    Communities of practice in academia

    Up to now, the relationships among the fundamental notions of communities of practice (CoPs), i.e. knowledge, participation, identity, and artefact development have been based mainly on results from qualitative studies; they are not yet sufficiently based on quantitative evidence. Starting from a literature review, we formulate a quantitative, causal model of CoPs that describes these variables in the context of academic communities, and aim to validate this model in two academic CoPs with a total of N = 208 participants. A cluster analysis classifies the participants into clusters that are in line with the core-periphery structure known from previous qualitative studies. A regression analysis provides evidence for the hypothesized model on the basis of quantitative data. Suggested directions for future research are to focus on factors that determine CoP participants’ contributions to artefact development and on approaches to automated monitoring of virtual CoPs

    Social and municipal influences on electric vehicle purchases

    Growth in electric vehicle ownership in Canada has been slow relative to policy imperatives to reduce greenhouse gas (GHG) emissions. According to the 2014 IPCC report the global transportation sector was responsible for approximately 23% of all energy related CO2 emissions - second only to the energy supply sector. Further, transportation-related GHG emissions have risen despite growing availability of more efficient modes of transportation (IPCC, 2014). In Ontario, where there is an emphasis on renewable energy production, the transportation sector is the largest emitter of GHGs at 35% (Ontario Government, 2016). We conducted our case study in London, Ontario where vehicle ownership has grown 40% from 2010 – 2018 while combined ownership of plug in hybrid electric vehicles (PHEV) and plug in electric vehicles (EV) grew only 2.8% (City of London, 2019). While existing research on hybrid and EV purchasing focuses mainly on market-level economic models, few studies investigate how either social influence or municipal intervention impact electric vehicle purchasing behaviour. This study uses a survey of 257 Londoners, randomly selected from four different postal codes to determine if their likelihood to purchase a hybrid or EV is influenced by social (e.g. talking with family and friends) and municipal factors (e.g., municipally sponsored test drive events, preferential parking). Those factors were included in a model with several other predictors of electric vehicle purchasing behaviour including: cost/financial concerns, environmental concerns, and sociodemographic factors. As hypothesized both social and municipal variables were significantly associated with likelihood to purchase a hybrid or EV as expected. There are two key implications. First, those interested in promoting hybrid/EV should consider messaging that centers on current owners of hybrid/EVs interacting with family and friends. Second, cities should consider their role in providing relatively low-cost incentives and promotion for hybrid or EV. Future research might include direct municipal interventions meant to increase hybrid/EV uptake along with measurement of the impacts using before and after questionnaire

    Analysis of surgical options for patients with bilateral carpal tunnel syndrome.

    Background: Bilateral endoscopic carpal tunnel release is a modality offered for the treatment of the median nerve compression neuropathy. This retrospective study compares outcomes for patients undergoing open carpal tunnel release versus bilateral endoscopic carpal tunnel release. We hypothesized that there is no significant difference in postoperative complication rates between unilateral open and bilateral endoscopic carpal tunnel release surgery. Methods: The authors identified all patients who underwent open carpal tunnel release, unilateral endoscopic carpal tunnel release and bilateral endoscopic carpal tunnel release at a university hospital from 2012 to 2014. Cases were identified using CPT billing codes and the data was assessed using an analysis of variance (ANOVA). All endoscopic carpal tunnel releases were done by the same surgeon (AP), and greater than 90% of open procedures were done by a different same surgeon (DF). Results: The total combined complication rate was 24.7% with no significant difference (p \u3e .05) between techniques. There were no major complications necessitating a return to the operating room. Variables that had a statistically significant difference between groups (p \u3c .05) included mean tourniquet time, mean total procedure time, and return to work as determined from the number of follow-up appointments. Conclusions: The study demonstrates equivocal complication profiles and decreased cost associated with bilateral endoscopic tunnel release as compared to sequential open carpal tunnel release. Endoscopic bilateral carpal tunnel release for patients with bilateral carpal tunnel syndrome offers a safe and effective alternative to open carpal tunnel release

