    Sol–gel synthesis and characterization of Co-doped LSGM perovskites

    One of the major requirements for the development and commercialization of low-cost SOFCs is the reduction in the operating temperature. One of the methods to reach this aim is the use of solid electrolytes which exhibit superior ionic conductivity at intermediate temperatures (IT, T < 800 °C). Among these ionic conductors, doped LaGaO3 materials show high oxide ionic conductivity in the 600–800 °C range. These perovskites are usually prepared by time- and energy-consuming solid state reaction. In this paper, La0.8Sr0.2Ga0.8Mg0.2O2.8 (LSGM) and La0.8Sr0.20Ga0.8Mg0.2−xCoxO3−δ (LSGMC) powders containing different amounts of Co (x = 0.05, 0.085 and 0.10) were prepared from precursors synthesised by citrate sol–gel method. The precursors were calcined at 1000 °C (10 h) and dense high-purity pellets were obtained by pressing (300 MPa) and sintering in air at 1475 °C (5, 10 and 20 h). Sintered pellets of LSGM and LSGMC contained very small amounts (<1%) of SrLaGa3O7 and SrLaGaO4, respectively, as detected by X-ray diffraction (XRD) and by the combined use of scanning electron microscopy (SEM) and energy dispersive spectroscopy (EDS).     Sol–gel synthesis, X-ray photoelectron spectroscopy and electrical conductivity of Co-doped (La, Sr)(Ga, Mg)O3−δ perovskites

    La0.8Sr0.2Ga0.8Mg0.2−xCoxO3−δ (LSGMC) powders containing different amounts of Co (x = 0.05 and 0.085) were prepared by a citrate sol–gel method. The powders were used to prepare highly phase-pure LSGMC sintered pellets with controlled composition and fractional densities larger than 95%. For the first time, LSGMC materials were subjected to X-ray photoelectron spectroscopy (XPS) characterization. XPS data confirmed the presence of the dopants in the material and allowed to identify two different chemical states for Sr2+ and oxygen, both related to the oxygen-deficient perovskite structure of LSGMC. The conductivity of LSGMC sintered pellets containing different amounts of Co ions in the B sites of the perovskite lattice was assessed by electrochemical impedance spectroscopy (EIS) in the 250–750 °C temperature range. Conductivity values and apparent activation energies were in good agreement with previously published data referring to materials with same composition, but prepared by solid-state route. Therefore, the physicochemical and electrochemical characterization clearly demonstrated the ability of sol–gel methods to produce high-purity Co-doped LSGM perovskites, which represent promising solid electrolytes for intermediate-temperature SOFCs

    Avaliação do risco cardiovascular em pacientes obesos hipertensos com síndrome metabólica: estudo ARCOS

    Weight loss improves metabolic abnormalities and reduces cardiovascular risk in obese hypertensive patients. To evaluate the impact of a sustained weight loss on coronary risk, 181 hypertensive patients with metabolic syndrome underwent to orlistat therapy, 120 mg, t.i.d., plus diet for 36 weeks. During therapy, Framingham risk scores (FRS) were calculated for determination of coronary heart disease risk in ten years. Body mass index decreased from 35.0 ± 4.2 to 32.6 ± 4.5 kg/m² (p 5% occurred in 64.6% of all patients, associated with improvement in glucose metabolism. Among those with abnormal glucose metabolism, 38 out 53 patients (71.7%) improved their glucose tolerance (p 5% em 64,6% de todos os pacientes, associada com melhora no metabolismo da glicose. Entre os 53 pacientes com metabolismo de glicose anormal, 38 (71,7%) melhoraram sua tolerância à glicose (p< 0,0005).     Impact of atrial fibrillation in critically ill patients admitted to a stepdown unit

    Background: Limited data are available on the clinical course of patients with history of atrial fibrillation (AF) when admitted in an intensive care environment. We aimed to describe the occurrence of major adverse events in AF patients admitted to a stepdown care unit (SDU) and to analyse clinical factors associated with outcomes, impact of dicumarolic oral anticoagulant (OAC) therapy impact and performance of clinical risk scores in this setting. Materials and methods: Single-centre, observational retrospective analysis on a population of subjects with AF history admitted to a SDU. Therapeutic failure (composite of transfer to ICU or death) was considered the main study outcome. Occurrence of stroke and major bleeding (MH) was considered as secondary outcomes. The performance of clinical risk scores was evaluated. Results: A total of 1430 consecutive patients were enrolled. 194 (13.6%) reported the main outcome. Using multivariate logistic regression, age (odds ratio [OR]: 1.03, 95% confidence interval [CI]: 1.01-1.05), acute coronary syndrome (OR:3.10, 95% CI: 1.88-5.12), cardiogenic shock (OR:10.06, 95% CI: 5.37-18.84), septic shock (OR:5.19,95%CI:3.29-18.84), acute respiratory failure (OR:2.49, 95% CI: 1.67-3.64) and OAC use (OR: 1.61, 95% CI: 1.02-2.55) were independently associated with main outcome. OAC prescription was associated with stroke risk reduction and to both MH and main outcome risk increase. CHA2 DS2 -VASc (c-index: 0.545, P = .117 for stroke) and HAS-BLED (c-index:0.503, P = .900 for MH) did not significantly predict events occurrence. Conclusions: In critically ill AF patients admitted to a SDU, adverse outcomes are highly prevalent. OAC use is associated to an increased risk of therapeutic failure, clinical scores seem unhelpful in predicting stroke and MH, suggesting a highly individualized approach in AF management in this setting

