    Geometrically Induced Gauge Structure on Manifolds Embedded in a Higher Dimensional Space

    We explain in a context different from that of Maraner the formalism for describing motion of a particle, under the influence of a confining potential, in a neighbourhood of an n-dimensional curved manifold M^n embedded in a p-dimensional Euclidean space R^p with p >= n+2. The effective Hamiltonian on M^n has a (generally non-Abelian) gauge structure determined by geometry of M^n. Such a gauge term is defined in terms of the vectors normal to M^n, and its connection is called the N-connection. In order to see the global effect of this type of connections, the case of M^1 embedded in R^3 is examined, where the relation of an integral of the gauge potential of the N-connection (i.e., the torsion) along a path in M^1 to the Berry's phase is given through Gauss mapping of the vector tangent to M^1. Through the same mapping in the case of M^1 embedded in R^p, where the normal and the tangent quantities are exchanged, the relation of the N-connection to the induced gauge potential on the (p-1)-dimensional sphere S^{p-1} (p >= 3) found by Ohnuki and Kitakado is concretely established. Further, this latter which has the monopole-like structure is also proved to be gauge-equivalent to the spin-connection of S^{p-1}. Finally, by extending formally the fundamental equations for M^n to infinite dimensional case, the present formalism is applied to the field theory that admits a soliton solution.     X-Ray Diffuse Scattering Study on Ionic-Pair Displacement Correlations in Relaxor Lead Magnesium Niobate

    Ionic-pair equal-time displacement correlations in relaxor lead magnesium niobate, Pb(Mg1/3Nb2/3)O3Pb(Mg_{1/3}Nb_{2/3})O_{3}, have been investigated at room temperature in terms of an x-ray diffuse scattering technique. Functions of the distinct correlations have been determined quantitatively. The results show the significantly strong rhombohedral-polar correlations regarding Pb-O, Mg/Nb-O, and O-O' pairs. Their spatial distribution forms an ellipse or a sphere with the radii of 30-80AËš\AA. This observation of local structure in the system proves precursory presence of the polar microregions in the paraelectric state which leads to the dielectric dispersion.Comment: 11 pages, 3 figure

    Perceptions and attitudes of users and non-users of mental health services concerning mental illness and services in Japan

    ObjectivesThere is a global movement to develop and implement community-based integrated mental health systems. The present study attempted to clarify the perceptions and attitudes of users and non-users of mental health services concerning mental illness and services in Japan.MethodsA new questionnaire was developed for this internet survey. Data from 500 outpatients with depression and 500 healthy subjects were sampled according to the demographics of the Japanese population.ResultsOver 90% of healthy subjects and over 70% of patients were unaware of the common age of onset or lifetime prevalence of mental illness. Over 90% of the healthy subjects and about 70% of the patients could not describe any services where they would feel comfortable discussing mental health problems. In both groups, “adolescents and young adults” were ranked first as a target population for mental health and illness policies. The top requirement for the integrated care systems was the promotion and awareness of correct knowledge of mental illness in both the healthy subjects and patients.ConclusionSocietal requirements could include disseminating correct knowledge, awareness-raising actions for society, and implementing services where people, especially young people, can easily consult and receive support in the community

    EOS789, a broad-spectrum inhibitor of phosphate transport, is safe with an indication of efficacy in a phase 1b randomized crossover trial in hemodialysis patients

    The treatment of hyperphosphatemia remains challenging in patients receiving hemodialysis. This phase 1b study assessed safety and efficacy of EOS789, a novel pan-inhibitor of phosphate transport (NaPi-2b, PiT-1, PiT-2) on intestinal phosphate absorption in patients receiving intermittent hemodialysis therapy. Two cross-over, randomized order studies of identical design (ten patients each) compared daily EOS789 50 mg to placebo with meals and daily EOS789 100 mg vs EOS789 100 mg plus 1600 mg sevelamer with meals. Patients ate a controlled diet of 900 mg phosphate daily for two weeks and began EOS789 on day four. On day ten, a phosphate absorption testing protocol was performed during the intradialytic period. Intestinal fractional phosphate absorption was determined by kinetic modeling of serum data following oral and intravenous doses of 33Phosphate ( 33P). The results demonstrated no study drug related serious adverse events. Fractional phosphate absorption was 0.53 (95% confidence interval: 0.39,0.67) for placebo vs. 0.49 (0.35,0.63) for 50 mg EOS789; and 0.40 (0.29,0.50) for 100 mg EOS789 vs. 0.36 (0.26,0.47) for 100 mg EOS789 plus 1600 mg sevelamer (all not significantly different). The fractional phosphate absorption trended lower in six patients who completed both studies with EOS789 100 mg compared with placebo. Thus, in this phase 1b study, EOS789 was safe and well tolerated. Importantly, the use of 33P as a sensitive and direct measure of intestinal phosphate absorption allows specific testing of drug efficacy. The effectiveness of EOS789 needs to be evaluated in future phase 2 and phase 3 studies