    Products of Vitamin D3 or 7-Dehydrocholesterol Metabolism by Cytochrome P450scc Show Anti-Leukemia Effects, Having Low or Absent Calcemic Activity

    BACKGROUND. Cytochrome P450scc metabolizes vitamin D3 to 20-hydroxyvitamin D3 (20(OH)D3) and 20,23(OH)2D3, as well as 1-hydroxyvitamin D3 to 1a,20-dihydroxyvitamin D3 (1,20(OH)2D3). It also cleaves the side chain of 7-dehydrocholesterol producing 7-dehydropregnenolone (7DHP), which can be transformed to 20(OH)7DHP. UVB induces transformation of the steroidal 5,7-dienes to pregnacalciferol (pD) and a lumisterol-like compounds (pL). METHODS AND FINDINGS. To define the biological significance of these P450scc-initiated pathways, we tested the effects of their 5,7-diene precursors and secosteroidal products on leukemia cell differentiation and proliferation in comparison to 1a,25-dihydroxyvitamin D3 (1,25(OH)2D3). These secosteroids inhibited proliferation and induced erythroid differentiation of K562 human chronic myeloid and MEL mouse leukemia cells with 20(OH)D3 and 20,23(OH)2D3 being either equipotent or slightly less potent than 1,25(OH)2D3, while 1,20(OH)2D3, pD and pL compounds were slightly or moderately less potent. The compounds also inhibited proliferation and induced monocytic differentiation of HL-60 promyelocytic and U937 promonocytic human leukemia cells. Among them 1,25(OH)2D3 was the most potent, 20(OH)D3, 20,23(OH)2D3 and 1,20(OH)2D3 were less active, and pD and pL compounds were the least potent. Since it had been previously proven that secosteroids without the side chain (pD) have no effect on systemic calcium levels we performed additional testing in rats and found that 20(OH)D3 had no calcemic activity at concentration as high as 1 µg/kg, whereas, 1,20(OH)2D3 was slightly to moderately calcemic and 1,25(OH)2D3 had strong calcemic activity. CONCLUSIONS. We identified novel secosteroids that are excellent candidates for anti-leukemia therapy with 20(OH)D3 deserving special attention because of its relatively high potency and lack of calcemic activity.National Institutes of Health (R01A052190

    2005 Revised Omaha Economic Development Plan: Plan for Northeast Omaha

    The 2005 Revised OEDP provides OEDC with current data and analysis to initiate and complete a strategic process to develop an action plan for community and economic development activities and projects. The 2005 Revised Overall Economic Development Plan (OEDP) prepared by the Center for Public Affairs Research (CPAR) at the University of Nebraska at Omaha (UNO) for the Omaha Economic Development Corporation (OEDC) replaces the original OEDP first prepared in 1978. While a variety of revisions and updates over the years helped keep the original OEDP relatively current and helpful in development activities, in 2005 OEDC President Alvin Goodwin commissioned CPAR to provide a new basic planning document for the organization. Working with OEDC staff, and using the original OEDP as a framework, faculty and staff from CPAR, with the assistance of UNO graduate students, collected and analyzed data for the 2005 Revised Overall Economic Development Plan. The Revised OEDP retains and updates several of the sections from the original Plan but also adds new sections. Overall economic development plans contain a wealth of information about the demographic, social, and economic characteristics of the geographic area targeted for development by a community development corporation (CDC). CDCs, like the Omaha Economic Development Corporation, engage in many and varied community and economic development projects in its service area, and the overall economic development plan provides needed information to implement these projects. For example, the OEDP helps the community development corporation identify community needs as well as opportunities for business and economic development. Data from the OEDP can also be utilized for marketing and promotional purposes to attract new firms and customers. The OEDP serves as a source of information for formulating broad, long-term organizational goals, as well as specific, short-term development strategies and actions. Finally, the OEDP helps coordinate development activities, plans, and projects among the range of other organizations working in or adjacent to the target area

    Prediction of Pathological Complete Response to Neoadjuvant Chemotherapy for Primary Breast Cancer Comparing Interim Ultrasound, Shear Wave Elastography and MRI

