    Maximising ophthalmic theatre time : part 1

Methods: All ophthalmic surgeries under the care of Mr Franco Mercieca carried out in Theatre 13 in the sample months of May and October 2009, were retrospectively analyzed. Average times were estimated for all types of ophthalmic procedures. The total number of cataract operations and patients listed for surgery were noted from beginning of 2006 till end of 2009. Results: In 2009, this firm has carried out 572 cataract operations, 81% increase from 2008, of which 52% were performed on weekends and public holidays. The theatre usage time in the sample months under study was 89.25% on weekdays and over 94% on extra sessions. Theatre time wastage was an average of 8%, half of which being due to cancellations on the day. On average over 71% of our theatre time is used for cataract surgery. Conclusions: In order to maximize our theatre time efficiency, we need to set up a Preoperative Assessment clinic and encourage more patients to have their surgery under local anaesthesia. To reduce the waiting list the need for a parallel theatre dedicated solely to cataract surgery under local anaesthesia is of paramount importance.peer-reviewe

    The challenge of detecting intracluster filaments with Faraday Rotation

    The detection of filaments in the cosmic web will be crucial to distinguish between the possible magnetogenesis scenarios and future large polarization surveys will be able to shed light on their magnetization level. In this work, we use numerical simulations of galaxy clusters to investigate their possible detection. We compute the Faraday Rotation signal in intracluster filaments and compare it to its surrounding environment. We find that the expected big improvement in sensitivity with the SKA-MID will in principle allow the detection of a large fraction of filaments surrounding galaxy clusters. However, the contamination of the intrinsic Faraday Rotation of background polarized sources will represent a big limiter to the number of objects that can be significantly detected. We discuss possible strategies to minimize this effect and increase the chances of detection of the cosmic web with the large statistics expected from future surveys.Comment: 16 pages, accepted to Galaxie

    Para las empresas mexicanas es muy importante abrirse al mundo y arriesgarse para obtener mejores estrategias y lograr aumentar sus ventas. Las pequeñas y medianas empresas (pymes), además de aumentar sus nichos de mercado también aumentan sus estrategias para crecer, pues con todo eso ganan mucha experiencia y logran volverse más competitivos. El objetivo de este trabajo es brindarle a la empresa manufacturera textil Franco´s las herramientas necesarias para poder lograr una mejora en sus ventas de una manera eficaz, innovando y siempre siendo eficaces. La empresa esta consiente del rápido crecimiento en cuanto a la competitividad e innovación entre ellas, así como de las ventas de los textiles, es por eso que estas estrategias ayudarán a mejorar la calidad en su producto y en su proceso administrativo. Por estas razones, la empresa manufacturera textil Franco´s, debe realizar un diagnóstico para conocer sus áreas de oportunidad y así contar con las bases necesarias para realizar observaciones y recomendaciones con el fin de llegar a obtener las mejores estrategias mercadológicas y mejorar su posicionamiento. En este sentido, la empresa debe incrementar y mantener sus ventas dentro del mercado, elevar la satisfacción de sus clientes, diferenciarse de la competencia y ser la preferencia de los clientes. Por lo tanto, se pretende desarrollar estrategias mercadológicas, con el fin de elevar la calidad de su servicio. Para las empresas que no se han atrevido a mejorar sus estrategias mercadológicas es muy importante vencer el temor a hacerlo, pues es fundamental mostrarles con claridad, cuáles serán esos procedimientos, requisitos y medios para lograrlo, por tanto, deben ser ellas mismas las que gestionen todos sus recursos para lograrlo. Es importante mencionar que una de las principales herramientas que se tomaran en cuenta dentro de la empresa será su imagen, ya quien como sabemos, lo principal para que una empresa pueda ser identificada de la competencia y reconocida más fácil, es necesario que cuente con un logo, eslogan, emblema, entre otros elementos, es por eso que se analizara más detenidamente durante todo los capítulos siguientes. El trabajo de campo permitió dar a conocer las condiciones para una acertada interpretación de lo estudiado. Se determinara un perfil de los mayores clientes de la empresa, ya que ellos son los que llevan más años conociendo a la empresa y serán de gran ayuda para poder decidir sobre la nueva imagen que tendrá la empresa y para que a ellos les sea más fácil poder elegir y comprar sus productos ya sea por vía Facebook u otro medio. También el tipo de publicidad les llama la atención para atraer a los consumidores. Para determinar la muestra en la investigación, se obtendrá de los pedidos con más frecuencia que se hacen dentro de la empresa y de la lista de los consumidores más frecuentes, para aplicar un cuestionario y obtener datos que permitirán el desarrollo y la creación de estrategias que mejoren la empresa. Este cuestionario se les hará llegar por medio de un correo electrónico, con el fin de que si por el momento no pueden acudir a la empresa, lo puedan contestar de manera más sencilla, sin tener que trasladarse hasta la empresa

