Granuloma reparativo de células gigantes en el escafoides tarsiano: a propósito de 1 caso

Se presenta el caso de una mujer de 22 años con dolor en la región medialarsiana del pie izquierdo de 1 año de evolución sin antecedente traumático. La paciente presentaba una tumoración dolorosa en la cara medial de escafoides. La movilidad del tobillo era completa. La radiología mostraba una lesión radiolucente insuflante en el escafoides. En la TAC se apreciaba una ruptura de la cortical y la ausencia de afectación de partes blandas. La captación con 99Tc mostraba una alta concentración de radioisótopo en la lesión. El tratamiento quirúrgico incluyó curetaje e injerto óseo. La microscopía de la muestra quirúrgica ofreció el diagnóstico de granuloma reparativo de células gigantes. Se discute el diagnóstico diferencial de esta lesión.A 22-year-old women with pain in her left midle foot since one year before the first visit to the hospital, without any previous trauma is presented. At physical examination there was a tender point and tumor at the medial aspect of the foot, over the navicular bone. Complete range of motion was present. Plain X-rays showed a radiolucent lesion in the navicular. Computed tomography showed cortical bone disruption and no soft tissue involvement. The tecnetium-99 bone scan disclosed a high concentration of radioisotope at the lesion. Surgical treatment included curetagge and bone grafting. Microscopic assesment of the resected tissue was compatible with giant cell reparative granuloma. The differential diagnosis is discussed

A case study of AstraZeneca’s omeprazole/esomeprazole chiral switch strategy

Introduction/Objective: To describe the chiral switch, an evergreening strategy used by AstraZeneca to position enantiopure esomeprazole as the new proton pump inhibitor market leader, displacing its predecessor omeprazole.Methodology: A four-stage systematic review which included: a preliminary review, bibliographic review using databases, classification of the body of literature, and content analysis.Results: Using different legal and commercial strategies, such as patent thickets and aggressive publicity campaigns, AstraZeneca transferred consumer loyalty from their successful omeprazole to esomeprazole, its new and more expensive patent protected product which has the same therapeutic value as its predecessor. This chiral switch allowed AstraZeneca to maintain monopoly prices, which increased the financial burden experienced by consumers and payors and may have also had a negative impact on access to the medication.Conclusions: This case study exemplifies how the current patent system, including patent thickets, can be used to enhance the profits of pharmaceutical companies while stalling innovation and placing undue financial burdens on the consumer.Fil: Piñeiro, Federico Jesús. Tradiciones equívocas: sobre la controversia entre Ingarden, Wellek y Hamburger

This paper deals with the recovery of the phenomenological and ontological legacy of Roman Ingarden given the controversies of his work with prestigious names in literature theory such as René Wellek and Käte Hamburger. Después de haber usado habitualmente las webs mas conocidas llegamos a la conclusión de que faltaba en el mercado de la construcción un nuevo directorio que a la vez que fuese amplio en la información, esta fuese muy sencilla y rápida de manejar. Así nació la idea del directorio, que inicialmente recibió el nombre de AsturCons (Asturias construcción). Impacto de la rigidez de las uniones en el coste global de pórticos a dos aguas de acero

This paper studies the influence of joint stiffness value combinations in gabled steel frames. Twelve different joint stiffness values were combined for the column base, knee joint and ridge joint. Ten different geometries were combined for gabled steel frames (8-20 m span, 3.5-10 m column height and 10-20% roof slope). Finally, three different load levels for snow and wind were considered, with constant values of permanent and maintenance loads. A total of 46656 cases were analyzed using specific software for steel structure design, obtaining in each case the total cost. Graphical cost representation was obtained for each joint stiffness value combination. Joints represent an average of 17% of the total cost of gabled steel frame structures. In general, lower cost structures were obtained with low joint rigidity values. It is possible to reduce the total cost by around 18% on average, with appropriate selection of joint stiffness combinations.En este artículo se presenta un estudio sobre la influencia de la rigidez de las uniones en pórticos a dos aguas de acero. Se combinan doce valores diferentes de rigidez de placa de anclaje, hombro y cumbrera. Se combinan diez geometrías (8- 20 m de luz, 3.5-10 m de altura de pilares y 10-20% de pendiente de cubierta). Se consideran tres niveles de carga de nieve y viento con valores constantes de carga de mantenimiento. Se analizan un total de 46656 supuestos utilizando un programa informático de cálculo de estructuras para obtener en cada caso su coste total. Se obtiene la representación gráfica del coste para cada combinación de rigideces. En general, los costes más bajos de las estructuras se corresponden con valores bajos de rigidez. Es posible reducir el coste total un 18% de media, con la selección apropiada de la combinación de valores de rigidez de las uniones

Chitosan-Based Thermosensitive Materials

Thermosensitive polymers are materials capable of undergoing a reversible phase transition in aqueous media in response to a variation of the temperature. They have attracted high scientific interest for advanced applications in diverse areas, such as biotechnology, biomedical, environmental, food industry and other fields. At the same time, chitosan is a promising marine polysaccharide that has long been used in applications such as drug, peptide or gene delivery systems. Being the most abundant marine polysaccharide, chitin and chitosan do not exhibit thermoresponsive properties, but some of their derivatives do. In the present chapter, the efforts to produce chitosan-based thermosensitive materials are reviewed. Particularly, the properties and applications of chitosan-glycerophosphate thermogelling system are examined; the methods of synthesis of chitosan copolymers grafted with poly(N-isopropylacrylamide) or poly(N-vinylcaprolactam), their physicochemical properties and most of their prominent applications are discussed as well

Proyecto de investigación: evaluación de una actualización de un protocolo sobre cuidados de enfermería al paciente con hipotermia inducida

La hipotermia inducida es una técnica utilizada para el tratamiento de pacientes neurocríticos que precisa de profesionales altamente entrenados en su manejo. Aunque existen protocolos que guían su aplicación y que han demostrado su utilidad en el largo periodo de su utilización, pensamos que una actualización de los mismos centrada en el manejo enfermero de la detección y tratamiento precoces de las complicaciones que pueden surgir, mejoraría los resultados relacionados con la morbi-mortalidad de estos pacientes. Si la investigación propuesta ligada a la actualización de los protocolos disponibles demuestra el beneficio buscado de la reducción de la morbi-mortalidad se podría implantar en breve en los hospitales de nuestro entorno. Esto es debido a que tenemos evidencias racionales de que es coste-efectiva. Adicionalmente, al haber sido evaluada en centros de distinto nivel y experiencia, demostraría su aplicabilidad a la práctica clínica habitual en el manejo enfermero del paciente neurocrítico.Grado en Enfermerí

Measuring Oscillations with A Million Atmospheric Neutrinos

We analyze the sensitivity achievable by the current and near-future water(ice)-Cherenkov atmospheric neutrino experiments in the context of standard three-flavor neutrinos oscillations. In this study, we perform an in-depth analysis of the current shared systematic uncertainties arising from the common flux and neutrino-water interactions. We then implement the systematic uncertainties of each experiment and develop the atmospheric neutrino simulations for Super-Kamiokande (SK), with and without neutron-tagging capabilities (SuperK-Gd), IceCube-Upgrade, and ORCA experiments. A careful review of the synergies and features of these experiments is carried out to examine the potential of a joint analysis of these atmospheric neutrino data in resolving the $\theta_{23}$ octant and the neutrino mass ordering. Finally, we assess the capability to constraint $\theta_{13}$ and the CP-violating phase ($\delta_{CP}$ ) in the leptonic sector independently from reactor and accelerator neutrino data.Comment: 39 pages, 33 figure
