. It must be stated with all conviction that the formulation of the concept of medical error is still ongoing and will evolve with the development of medicine as well as the law, which will strive to delineate the framework of its occurrence. The majority of doubts, which results from the obtained research, raises the issue of provision of medical help without the consent of the patient and the right to refuse treatment, but above all, what is quite surprising is the question of accepting gifts from pharmaceutical companies. The most frequently cited reasons influencing the lack of updating legal knowledge in the field of the performed profession were lack of time and a large number of duties, whereas the research group, in order to deepen their knowledge on that matter, most frequently used the Internet resources, industry articles, and on the third place was the training related to the subject matter of the medical law

    Awareness of the role of cardiovascular risk factors and their prevention from the perspective of Tricity adolescents

    Introduction. Arterial hypertension (HT) affects 10.4 million adult Poles, and the blood pressure (BP) control rate is only 26%. Beyond any doubt, high blood pressure results in cardiovascular (CV) target organ damage, which markedly influences national healthcare programs. Childhood introduced cardiovascular prophylaxis offers opportunities to decrease incident HT and delay or even eliminate its consequences. Therefore we have decided to study the level of knowledge on HT and CV disease (CVD) among random 1st and 2nd grade high school students in Tricity, Poland. Material and methods. Questionnaire-based study was conducted voluntarily in two selected high schools of the Tricity agglomeration, Poland, in 2000 and repeated in 2016. All participants were presented with a 38-item questionnaire on cardiovascular risk factors knowledge as well as students’ health habits. Results. Studied group consisted of 615 students (57.6% females) at the age of 16, mean BMI 20.89 ± 2.95 kg/m2 (78% BMI norm). The evaluation of the several habits in two time-points was as follows: additional salt intake was reported by 38.3% vs. 35.1% adolescents in 2000 and 2016, respectively; p = 0.43, dining in fast food restaurants at least once a week: 13.9% vs. 44.8%; 2000/2016; p < 0.01; hours per week spent on physical activity: 5.6 ± 4.4 vs. 5.5 } 4.5; p = 0.85; students who never drank alcohol: 19.7% vs. 31.1%; p < 0.01; cigarettes ever smoking: 39.6% vs. 21.2%; p < 0.01; regular smokers: 10.2% vs. 4.1%; p < 0.05. The awareness of CV-preventive measures such as body weight reduction was presented by 88.1% vs. 93.9% students in 2000 vs. 2016; p = 0.01; regular physical activity 92.6% vs. 97.1%; P = 0.01, limiting smoking and alcohol consumption 84.2% vs. 91.2%; p = 0.01; salt reduction: 62.6% vs. 82.3%; p < 0.001. Interestingly, in 2016 the majority of students identified myocardial infarction as a consequence of AH 92.4%, which was markedly less evident in case of stroke and kidney disease (46.2%, and 28%, respectively). Conclusion. Although the awareness of negative consequences of HT and other CV-risk factors has grown over the past 15 years, the implementation of this knowledge is insufficient among high-school adolescents.

    Amiodarone-induced thyroid dysfunction in the developmental period: prenatally, in childhood, and adolescence — case reports and a review of the literature

    Introduction: Amiodarone is an important antiarrhythmic drug used in paediatric practice, mainly in children with complex congenital cardiac diseases and/or severe arrhythmias. One of the side effects of amiodarone therapy is thyroid dysfunction, which is observed in about 20% of patients. The thyroid dysfunction may present with various forms: from subclinical changes in hormone levels to amiodaroneinduced thyrotoxicosis (AIT) and amiodarone-induced hypothyroidism (AIH). Material and methods: We reported six patients in the age range from two weeks to 14 years, with complex congenital cardiac diseases and severe arrhythmias, who developed amiodarone-induced thyroid dysfunctions: thyrotoxicosis or hypothyroidism or both together. The clinical signs and symptoms of all thyroid dysfunctions were atypical, most patients presented with an aggravation of heart insufficiency. Our patients with thyrotoxicosis were treated with combined therapy including thionamides and corticosteroids due to the presentation of mixed-identified type of AIT. Results: Currently, five patients (one patient’s status is unknown) are in biochemical and clinical euthyreosis; however, in one of them it was impossible to discharge amiodarone treatment. Three of them are still treated with levothyroxine, and two do not need thyroid treatment. Conclusions: Amiodarone-induced thyroid dysfunction is usually atypical; therefore, monitoring of thyroid status before, during, and after amiodarone is demanded. AIH could significantly influence the development of the child, while AIT could significantly deteriorate the clinical status of children with complex cardiac diseases. Early and proper diagnose of AIT and AIH allows the introduction of immediate and appropriate treatment considering the cardiac condition of the young patient

