how the mediterranean diet and some of its components modulate inflammatory pathways in arthritis
Arthritis encompasses a heterogeneous group of diseases characterised by inflammation that leads not only to joint damage, bone erosion, severe pain and disability, but also affects other organs of the body, resulting in increased morbidity and mortality. Although the mechanisms underlying the pathogenesis of joint diseases are for the most part unknown, a number of nutrient and non-nutrient components of food have been shown to affect the inflammatory process and, in particular, to influence clinical disease progression. The Mediterranean diet model has already been linked to a number of beneficial health effects: both fat and non-fat components of the Mediterranean dietary pattern have been shown to exert important anti-inflammatory activities by affecting the arachidonic acid cascade, the expression of some proinflammatory genes, and the activity of immune cells. N-3 polyunsaturated fatty acids, in particular, have been shown to affect lymphocyte and monocyte functions, crucially involved in adaptive and innate immunity. Although some aspects concerning the mechanisms of action through which the Mediterranean diet pattern exerts its beneficial effects remain to be elucidated, arthritis patients may potentially benefit from it in view of their increased cardiovascular risk and the treatment they require which may have side effects
Detection of a slow-flow component in contrast-enhanced ultrasound of the synovia for the differential diagnosis of arthritis
Contrast Enhanced Ultrasound (CEUS) is a sensitive imaging technique to assess tissue vascularity, that can be useful in the quantification of different perfusion patterns. This can particularly important in the early detection and differentiation of different types of arthritis. A Gamma-variate can accurately quantify synovial perfusion and it is flexible enough to describe many heterogeneous patterns. However, in some cases the heterogeneity of the kinetics can be such that even the Gamma model does not properly describe the curve, especially in presence of recirculation or of an additional slowflow component. In this work we apply to CEUS data both the Gamma-variate and the single compartment recirculation model (SCR) which takes explicitly into account an additional component of slow flow. The models are solved within a Bayesian framework. We also employed the perfusion estimates obtained with SCR to train a support vector machine classifier to distinguish different types of arthritis. When dividing the patients into two groups (rheumatoid arthritis and polyarticular RA-like psoriatic arthritis vs. other arthritis types), the slow component amplitude was significantly different across groups: mean values of a1 and its variability were statistically higher in RA and RA-like patients (131% increase in mean, p = 0.035 and 73% increase in standard deviation, p = 0.049 respectively). The SVM classifier achieved a balanced accuracy of 89%, with a sensitivity of 100% and a specificity of 78%. © 2017 SPIE
The impact of alkaline treatments on elasticity in spruce tonewood
It is commonly believed that violins sound differently when finished. However, if the role of varnishes on the vibrational properties of these musical instruments is well-established, how the first components of the complete wood finish impact on the final result is still unclear. According to tradition, the priming process consists of two distinct stages, called pre-treatment and sizing. The literature reports some recipes used by old Cremonese luthiers as primers, mainly based on alkaline aqueous solutions and protein-based glues. In this manuscript, we analyze the impact of these treatments on the mechanical properties of the material. The combination of two pre-treatments and three sizes is considered on nine different plates. We compare the vibrational properties before and after the application and assess the effects of the different primers, also supported by finite element modeling. The main outcome is that the combination of particular treatments on the violin surface before varnishing leads to changes not only to the wood appearance, but also to its vibrational properties. Indeed pre-treatments, often considered negligible in terms of vibrational changes, enhance the penetration of the size into the wood structure and strengthen the impact of the latter on the final rigidity of the material along the longitudinal and radial directions
From macro to nano: Linking quantitative CEUS perfusion parameters to CD4+ T cells subtypes in spondyloarthtitis
