Italijanske politike sprejemanja in pandemija: Od izključenosti do zapuščenosti
The article analyzes how the COVID-19 pandemic has affected the management of reception centers for refugees and asylum seekers in Italy. By analyzing the transformation of Italian reception policies in the last years, the article shows the relationship between these changes and the condition of refugees and asylum seekers in these centers during the COVID-19 pandemic. Overcrowded housing, the absence of institutional guidance on managing the situation, and the interruption of many migrants’ migratory projects are the main findings that emerged. The article is based on digital ethnographic techniques, in addition to phone interviews with key speakers of the social contexts monitored online.Članek obravnava vpliv pandemije Covida-19 na upravljanje sprejemnih centrov za begunce in prosilce za azil v Italiji. Na podlagi analize sprememb v politiki sprejemanja v zadnjih letih prikazuje, kako so te v omenjenih centrih med pandemijo vplivale na življenjske razmere beguncev in prosilcev za azil. Rezultati analize poudarjajo predvsem prenatrpane nastanitvene zmogljivosti, odsotnost institucionalnih smernic za obvladovanje položaja, pa tudi prekinitev selitvenih načrtov številnih migrantov. Članek temelji na digitalnih etnografskih tehnikah in telefonskih intervjujih s ključnimi govorci družbenih kontekstov, ki se spremljajo na spletu
Microstructure and rheological response of laboratory-aged SBS-modified bitumens
Polymer additives are widely used to improve the performance of road bitumens including their resistance to hardening during oxidation, although their oxidative inhibitor effect has not been well documented. This study aims to investigate the effect of laboratory-simulated ageing on the microstructure, and rheological properties of Polymer Modified Bitumens (PMBs) prepared with a Styrene-Butadiene-Styrene (SBS) copolymer and sulphur as a cross-linker. The laboratory ageing was conducted through the Rolling Thin Film Oven (RTFO) and the Pressure Aging Vessel (PAV) procedures. The unaged and aged binders were characterised using the Multiple Stress Creep and Recovery (MSCR) test, Gel Permeation Chromatography (GPC) and fluorescence microscopy. The MSCR test showed that the cross-linked polymer had a higher capability of mitigating the effect of bitumen hardening on the rheological response, while for the PMBs without cross-linking, the polymer effects decreased significantly after RTFOT ageing. The GPC results showed that the aromatics, resins and asphaltenes fractions of the unmodified bitumens shifted towards heavier molecules after one cycle of PAV ageing but showed limited variation with further ageing. Similar behaviour was observed for the uncross-linked polymer binders, while for the cross-linked polymer binder, the behaviour depended on the polymer concentration

METHODS. Propranolol was administered subcutaneously and picropodophyllin (PPP) intraperitoneally. Intravitreal injections of vascular endothelial growth factor (VEGF) were performed. Messengers of -ARs, VEGF, its receptors, IGF-1 and IGF-1R were measured with quantitative RT-PCR. VEGF content was determined with ELISA. -ARs, hypoxia-inducible factor (HIF)-1 , occludin, and albumin were measured with Western blot. Retinal localization of 3-ARs was determined by immunohistochemistry. Retinopathy was assessed by scoring fluorescein-perfused retinas, and plasma extravasation was visualized by Evans blue dye
Critical appraisal of the role of volumetric modulated arc therapy in the radiation therapy management of breast cancer
Background: The aim of this review is the critical appraisal of the current use of volumetric modulated arc therapy for the radiation therapy management of breast cancer. Both clinical and treatment planning studies were investigated. Material and methods: A Pubmed/MEDLINE search of the National Library of Medicine was performed to identify VMAT and breast related articles. After a first order rejection of the irrelevant findings, the remaining articles were grouped according to two main categories: clinical vs. planning studies and to some sub-categories (pointing to significant technical features). Main areas of application, dosimetric and clinical findings as well as areas of innovations were defined. Results: A total of 131 articles were identified and of these, 67 passed a first order selection. Six studies reported clinical results while 61 treatment dealed with treatment planning investigations. Among the innovation lines, the use of high intensity photon beams (flattening filter free), altered fractionation schemes (simultaneous integrated boost, accelerated partial breast irradiation, single fraction), prone positioning and modification of standard VMAT (use of dynamic trajectories or hybrid VMAT methods) resulted among the main relevant fields of interest. Approximately 10% of the publications reported upon respiratory gating in conjunction with VMAT. Conclusions: The role of VMAT in the radiation treatment of breast cancer seems to be consolidated in the in-silico arena while still limited evidence and only one phase II trial appeared in literature from the clinical viewpoint. More clinical reports are needed to fully proove the expected dosimetric benefits demonstrated in the planning investigations
