    Chest computed tomography in severe bronchopulmonary dysplasia:Comparing quantitative scoring methods

    Purpose: Bronchopulmonary dysplasia (BPD) is the most common complication of extreme preterm birth and structural lung abnormalities are frequently found in children with BPD. To quantify lung damage in BPD, three new Hounsfield units (HU) based chest-CT scoring methods were evaluated in terms of 1) intra- and inter-observer variability, 2) correlation with the validated Perth-Rotterdam-Annotated-Grid-Morphometric-Analysis (PRAGMA)-BPD score, and 3) correlation with clinical data. Methods: Chest CT scans of children with severe BPD were performed at a median of 7 months corrected age. Hyper- and hypo-attenuated regions were quantified using PRAGMA-BPD and three new HU based scoring methods (automated, semi-automated, and manual). Intra- and inter-observer variability was measured using intraclass correlation coefficients (ICC) and Bland-Altman plots. The correlation between the 4 scoring methods and clinical data was assessed using Spearman rank correlation. Results: Thirty-five patients (median gestational age 26.1 weeks) were included. Intra- and inter-observer variability was excellent for hyper- and hypo-attenuation regions for the manual HU method and PRAGMA-BPD (ICCs range 0.80–0.97). ICC values for the semi-automated HU method were poorer, in particular for the inter-observer variability of hypo- (0.22–0.71) and hyper-attenuation (-0.06–0.89). The manual HU method was highly correlated with PRAGMA-BPD score for both hyper- (ρs0.92, p &lt; 0.001) and hypo-attenuation (ρs0.79, p &lt; 0.001), while automated and semi-automated HU methods showed poor correlation for hypo- (ρs &lt; 0.22) and good correlation for hyper-attenuation (ρs0.72–0.74, p &lt; 0.001). Several scores of hyperattenuation correlated with the use of inhaled bronchodilators in the first year of life; two hypoattenuation scores correlated with birth weight. Conclusions: PRAGMA-BPD and the manual HU method have the best reproducibility for quantification of CT abnormalities in BPD.</p

    Lessons from Love-Locks: The archaeology of a contemporary assemblage

Those who research ritual frequently encounter difficulties in the interpretation of its often tantalisingly incomplete material record. Careful analysis of material remains may afford us glimpses into past ritual activity, but our often vast chronological separation from the ritual practitioners themselves prevent us from seeing the whole picture. The archaeologist engaging with structured deposits, for instance, is often forced to study ritual assemblages post-accumulation. Many nuances of its formation, therefore, may be lost in interpretation. This paper considers what insights an archaeologist could gain into the place, people, pace, and purpose of deposition by recording an accumulation of structured deposits during its formation, rather than after. To answer this, the paper will focus on a contemporary depositional practice: the love-lock. This custom involves the inscribing of names/initials onto a padlock, its attachment to a bridge or other public structure, and the deposition of the corresponding key into the water below; a ritual often enacted by a couple as a statement of their romantic commitment. Drawing on empirical data from a three-year diachronic site-specific investigation into a love-lock bridge in Manchester, UK, the author demonstrates the value of contemporary archaeology in engaging with the often enigmatic material culture of ritual accumulations.Peer reviewe

    Performance of a new hand-held device for exhaled nitric oxide measurement in adults and children

    BACKGROUND: Exhaled nitric oxide (NO) measurement has been shown to be a valuable tool in the management of patients with asthma. Up to now, most measurements have been done with stationary, chemiluminescence-based NO analysers, which are not suitable for the primary health care setting. A hand-held NO analyser which simplifies the measurement would be of value both in specialized and primary health care. In this study, the performance of a new electrochemical hand-held device for exhaled NO measurements (NIOX MINO) was compared with a standard stationary chemiluminescence unit (NIOX). METHODS: A total of 71 subjects (6–60 years; 36 males), both healthy controls and atopic patients with and without asthma were included. The mean of three approved exhalations (50 ml/s) in each device, and the first approved measurement in the hand-held device, were compared with regard to NO readings (Bland-Altman plots), measurement feasibility (success rate with 6 attempts) and repeatability (intrasubject SD). RESULTS: Success rate was high (≥ 84%) in both devices for both adults and children. The subjects represented a FE(NO )range of 8–147 parts per billion (ppb). When comparing the mean of three measurements (n = 61), the median of the intrasubject difference in exhaled NO for the two devices was -1.2 ppb; thus generally the hand-held device gave slightly higher readings. The Bland-Altman plot shows that the 95% limits of agreement were -9.8 and 8.0 ppb. The intrasubject median difference between the NIOX and the first approved measurement in the NIOX MINO was -2.0 ppb, and limits of agreement were -13.2 and 10.2 ppb. The median repeatability for NIOX and NIOX MINO were 1.1 and 1.2 ppb, respectively. CONCLUSION: The hand-held device (NIOX MINO) and the stationary system (NIOX) are in clinically acceptable agreement both when the mean of three measurements and the first approved measurement (NIOX MINO) is used. The hand-held device shows good repeatability, and it can be used successfully on adults and most children. The new hand-held device will enable the introduction of exhaled NO measurements into the primary health care

