    Pregnancy after Gastric Banding for the Treatment of Morbid Obesity

    Obesity is known to have a negative impact on pregnancy outcome, as it is associated with an increase in the incidence of gestational diabetes, hypertension, preeclampsia, neural tube defects, macrosomia, and late fetal death. Gastric banding is considered an appropriate intervention for morbid obesity when other weight-loss measures are unsuccessful, and this treatment has been shown to be effective in causing a sustainable weight loss. Some women will become pregnant after bariatric surgery, and the nutritional and metabolic challenges brought by gastric banding may have a profound impact on maternal health and pregnancy outcome. The authors report the case of a 27 year old pregnant woman, with a past medical history of gastric banding surgery for morbid obesity. At 18 weeks of gestation, the patient started complaining of severe nausea and vomiting, The situation deteriorated three weeks later when she rapidly developed severe desnutrition, dehydration and early signs of liver and renal failure. Migration of the gastric band was diagnosed, and laparoscopy conducted to remove it. In the day following surgery the patient complained of absent fetal movements, and an intrauterine demise was diagnosed on ultrasound. Pathological examination of the fetus and placenta failed to reveal the cause of death, but no growth restriction was documented, suggesting the occurrence of an acute event

    In a market where the experience of senior consultants in the public sector is highly valued by the convenors, a small consulting company is unable to permanently retain professionals who know the methodology and are well qualified for tenders. Being this a constraining factor for company growth, Xperta thought it was necessary to shift its value curve in order to change its offer, be recognized in the market and thus achieve sustainability. A project using the Karagabi KMModel as a basis for modeling the knowledge management of the company was run. Basically, the model is a reference guide to build what is called “knowledge-oriented organizations” and has three main components: an implementation strategy, a library of knowledge models and an experience knowledge base. With development of the project a new line of business arose, which is now being offered to the market. It is based on a validated and operational model of knowledge management and innovation. Consulting services in knowledge management integrate products and processes obtained and generated across the various experiences of the company, and it is supported by a protocol that allows the adaptation of the Karagabi model to the current national context.En un mercado donde la experiencia de consultores de alto nivel del sector público es valorada por los convocantes, una pequeña empresa consultora no puede retener de manera permanente a los profesionales que conocen la metodología empleada y que están bien calificados en los concursos. Al ser un factor limitante para el crecimiento de la empresa, Xperta advirtió la necesidad de desplazar su curva de valor para cambiar su oferta, ser reconocida en el mercado y, así, alcanzar sostenibilidad. Se ejecutó un proyecto empleando el modelo KARAGABI KMModel como base para modelar la gestión del conocimiento de la empresa. En esencia, el modelo es una guía de referencia para construir lo que se denomina las “organizaciones orientadas al conocimiento”, y tiene tres componentes principales: una estrategia de implementación, una librería de modelos de conocimiento y una base de conocimiento de experiencias. Con el desarrollo del proyecto surgió una nueva línea de negocio, la misma que se ha empezado a ofrecer en el mercado, con base en un modelo de gestión de conocimiento e innovación, validado y operativo. El servicio de consultoría en gestión del conocimiento integra productos y procesos obtenidos y generados en las diversas experiencias de la empresa, y está soportado por un protocolo que permite la adaptación del modelo KARAGABI a la realidad nacional.

    Phenolic acids and derivatives: studies on the relationship among structure, radical scavenging activity, and physicochemical parameters

    The antiradical activity of caffeic acid (1), dihydrocaffeic acid (5), and their corresponding n-alkyl esters was evaluated by using the 2,2-diphenyl-1-picrylhydrazyl radical (DPPH(*)) method. Dihydrocaffeic acid (5) was the most potent compound, having an antiradical effect higher than that of (+/-)-alpha-tocopherol, whereas caffeic acid (1) was less efficient. Esterification of the carboxyl group of dihydrocaffeic acid (5) had a dramatic effect on its antiradical potency, but similar effects were not observed for caffeic acid (1) derivatives. The n-alkyl esters of both phenolic series had similar potencies, and their antiradical activities were independent of the alkyl chain length. Dose-dependent scavenger effects were found in both series. Acid-base properties of the compounds, evaluated by using potentiometry and spectrophotometry, showed that the catechol moiety had pK(a2) and pK(a3) values of 9. 24-9.02 and 11.38-10.99 in the dihydrocaffeic series and 8.48-8.24 and 11.38-11.07 in the caffeic series, respectively.     Histomorphometric evaluation of bone healing in rabbit fibular osteotomy model without fixation

    <p>Abstract</p> <p>Background</p> <p>Animal models of fracture consolidation are fundamental for the understanding of the biological process of bone repair in humans, but histological studies are rare and provide only qualitative results. The objective of this article is to present the histomorphometric study of the bone healing process using an experimental model of osteotomy in rabbit fibula without interference of synthesis material.</p> <p>Methods</p> <p>Fifteen rabbits were submitted to fibular osteotomy without any fixation device. Groups of five animals were submitted to pharmacological euthanasia during a period of one (group A), two (group B) and four weeks (group C) after osteotomy. Histomorphometric evaluation was performed in the histological sections.</p> <p>Results</p> <p>During week one there was intense cellularity (67/field), a large amount of woven bone (75.7%) and a small amount of lamellar bone (7.65%). At two weeks there was a decrease in woven bone (41.59%) and an increase in lamellar bone (15.16%). At four weeks there was a decrease of cellularity (19.17/field) and lamellar bone (55.56%) exceeded the quantity of woven bone (31.68%).</p> <p>Conclusion</p> <p>Histomorphometric (quantitative) evaluation of the present study was shown to be compatible with bone healing achieved in qualitative experimental models that have been commended in the literature.</p

    Aloenxertos ósseos granulados esponjosos criopreservados na revisão do componente acetabular de artroplastias da anca: protocolos de preparação, de processamento e técnica de impactação

    Os defeitos ósseos acetabulares presentes numa falência de uma artroplastia total da anca podem ser reconstruídos com enxertos ósseos granulados impactados, quer se reimplante uma cúpula cimentada quer uma cúpula não cimentada. Na maioria das publicações ortopédicas, os enxertos ósseos são preparados a partir de cabeças femorais colhidas em dadores vivos submetidos a uma artroplastia da anca. O propósito deste trabalho é descrever os métodos de preparação e processamento e a técnica de impactação de aloenxertos esponjosos granulados congelados, usados na reconstrução acetabular de revisões de artroplastias da anca. O osso esponjoso colhido nos pratos da tíbia e nos côndilos femorais do dador de órgãos, é fragmentado com instrumentos manuais e não com um moinho de osso, é submetido a soluções de peróxido de hidrogénio e de etanol e conservado a -80ºC. O tamanho dos fragmentos (7-10 mm), a lavagem dos enxertos, a contenção dos defeitos ósseos, a técnica de impactação convencional e a vascularização do osso hospedeiro, são fatores críticos para o sucesso das reconstruções acetabulares biológicas.info:eu-repo/semantics/publishedVersio
