    La diagnosi infermieristica, seconda fase del processo infermieristico, è un giudizio clinico riguardante le risposte della persona, della famiglia o della comunità a problemi di salute/processi vitali attuali o potenziali. Essa costituisce la base sulla quale scegliere gli interventi infermieristici volti a raggiungere dei risultati di cui l'infermiere è responsabile. La diagnosi infermieristica esprime il giudizio professionale sulle condizioni del paziente, sulle sue risposte ai trattamenti ricevuti e sulle necessità di assistenza infermieristica. La NANDA propone tre modelli di diagnosi: reali, di rischio, di benessere. La struttura della diagnosi infermieristica si compone di tre elementi utili essenzialmente per l'adozione di un linguaggio infermieristico condiviso. Per questo motivo è usata una terminologia specifica per diagnosticare in modo infermieristico. Gli elementi componenti sono tre: titolo, caratteristiche definenti, fattori correlati. Il titolo deve "qualifcare" la tipologia del problema; le caratteristiche definenti sono l'equivalente dei segni e dei sintomi soggettivi ed oggettivi presenti in relazione ad una determinata diagnosi; i fattori correlati sono in pratica le cause, i fattori eziologici che determinano una certa situazione; si possono raggruppare in quattro categorie: fisiopatologici, situazionali, fasi maturative, trattamenti.     Solid organ transplant recipients are at risk of develop ing a wide range of viral-associated malignancies, in cluding skin tumours and lymphoproliferative disorders. The risk of a post-transplant lymphoproliferative disorder is 28–49 times the risk of a lymphoproliferative disorder in the normal population. Most cases are of B-cell phenotype and are associated with Epstein-Barr virus infection. Post-transplant lymphoproliferative disorders presenting clinically in the skin are rare and usually of B-cell phenotype. Only rare cases of cutaneous T-cell post-transplant lymphoproliferative disorder have been reported previously, mostly mycosis fungoides type. We describe here a rare primary cutaneous T-cell lymphoma CD30+ arising in a heart transplant patient who had a nodule on the right leg, several years after heart transplantation. The morphology and immunohistochemical findings were consistent with a CD30+ anaplastic large cell lymphoma with a T-cell phenotype.     We deal with positive solutions for the Neumann boundary value problem associated with the scalar second order ODE u"+q(t)g(u)=0,t[0,T], u" + q(t)g(u) = 0, \quad t \in [0, T], where g:[0,+[Rg: [0, +\infty[\, \to \mathbb{R} is positive on ]0,+[\,]0, +\infty[\, and q(t)q(t) is an indefinite weight. Complementary to previous investigations in the case 0Tq(t)<0\int_0^T q(t) < 0, we provide existence results for a suitable class of weights having (small) positive mean, when g(x)<0g'(x) < 0 at infinity. Our proof relies on a shooting argument for a suitable equivalent planar system of the type x=y,y=h(x)y2+q(t), x' = y, \qquad y' = h(x)y^2 + q(t), with h(x)h(x) a continuous function defined on the whole real line.Comment: 17 pages, 3 figure

    The effect of amoxicillin on dental enamel development in vivo

    The exposure to amoxicillin has been associated with molar incisor hypomineralization. This study aimed to determine if amoxicillin disturbs the enamel mineralization in in vivo experiments. Fifteen pregnant rats were randomly assigned into three groups to received daily phosphatase-buffered saline or amoxicillin as either 100 or 500 mg/kg. Mice received treatment from day 13 of pregnancy to day 40 postnatal. After birth, the offsprings from each litter continued to receive the same treatment according to their respective group. Calcium (Ca) and phosphorus (P) content in the dental hard tissues were analyzed from 60 upper first molars and 60 upper incisors by the complexometric titration method and colorimetric analysis using a spectrophotometer at 680 nm, respectively. Lower incisors were analyzed by X-ray microtomography, it was measured the electron density of lingual and buccal enamel, and the enamel and dentin thickness. Differences in Ca and P content and electron density among the groups were analyzed by one-way ANOVA. There was no significant difference on enamel electron density and thickness among the groups (p > 0.05). However, in incisors, the higher dose of amoxicillin decreased markedly the electron density in some rats. There were no statistically significant differences in Ca (p = 0.180) or P content (p = 0.054), although the higher dose of amoxicillin could affect the enamel in some animals. The amoxicillin did not significantly alter the enamel mineralization and thickness in rats. © 2020Peer reviewe

    Molecular Epidemiology of mcr-Encoded Colistin Resistance in Enterobacteriaceae From Food-Producing Animals in Italy Revealed Through the EU Harmonized Antimicrobial Resistance Monitoring

    Colistin resistance by mobilisable mcr genes has been described in bacteria of food-animal origin worldwide, which has raised public health concerns about its potential foodborne transmission to human pathogenic bacteria. Here we provide baseline information on the molecular epidemiology of colistin-resistant, mcr-positive Escherichia coli and Salmonella isolates in food-producing animals in Italy in 2014-2015. A total 678, 861 and 236 indicator E. coli, Extended Spectrum Beta-Lactamase (ESBL)/AmpC-producing E. coli, and Salmonella isolates, respectively, were tested for colistin susceptibility. These isolates were collected according to the EU harmonized antimicrobial resistance monitoring program and are representative of at least 90 and 80% of the Italian poultry (broiler chickens and turkeys) and livestock (pigs and bovines &lt; 12 months) production, respectively. Whole genome sequencing by Illumina technology and bioinformatics (Center for Genomic Epidemiology pipeline) were used to type 42 mcr-positive isolates by PCR. Colistin resistance was mainly observed in the ESBL/AmpC E. coli population, and was present in 25.9, 5.3, 6.5, and 3.9% of such isolates in turkeys, broilers, pigs, and bovines, respectively. Most colistin-resistant isolates (141/161, 87.5%) harbored genes of the mcr-1 group. mcr-1 was also detected in a small proportion of Salmonella isolates (3/146, 2.0%) in turkeys. Additional mcr types were mcr-3 in four ESBL-producing E. coli from bovines, and two mcr-4 in ESBL (n = 1) and indicator E. coli (n = 1) from pigs and bovines. We describe notable diversity of mcr variants with predominance of mcr-1.1 and mcr-1.2 on conjugative IncX4 plasmids in E. coli and in Salmonella serovars Typhimurium, Newport, Blockley from turkey. A new variant, mcr-1.13 was detected in the chromosome in E. coli in turkey and pig isolates. Additionally, we describe mcr-3.2 and mcr-4.3 in E. coli from bovines, and mcr-4.2 in E. coli from pigs. These findings elucidate the epidemiology of colistin resistance in food-producing animals in Italy along with its genetic background, and highlight the likelihood of mcr horizontal transfer between commensal bacteria and major food-borne pathogens (Salmonella) within the same type of productions. Thorough action and strategies are needed in order to mitigate the risk of mcr transfer to humans, in a “One Health” perspective