Economic inequality increases the number of hours worked and decreases work-life balance perceptions: longitudinal and experimental evidence
International institutions' attention to work-life balance (WLB) demonstrates the global breadth of this issue. Yet the scientific community has thus far paid little attention to its structural underpinnings and to the interplay between these macro-level underpinnings and individual psychological factors. We examine the contextual role of economic inequality at the national level as a significant factor influencing working time and WLB perceptions using multiple empirical strategies. In the first set of studies (1a and 1b), we compared countries with different levels of inequality (Study 1a with 37 countries, Study 1b with longitudinal data from 34 countries, N = 254) and found increased working time and reduced WLB in highly unequal countries. In a pilot study ( N = 81) and in the pre-registered Studies 2 ( N = 338) and 3 ( N = 499) we corroborated this evidence with an experimentally induced inequality perception, reporting an indirect effect of inequality on WLB (Studies 2 and 3) and working time (Study 3) through status anxiety and competitiveness. In Study 2, we manipulated socio-economic class in addition to economic inequality, showing that the detrimental effect of inequality on WLB is especially marked for participants assigned to a low-class condition. This research contributes to an integrated understanding of the impact of economic inequality and socio-economic class in shaping WLB and provides useful insights for organizations to develop context-specific policies to improve employees' WLB that take both individual and structural factors into account
Effect of ageing on the morphology and creep and recovery of polymer-modified bitumens
Polymer additives are used to improve the properties of road bitumens including their oxidative resistance. However, their usage as anti-oxidative materials remains relatively unclear. This study aims to investigate the changes in the morphology and the rheological response of polymer modified bitumens used in road pavement construction caused by ageing. An elastomer (radial styrene butadiene styrene, SBS) and a plastomer (ethyl vinyl acetate, EVA) polymer were mixed with one base bitumen at three polymer concentrations. The bitumens were RTFO and PAV aged. The morphology of the bitumens was captured by fluorescence microscopy while the rheological properties were measured by means of the multiple stress creep and recovery (MSCR) test. The results show that the morphology of the SBS modified bitumen degrades with ageing as a function of polymer concentration and dispersion, with higher dispersion being more resistant. The morphology of the EVA modified bitumen has a low ageing susceptibility irrespective of polymer concentration. The MSCR response of EVA modified bitumens does not differ from that found for unmodified bitumen, where the hardening produces a decrease in the non-recoverable compliance. In the case of SBS modified bitumen, the degradation of the polymer backbone affects the bitumen hardening as much as the polymer phase dispersed and networked in the bitumen phase. Furthermore, in the case of the elastomer, the average percent recovery is in agreement with the variation of the morphology with ageing. Therefore, the use of the average percent recovery as a valuable rheological index of the integrity of the polymer network can be advocated
Expression of endothelial nitric oxide synthase in blood vessels and silicic acid consumption
NO produces by endothelial nitric oxide synthase (eNOS) represents one of the most representative vasoactive molecules able to regulate vascular tone; it is released by endothelial cells and it diffuses to adjacent vascular smooth muscle cells causing vasorelaxation. In addition, endothelium-derived NO is know to be involved in multiple ways to prevent the progression of age-related vascular diseases. Senescent endothelial cells are characterized by a decreased production of endothelium-derived NO due to a decrease of eNOS activity that could be attributable to a reduction in eNOS protein expression as well as in eNOS phosphorylation (Matsushita et al., 2001). Previous studies showed that silicon, mainly as silicic acid, plays an important role as protective factor against