    Training Hospital Readiness in Speech-Language Pathology Students through Simulation

    Simulated learning environments allow students to develop technical and clinical decision-making skills in a safe and realistic setting. The aim of this study was to evaluate speech-language pathology students’ perception of hospital readiness following a one-day simulation-based training day on swallowing management. Nineteen students attended the training day. Training included part-task skill learning and immersive simulated scenarios. Students were asked to complete course evaluation forms and participated in focus groups immediately after the day. Seven students participated in a further focus group after a five-week hospital placement within a month of the training day. Four students participated in a focus group after a five-week hospital placement three months after the training day. The training day was positively accepted by all students. Analyses revealed three global themes: (1) preparation for hospital environment, (2) speech-language pathology skills, and (3) impact of simulated learning environments. Students directly attributed increased confidence in working in the hospital environment and increased clinical competency to the training day. These themes continued up to three months post training. Simulated learning environments may have long lasting benefits in developing hospital readiness in speech-language pathology students

    A prática de mindfulness (MBSR) como dispositivo terapêutico no tratamento da depressão em pacientes HIV-positivo: resultados de um estudo-piloto

    This pilot-study investigated the usefulness of a mindfulness-based stress reduction (MBSR) 8-week programme for HIV-positive individ­uals experiencing mild-moderate symptoms of depression. A total of 17 participants, aged 18-65 were recruited. This study used a mixed research design (One-way repeated measures; thematic analysis). Outcome mea­sures included the Hospital Anxiety Depression Scale (depression sub-scale) assessing severity of depressive symptoms and the Acceptance and Action Questionnaire (AAQ-II) assessing levels of psychological flexibility. In addition participants were asked to share experiences of how treatment had affected their ability to manage symptoms of depres­sion and HIV-diagnosis (acceptance, control, coping and relationships were amongst the themes extracted). Results suggested that there was significant reduction in depressive symptoms between post-treatment and all other three conditions (beginning of treatment, baseline one and baseline two). Similarly a significant increase in psychological flexibil­ity was reported at post-treatment phase. The theoretical, clinical and research implications are discussed.Este estudo-piloto investigou a utilidade do Programa Mindfulness de Redução de Estresse (MBSR) de 8 semanas em 17 participantes HIV­-positivos apresentando sintomas de depressão leve à moderada. Foram utilizados questionários para avaliar os sintomas de depressão (escala HADS) e flexibilidade psicológica (AAQ-II), bem como uma análise qualitativa de como o tratamento afetou a capacidade dessas pessoas em lidar com sintomas de depressão e o diagnóstico de HIV. Os resultados sugerem que houve uma redução significativa dos sintomas de depressão entre pós-tratamento, início de tratamento e fase de espera. Do mesmo modo houve um aumento significativo na flexibilidade psicológica após tratamento. Os resultados desse estudo indicam que o Programa Min­dfulness de Redução de Estresse é altamente eficaz no âmbito clínico, oferecendo novas perspectivas terapêuticas no tratamento da depressão para portadores de HIV. Discutem-se os processos cognitivos en­volvidos na prática de mindfulness que mediam essas mudanças nos pacientes, e são oferecidas recomendações de como integrar práticas de mindfulness dentro do contexto terapêuti­co da clínica para atendimento a pessoas que vivem com HIV/ AIDS

    Speaking up in Healthcare: An Exploration of the Allied Health New Graduate Workforce

    Introduction: All healthcare workers are responsible for patient safety and quality improvement and need to “speak up” to communicate issues. As healthcare systems strain under the impact of reduced staffing and workloads increase, allied health new graduates are feeling under pressure and unsupported. Understanding their experiences of speaking up as they transition into the workforce will identify what support they require to fulfil their patient safety and quality improvement responsibilities. Method: An exploratory study was conducted to investigate how new graduates in allied health speak up. Informed by a realist theoretical position, this study was interested in what contexts and resources support new graduates to speak up or not. Two different focus groups with allied health new graduates were used to collect data. Reflexive thematic analysis was employed to draw out key themes and sub-themes. Results: Three main themes were generated –advocacy drives speaking up, scaffolding, and transition impact. Further analysis identified context-mechanism-outcome configurations which were then developed into an initial programme theory. Conclusion: Further in-depth exploration of speaking up behaviour with allied health new graduates will inform leaders within education and workplace settings about ways to develop confident and competent professionals who can speak up for patient safety and quality improvement

