    Field Test of the Cougar Home Safety Assessment for Older Persons Version 1.0

    After reviewing the literature on environmental safety and existing home safety evaluation tools, the researchers developed the Cougar Home Safety Assessment Version 1.0 (Cougar 1.0). This assessment is a 56-item tool for identifying environmental safety hazards in the homes of older persons. The first purpose of this study was to develop and field test the Cougar 1.0 to determine its inter-rater reliability and validity. The second purpose was to identify specific environmental safety hazards in the residences of older persons during the field test. Three graduate occupational therapy students administered the Cougar 1.0 in the homes of 14 older persons within one county of northeastern Pennsylvania. The instrument was found to have a moderate level of inter-rater reliability, as Cohen’s Kappa for the ratings of the 3 raters was .506. Percent agreement between the three raters was 90 percent. The field test found that the most frequently unsafe aspects of the homes were: a lack of grab bars near toilets; no emergency numbers posted near phones; the presence of non-grip throw rugs; lack of fire extinguishers; and lack of step stools. Participating residents and occupational therapy home safety specialists provided positive feedback regarding the content validity of the Cougar 1.0. This research suggests that the use of this instrument by occupational therapists may prevent client injuries in the home

    Exploring Occupational Therapy Student Stress: Professor and Student Perspectives

    The purpose of this study was to identify occupational therapy student stress levels and determine factors influencing this emotional response. The study also sought to obtain the perspective of occupational therapy professors on this issue. This mixed-methods study utilized an online quantitative descriptive survey and a telephone or face-to-face qualitative open-ended interview. Online survey respondents included a nationwide sample of 340 occupational therapy professors and 459 occupational therapy students. Additionally, nine professors and five students served as the interviewees. All professors were certified by the National Board for Certification in Occupational Therapy (NBCOT) and employed either full or part time as instructors in an Accreditation Council of Occupational Therapy Education (ACOTE) accredited entry level Master of Science or entry level doctoral educational program. All students were enrolled in an ACOTE accredited entry level master’s or entry level occupational therapy doctoral degree program. Findings demonstrated high stress levels in students caused by personal, financial, and academic pressures. Professors acknowledged student stress; however, the results indicated the need for enhanced understanding and communication regarding student stress. Students may benefit from more intense counseling and stress reduction measures. Additionally, professors and universities may need to enhance existing support systems for students. Professors are advised to be attuned to student stress levels and may need to adjust academic requirements accordingly. Further research is needed to determine avenues for diminishing student stress

    Home Modification Outcomes in the Residences of Older People as a Result of Cougar Home Safety Assessment (Version 4.0) Recommendations

    The purpose of this study was to determine if recommendations made as a result of the administration of the Cougar Home Safety Assessment Version 4.0 (CHSA 4.0) were effective in causing older residents to make environmental safety modifications in their homes. Initial data were collected during the administration of the CHSA 4.0 in the homes of 40 older people in four northeastern states. After completing the initial assessments, the researchers provided the participants with recommendations for improving the environmental safety of their homes. Approximately one month later, each home was reassessed with the CHSA 4.0. Overall, improvement in environmental safety was demonstrated in the homes with the greatest increases being in fire safety, emergency phone number placement, and bathroom safety. A t-test demonstrated a significant gain in compliance (t = 7.8, p < .001) by comparing the difference between the mean initial and reassessment home safety scores. A very large effect size (Cohen’s d = 1.7) was also found, indicating a high magnitude of difference between initial and reassessment safety ratings. The assessment proved to be beneficial in increasing the environmental safety of homes

    Focus Group Study of Diverse Local Populations and Their Health Care Experiences in Northeastern Pennsylvania: Cross-Cultural Issues