    Isolation and characterization of the TIGA genes, whose transcripts are induced by growth arrest

    We report here the isolation of 44 genes that are upregulated after serum starvation and/or contact inhibition. These genes have been termed TIGA, after Transcript Induced by Growth Arrest. We found that there are two kinds of G0 phases caused by serum starvation, namely, the shallow G0 (or G0/G1) and the deep G0 phases. The shallow G0 is induced by only a few hours of serum starvation, while deep G0 is generated after 3 days of serum starvation. We propose that mammalian cells enter deep G0 through a G0 gate, which is only opened on the third day of serum starvation. TIGA1, one of the uncharacterized TIGA genes, encodes a homolog of cyanate permease of bacteria and localizes in mitochondria. This suggests that Tiga1 is involved in the inorganic ion transport and metabolism needed to maintain the deep G0 phase. Ectopic expression of TIGA1 inhibited not only tumor cell proliferation but also anchorage-independent growth of cancer cell lines. A microsatellite marker, ENDL-1, allowed us to detect loss of heterozygosity around the TIGA1 gene region (5q21–22). Further analysis of the TIGA genes we have identified here may help us to better understand the mechanisms that regulate the G0 phase

    Relationship between the Arthroscopic Findings and Pathology of Greater Trochanteric Pain Syndrome

    In recent publications on greater trochanteric pain syndrome (GTPS), the pathology receiving the most attention has been gluteus medius muscle tendinous injury, and surgical techniques such as gluteus medius tendon repair and their outcomes for GTPS have been reported. In our department-related facilities, arthroscopic surgeries are routinely performed for the patients with recalcitrant GTPS. A total of 51 patients were diagnosed with GTPS. Surgical treatment was carried out 22 patients (24 joints; 4 males and 18 females; mean age at surgery of 52.0 years). Arthroscopic findings confirmed bursitis in all 24 joints. In all cases, debridement of the greater trochanter bursa provided rapid relief of greater trochanter pain. The Numerical Rating Scale showed significant improvement, from the preoperative mean of 7.8 (range, 6-10) to the postoperative day 7 mean of 1.6 (range, 0-3). The modified Harris Hip Score was significantly improved from the preoperative mean of 65.5 (range, 52.5-78.3) to the final follow-up (average 2.9 months) mean of 96.0 (range, 85.2-100). Fascial damage of the gluteus medius muscle was observed in 21 joints while only 2 patients had a gluteus medius tendinous injury. Greater trochanteric bursitis and fascia or muscle-fiber injury of the gluteus medius muscle are the most common pathologies in patients with lateral hip pain

    Characteristics of adolescent athletes with pain during sports activities: evaluation of flexibility using ultrasound elastography

    Medical checkups for adolescent athletes aim to prevent or detect sports injuries and manage them before they hamper future sports activities and daily life. However, effective items for evaluation, as well as the areas of intervention to prevent sports injuries, remain unclear. We aimed to clarify the checkup items and intervention areas to prevent sports injuries in adolescent athletes. This is a cross-sectional observational study that investigated the presence or absence of pain in adolescent athletes and associated factors at the time of checkup. We investigated joint laxity, range of motion (ROM), finger-floor distance (FFD), heel–buttock distance (HBD), straight leg raising (SLR) angle, too many toes sign, and presence or absence of low back pain during lumbar extension in 301 junior high school athletes. Additionally, after confirming the developmental stage of the tibial tuberosity using ultrasonography, ultrasound elastography was used to quantify the elasticity of the quadriceps femoris, and items related to pain during sports activities were extracted. Items related to pain included the too many toes sign, low back pain during lumbar extension, and elasticity of the quadriceps femoris. Athletes with a positive too many toes sign were less likely to experience pain; conversely, the harder the quadriceps femoris, the more likely they were to experience pain. Medical checkups focusing on these items are effective for adolescent athletes who are prone to knee pain
