    Retribución en farmacia comunitaria. Visión en otros países

    INTRODUCCIÓN La retribución, que según la RAE, es la recompensa o pago por algo, constituye un factor de máxima relevancia en la determinación de la eficiencia de una actividad. Como en cualquier actividad profesional, en la farmacia comunitaria es un determinante de la consecución de los objetivos de la misma. En base a ello, el objetivo del presente estudio es analizar la estructura de la retribución de la dispensación en la farmacia comunitaria en España, comparándola con la de otros países representativos a nivel internacional.MÉTODOS Se realizó una búsqueda en internet de las disposiciones oficiales sobre los diferentes sistemas retributivos actualmente en vigor para cada país seleccionado. Se seleccionaron las estructuras de remuneración de la farmacia comunitaria en diversos países de Europa (Alemania, Bélgica, Francia, Gran Bretaña, Holanda, Irlanda y Suiza), América (Canadá y Estados Unidos) y Australia (Australia).RESULTADOS En España, la retribución de la dispensación en la farmacia comunitaria se basa exclusivamente en un porcentaje sobre el precio de venta del medicamento. En los demás países analizados, dicha retribución ha ido evolucionando hacia el pago de honorarios por las intervenciones farmacéuticas desarrolladas, desligándose así del precio del medicamento y asimilando el trabajo del farmacéutico más a una actividad profesional que mercantil.CONCLUSIONES El sistema retributivo de la dispensación de la farmacia comunitaria en España debería evolucionar, al igual que ocurre en el resto de los países analizados, hacia un sistema basado en un honorario profesional por las actividades profesionales realizadas, desterrando definitivamente el actual sistema retributivo basado en un margen sobre el precio de venta del medicamento

    Spike-based control monitoring and analysis with Address Event Representation

    Neuromorphic engineering tries to mimic biological information processing. Address-Event Representation (AER) is a neuromorphic communication protocol for spiking neurons between different chips. We present a new way to drive robotic platforms using spiking neurons. We have simulated spiking control models for DC motors, and developed a mobile robot (Eddie) controlled only by spikes. We apply AER to the robot control, monitoring and measuring the spike activity inside the robot.     Bio-Inspired and Neuro-Inspired systems or circuits are a relatively novel approaches to solve real problems by mimicking the biology in its efficient solutions. Robotic also tries to mimic the biology and more particularly the human body structure and efficiency of the muscles, bones, articulations, etc. Address-Event-Representation (AER) is a communication protocol for transferring asynchronous events between VLSI chips, originally developed for neuro-inspired processing systems (for example, image processing). Such systems may consist of a complicated hierarchical structure with many chips that transmit data among them in real time, while performing some processing (for example, convolutions). The information transmitted is a sequence of spikes coded using high speed digital buses.     Is Sustainable Performance Explained by Firm Effect in Small Business?

    To what extent a firm’s resources (firm effect) and the structure of the sector (industry effect) are sources of a firm’s competitiveness has been debated for years in strategic management. Most of the empirical studies carried out have focused on large firms and have used static performance measures, and in them the firm effect generally outweighs the industry effect. This research contributes to this debate in trying to verify whether the competitive advantage that relies on the firm’s resources is sustainable, especially in small firms. We used a sample of almost 15,000 Spanish firms to test the impact that the firm and the industry effects have on sustainable performance, for both small and large firms, applying hierarchical linear modelling with a variable measured through time-varying parameters. Our results confirm the absolute importance of the firm effect on sustainable organizational performance, regardless the firm size, and show that, even though the industry effect has little weight in explaining sustainability, it is significantly higher in the case of small firms. This means that managers must concentrate efforts on providing their firm with the necessary resources to achieve a competitive advantage while choosing a good sector to position itselfS

    A Protocol Generator Tool for Automatic In-Vitro HPV Robotic Analysis

    Human Papilloma Virus (HPV) could develop precancerous lesions and invasive cancer, as it is the main cause of nearly all cases of cervical cancer. There are many strains of HPV and current vaccines can only protect against some of them. This makes the detection and genotyping of HPV a research area of utmost importance. Several biomedical systems can detect HPV in DNA samples; however, most of them do not have a procedure as fast, automatic or precise as it is actually needed in this field. This manuscript presents a novel XML-based hierarchical protocol architecture for biomedical robots to describe each protocol step and execute it sequentially, along with a robust and automatic robotic system for HPV DNA detection capable of processing from 1 to 24 samples simultaneously in a fast (from 45 to 162 min), efficient (100% markers effectiveness) and precise (able to detect 36 different HPV genotypes) way.     Building Blocks for Spikes Signals Processing

    Neuromorphic engineers study models and implementations of systems that mimic neurons behavior in the brain. Neuro-inspired systems commonly use spikes to represent information. This representation has several advantages: its robustness to noise thanks to repetition, its continuous and analog information representation using digital pulses, its capacity of pre-processing during transmission time, ... , Furthermore, spikes is an efficient way, found by nature, to codify, transmit and process information. In this paper we propose, design, and analyze neuro-inspired building blocks that can perform spike-based analog filters used in signal processing. We present a VHDL implementation for FPGA. Presented building blocks take advantages of the spike rate coded representation to perform a massively parallel processing without complex hardware units, like floating point arithmetic units, or a large memory.     Automatic selection of the Groebner Basis' monomial order employed for the synthesis of the inverse kinematic model of non-redundant open-chain robotic systems

In a previous work we presented a systematic procedure that relies on Groebner Bases to synthesize the IKM of non-redundant open-chain robots. This study expands the developed procedure with a methodology for the automatic selection of the basis' monomial order. The procedure's inputs are the robot's Denavit-Hartenberg parameters and the movement range of its actuators, while the output is the synthesized IKM, ready to be used in the robot's control system or in a simulation of its behavior. This procedure can synthesize the IKM of a wide range of open-chain robotic systems, such as Cartesian robots, SCARA, non-redundant multi-legged robots, and all non-redundant manipulators that satisfy the in-line wrist condition. The procedure's performance is assessed through two study cases of open-chain robots: a walking hexapod and a PUMA manipulator. The optimal monomial order is successfully identified for all cases. Protease inhibitors (PI) are commonly used as part of triple combinations, given that to antiviral potency, better tolerance and convenience has been achieved in recent years.Objective: To summarize and update evidence-based information about atazanavir (ATV) on initial, simplification, and rescue interventions in HIV patients.Methods: Review of observational and randomized trials reported in medical conferences, peer-reviewed journals, and treatment guidelines.Results: ATV is a second-generation PI, which has shown across studies potent antiviral activity and high genetic barrier, both in HAART-naïve patients or after virological failure. Indulgent metabolic profile, in terms of insulin glucose and lipid levels, adds value to this drug for the long-term management of HIV infection.Keywords: atazanavir, HAART, protease inhibitor