    The 600 yr eruptive history of Villarrica Volcano (Chile) revealed by annually laminated lake sediments

    Lake sediments contain valuable information about past volcanic and seismic events that have affected the lake catchment, and they provide unique records of the recurrence interval and magnitude of such events. This study uses a multilake and multiproxy analytical approach to obtain reliable and high-resolution records of past natural catastrophes from similar to 600-yr-old annually laminated (varved) lake sediment sequences extracted from two lakes, Villarrica and Calafquen, in the volcanically and seismically active Chilean Lake District. Using a combination of micro-X-ray fluorescence (mu XRF) scanning, microfacies analysis, grain-size analysis, color analysis, and magnetic-susceptibility measurements, we detect and characterize four different types of event deposits (lacustrine turbidites, tephra-fall layers, runoff cryptotephras, and lahar deposits) and produce a revised eruption record for Villarrica Volcano, which is unprecedented in its continuity and temporal resolution. Glass geochemistry and mineralogy also reveal deposits of eruptions from the more remote Carran-Los Venados volcanic complex, Quetrupillan Volcano, and the Huanquihue Group in the studied lake sediments. Time-series analysis shows 112 eruptions with a volcanic explosivity index (VEI) >= 2 from Villarrica Volcano in the last similar to 600 yr, of which at least 22 also produced lahars. This significantly expands our knowledge of the eruptive frequency of the volcano in this time window, compared to the previously known eruptive history from historical records. The last VEI >= 2 eruption of Villarrica Volcano occurred in 1991. Based on the last similar to 500 yr, for which we have a complete record from both lakes, we estimate the probability of the occurrence of future eruptions from Villarrica Volcano and statistically demonstrate that the probability of a 22 yr repose period (anno 2013) without VEI >= 2 eruptions is <= 1.7%. This new perspective on the recurrence interval of eruptions and historical lahar activity will help improve volcanic hazard assessments for this rapidly expanding tourist region, and it highlights how lake records can be used to significantly improve historical eruption records in areas that were previously uninhabited

    Event trees and epistemic uncertainty in long‐term volcanic hazard assessment of Rift Volcanoes: the example of Aluto (Central Ethiopia)

    Aluto is a peralkaline rhyolitic caldera located in a highly populated area in central Ethiopia. Its postcaldera eruptive activity has mainly consisted of self‐similar, pumice‐cone‐building eruptions of varying size and vent location. These eruptions are explosive, generating hazardous phenomena that could impact proximal to distal areas from the vent. Volcanic hazard assessments in Ethiopia and the East African Rift are still limited in number. In this study, we develop an event tree model for Aluto volcano. The event tree is doubly useful: It facilitates the design of a conceptual model for the volcano and provides a framework to quantify volcanic hazard. We combine volcanological data from past and recent research at Aluto, and from a tool to objectively derive analog volcanoes (VOLCANS), to parameterize the event tree, including estimates of the substantial epistemic uncertainty. Results indicate that the probability of a silicic eruption in the next 50 years is highly uncertain, ranging from 2% to 35%. This epistemic uncertainty has a critical influence on event‐tree estimates for other volcanic events, like the probability of occurrence of pyroclastic density currents (PDCs) in the next 50 years. The 90% credible interval for the latter is 5–16%, considering only the epistemic uncertainty in conditional eruption size and PDC occurrence, but 2–23% when adding the epistemic uncertainty in the probability of eruption in 50 years. Despite some anticipated challenges, we envisage that our event tree could be translated to other rift volcanoes, making it an important tool to quantify volcanic hazard in Ethiopia and elsewhere

    Reinventing 'The Invention of Tradition'? Indigenous Pasts and the Roman Present

    Thirty years ago Eric Hobsbawm and Terence Ranger introduced The invention of tradition as a concept to explain the creation and rise of certain traditions in times of profound cultural change. Taking stock of current theoretical understandings and focusing on the Roman world, this volume explores the concept of 'inventing traditions' as a means to understand processes of continuity, change and cultural innovation. The notion has been highly influential among studies concerned with the Greek and Roman eastern Mediterranean. Elsewhere in the Roman world and traditions other than Greek, however, have been neglected. This volume aims to evaluate critically the usefulness of the idea of 'inventing traditions' for the successor culture that was Rome. It focuses on the western part of the Roman Empire, which has been virtually ignored by such studies, and on non-Greek traditions. Why, in the Roman present, were some (indigenous) traditions forgotten while others invented or maintained? Using the past for reasons of legitimation in a highly volatile present is a cultural strategy that (also) characterises our present-day, globalized world. Can 'inventing traditions' be regarded as a common human characteristic occurring throughout world history
