Myocardial viability and survival in ischemic left ventricular dysfunction
BACKGROUND The assessment of myocardial viability has been used to identify patients with coronary artery disease and left ventricular dysfunction in whom coronary-artery bypass grafting (CABG) will provide a survival benefit. However, the efficacy of this approach is uncertain. METHODS In a substudy of patients with coronary artery disease and left ventricular dysfunction who were enrolled in a randomized trial of medical therapy with or without CABG, we used single-photon-emission computed tomography (SPECT), dobutamine echocardiography, or both to assess myocardial viability on the basis of pre-specified thresholds. RESULTS Among the 1212 patients enrolled in the randomized trial, 601 underwent assessment of myocardial viability. Of these patients, we randomly assigned 298 to receive medical therapy plus CABG and 303 to receive medical therapy alone. A total of 178 of 487 patients with viable myocardium (37%) and 58 of 114 patients without viable myocardium (51%) died (hazard ratio for death among patients with viable myocardium, 0.64; 95% confidence interval [CI], 0.48 to 0.86; P = 0.003). However, after adjustment for other baseline variables, this association with mortality was not significant (P = 0.21). There was no significant interaction between viability status and treatment assignment with respect to mortality (P = 0.53). CONCLUSIONS The presence of viable myocardium was associated with a greater likelihood of survival in patients with coronary artery disease and left ventricular dysfunction, but this relationship was not significant after adjustment for other baseline variables. Experience with Magnetically-Levitated Centrifugal Flow Pump in Patients with Cardiogenic Shock (INTERMACS 1)
IntroductionShort term use of magnetically-levitated ventricular assist devices offers hemodynamic stabilization of patients with refractorycardiogenic shock in INTERMACS stage 1, enablinga therapeutic strategy.ObjectiveThe aim of this study was to assess in a single centre the results with second generation centrifugal flow pumps inpatients with refractory cardiogenic shock.Methods Fifteen patients with Levitronix CentriMag ventricular assistdevice implantation were retrospectively analyzed from 2006 to 2011. All patients presented refractory cardiogenic shock under two inotropic agents and 13 patients were also assisted with intra aortic balloon pump. The indications were:end stage cardiomyopathy in 8 patients, viral myocarditis in 1 patient, postpartum cardiomyopathy in 1 patient, postcardiotomycardiogenic shock in 3 patients and post heart transplantation graft failure in 2 patients.ResultsMean age was 49 ± 13 years, and 66% (10/15) were men. Only 1 patient underwent left ventricular assist device implantation(LVA) and 14 patients underwent biventricular assistance (BVA). Mean support duration was 6 ± 4 days(2-19). Final post-implant therapeutic decision was bridgeto heart transplantation in 12 patients (80%), bridge to recovery in 1 patient (7%) and bridge to decision in 2 patients(13%). One patient was successfully weaned from BVA dueto ventricular function recovery and 8 patients were transplanted,with a survival rate of 60% (9/15). Reoperation dueto bleeding was performed in 6 patients (40%) and 1 patient presented cannulae thrombosis. None of the patients hadstroke or technical system failures. Six patients died whilereceiving circulatory assistance (40%) (5 BVA and 1 LVA),1 patient due to sepsis, 1 patient due to coagulopathy and4 patients due to multiple system organ failure. Out of the 6 deaths, 2 