    Reversed cortical over-activity during movement imagination following neurofeedback treatment for central neuropathic pain

    Objective: One of the brain signatures of the central neuropathic pain (CNP) is the theta band over-activity of wider cortical structures, during imagination of movement. The objective of the study was to investigate whether this over-activity is reversible following the neurofeedback treatment of CNP. Methods: Five paraplegic patients with pain in their legs underwent from twenty to forty neurofeedback sessions that significantly reduced their pain. In order to assess their dynamic cortical activity they were asked to imagine movements of all limbs a week before the first and a week after the last neurofeedback session. Using time–frequency analysis we compared EEG activity during imagination of movement before and after the therapy and further compared it with EEG signals of ten paraplegic patients with no pain and a control group of ten able-bodied people. Results: Neurofeedback treatment resulted in reduced CNP and a wide spread reduction of cortical activity during imagination of movement. The reduction was significant in the alpha and beta band but was largest in the theta band. As a result cortical activity became similar to the activity of other two groups with no pain. Conclusions: Reduction of CNP is accompanied by reduced cortical over-activity during movement imagination.     Prevalence and predictors of postdischarge antibiotic use following mastectomy

    OBJECTIVESurvey results suggest that prolonged administration of prophylactic antibiotics is common after mastectomy with reconstruction. We determined utilization, predictors, and outcomes of postdischarge prophylactic antibiotics after mastectomy with or without immediate breast reconstruction.DESIGNRetrospective cohort.PATIENTSCommercially insured women aged 18–64 years coded for mastectomy from January 2004 to December 2011 were included in the study. Women with a preexisting wound complication or septicemia were excluded.METHODSPredictors of prophylactic antibiotics within 5 days after discharge were identified in women with 1 year of prior insurance enrollment; relative risks (RR) were calculated using generalized estimating equations.RESULTSOverall, 12,501 mastectomy procedures were identified; immediate reconstruction was performed in 7,912 of these procedures (63.3%). Postdischarge prophylactic antibiotics were used in 4,439 procedures (56.1%) with immediate reconstruction and 1,053 procedures (22.9%) without immediate reconstruction (P&lt;.001). The antibiotics most commonly prescribed were cephalosporins (75.1%) and fluoroquinolones (11.1%). Independent predictors of postdischarge antibiotics were implant reconstruction (RR, 2.41; 95% confidence interval [CI], 2.23–2.60), autologous reconstruction (RR, 2.17; 95% CI, 1.93–2.45), autologous reconstruction plus implant (RR, 2.11; 95% CI, 1.92–2.31), hypertension (RR, 1.05; 95% CI, 1.00–1.10), tobacco use (RR, 1.07; 95% CI, 1.01–1.14), surgery at an academic hospital (RR, 1.14; 95% CI, 1.07–1.21), and receipt of home health care (RR, 1.11; 95% CI, 1.04–1.18). Postdischarge prophylactic antibiotics were not associated with SSI after mastectomy with or without immediate reconstruction (bothP&gt;.05).CONCLUSIONSProphylactic postdischarge antibiotics are commonly prescribed after mastectomy; immediate reconstruction is the strongest predictor. Stewardship efforts in this population to limit continuation of prophylactic antibiotics after discharge are needed to limit antimicrobial resistance.Infect Control Hosp Epidemiol2017;38:1048–1054</jats:sec

    Interface characteristics in an {\alpha}+{\beta} titanium alloy

    The alpha/beta interface in Ti-6Al-2Sn-4Zr-6Mo (Ti-6246) is investigated via centre of symmetry analysis, both as-grown and after 10% cold work. Semi-coherent interface steps are observed at a spacing of 4.5 +/-1.13 atoms in the as-grown condition, in good agreement with theory prediction (4.37 atoms). Lattice accommodation is observed, with elongation along [-1 2 -1 0]alpha and contraction along [1 0 -1 0]alpha . Deformed alpha exhibited larger, less coherent steps with slip bands lying in {110}beta. This indicates dislocation pile-up at the grain boundary, a precursor to globularisation, offering insight into the effect of deformation processing on the interface, which is important for titanium alloy processing route design.Comment: Revised after revie

    Mitofusins Mfn1 and Mfn2 coordinately regulate mitochondrial fusion and are essential for embryonic development

    Mitochondrial morphology is determined by a dynamic equilibrium between organelle fusion and fission, but the significance of these processes in vertebrates is unknown. The mitofusins, Mfn1 and Mfn2, have been shown to affect mitochondrial morphology when overexpressed. We find that mice deficient in either Mfn1 or Mfn2 die in midgestation. However, whereas Mfn2 mutant embryos have a specific and severe disruption of the placental trophoblast giant cell layer, Mfn1-deficient giant cells are normal. Embryonic fibroblasts lacking Mfn1 or Mfn2 display distinct types of fragmented mitochondria, a phenotype we determine to be due to a severe reduction in mitochondrial fusion. Moreover, we find that Mfn1 and Mfn2 form homotypic and heterotypic complexes and show, by rescue of mutant cells, that the homotypic complexes are functional for fusion. We conclude that Mfn1 and Mfn2 have both redundant and distinct functions and act in three separate molecular complexes to promote mitochondrial fusion. Strikingly, a subset of mitochondria in mutant cells lose membrane potential. Therefore, mitochondrial fusion is essential for embryonic development, and by enabling cooperation between mitochondria, has protective effects on the mitochondrial population

    Consumption of Solid Food by Suckling Pigs: Individual Variation and Relation to Weight Gain

