    Tratamiento rehabilitador del paciente con sindrome de Guillain Barré y nivel de independencia funcional alcanzado durante du hospitalizacion en Hospital de rehabilitacion Aldo Chavarria Enero a Diciembre 2015

    El Síndrome de Guillain Barré es la causa más frecuente de parálisis aguda generalizada ocasionalmente fatal, es un grupo de síndromes de etiología autoinmune con características clínicas, fisiopatológicas, electrofisiológicas, pronóstico y evolución diferentes. Objetivos: Conocer el tratamiento rehabilitador de pacientes con síndrome de Guillain Barré y el nivel de independencia funcional alcanzado. Materiales y métodos: El estudio es descriptivo, observacional, retroprospectivo y transversal de pacientes hospitalizados con Síndrome de Guillain Barré en periodo de 3 años, utilizando las escalas de clasificación funcional Wines Hughes, valoración de fuerza muscular Lovett y de independencia en AVD Barthel. Resultados: Edades más afectadas de 35 – 49 años, predominio sexo masculino, procedencia urbana, ocupación estudiantes y amas de casa, planes terapéuticos acorde a la literatura. con mejoría funcional egresando con Winer Hughes de 3, 2 y 1. Mejoría de fuerza muscular en miembros superiores e miembros inferiores con Lovett de 3, 4 y 5. Se observó disminución de casos con dependencia severa en AVD incrementando los casos con dependencia moderada en AVD. Conclusiones: Grupo etareo más afectado 35 a 49 años, sexo masculino, del área urbana, profesión estudiantes y amas de casa. Variante clínica principal fue Neuropatía axonal motora aguda. Diagnóstico funcional al ingreso y egreso cuadriparesia flácida. Planes terapéuticos instaurados contribuyeron en mejoría funcional e independencia reflejado en incremento de egresos en escala Winer Hughes 3, 2 y 1. Hubo recuperación funcional de fuerza muscular con Lovett de 3, 4 y 5. Disminuyó la dependencia severa en AVD incrementando la dependencia moderada en AVD. Estancia hospitalaria promedio 12 a 15 semanas. Recomendaciones: Consignar la valoración del paciente con escala de Hughes, Lovett y Barthel para evaluar la progresión de la enfermedad, mejoría funcional y respuesta al tratamiento rehabilitador. Promover estudios de conducción nerviosa en estos pacientes para mejorar calidad de atenció

    Human Trial of a Genetically Modified Herpes Simplex Virus for Rapid Detection of Positive Peritoneal Cytology in the Staging of Pancreatic Cancer

    AbstractIntroductionPatients with peritoneal dissemination of pancreatic adenocarcinoma do not benefit from surgical resection, but radiologic and cytologic detection of peritoneal cancer lack sensitivity. This trial sought to determine if an oncolytic virus may be used as a diagnostic agent to detect peritoneal cancer.MethodsPeritoneal washings from patients with pancreatic adenocarcinoma were incubated with the enhanced green fluorescent protein (eGFP)-expressing oncolytic herpes simplex virus (HSV) NV1066. eGFP-positive or negative status was recorded for each specimen and compared to results obtained by conventional cytologic evaluation. These results were correlated with recurrence and survival for patients who underwent R0 resection.ResultsOf 82 patients entered in this trial, 12 (15%) had positive cytology and 50 (61%) had virally-mediated eGFP positive cells in peritoneal washings. All cytology-positive patients were also eGFP positive. HSV-mediated fluorescence detection had sensitivities of 94% and 100% for detection of any and peritoneal metastatic disease; respectively. Median recurrence free and disease specific survival were 6.5 and 18.3months for eGFP positive patients, versus 12.2 and 36.2months for eGFP negative patients (P=0.01 and 0.19); respectively.ConclusionsA genetically modified HSV can be used as a highly sensitive diagnostic agent for detection of micro-metastatic disease in patients with pancreatic adenocarcinoma and may improve patient selection for surgery

    Protocol of a systematic review and network meta-analysis for the prevention and treatment of perinatal depression

    Introduction Perinatal depression is common and can often lead to adverse health outcomes for mother and child. Multiple pharmacological and non-pharmacological treatments have been evaluated against usual care or placebo controls in meta-analyses for preventing and treating perinatal depression compared. It is not yet established which of these candidate treatments might be the optimal approach for prevention or treatment. Methods and analysis A systematic review and Bayesian network meta-analyses will be conducted. Eight electronic databases shall be searched for randomised controlled trials that have evaluated the effectiveness of treatments for prevention and/or treatment of perinatal depression. Screening of articles shall be conducted by two reviewers independently. One network meta-analysis shall evaluate the effectiveness of interventions in preventing depression during the perinatal period. A second network meta-analysis shall compare the effectiveness of treatments for depression symptoms in women with perinatal depression. Bayesian 95% credible intervals shall be used to estimate the pooled mean effect size of each treatment, and surface under cumulative ranking area will be used to rank the treatments\u27 effectiveness. Ethics and dissemination We shall report our findings so that healthcare providers can make informed decisions on what might be the optimal approach for addressing perinatal depression to prevent cases and improve outcomes in those suffering from depression through knowledge exchange workshops, international conference presentations and journal article publications.     In vitro antioxidant and anticancer activities of Clinacanthus nutans extracts

    Clinacanthus nutans (CN), from the Acanthaceae family is a medicinal plant widely used in Thailand and Malaysia. CN is commonly used as a treatment of inflammation, cancer, and herpes virus infection. This study aimed to determine the antioxidant activity and anticancer properties of CN leaves extracts on human colorectal cancer cell lines, HCT 116 and HT-29. In this study, CN leaves powders were extracted in methanol, chloroform, and acetone at different durations. The crude extracts were assessed for total phenolic content (TPC), total flavonoid contents (TFC), 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging activity and MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) tetrazolium reduction assay. The extract using acetone showed with the highest TPC. The highest total flavonoid content was the methanol extract. The DPPH radical scavenging activity, IC50 of methanol extract was 19.67 µg/mL and exhibited the most efficacious antioxidant property among the others. The 24-hour methanol extract showed the most promising results on MTT assay. Therefore, CN methanol extract is a promising candidate to proceed in other anticancer studies such as cell cycle arrest and apoptosis assays

    Designing tools for teachers to orchestrate computer supported collaborative learning activities in their classrooms requires that attention be paid to the range of roles and activities a teacher must take throughout the process. Drawing on the Implementing Collaborative Learning in the Classroom framework proposed by Kaendler, Wiedmann, Rummel and Spada (2014), the contributors to this symposium will speak to the way their designs address the various parts of this framework, allowing us to draw conclusions about what has been successful for different parts of this process, and identifying future directions for development and research