patients were in postcardiotomy cardiogenic shock and 4 were on heart transplantation waiting list.ConclusionsIn this series, circulatory support with Levitronix CentriMagcentrifugal flow pump was effective in critical patients with asurvival rate of 60%. Reoperation for bleeding was the mostfrequent complication.Introducción: El uso de dispositivos de asistencia ventricular a corto plazo con levitación magnética permite estabilizar hemodinámicamente a  pacientes (p) en shock cardiogénico   refractario en estadÃo INTERMACS 1 y definir la estrategia  terapéutica.Objetivos: Evaluar los resultados del uso de bomba centrifuga de 2ª generación en p con shock cardiogénico refractario en el Hospital Universitario de la Fundación Favaloro.Métodos: Se analizaron  retrospectivamente 15 pacientes (p) con  asistencia ventricular con bomba Levitonix CentriMag desde  2006 al 2011. Todos los p presentaban shock cardiogénico refractario con dos inotrópicos y 13 p tenÃan  balón intra aórtico de contrapulsación previo a la asistencia. Las indicaciones fueron:  miocardiopatÃas avanzadas, 8 pacientes , miocarditis viral, 1 p ; miocardiopatia periparto, 1 p ; shock cardiogénico pos cardiotomÃa (SCPC), 3 p  y falla del injerto pos trasplante cardÃaco 2 p .Resultados: La edad media en adultos fue 49±13 años y el 66% (10/15) eran hombres. A 1 p  se le implanto asistencia ventricular izquierda (AVI) y asistencia biventricular (ABV) a 14 p. El tiempo medio de asistencia fue de 6±4 dÃas (1-19). La decisión terapéutica final pos implante fue: puente al trasplante cardÃaco en 12 p (80%), puente a la recuperación 1 p (7%) y puente a la decisión 2p (13%). Se explantó la asistencia en 1 p  por recuperación de la función ventricular (ABV) y 8 p fueron trasplantados, con una supervivencia del 60% (9/15). Requirieron reoperación por sangrado 6 p (40%) y 1 p presentó  trombosis de las cánulas, ninguno ACV ni fallas técnicas del sistema. Fallecieron bajo asistencia 6 p (40%) (5 ABV y 1 AVI): 1 p por sepsis, 1 p con coagulopatÃa severa y 4 p por falla multiorgánica. De los 6 p fallecidos, 2 eran SCPC y 4 eran candidatos previos a trasplante cardÃaco.Conclusiones: En esta serie, el soporte circulatorio con bomba centrifuga Levitronix CentriMag® fue efectivo en pacientes crÃticos  con una supervivencia del 60%. Infective Endocarditis in Hypertrophic Cardiomyopathy
Background: Infective endocarditis (IE) is a well-known complication of hypertrophic cardiomyopathy (HCM). Intracardiacdevice (ID) implantation for the treatment of HCM is an additional factor that increases the risk of IE.Objective: The aim of this study was to assess the clinical manifestations and prognosis of IE in patients with HCM.Methods: A retrospective, descriptive and observational study assessed the occurrence of IE and the clinical characteristicsof a population with HCM from June 1992 to January 2014, with median follow-up of 7.5 years.Results: The study evaluated 646 patients with HCM. Left ventricular outflow tract obstruction (LVOTO) was present in38.5% (n=230) of patients and 22% (n=129) had an ID. The incidence of IE was 1.9%. Twelve episodes were confirmed, 7valvular [7/230 (3.04%)] and 5 in ID, 3 in pacemakers and 2 in implantable cardioverter defibrillators [5/129 (6.45%)]. Patientswith valvular IE had resting mean gradient of 48±37 mmHg and during Valsalva maneuver of 126±44 mmHg, respondingto medical treatment in all cases. Infective endocarditis in ID was resolved with percutaneous removal in 5 patients. Onepatient of the valvular group (8%) required valve replacement. No deaths were reported.Conclusions: The incidence of IE in HCM is