    Učinak ketoprofena na koncentraciju u plazmi i farmakokinetičke pokazatelje ciprofloksacina u pilića

    Ciprofloxacin (CFX) and ketoprofen (KPN) are used widely in combination in veterinary interventions for bacterial infections, so in this study the effect of KPN was studied on the efficacy of CFX, by measuring its plasma concentration and pharmacokinetic parameters in 7-10 day-old chickens. The analgesic median effective dose (ED50) of KPN was determined to be 1.62 mg/kg, IM, in the chickens. The preferable analgesic dose of KPN to be used with CFX was 4 mg/kg, IM, which differs significantly from KPN 2 mg/kg, IM,. The CFX plasma concentrations alone (8 mg/kg, IM) measured at different times (0.5, 1, 2, 4 and 24 hours) were 3.31, 3.60, 3.21, 2.70 and 0.17 μg/mL while its concentration was elevated by 53, 54, 90, 107 and 418 % when coadministered with KPN (4 mg/kg, IM) to 5.05, 5.53, 6.10, 5.59 and 0.88 μg/mL in the chickens, respectively. CFX pharmacokinetic parameters, such as the area under the curve (AUC), the area under the moment curve (AUMC), mean residence time (MRT), half-life (t1/2β), Tmax, and Cmax increased when KPN was coadministered with CFX by 129, 289, 70, 49, 100 and 69 %, whereas the elimination rate constant (Kel), the volume of distribution at steady state (Vss) and clearance (Cl) decreased by 36, 34 and 58 %, respectively. It was concluded that coadministration of KPN alters the plasma concentration and the pharmacokinetic parameters of CFX, suggesting that the CFX dose can be reduced when used with KPN to achieve the desired concentration of CFX in the plasma, as an antibacterial for treatment of infected animals.Kombinacija ciprofloksacina (CFX) i ketoprofena (KPN) u širokoj je upotrebi u veterinarskom liječenju bakterijskih infekcija. Ovo je istraživanje provedeno kako bi se ustanovio utjecaj KPN-a na učinkovitost CFX-a mjerenjem njegove koncentracije u plazmi i farmakokinetičkih pokazatelja u pilića starih između 7 i 10 dana. Procijenjeno je da je prosječna doza analgetičke učinkovitosti (ED50) KPN-a u pilića 1,62 mg/kg, intramuskularno. Poželjna analgetička doza KPN-a bila je 4 mg/kg, intramuskularno, što se znakovito razlikuje od doze KPN-a od 2 mg/kg, intramuskularno,koja se daje s CFX-om. Koncentracije CFX-a u plazmi (8 mg/kg, im.) u različitim vremenima mjerenja (0, 5, 1, 2, 4 i 24 sata) bile su 3,31, 3,60, 3,21, 2,70 i 0,17 μg/mL, a primijenjene zajedno s KPN-om (4 mg/kg, im) porasle su za 53, 54, 90, 107 i 418 % i iznosile 5,05, 5,53, 6,10, 5,59 i 0,88 μg/mL. Farmakokinetički pokazatelji CFX-a, koji uključuju područje ispod krivulje (AUC), područje ispod krivulje momenta (AUMC), prosječno vrijeme zadržavanja (MRT),poluživot (t1/2β), Tmax, i Cmax, porasli su kad je KPN primijenjen s CFX-om za 129, 289, 70, 49, 100 i 69 %, dok su se konstanta brzine eliminacije (Kel), volumen distribucije u stabilnom stanju (Vss) i klirens (Cl) smanjili za 36, 34 i 58 %. Zaključeno je da zajednička primjena KPN-a i CFX-a mijenja koncentraciju u plazmi i farmakokinetička svojstva CFX-a. Navedeno upućuje na to da bi se doza CFX-a mogla smanjiti kad se primjenjuje u kombinaciji s KPN-om, pri čemu se postigla željena koncentracija CFX-a u plazmi kao antibakterijskog lijeka za zaražene živoitnje