    This investigation was conducted through the support of the College Misericordia Diversity Institute and a grant from the Blue Ribbon Foundation of Blue Cross of Northeastern Pennsylvania. Focus group participants were 49 adults from seven minority populations residing in northeastern Pennsylvania’s Luzerne and Lackawanna counties. Data was collected by 11 focus group leaders who were members of a Blue Ribbon Grant Core Committee at College Misericordia in 2004. The seven populations studied were African Americans, Arabic Muslims, Asian-Chinese and Korean, Gay and Lesbian, Hispanic, Jewish, and Asian Indian. A 30-question survey was used to collect data during one to two hour focus group interviews. Through content analysis, six problematic issues faced by many of the participants were identified. All of the findings were validated by a review process. The six issues faced by the 7 groups were: 1) Economics, Education, and Employment Influence Life for Newcomers, 2) Customs and Traditions Sometimes Sacrificed- The Influence of American Culture, 3) Socialization Often Limited to Same Population Group, 4) Mixed Acceptance Level from Area Natives, 5) Bilingual Challenges Impede Optimal Inclusion, 6) Health Care Access Problems. The aforementioned cross-groups study is explored in this report. The study also yielded seven other reports (one for each diverse population) which provide a description of that particular focus group’s perspective on topics such as religion, food, family, customs, and health care (see Appendices A, B, C, D, E, F, and G). Findings of this study are being disseminated in a local effort to educate health care professionals. Future research will be needed to determine if progress is being made in fulfilling the health care needs of all diverse populations living in Luzerne and Lackawanna County, as well as other parts of northeastern Pennsylvania

    Barriers to ecological restoration in Europe: expert perspectives

    Ecological restoration is key to counteracting anthropogenic degradation of biodiversity and to reducing disaster risk. However, there is limited knowledge of barriers hindering the wider implementation of restoration practices, despite high-level political priority to halt the loss of biodiversity. In Europe, progress on ecological restoration has been slow and insufficient to meet international agreements and comply with European Union Nature Directives. We assessed European restoration experts' perceptions on barriers to restoration in Europe, and their relative importance, through a multiple expert consultation using a Delphi process. We found that experts share a common multi-dimensional concept of ecological restoration. Experts identified a large number of barriers (33) to the advancement of ecological restoration in Europe. Major barriers pertained to the socio-economic, not the environmental, domain. The three most important being insufficient funding, conflicting interests among different stakeholders, and low political priority given to restoration. Our results emphasize the need to increase political commitment at all levels, comply with existing nature laws, and optimize the use of financial resources by increasing funds for ecological restoration and eradicate environmentally harmful subsidies. The experts also call for the integration of ecological restoration into land-use planning and facilitating stakeholders' collaboration. Our study identifies key barriers, discusses ways to overcome the main barriers to ER in Europe, and contributes knowledge to support the implementation of the European Biodiversity Strategy for 2030, and the EU 2030 Restoration Plan in particular. © 2021 The Authors.     Methamphetamine and Viagra Use: Relationship to Sexual Risk Behaviors

    Recent studies show that Viagra and methamphetamine use are associated with unprotected anal intercourse among men who have sex with men (MSM). In Long Beach, California, we have reported on an association between Viagra use and the use of amphetamines during sex. The current research investigated the use of both Viagra and amphetamine in men in Long Beach, California. Data on 1,839 men recruited into HIV prevention and testing programs were collected using the Risk Behavior Assessment. A generalized logit model was constructed comparing ever having used both amphetamine and Viagra together and separately, as compared to never having used either (referent). Men who used both methamphetamine and Viagra showed a significantly higher prevalence of hepatitis B, syphilis, and HIV compared to those who used only one or neither drug. Of the 1,794 complete cases, 11.1% (199/1794) had used both amphetamine and Viagra. Of 20 potential risk and protective factors for use of amphetamine and Viagra, 12 were significant predictors: ever used gamma-hydroxybutyrate (GHB), ever used cocaine, ever used ecstasy, being infected with HIV, race = White compared to other, ever having hepatitis B, ever using crack, ever given money to have sex, living in a hotel, ever been in drug treatment, and ever using heroin. The protective factor was being heterosexual. Viagra use was associated with insertive, and methamphetamine was associated with receptive, anal intercourse. GHB use appears to play a more important role than previously thought

    The Functional DRD3 Ser9Gly Polymorphism (rs6280) Is Pleiotropic, Affecting Reward as Well as Movement