the development of age-related vascular diseases, maintaining integrity, stability and elastic properties of arterial walls (Schwarz et al., 1977). So, the aim of this study was to evaluate the relationship between the expression of eNOS and silicic acid consumption in a mouse model of early physiological aging. We evaluated qualitatively and quantitatively by immunohistochemical method, the expression of eNOS in the vessel wall of aorta and renal vessels in relation with the administration of silicic acid in drinking water. The results showed that loss of eNOS expression was prevented by regular consumption of silicic acid rich water, supporting the potential protective role of silicon against age related-vascular disorders
A Real-World, Multicenter, Observational Retrospective Study of Durvalumab After Concomitant or Sequential Chemoradiation for Unresectable Stage III Non-Small Cell Lung Cancer
Introduction: For unresectable stage III non-small cell lung cancer (NSCLC), the standard therapy consists of chemoradiotherapy (CRT) followed by durvalumab maintenance for responding patients. The present study reports on the safety and outcome of durvalumab use after CRT in a real-world, multicenter, retrospective cohort. Methods: Two hundred thirty-eight patients have been included. We collected data on systemic therapy, radiation therapy, the timing between CRT and durvalumab, number of durvalumab cycles, reasons for non-starting or discontinuation, incidence and grade of adverse events (AEs), and progression-free survival (PFS) and overall survival (OS). Results: One hundred fifty-five patients out of 238 (65.1%) received at least one durvalumab dose: 91 (58.7%) after concomitant CRT (cCRT) and 64 (41.3%) after sequential CRT (sCRT). Programmed-death ligand 1 (PD-L1) status was unknown in 7/155 (4.5%), negative in 14 (9.1%), and positive ≥1% in 134/155 (86.4%). The main reasons for non-starting durvalumab were progression (10.1%), PD-L1 negativity (7.5%), and lung toxicity (4.6%). Median follow-up time was 14 months (range 2–29); 1-year PFS and OS were 83.5% (95%CI: 77.6–89.7) and 97.2% (95%CI: 94.6–99.9), respectively. No significant differences in PFS or OS were detected for cCRT vs. sCRT, but the median PFS was 13.5 months for sCRT vs. 23 months for cCRT. Potentially immune-related AEs were recorded in 76/155 patients (49.0%). Pneumonitis was the most frequent, leading to discontinuation in 11/155 patients (7.1%). Conclusions: Durvalumab maintenenace after concurrent or sequential chemoradiation for unresectable, stage III NSCLC showed very promising short-term survival results in a large, multicenter, restrospective, real-world study. Durvalumab was the first drug obtaining a survival benefit over CRT within the past two decades, and the present study contributes to validating its use in clinical practice
Long-term disability trajectories in relapsing multiple sclerosis patients treated with early intensive or escalation treatment strategies
Background and aims: No consensus exists on how aggressively to treat relapsing-remitting multiple sclerosis (RRMS) nor on the timing of the treatment. The objective of this study was to evaluate disability trajectories in RRMS patients treated with an early intensive treatment (EIT) or with a moderate-efficacy treatment followed by escalation to higher-efficacy disease modifying therapy (ESC).
Methods: RRMS patients with â©ľ5-year follow-up and â©ľ3 visits after disease modifying therapy (DMT) start were selected from the Italian MS Registry. EIT group included patients who received as first DMT fingolimod, natalizumab, mitoxantrone, alemtuzumab, ocrelizumab, cladribine. ESC group patients received the high efficacy DMT after â©ľ1 year of glatiramer acetate, interferons, azathioprine, teriflunomide or dimethylfumarate treatment. Patients were 1:1 propensity score (PS) matched for characteristics at the first DMT. The disability trajectories were evaluated by applying a longitudinal model for repeated measures. The effect of early versus late start of high-efficacy DMT was assessed by the mean annual Expanded Disability Status Scale (EDSS) changes compared with baseline values (delta-EDSS) in EIT and ESC groups.
Results: The study cohort included 2702 RRMS patients. The PS matching procedure produced 363 pairs, followed for a median (interquartile range) of 8.5 (6.5-11.7) years. Mean annual delta-EDSS values were all significantly (p < 0.02) higher in the ESC group compared with the EIT group. In particular, the mean delta-EDSS differences between the two groups tended to increase from 0.1 (0.01-0.19, p = 0.03) at 1 year to 0.30 (0.07-0.53, p = 0.009) at 5 years and to 0.67 (0.31-1.03, p = 0.0003) at 10 years.
Conclusion: Our results indicate that EIT strategy is more effective than ESC strategy in controlling disability progression over time
