    Interaoperative frozen section consultation: an analysis of accuracy in a teaching hospital.

    This Is a retrospective quality assurance study of all frozen sections done at The Aga Khan University Hospital during a six year period (1986 to 1991). There were 1,031 frozen sections out of a cumulative total of 42,985 surgical specimens (2.39%). Nine hundred and seventy-six (94.66%) were concordant. In 92(8.9%) fresh specimens were brought from other hospitals of Karachi, in 37 cases (3.58%) the diagnosis was deferred till the evaluation of permanent paraffin sections and 18 (1,74%) were discordant with 7 (0.67%) false positive and 11(1,06%) false negative. Among the discordant cases, 9 were attributed to misinterpretation, 7 due to sampling errors and 2 due to technical reasons. Some of these errors might have been avoided, but appear to be an Irreducible minimum

    Enhancing the catalytic activity of eggshell-derived CaO catalyst and its application in biodiesel production from waste chicken fat.

    The comparatively greater cost of producing biodiesel in comparison to petroleum diesel is one of the key drawbacks. Eggshells and leftover chicken fat are examples of poultry wastes that can be used to produce biodiesel at a low cost as catalysts and oil, respectively. In this study, eggshell-derived CaO and its doping with sodium methoxide catalyst for enhancing catalytic activity was synthesized for the transesterification of waste chicken fat and characterized by FT-IR and XRD analyses. XRD studies confirmed the crystalline structure of the developed catalyst and doping of sodium with eggshell-derived CaO. The transesterification reaction was performed at different reaction parameters such as the catalyst loading, the methanol to oil ratio, the reaction temperature, and the reaction time. The biodiesel produced at the maximum yield was evaluated by gas chromatography mass spectrometry analysis. A maximum yield of 96% biodiesel was obtained with catalyst loading of 2 wt% of oil, as well as a methanol to oil ratio of 13:1 at 60 °C in 1 h. The output demonstrates that eggshell waste is a potentially accessible source of biomass-derived nano catalyst for the synthesis of biodiesel using chicken fat as a feedstock

    The development and validation of a scoring tool to predict the operative duration of elective laparoscopic cholecystectomy

    Background: The ability to accurately predict operative duration has the potential to optimise theatre efficiency and utilisation, thus reducing costs and increasing staff and patient satisfaction. With laparoscopic cholecystectomy being one of the most commonly performed procedures worldwide, a tool to predict operative duration could be extremely beneficial to healthcare organisations. Methods: Data collected from the CholeS study on patients undergoing cholecystectomy in UK and Irish hospitals between 04/2014 and 05/2014 were used to study operative duration. A multivariable binary logistic regression model was produced in order to identify significant independent predictors of long (> 90 min) operations. The resulting model was converted to a risk score, which was subsequently validated on second cohort of patients using ROC curves. Results: After exclusions, data were available for 7227 patients in the derivation (CholeS) cohort. The median operative duration was 60 min (interquartile range 45–85), with 17.7% of operations lasting longer than 90 min. Ten factors were found to be significant independent predictors of operative durations > 90 min, including ASA, age, previous surgical admissions, BMI, gallbladder wall thickness and CBD diameter. A risk score was then produced from these factors, and applied to a cohort of 2405 patients from a tertiary centre for external validation. This returned an area under the ROC curve of 0.708 (SE = 0.013, p  90 min increasing more than eightfold from 5.1 to 41.8% in the extremes of the score. Conclusion: The scoring tool produced in this study was found to be significantly predictive of long operative durations on validation in an external cohort. As such, the tool may have the potential to enable organisations to better organise theatre lists and deliver greater efficiencies in care

    In this article, a new "odds generalized gamma-G" family of distributions, called the GG-G family of distributions, is introduced. We propose a complete mathematical and statistical study of this family, with a special focus on the Fréchet distribution as baseline distribution. In particular, we provide infinite mixture representations of its probability density function and its cumulative distribution function, the expressions for the Rényi entropy, the reliability parameter and the probability density function of ith order statistic. Then, the statistical properties of the family are explored. Model parameters are estimated by the maximum likelihood method. A regression model is also investigated. A simulation study is performed to check the validity of the obtained estimators. Applications on real data sets are also included, with favorable comparisons to existing distributions in terms of goodness-of-fit