    Intervenciones para mejorar la adherencia al tratamiento: revisión de los resultados publicados

    La adherencia al tratamiento es definida por la Organización Mundial de la Salud (OMS) como el grado de seguimiento por parte del paciente de las recomendaciones aportadas por su facultativo. Existen va rios métodos para medirla y se clasifican en dos grandes bloques, directos e indirectos pero , sin embargo, ninguno se considera de referencia, por lo que se r ecomienda la combinación de varios de ellos. Por otro lado, hay múltiples factores y frecuentemente interrelacionados que provocan que la adhesión a los fármacos sea un verdadero problema en algunos pacientes. En la actualidad, se estima que el 50% de los pacientes crónicos no son adherentes a los tratamientos prescritos y esta ausencia de adherencia conlleva un peor control de la enfermedad, la aparición de un mayor número de complicaciones, a lo que le sigue un aumento de hospitalizaciones y pr uebas compl ementarias, es decir , relevantes repercusiones clínicas y económicas. Por ello, surge la necesidad de desarrollar intervenciones que consigan suavizar o disminuir esta importante cuestión. En el presente trabajo, se hace un estudio acerca de la efectividad de las intervenciones aplicadas hasta el momento con el fin previamente nombrado. Hay ocho prototipos diferentes de intervenciones (técnicas, educativas, conductuales, de apoyo social y familiar, terapia s directamente observable s , psicológicas, combinadas y dirigidas a los profesionales sanitarios) , así como diversas y diferentes estrategias encuadradas en alguno de esos tipos. Los resultados fueron que un gran número de ellas sí conseguían el objetivo perseguido, mejorar los niveles de adherencia en los p acientes mientras que otras no llegando incluso en alguna ocasión a empeorarla. Sin embargo, es de importancia destacar que muchos de esos estudios presentaban un alto riesgo de sesgo, lo que sugiere la necesidad de redefinir los diseños experimentales par a mejorar la calidad de la evidencia que avalen intervenciones sobre los pacientes.Universidad de Sevilla.     Dynamical evolution of magnetic fields in the intracluster medium

    We investigate the evolution of magnetic fields in galaxy clusters starting from constant primordial fields using highly resolved (4 kpc\approx \rm 4 ~kpc) cosmological MHD simulations. The magnetic fields in our sample exhibit amplification via a small-scale dynamo and compression during structure formation. In particular, we study how the spectral properties of magnetic fields are affected by mergers, and we relate the measured magnetic energy spectra to the dynamical evolution of the intracluster medium. The magnetic energy grows by a factor of \sim 40-50 in a time-span of 9\sim 9 Gyr and equipartition between kinetic and magnetic energy occurs on a range of scales (<160 kpc< 160 \rm ~kpc at all epochs) depending on the turbulence state of the system. We also find that, in general, the outer scale of the magnetic field and the MHD scale are not simply correlated in time. The effect of major mergers is to shift the peak magnetic spectra to it smaller scales, whereas the magnetic amplification only starts after \lesssim 1 Gyr. In contrast, continuous minor mergers promote the steady growth of the magnetic field. We discuss the implications of these findings in the interpretation of future radio observations of galaxy clusters.Comment: Accepted in MNRAS; 16 pages, 34 figure

    Corporate culture and shareholder value in banking industry

    This paper analyses the casual relationship between corporate culture and shareholder value using a sample of large banks in the French, German, Italian and U.K. banking systems over the 2000 to 2003 period. Firstly, we measure shareholder value using an Economic Value Added estimated through a procedure tailored to account for banking peculiarities. Secondly, we measure corporate culture using language as its particular artifact and developing a cultural survey based on the application of a text-analysis model to a corpus of reference texts produced by the sample of banks. We posit six hypotheses regarding the relationship between corporate culture and bank profits and shareholder value. Our results noticeably show that bank profits and shareholder value benefit from different orientations of banking corporate culture.

Si tratta di viste 3D navigabili, analisi di visibilità e analisi per il riconoscimento delle unità sceniche di paesaggio: sono state elaborate a partire da dati open avvalendosi di un software GIS open, in una sperimentazione didattica universitaria presso il Politecnico di Torino.The paper presents some methods to produce GIS views to support policies for enhancing landscape and cultural heritage, and their related decision-making processes.