    Os efeitos de intervenções baseadas em mindfulness em tempos da COVID-19: uma revisão sistemática

    Introduction: the public health crisis caused by COVID-19, a disease caused by SARS-CoV-2, has imposed physical threats and psychological suffering on both infected patients and individuals who experience social isolation and various governmental restrictions, leading to the appearance of symptoms like anxiety, depression, as well as insomnia, stress, and changes in biological rhythm. In face of this stressful scenario, interventions based on mindfulness (MBIs) have proved to be potentially adequate tools in reducing psychological suffering and generating well-being in the general population. Objective: to describe the effects of mindfulness-based interventions during the COVID-19 pandemic. Methods: A systematic literature review was conducted on the effects of mindfulness intervention in times of COVID-19. The articles were searched in four databases (Pubmed, Embase, Scopus, and Science direct) and the PRISMA protocol was used to conduct this review. In total, fourteen articles were included in the study. Results: the use of mindfulness techniques in the population with impaired mental health because of the COVID-19 pandemic proved to be beneficial, with improvement in emotional stress scores and reduction in anxiety symptoms, through formal mindfulness meditation practices such as mindful breathing, body scanning, and application of the mindfulness-based stress reduction strategy (MBSR). Strategies were also applied through smartphone applications that had the objective of promoting the increase of mindfulness and the development of the acceptance without judgment of the traumatic experiences already lived, in addition to an integrated intervention on the internet with the use of mindfulness, the training of respiratory relaxation, refuge skills and Butterfly Hug Method. Conclusion: with the pandemic going through, still with high mortality, continuity of social isolation, and, consequently, intensification of psychic suffering in the population and health professionals, interventions based on mindfulness (MBIs) are being effective to reduce this suffering, prevent the appearance of of chronic mental disorders and promote positive impacts on physical and mental well-being.Introdução: a crise de saúde pública ocasionada pela COVID-19, doença causada pelo SARS-CoV-2, impôs ameaças físicas e sofrimento psíquico tanto aos pacientes infectados quanto aos indivíduos que vivenciam o isolamento social e as diversas restrições governamentais, propiciando o aparecimento de sintomas de ansiedade, depressão, além de insônia, estresse e alterações do ritmo biológico. Diante desse cenário estressor, as intervenções baseadas em atenção plena (MBIs), se mostraram ferramentas potencialmente adequadas na redução do sofrimento psicológico e na geração de bem-estar da população em geral. Objetivo: descrever os efeitos das intervenções baseadas em mindfulness em tempos da COVID-19. Método: foi realizada uma revisão sistemática da literatura sobre os efeitos da intervenção mindfulness em tempos da COVID-19. Os artigos foram pesquisados em quatro bases de dados (Pubmed, Embase, Scopus e Science direct) e o protocolo PRISMA foi utilizado para realização desta revisão. No total, quatorze artigos foram incluídos no estudo. Resultados: a utilização de técnicas de mindfulness na população com prejuízos na saúde mental em consequência da pandemia da COVID-19 mostraram-se benéficas, ocorrendo melhora no escore de estresse emocional e redução dos sintomas de ansiedade, através de práticas formais de meditação tipo mindfulness, como respiração consciente, escaneamento corporal, e aplicação da estratégia de redução do estresse baseada em atenção plena (MBSR). Estratégias também foram aplicadas através de aplicativos de smartphone que tiveram o objetivo de promover o aumento da atenção plena e o desenvolvimento da aceitação, sem julgamentos, das experiências traumáticas já vividas, além de intervenção integrada na internet com o uso da atenção plena, treinamento de relaxamento respiratório, habilidades de refúgio e método do abraço de borboleta. Conclusão: com o perpassar da pandemia, ainda com elevada mortalidade, continuidade dos isolamentos sociais, e, consequentemente, intensificação do sofrimento psíquico na população e profissionais de saúde, as intervenções baseadas em atenção plena (MBIs) estão sendo eficazes para diminuir esse sofrimento, prevenir aparecimento de transtornos mentais crônicos e promover impactos positivos no bem-estar físico e mental