    Abstract Background Prediction of pathological complete response (pCR) of primary breast cancer to neoadjuvant chemotherapy (NACT) may influence planned surgical approaches in the breast and axilla. The aim of this project is to assess the value of interim shear wave elastography (SWE), ultrasound (US) and magnetic resonance imaging (MRI) after 3 cycles in predicting pCR. Methods 64 patients receiving NACT had baseline and interim US, SWE and MRI examinations. The mean lesion stiffness at SWE, US and MRI diameter was measured at both time points. We compared four parameters with pCR status: a) Interim mean stiffness ≤ or &gt; 50 kPa; b) Percentage stiffness reduction; c) Percentage US diameter reduction and d) Interim MRI response using RECIST criteria. The Chi square test was used to assess significance. Results Interim stiffness of ≤ or &gt; 50 kPa gave the best prediction of pCR with pCR seen in 10 of 14 (71 %) cancers with an interim stiffness of ≤ 50 kPa, compared to 7 of 50 (14 %) of cancers with an interim stiffness of &gt; 50 kPa, (p &lt; 0.0001) (sensitivity 59 %, specificity 91 %, PPV 71 %, NPV 86 % and diagnostic accuracy 83 %). Percentage reduction in stiffness was the next best parameter (sensitivity 59 %, specificity 85 %, p &lt; 0.0004) followed by reduction in MRI diameter of &gt; 30 % (sensitivity 50 % and specificity 79 %, p = 0.03) and % reduction in US diameter (sensitivity 47 %, specificity 81 %, p = 0.03). Similar results were obtained from ROC analysis. Conclusion SWE stiffness of breast cancers after 3 cycles of NACT and changes in stiffness from baseline are strongly associated with pCR after 6 cycles.</jats:p

    Estimating the Health Effects of Adding Bicycle and Pedestrian Paths at the Census Tract Level: Multiple Model Comparison

    Background: Adding additional bicycle and pedestrian paths to an area can lead to improved health outcomes for residents over time. However, quantitatively determining which areas benefit more from bicycle and pedestrian paths, how many miles of bicycle and pedestrian paths are needed, and the health outcomes that may be most improved remain open questions. Objective: Our work provides and evaluates a methodology that offers actionable insight for city-level planners, public health officials, and decision makers tasked with the question “To what extent will adding specified bicycle and pedestrian path mileage to a census tract improve residents’ health outcomes over time?” Methods: We conducted a factor analysis of data from the American Community Survey, Center for Disease Control 500 Cities project, Strava, and bicycle and pedestrian path location and use data from two different cities (Norfolk, Virginia, and San Francisco, California). We constructed 2 city-specific factor models and used an algorithm to predict the expected mean improvement that a specified number of bicycle and pedestrian path miles contributes to the identified health outcomes. Results: We show that given a factor model constructed from data from 2011 to 2015, the number of additional bicycle and pedestrian path miles in 2016, and a specific census tract, our models forecast health outcome improvements in 2020 more accurately than 2 alternative approaches for both Norfolk, Virginia, and San Francisco, California. Furthermore, for each city, we show that the additional accuracy is a statistically significant improvement (P2 weeks of poor physical health days in the census tract within 1.83% (SD 0.57%). For San Francisco (n=49 census tracts), our approach estimates, on average, that the percentage of individuals who had a stroke in the census tract is within 1.81% (SD 0.52%), and the percentage of individuals with diabetes in the census tract is within 1.26% (SD 0.91%). Conclusions: We propose and evaluate a methodology to enable decision makers to weigh the extent to which 2 bicycle and pedestrian paths of equal cost, which were proposed in different census tracts, improve residents’ health outcomes; identify areas where bicycle and pedestrian paths are unlikely to be effective interventions and other strategies should be used; and quantify the minimum amount of additional bicycle path miles needed to maximize health outcome improvements. Our methodology shows statistically significant improvements, compared with alternative approaches, in historical accuracy for 2 large cities (for 2016) within different geographic areas and with different demographics