    Development and Validation of the Jordanian Diabetic Health Literacy Questionnaire: Enhancing Diabetes Management in Arabic-Speaking Populations

    (1) Background: Amidst the global rise in type 2 diabetes mellitus (T2DM), effective management of the disease has become increasingly important. Health literacy, particularly in non-English speaking populations, plays a crucial role in this management. To address the lack of suitable tools for Arabic-speaking diabetic patients, this study developed and validated the Jordanian Diabetic Health Literacy Questionnaire (JDHLQ). (2) Methods: A sample of 400 diabetic patients from Jordan, with a balance in gender, age, and educational background, was recruited from an endocrinology outpatient clinic. The JDHLQ, consisting of informative and communicative sections, underwent rigorous validation. Utilizing principal component analysis and Rasch analysis, the JDHL's reliability and validity were evaluated. (3) Results: The results showed moderate proficiency in understanding and communicating diabetes-related information and confirmed the reliability and validity of the JDHLQ. (4) Conclusions: These findings emphasize the importance of culturally appropriate health literacy tools in enhancing patient understanding, engagement, and overall management of T2DM in Arabic-speaking communities

    Automatic alignment of surgical videos using kinematic data

    Over the past one hundred years, the classic teaching methodology of "see one, do one, teach one" has governed the surgical education systems worldwide. With the advent of Operation Room 2.0, recording video, kinematic and many other types of data during the surgery became an easy task, thus allowing artificial intelligence systems to be deployed and used in surgical and medical practice. Recently, surgical videos has been shown to provide a structure for peer coaching enabling novice trainees to learn from experienced surgeons by replaying those videos. However, the high inter-operator variability in surgical gesture duration and execution renders learning from comparing novice to expert surgical videos a very difficult task. In this paper, we propose a novel technique to align multiple videos based on the alignment of their corresponding kinematic multivariate time series data. By leveraging the Dynamic Time Warping measure, our algorithm synchronizes a set of videos in order to show the same gesture being performed at different speed. We believe that the proposed approach is a valuable addition to the existing learning tools for surgery.Comment: Accepted at AIME 201

    Emergent Surgical Airway Skills: Time to Re-evaluate the Competencies

    Introduction: One of the most challenging scenarios an anesthesia provider can face is treating a can\u27t intubate can\u27t ventilate (CICV) patient. The incidence of CICV is estimated to be around one in 10,000 cases. According to the American Society of Anesthesiology Closed Claims Study, adverse respiratory events are the most common type of injury, with difficult intubation and ventilation contributing to the majority of these cases. The objective of this non-interventional quality improvement project was to evaluate the prior training, exposure, and self-reported confidence in handling the CICV scenario among anesthesia providers at Henry Ford Hospital in Detroit, MI. Methods: An online questionnaire was distributed via email to all residents, certified registered nurse anesthetists (CRNAs), and attending anesthesiologists in March 2021. The email contained a link to an online questionnaire via Microsoft Forms (Microsoft Corporation, Redmond, WA). Univariate group comparisons were carried out between the respondents\u27 role (attending, CRNA, or resident), as well as between the number of years that the respondents were in practice (\u3c 5 years, 5-10 years, \u3e 10 years). Results: Out of the total 170 anesthesia providers, 119 participated in the study where 54 (45%) were attendings, 44 (37%) were residents, and 21 (18%) were CRNAs. The majority (75%) did not know the surgical airway kit location, and 87% had not performed the surgical airway procedure before. The vast majority (96.7%) recommended simulation training compared to online training or lecture series, and just over 50% recommended annual training frequency. When looking at the differences in responses based on years of experience as an anesthesia provider, the majority of those with \u3e 10 years in practice knew how to perform the surgical airway technique while respondents with \u3c 5 years did not know how to perform the technique, and 50% of those with five to 10 years experience knew how to perform the surgical airway procedure for a CICV scenario. Conclusion: Although there were many significant differences observed between the various provider roles and years in practice, surprisingly, the responses revealed both a lack of experience and confidence in performing the surgical airway procedure in all provider roles. These findings highlight a need for better emergency airway teaching and training. These findings will be used to guide the design and implementation of improved surgical airway training for residents, CRNAs, and attending anesthesiologists with the goal of better preparedness for handling a CICV scenario