    Abnormalities of motivation and behavior in the context of reward are a fundamental component of addiction and mood disorders. Here we test the effect of a functional missense mutation in the dopamine 3 receptor (DRD3) gene (ser9gly, rs6280) on reward-associated dopamine (DA) release in the striatum. Twenty-six healthy controls (HCs) and 10 unmedicated subjects with major depressive disorder (MDD) completed two positron emission tomography (PET) scans with [11C]raclopride using the bolus plus constant infusion method. On one occasion subjects completed a sensorimotor task (control condition) and on another occasion subjects completed a gambling task (reward condition). A linear regression analysis controlling for age, sex, diagnosis, and self-reported anhedonia indicated that during receipt of unpredictable monetary reward the glycine allele was associated with a greater reduction in D2/3 receptor binding (i.e., increased reward-related DA release) in the middle (anterior) caudate (p<0.01) and the ventral striatum (p<0.05). The possible functional effect of the ser9gly polymorphism on DA release is consistent with previous work demonstrating that the glycine allele yields D3 autoreceptors that have a higher affinity for DA and display more robust intracellular signaling. Preclinical evidence indicates that chronic stress and aversive stimulation induce activation of the DA system, raising the possibility that the glycine allele, by virtue of its facilitatory effect on striatal DA release, increases susceptibility to hyperdopaminergic responses that have previously been associated with stress, addiction, and psychosis

    Protection of rat renal vitamin E levels by ischemic-preconditioning

    BACKGROUND: During renal transplantation, the kidney remains without blood flow for a period of time. The following reperfusion of this ischemic kidney causes functional and structural injury. Formation of oxygen-derived free radicals (OFR) and subsequent lipid peroxidation (LP) has been implicated as the causative factors of these injuries. Vitamin E is known to be the main endogenous antioxidant that stabilizes cell membranes by interfering with LP. The present study was designed to examine the role of ischemic-preconditioning (repeated brief periods of ischemia, IPC) in prevention of renal injury caused by ischemia-reperfusion (IR) in rats. METHODS: IPC included sequential clamping of the right renal artery for 5 min and release of the clamp for another 5 min for a 3 cycles. IR was induced by 30 min ischemia followed by 10 min reperfusion. Four groups of male rats were used: Control, IPC, IR and IPC-IR. Vitamin E, an endogenous antioxidant and as an index of LP, was measured by HPLC and UV detection in renal venous plasma and tissue. Renal function was assessed by serum creatinine and BUN levels. Renal damage was assessed in sections stained with Haematoxylin and Eosin. RESULTS: In the IR group, there was a significant decrease in vitamin E in plasma and tissue compared to a control group (p,0.05). In the IPC-IR group, vitamin E concentration was significantly higher than in the IR group (p,0.01). The results showed that 30 min ischemia in the IR group significantly (p,0.05) reduced renal function demonstrated by an increase in serum creatinine levels as compared with the control group. These results in the IPC group also showed a significant difference with the IR group but no significant difference in serum BUN and creatinine between IR and IPC-IR group were detected. Histological evaluation showed no structural damage in the IPC group and an improvement in the IPC-IR group compared to IR alone. CONCLUSIONS: In this study, IPC preserved vitamin E levels, but it could not markedly improve renal function in the early phase (1–2 h) of reperfusion. IPC may be a useful method for antioxidant preservation in organ transplantation

    Planktonic foraminiferal assemblages reflect warming during two recent mid-latitude marine heatwaves

    Under future climate scenarios, ocean temperatures that are presently extreme and qualify as marine heatwaves (MHW) are forecasted to increase in frequency and intensity, but little is known about the impact of these events on one of the most common paleoproxies, planktonic foraminifera. Planktonic foraminifera are globally ubiquitous, shelled marine protists. Their abundances and geochemistry vary with ocean conditions and fossil specimens are commonly used to reconstruct ancient ocean conditions. Planktonic foraminiferal assemblages are known to vary globally with sea surface temperature, primary productivity, and other hydrographic conditions, but have not been studied in the context of mid-latitude MHWs. For this study, the community composition and abundance of planktonic foraminifera were quantified for 2010-2019 along the Newport Hydrographic Line, a long-term monitoring transect at 44.6°N in the Northern California Current (NCC). Samples were obtained from archived plankton tows spanning 46 to 370 km offshore during annual autumn (August – October) cruises. Two MHWs impacted the region during this timeframe: the first during 2014-2016 and a second, shorter duration MHW in 2019. During the 2014-2016 MHW, warm water subtropical and tropical foraminifera species were more prevalent than the typical polar, subpolar, and transitional species common to this region. Cold water species were abundant again after the first MHW dissipated in late 2016. During the second, shorter-duration MHW in 2019, the assemblage consisted of a warm water assemblage but did not include tropical species. The foraminiferal assemblage variability correlated with changes in temperature and salinity in the upper 100 meters and was not correlated with distance offshore or upwelling. These results suggest that fossil foraminiferal assemblages from deep sea sediment cores may provide insight into the magnitude and frequency of past MHWs