    Individual daily consumption of supplementary solid food (\u27creep feed\u27) was measured from Day 10 to weaning at Day 28 for 39 piglets in four litters, and its relationship to body weight and weight gain up to Day 42 was investigated. Individual consumption was measured by combining the weight of the feed removed from the dispensers (monitored electronically) and a video image of piglet activity at the feeder. Creep feed consumption varied greatly, both between and within litters. On average, pigs began feeding on Day 12 (range Day 10-28), intake was relatively low (usually \u3c 5 g day β€’1) until Day 20 but increased considerably in the week before weaning, with a mean intake of 63 g day β€’1 (range 2-205 g day β€’1) during that week. Over the entire creep-feeding period, total feed consumption ranged from 13-1911 g per pig. Within litters, intake was positively correlated with birth weight (P \u3c 0.05) and the correlation with weight gains to Day 20 tended to be positive rather than negative. This suggests that greater creep feed intake was typical of the larger and more mature piglets, rather than serving as compensation for poor milk intake among the more deprived litter-mates. However, one exceptional pig began, on Day 14 after several days of weight loss, to eat more creep feed than any other piglet studied, suggesting that compensatory creep feeding can occur at a young age in exceptional cases. In a multiple regression analysis, creep feed intake accounted for 37% of the variation in weight gain in the week before weaning (P \u3c 0.001) and 7% of the variation in gain from Day 10 to weaning (P \u3c 0.01) after variation attributable to antecedent variables had been taken into account. Within-litter differences in weight gain during the 2 weeks after weaning were correlated with weight at birth and weight gain before weaning (P \u3c 0.05), but not with pre-weaning creep feed intake. Hence, creep feed intake appeared to contribute to pre-weaning gains and these in turn were correlated with post-weaning gains; however, a more direct effect of pre-weaning creep feed intake on post-weaning gain could not be detected

    The Relationship between Creep Feeding Behavior of Piglets and Adaptation to Weaning: Effect of Diet Quality

    Individual variation in creep feed intake and its relation to adaptation to weaning were studied in piglets weaned at 4 wk of age. The animals received either a low-complexity creep-starter diet based on corn, barley and soybean meal (12 litters), or a high-complexity , medicated, commercial diet without soybean meal (12 litters). Diets were fed as creep feed during the 2 wk before weaning, and as the sole diet during the 2 wk after weaning. Creep feeding behavior of piglets was monitored by video recording. Pigs fed the high-complexity diet consumed more creep feed (P \u3c 0.05), tended to gain more during the week before weaning (P \u3c 0.10), and converted feed more efficiently and gained more weight in the 2 wk after weaning (P \u3c 0.01). Use of creep feed varied greatly among individual littermates. Multiple regression analysis showed that on the high-complexity diet, pigs that used creep feed more than their littermates tended to be those with low gains in weeks 1-3 after birth (P \u3c 0.001), and tended to gain more weight during the week before and during the 2 wk after weaning (P \u3c 0.01). The trends were consistent but weaker with the low-complexity diet. However, predictive power was low, with creep feeding accounting for only 4% of individual variation in post-weaning gain on the high-complexity diet and 1% on the low. Regardless of diet quality, therefore, creep feeding remained highly variable and only weakly related to weight gains during the 2 wk after weaning

    Attitudes of Canadian Pig Producers Toward Animal Welfare

    As part of a larger study eliciting Canadian producer and non-producer views about animal welfare, open-ended, semi-structured interviews were used to explore opinions about animal welfare of 20 Canadian pig producers, most of whom were involved in confinement-based systems. With the exception of the one organic producer, who emphasized the importance of a β€˜β€˜natural’’ life, participants attached overriding importance to biological health and functioning. They saw their efforts as providing pigs with dry, thermally regulated, indoor environments where animals received abundant feed, careful monitoring and where prospective disease outbreaks could be minimized and controlled. Emphasis was also placed on low-stress handling and agreeable working conditions which were believed to promote good animal care. The fact that pigs tend to respond to such conditions with steady growth reinforced the belief that good welfare was provided. Participants supported the use of sow gestation stalls, but with some reservations, and expressed concern about welfare problems that could occur if sows were grouped. Invasive procedures (castration, tail-docking, teeth clipping) were recognized as painful but were accepted because they were seen as: (1) necessary for sales or management; (2) satisfactory trade-offs to prevent worse welfare problems such as injury or infection; or (3) sufficiently short-term to be relatively unimportant. Participants were adamantly opposed to animal neglect and some welcomed actions of animal protectionists that expose poor care. Producers also welcomed natural-science-based approaches to improving animal welfare. The findings contribute to a broader effort to identify overlapping values among different stakeholder groups as a basis for formulating mutually agreeable, farm animal care and handling polices

    Sow Preference for Farrowing-Crate Width

    A preference-testing apparatus was designed to provide sows with continuous access to three farrowing crates of different widths. The crates radiated from a central hub area sufficiently large for sows to enter or leave any crate freely. In exp. 1, nine sows were offered crates of 450-, 600- and 750-mm width at standing height. In exp. 2, 12 sows were offered widths of 450, 750 and 1200 mm, the largest being of sufficient width for the sow to turn around. Video recording was used to determine sow position from 3 d before to 6 d after farrowing. In exp. 1, sows showed a preference for the widest crate of 750 mm during and after farrowing (P \u3c 0.05) but not before. ln exp. 2, the 1200-mm width was preferred before, during and after farrowing (P \u3c 0.005) over the widths that were too narrow to permit turning. Sufficient space for turning appears to be preferred by sows around the time of farrowing