    Nurturing Micro, Small, and Medium Enterprises (MSMEs/UMKM): Unleashing the Power of Islamic Economic Innovation through a Historical Lens

    The focal point of this study lies in the impact of the Covid-19 pandemic on the economic landscape of Micro, Small, and Medium Enterprises (MSMEs) within the Yogyakarta community, particularly in the Kotagede area. Numerous MSMEs and local trade stalls have become non-operational due to the adverse effects of the pandemic, leading to financial distress and, in some cases, bankruptcy. This article addresses this issue within the context of Islamic economics by drawing insights from its foundational theories and proponents. The aim is to provide inspiration for MSMEs and entrepreneurs in Indonesia. The research employs a literature-based qualitative methodology, utilizing sources from Google Scholar, Scopus, and relevant books on Islamic economics, MSMEs, INOVASI, and Indonesian traders. The data analysis is conducted through a robust content analysis approach. The findings reveal an innovative model within Islamic economics that fosters creativity and initiative among MSMEs, particularly in Kotagede, Yogyakarta. Despite the intermission experienced by many small businesses during the pandemic, the encouragement for innovation from local authorities, including the Minister of Tourism and Culinary, has facilitated their return to business operations. Additionally, the study sheds light on the historical perspectives of Islamic economics by examining the contributions of experts such as Boediyono, Ibnu Khaldun, Abu Hanifa, and Zaid bin Ali. The implications of this research are significant in fortifying the Islamic economic sector, specifically for MSMEs in Indonesia

    Applying Best Practices for The Prevention of Surgical Site Infection (SSI) and Reducing Risk Factors for Patients: Meta-Analysis Theoretical Review

    This study aimed at providing a comprehensive range of evidence-based recommendations for interventions to be applied during the pre-, intra- and postoperative periods for the prevention of the surgical site infection SSI, while also considering aspects related to resource availability and values and preferences. An initial search identified more than 200 titles published in 2006-2017 as relevant for data extraction, the researchers dealt with the extracted information included study design and methodology, reported cumulative incidence and post-surgical time until onset of SSI, and odds ratios and associated variability for all factors considered in univariate and/or multivariable analyses. And then the study recommended the best producers for reducing the risk factors for patients by conducting the findings of the meta-analysis theoretical review.     Numerical treatment for a novel crossover mathematical model of the COVID-19 epidemic

    This paper extends a novel piecewise mathematical model of the COVID-19 epidemic using fractional and variable-order differential equations and fractional stochastic derivatives in three intervals of time. The deterministic models are augmented with hybrid fractional order and variable order operators, while the stochastic differential equations incorporate fractional Brownian motion. To probe the behavior of the proposed models, we introduce two numerical techniques: the nonstandard modified Euler Maruyama method for the fractional stochastic model, and the Caputo proportional constant-Grünwald-Letnikov nonstandard finite difference method for the fractional and variable-order deterministic models. Several numerical experiments corroborate the theoretical assertions and demonstrate the efficacy of the proposed approaches

    Design and analysis of moment-resisting nature-inspired-design structure using graphic statics methods

    Based on Kiesler’s perception of space and the idea of continuity, the design proposal from DARC (Digital Architecture Research Centre at Kent School of Architecture and Planning) follows a nature-inspired scheme discussing the notion of continuity of force through the study of combined loading. Geometric intuitive methods are used to examine and determine axial forces, bending moments, torsion values and deflections. These include the use of Kangaroo, mesh subdivision, and traditional and latest developments in Graphic Statics. The study originally explores the use of the Doo-Sabin subdivision method for determining force, and, through seeking parallelisms of structural design with biological systems, aims to detect the benefits and limitations of expanding the application of advanced Graphic Statics and Graphic Kinematics methods for moments and deflections in nature-inspired design vocabularies
