    Some properties of skew Hurwitz series

    In this paper we show that, if R is a ring and σ an endomorphism of R, then the skew Hurwitz series ring T = (HR, σ ) is an n-clean ring if and only if R is an n-clean ring. Moreover, if R is an integral domain and a torsion-free Z-module, then T = (HR, σ ) is a Prufer domain if and only if R is a field. Also, we investigate when the ring T = (HR, σ ) is g(x)-clean, (n, g(x))-clean and a Neat ring

    Effect of magnesium sulphate on emergence agitation in children undergoing adenotonsillectomy

    Background: There is some evidence that intraoperative magnesium infusions prevent postoperative delirium in children.Objective: The aim of the work was to study the effects of intra-operative magnesium sulphate administration on the children's risk of post-adenotonsillectomy post-agitation.Patients and Methods: At Anesthesia & Surgical Intensive Care Department, Zagazig University Hospitals, this randomized, double-blind clinical trial included 90 children with American Society of Anesthesiologists (ASA) physical status classes I or II who were scheduled to undergo adenotonsillectomy. Patients were allocated randomly into two equal groups (45 for each group): Group C: received intraoperative 0.9% normal saline initial loading dose of 0.3ml/kg over 10 min as a bolus, then 0.1ml/kg/hr. of infusion for the length of the procedure. Group MG: Patients were given 30 mg/kg of a 10% solution of magnesium sulphate intraoperatively over the course of 10 minutes as an initial loading dose, then 0.1ml/kg/hr. of infusion for the length of the procedure.Results: Group MG was significantly lower than Group C regarding fentanyl needed and also total dose of fentanyl. As regard Pediatric Anesthesia Emergence Delirium (PAED) score, we found that there was No significant difference regarding pre but at post group MG was significantly lower at post also agitation was significantly associated with Group C.Conclusion: It could be concluded that sevoflurane anesthesia using magnesium sulphate is used for adenotonsillectomy surgery in children. A 30-mg/kg loading dose is given intraoperatively, followed by a continuous infusion at 0.1ml/kg/hr. for the duration of surgery. This combination reduces the incidence of emergent anxiety and the need for postoperative fentanyl

    Role of magnetic resonance imaging in the evaluation of the popliteus musclotendinous injuries as a part of the posterolateral corner injuries of the knee

    Introduction: The popliteus musclotendinous complex is considered to be one of the main stabilizers of the posterolateral corner of the knee having different attachments and variable sites of injuries, the use of MRI helps the radiologists to depict such injuries consistently.Purpose of the study: To study the role of MRI in evaluating the popliteus musclotendinous injuries as a part of the PLCof the knee.Patients and methods: Our study included 15 patients with trauma to the knee undergoing MRI using 1.5 T magnet using the following sequence: Axial, Coronal, sagittal proton density fat sat, T1, T2 and T2 gradient and conventional X-ray/CT [All patients did conventional X-ray Knee (AP and Lateral views), two patients did CT study of the knee].Results: Out of 15 studied patients with PLC injuries, eight (53.33%) patients had popliteus complex injuries, 7 (87.5%) patients showed combined injuries with other posterolateral structures while one (12.5) patient had isolated popliteus musclotendinous complex injury.Conclusion: MRI imaging is crucial in the evaluation of the different sites and patterns of injuries of the popliteus musclotendinous complex when suspecting PLC injury.Keywords: Popliteus muscle, Popliteus tendon, Posterolateral corner, Popliteofibular ligament, MR

    An Investigation of Multidimensional Voice Program Parameters in Three Different Databases for Voice Pathology Detection and Classification

    Background and Objective Automatic voice-pathology detection and classification systems may help clinicians to detect the existence of any voice pathologies and the type of pathology from which patients suffer in the early stages. The main aim of this paper is to investigate Multidimensional Voice Program (MDVP) parameters to automatically detect and classify the voice pathologies in multiple databases, and then to find out which parameters performed well in these two processes. Materials and Methods Samples of the sustained vowel /a/ of normal and pathological voices were extracted from three different databases, which have three voice pathologies in common. The selected databases in this study represent three distinct languages: (1) the Arabic voice pathology database; (2) the Massachusetts Eye and Ear Infirmary database (English database); and (3) the Saarbruecken Voice Database (German database). A computerized speech lab program was used to extract MDVP parameters as features, and an acoustical analysis was performed. The Fisher discrimination ratio was applied to rank the parameters. A t test was performed to highlight any significant differences in the means of the normal and pathological samples. Results The experimental results demonstrate a clear difference in the performance of the MDVP parameters using these databases. The highly ranked parameters also differed from one database to another. The best accuracies were obtained by using the three highest ranked MDVP parameters arranged according to the Fisher discrimination ratio: these accuracies were 99.68%, 88.21%, and 72.53% for the Saarbruecken Voice Database, the Massachusetts Eye and Ear Infirmary database, and the Arabic voice pathology database, respectively

    RURAL WOMEN ADOPTION OF ENVIRONMENTAL PRACTICES IN MENUFIYA GOVERNORATE

    This study aimed basically at identifying the level of rural women adoption of environmental practices in Menufiya Governorate. This main obective was achieved throw the followinge sub-objectives : determining the environmental knowledge level of rural women, identifying the dgree of rural women application of environmental practices, determining the factors which correlated  with each of the environmental knowledge level and dgree of rural women application of  environmental  practiece. The study was carried out in two villeges in Menufiya Governorate, the first villege was Monshiat Bakhaty which belongs to Shebien Elkom district and the sconede villege was Sahel Elgawaber which belongs to El shohadaa district. 100 rural women were selected randomly from each village. The study findings were: the environmental knowledge level was high as 83.5% from respondents. About 47% from respondents were applying the environmental practices. There were four independent variables (social participation, culturel cosmoplitness, geographical cosmoplitness, age) explained together 16.3 % of the total variance of the environmental knowledge level. There were six independent variables (family monthly incom, environmental situation at the village, cultural cosmoplitness, family education, geographical cosmoplitness, environmental knowledge) explained together 55.5% of the total variance of the environmental application dgree

    Formant analysis in dysphonic patients and automatic Arabic digit speech recognition

    <p>Abstract</p> <p>Background and objective</p> <p>There has been a growing interest in objective assessment of speech in dysphonic patients for the classification of the type and severity of voice pathologies using automatic speech recognition (ASR). The aim of this work was to study the accuracy of the conventional ASR system (with Mel frequency cepstral coefficients (MFCCs) based front end and hidden Markov model (HMM) based back end) in recognizing the speech characteristics of people with pathological voice.</p> <p>Materials and methods</p> <p>The speech samples of 62 dysphonic patients with six different types of voice disorders and 50 normal subjects were analyzed. The Arabic spoken digits were taken as an input. The distribution of the first four formants of the vowel /a/ was extracted to examine deviation of the formants from normal.</p> <p>Results</p> <p>There was 100% recognition accuracy obtained for Arabic digits spoken by normal speakers. However, there was a significant loss of accuracy in the classifications while spoken by voice disordered subjects. Moreover, no significant improvement in ASR performance was achieved after assessing a subset of the individuals with disordered voices who underwent treatment.</p> <p>Conclusion</p> <p>The results of this study revealed that the current ASR technique is not a reliable tool in recognizing the speech of dysphonic patients.</p