    Extrathyroidal congenital defects in children with congenital hypothyroidism : observations from a single paediatric centre in Central Europe with a review of literature

    Wprowadzenie: Pacjenci z wrodzoną niedoczynnością tarczycy (congenital hypothyroidism - CH) mają większe niż w populacji ogólnej ryzyko wystąpienia innych pozatarczycowych, wrodzonych nieprawidłowości. Nieprawidłowości te dotyczą głównie narządów i układów, w których prawidłowy rozwój i funkcjonowanie zaangażowane są geny odpowiedzialne również za prawidłowy przebieg organogenezy i syntezy hormonów gruczołu tarczowego. Cel pracy: Ocena częstości występowania innych wad towarzyszących CH, z uwzględnieniem możliwych przyczyn tej zależności i roli badań genetycznych. Materiał i metody: Do badania zostały włączone 54 noworodki objęte badaniem przesiewowym w kierunku CH w latach 2010-2017 w województwie małopolskim. Przeprowadzono dokładną retrospektywną analizę dokumentacji medycznej tych pacjentów. Wyniki: W grupie badanej u 20 z 54 pacjentów (37%) stwierdzono obecność współtowarzyszących CH, wrodzonych nieprawidłowości innych narządów. U 10 (18,5%) potwierdzono obecność wady serca, u 5 (9,25%) stwierdzono nieprawidłowe objawy ze strony układu oddechowego, u 7 (12,96%) stwierdzono nieprawidłowe objawy ze strony układu pokarmowego, u 5 (9,25%) rozpoznano nieprawidłowości ze strony układu moczowo-płciowego, u 3 (5,55%) stwierdzono nieprawidłowości ze strony układu nerwowego, u 6 (11,1%) rozpoznano nieprawidłowości ze strony układu mięśniowo-szkieletowego. Wnioski: Analiza uzyskanych danych i aktualnego piśmiennictwa wskazują, że największą rolę w powstawaniu pozatarczycowych nieprawidłowości u pacjentów z CH mogą mieć czynniki genetyczne. Zasadna wydaje się identyfikacja mutacji, która doprowadziła do CH. Znając mutacje mogące odpowiadać za wady współtowarzyszące CH, można by objąć pacjentów z CH odpowiednimi badaniami skriningowymi, mającymi na celu ich wczesne zdiagnozowanie, a w efekcie - odpowiednie, natychmiastowe leczenie.Introduction: Patients with congenital hypothyroidism (CH) can have an increased risk of occurrence of extrathyroidal defects compared to the general population, which could influence their development. The abnormalities occur mainly in organ systems whose development and function is dependent on genes that are also responsible for proper organogenesis of the thyroid gland and thyroid hormone synthesis. Aim of the study: The aim of the study was to evaluate the frequency of extrathyroidal defects in CH patients, taking into consideration the cause of this co-occurrence and the role of genetic tests. Material and methods: The study included 54 newborns with positive screening test for CH based on elevated TSH level, in the years 2010-2017, from South-Eastern Poland. The data was retrieved retrospectively from patients’ medical records. Results: Twenty of 54 newborns with CH (37%) had congenital defects of other organs. In 10 (18.5%) cardiac defects were found, in 5 (9.25%) abnormal symptoms of the respiratory system, 7 (12.96%) had abnormalities of the gastrointestinal system, five (9.25%) had genitourinary abnormalities, 3 (5.55%) had abnormalities of the nervous system, and 6 (11.1%) had musculoskeletal abnormalities. Conclusions: The analysis of our data and current literature suggests that genetic factors play the most important role in the development of extrathyroidal abnormalities in newborns with CH. Identifying the mutation causing CH, the potential defects that can accompany newborns with CH, screening could be offered for these patients in order to obtain an earlier diagnosis and implement early and appropriate treatment