low. There are two defined populations: left valvular IE confined to patients withLVOTO and IE for ID. Patients with HCM without LVOTO or ID did not present IE.Introducción: La endocarditis infecciosa (EI) es una complicación reconocida de la miocardiopatÃa hipertrófica (MCH); elimplante de dispositivos intracavitarios (DI) para el tratamiento de la MCH agrega un factor que incrementa el riesgo de EI.Objetivo: Analizar la incidencia, las manifestaciones clÃnicas y el pronóstico de la EI en pacientes con MCH.Material y métodos: Estudio retrospectivo, descriptivo y observacional. Se evaluaron la ocurrencia de EI y las caracterÃsticasclÃnicas de una población con diagnóstico de MCH desde junio de 1992 hasta enero de 2014, con una mediana de seguimientode 7,5 años.Resultados: Se evaluaron 646 pacientes con MCH. El 38,5% (n = 230) presentó obstrucción al tracto de salida del ventrÃculoizquierdo (OTSVI) y el 22% (n = 129) tenÃa un DI. La incidencia de EI fue del 1,9%. Se evidenciaron 12 episodios, 7 valvulares[7/230 (3,04%)] y 5 en DI, 3 en marcapasos y 2 en cardiodesfibrilador [5/129 (6,45%)]. Los pacientes con EI valvular presentabanun gradiente promedio de 48 ± 37 mm Hg en reposo y de 126 ± 44 mm Hg durante Valsalva. Todos respondieron altratamiento médico. En 5 pacientes con EI en el DI se efectuó la extracción percutánea. Un paciente (8%) del grupo valvularrequirió reemplazo; ningún paciente falleció.Conclusiones: La EI en la MCH tiene una incidencia baja. Existen dos poblaciones definidas: EI valvular izquierda, confinadaen pacientes con OTSVI y EI por DI. Los pacientes con MCH sin OTSVI ni DI no presentaron EI
Predictores de Mortalidad o trasplante cardÃaco en la miocardiopatÃa periparto
Introducción
La miocardiopatÃa periparto es una forma infrecuente de insuficiencia cardÃaca congestiva con
una evolución impredecible. Su verdadera incidencia y prevalencia no se conoce con certeza y
su etiologÃa aún no se ha aclarado, aunque se han involucrado varios mecanismos en los que
se reconocen diversos factores de riesgo.
Objetivo
Analizar predictores pronósticos de mortalidad o de requerimiento de trasplante cardÃaco.
Material y métodos
Entre 1992 y noviembre de 2011 se evaluaron retrospectivamente 23 pacientes. En aquellas
con insuficiencia cardÃaca descompensada se realizó monitorización hemodinámica. La mediana
de seguimiento fue de 7,3 años (3,2-17,5). El análisis univariado se realizó por regresión de
Cox y la supervivencia global se calculó con el método de Kaplan-Meier.
Resultados
La edad media fue de 28,7 ± 8,8 años, ocho pacientes eran multÃparas. El 73% estaban en
clase funcional III-IV. La presión arterial sistólica y diastólica fue de 103 ± 23 y 67 ± 11 mm
Hg, respectivamente, y la frecuencia cardÃaca, de 92 ± 19 lpm. El 100% se encontraba en
ritmo sinusal. El Ãndice cardiotorácico fue de 0,56 ± 0,07. El diámetro diastólico y sistólico
del ventrÃculo izquierdo fue de 67,5 ± 10,2 y 56,7 ± 10,1 mm, respectivamente, el diámetro
auricular izquierdo fue de 42,5 ± 6 mm y la fracción de eyección del ventrÃculo izquierdo, del
24,6% ± 10,8%. La presión media de la arteria pulmonar fue de 25 ± 9 mm Hg y la capilar
pulmonar, de 18,4 ± 7,8 mm Hg; el Ãndice cardÃaco fue de 2,6 ± 0,6 L/min/m2. Siete pacientes
fallecieron y tres fueron sometidas a trasplante cardÃaco. En el análisis univariado, la clase
funcional, el Ãndice cardÃaco, la presión arterial sistólica y diastólica, la presión capilar y
pulmonar media, el Ãndice cardiotorácico y el diámetro auricular izquierdo se asociaron con
mortalidad y trasplante cardÃaco. La supervivencia a 1, 3 y 6 años fue del 91%, 82% y 64%,
respectivamente.