The onset and progression of immune-mediated inflammatory arthritis, such as rheumatoid arthritis and spondyloarthritis, are linked to the IL23-IL17 immune axis, so that many therapeutic strategies aim at modulating this pathway. However, there is so far no possibility of an in vivo direct monitoring, without a biopsy, of the specific T cells involved in this modulation. Synovial perfusion, and thus synovial angiogenesis, has been recognized as a sensitive and early marker of inflammation that can be evaluated via quantitative analysis of contrast-enhanced ultrasound imaging data. © 2017 IEEE

A preliminary spectroscopic approach to evaluate the effectiveness of water-and silicone-based cleaning methods on historical varnished brass
Soiling deposition and wrong conservation practices are among the causes of the decay process that can affect the morphological, mechanical, and compositional features of the varnish, the most exposed layer of an artefact. In this perspective, the identification of the best cleaning practices is a priority. During the 18th century, scientific instruments of the highest quality were built, and peculiar varnishes were produced to confer protection and elegance to their metal elements. For this study, based on a historical recipe, we have reproduced a peculiar spirit varnish, enriched with natural resins and colorants, and we have applied on it a synthetic soiling mixture to simulate the aging conditions. We have then performed a non-invasive multi-analytical study to monitor the effectiveness of two water-based and a silicone-based, cleaning methods, namely, water in agarose, Tween 20 (3%) in agarose, and Velvesil Plus. The study includes colorimetry, Fourier transform infrared (FTIR) spectroscopy and X-ray fluorescence (XRF) mapping, coupled with chemometrics. Principal component analysis applied to FTIR spectral data has been demonstrated to be a powerful tool to enhance weak variations in the IR spectra, empowering the interpretation of cleaning effect versus the application time of each cleaning test
Transapical off-pump echo-guided mitral valve repair with neochordae implantation mid-term outcomes
Background: The NeoChord echo-guided transapical beating heart repair is a promising early-stage
minimally invasive surgical procedure for degenerative mitral valve (MV) regurgitation (DMR) correction.
The technique has been improved since its inception following procedure standardization, patient selection
optimization, and learning curve stabilization. We hereby present the mid-term clinical results through three
years of our large single center experience.
Methods: All consecutive patients with severe symptomatic DMR due to prolapse or flail of one or both
mitral leaflets that underwent the NeoChord procedure between November 2013 and June 2019 were
included. Patients were categorized according to MV anatomy; Type A isolated central posterior leaflet
prolapse and/or flail, Type B posterior multi-segment prolapse and/or flail, Type C anterior and/or bi-leaflet
prolapse or flail, Type D paracommissural prolapse and/or flail and/or significant leaflet and/or annular
calcifications. Patients underwent clinical and echocardiographic follow-up at one, three, six, twelve months
and yearly thereafter. Clinical outcomes and the composite primary endpoint (patient success) were defined
according to Mitral Valve Academic Research Consortium (MVARC) criteria. Mitral regurgitation (MR)
severity was graded as absent, mild, moderate and severe according to American Society of Echocardiography
(ASE) and European Society of Cardiology (ESC) guidelines.
Results: Two hundred and three patients were included; median follow-up was 24 months [interquartile
range (IQR), 9–36]. Median age was 64 years (IQR, 54–74 years), median Society of Thoracic Surgeons
(STS) Predicted Risk of Mortality (PROM) was 0.60% (IQR, 0.32–1.44%). There were 106 Type A patients
(52.2%), 68 Type B (33.5%), 16 Type C (7.9%), and 13 Type D (6.4%). Kaplan-Meier estimate of survival
was 99.0%±0.7% at one and two years and 94.0%±2.9% at three years. At one-year follow-up patient success
was 91.2%±2.0% and 111 patients (74%) presented a residual MR mild or less (1+). At three-year follow-up
patient success was 81.2%±3.8% and 32 patients (64%) had a residual MR mild or less (1+). Patient success
was significantly different according to anatomical type (P=0.001). Echocardiographic analysis showed a
significant acute left ventricle and left atrial reverse remodeling that was maintained up to three years.
Conclusions: The NeoChord echo-guided transapical beating heart repair procedure demonstrated good
clinical outcomes and echocardiographic results up to three-year follow-up

observations, in atmospheric dry conditions, in order to
validate radiative transfer models and test the quality of water
vapor continuum and line parameters. The project provides
the very first set of far-infrared spectral downwelling
radiance measurements, in dry atmospheric conditions,
which are complemented with Raman Lidar-derived
temperature and water vapor profiles