Conclusiones
La mortalidad hospitalaria fue del 4,3% y el requerimiento de trasplante cardÃaco o la muerte
en el seguimiento fueron del 39%. Los parámetros hemodinámicos al ingreso fueron los
principales predictores de mortalidad y de trasplante
Outcomes of heart transplantation based on recipient's clinical profile: 21-year experience
Introducción: El trasplante cardÃaco continúa siendo el tratamiento de elección en pacientes con miocardiopatÃas graves sin otras opciones terapéuticas. Los resultados alentadores del trasplante cardÃaco en términos de supervivencia han permitido ampliar los criterios de selección del receptor, lo que ha llevado a la inclusión de pacientes de mayor complejidad en lista de espera. Objetivo: Analizar los resultados del trasplante cardÃaco del Hospital Universitario Fundación Favaloro a lo largo de 21 años de seguimiento. Material y métodos: Entre febrero de 1993 y diciembre de 2014 se realizaron 442 trasplantes cardÃacos ortotópicos en un único centro. Se analizaron en forma retrospectiva las historias clÃnicas de los pacientes, excluyéndose los pediátricos y protegiendo la confidencialidad de los datos. Para el análisis se dividió la serie en perÃodo 1 (febrero 1993 - agosto 2003) y perÃodo 2 (septiembre 2003 - diciembre 2014). Resultados: Se observó durante el segundo perÃodo una prevalencia mayor de candidatos con miocardiopatÃa dilatada no coronaria versus coronaria y un incremento significativo de la indicación de trasplante cardÃaco en la miocardiopatÃa dilatada chagásica. La prevalencia de hipertensión pulmonar aumentó, realizándose un número mayor de trasplantes cardÃacos en condición de emergencia, con requerimiento de inotrópicos y soporte circulatorio mecánico con balón de contrapulsación intraaórtico. Conclusiones: Se han observado cambios en el perfil clÃnico de los receptores de trasplante cardÃaco, ingresando en lista de espera pacientes con un número mayor de comorbilidades. La cuidadosa elección de candidatos a trasplante requiere una revisión continua y un análisis individualizado de los diferentes factores que determinan la supervivencia de los pacientes y su impacto en los resultados de los programas de trasplante.Background: Heart transplantation is still the treatment of choice in patients with severe cardiomyopathies that do not have any therapeutic options. The promising results of heart transplantation in terms of survival have modified the criteria of recipients’ selection, including more complex patients on the waiting list. Objective: The aim of this report is to analyze the outcomes of heart transplantation performed at the Hospital Universitario Fundación Favaloro after 21 years of follow-up. Methods: Between February 1993 and December 2014, 442 orthotopic heart transplantations were performed at a single center. The clinical records of the recipients, excluding pediatric patients, were retrospectively analyzed, ensuring the confidentiality of the information. Two periods were considered: period 1 (from February 1993 to August 2003) and period 2 (from September 2003 to December 2014). Results: During the second period, the prevalence of recipients with non ischemic versus ischemic dilated cardiomyopathy was higher, and the indication of heart transplantation in Chagas dilated cardiomyopathy showed a significant increase. The prevalence of pulmonary hypertension increased and more emergency heart transplantations were performed, with requirements of inotropic agents and mechanical circulatory support with intraaortic balloon pump. Conclusions: The clinical profile of heart transplantation recipients has changed and more patients on the waiting list have a greater number of comorbidities. The careful selection of candidates for transplantation needs continuous revision and an individual analysis of the different factors that determine patients’ survival and its impact on the outcomes of transplantation programs.Fil: Peradejordi Lastras, Margarita Ana. Orthotopic Heart Transplantation: Results from the Hospital Universitario Fundación Favaloro
Background The favorable outcomes achieved during four decades of clinical experience have consolidated heart transplantation as the treatment of choice of refractory heart failure. ObjectiveTo report the outcomes of patients undergoing orthotopic heart transplantation at the Material and MethodsA total of 335 consecutive orthotopic heart transplantations performed in 333 patients from February 1993 to December 2009 were retrospectively analyzed. Follow-up was 94%. Seven percent corresponded to pediatric heart transplantation and 2% to heart-kidney transplantation. Thirty five percent of patients had ischemic cardiomyopathy. Transplants were elective procedures in 42% of cases (142 patients) and 58% (193 patients) were urgent/emergency procedures. Survival was analyzed using Kaplan-Meier curves.ResultsSurvival rates at 1, 5, 10 and 15 years were 80%, 74%, 71% and 65%, respectively. There were no significant differences between elective and urgent/emergency procedures. Survival was also similar for the different etiologies, even in patients with Chagas cardiomyopathy. In-hospital mortality was 12.5%, with no significant differences between elective and urgent/emergency procedures. After the first year, the incidence of severe cellular rejection was < 3%. The global incidence of humoral rejection was 4.5%. Lymphoproliferative syndrome was diagnosed in 11 patients (3.5%).