    Numerical investigation of bubble dynamics and flow boiling heat transfer in cylindrical micro-pin-fin heat exchangers

    Micro-pin-fin evaporators are a promising alternative to multi-microchannel heat sinks for two-phase cooling of high power-density devices. Within pin-fin evaporators, the refrigerant flows through arrays of obstacles in cross-flow and is not restricted by the walls of a channel. The dynamics of bubbles generated upon flow boiling and the associated heat transfer mechanisms are expected to be substantially different from those pertinent to microchannels; however, the fundamental aspects of two-phase flows evolving through micropin-fin arrays are still little understood. This article presents a systematic analysis of flow boiling within a micro-pin-fin evaporator, encompassing bubble, thin-film dynamics and heat transfer. The flow is studied by means of numerical simulations, performed using a customised boiling solver in OpenFOAM v2106, which adopts the built-in geometric Volume of Fluid method to capture the liquid–vapour interface dynamics. The numerical model of the evaporator includes in-line arrays of pin-fins of diameter of 50 μm and height of 100 μm, streamwise pitch of 91.7 μm and cross-stream pitch of 150 μm. The fluid utilised is refrigerant R236fa at a saturation temperature of 30 ◦C. The range of operating conditions simulated includes values of mass flux = 500–2000 kg∕(m2 s), heat flux = 200 kW∕m2 , and inlet subcooling = 0–5 K. This study shows that bubbles nucleated in a pin-fin evaporator tend to travel along the channels formed in between the pin-fin lines. Bubbles grow due to liquid evaporation and elongate in the direction of the flow, leaving thin liquid films that partially cover the pin-fins surface. The main contributions to heat transfer arise from the evaporation of this thin liquid film and from a cross-stream convective motion induced by the bubbles in the gap between the cylinders, which displace the hot fluid otherwise stagnant in the cylinders wakes. When the mass flow rate is increased, bubbles depart earlier from the nucleation sites and grow more slowly, which results in a reduction of the two-phase heat transfer. Higher inlet subcooling yields lower two-phase heat transfer coefficients because condensation becomes important when bubbles depart from the hot pin-fin surfaces and reach highly subcooled regions, thus reducing the two-phase heat transfer

    Uropathogenic Escherichia coli (UPEC) strains may carry virulence properties of diarrhoeagenic E-coli

    To analyze whether Escherichia coli strains that cause urinary tract infections (UPEC) share virulence characteristics with the diarrheagenic E. coli (DEC) pathotypes and to recognize their genetic diversity, 225 UPEC strains were examined for the presence of various properties of DEC and UPEC (type of interaction with HeLa cells, serogroups and presence of 30 virulence genes). No correlation between adherence patterns and serogroups was observed. Forty-five serogroups were found, but 64% of the strains belonged to one of the 12 serogroups (O1, O2, O4, O6, O7, O14, O15, O18, O21, O25, O75, and O175) and carried UPEC virulence genes (pap, hly, aer, sfa, cnf). the DEC genes found were: aap, aatA, aggC, agg3C, aggR, astA, eae, ehly, iha, irp2, lpfA(O113), pet, pic, pilS, and shf. Sixteen strains presented aggregative adherence and/or the aatA sequence, which are characteristics of enteroaggregative E. coli (EAEC), one of the DEC pathotypes. in summary, certain UPEC strains may carry DEC virulence properties, mostly associated to the EAEC pathotype. This finding raises the possibility that at least some faecal EAEC strains might represent potential uropathogens.     Geranylgeranyltransferase I Inhibitors Target RalB To Inhibit Anchorage-Dependent Growth and Induce Apoptosis and RalA To Inhibit Anchorage-Independent Growth

    Geranylgeranyltransferase I inhibitors (GGTIs) are presently undergoing advanced preclinical studies and have been shown to disrupt oncogenic and tumor survival pathways, to inhibit anchorage-dependent and -independent growth, and to induce apoptosis. However, the geranylgeranylated proteins that are targeted by GGTIs to induce these effects are not known. Here we provide evidence that the Ras-like small GTPases RalA and RalB are exclusively geranylgeranylated and that inhibition of their geranylgeranylation mediates, at least in part, the effects of GGTIs on anchorage-dependent and -independent growth and tumor apoptosis. To this end, we have created the corresponding carboxyl-terminal mutants that are exclusively farnesylated and verified that they retain the subcellular localization and signaling activities of the wild-type geranylgeranylated proteins and that Ral GTPases do not undergo alternative prenylation in response to GGTI treatment. By expressing farnesylated, GGTI-resistant RalA and RalB in Cos7 cells and human pancreatic MiaPaCa2 cancer cells followed by GGTI-2417 treatment, we demonstrated that farnesylated RalB, but not RalA, confers resistance to the proapoptotic and anti-anchorage-dependent growth effects of GGTI-2417. Conversely, farnesylated RalA but not RalB expression renders MiaPaCa2 cells less sensitive to inhibition of anchorage-independent growth. Furthermore, farnesylated RalB, but not RalA, inhibits the ability of GGTI-2417 to suppress survivin and induce p27Kip1 protein levels. We conclude that RalA and RalB are important, functionally distinct targets for GGTI-mediated tumor apoptosis and growth inhibition

    Idiopathic isolated clitoromegaly: A report of two cases

    BACKGROUND: Clitoromegaly is a frequent congenital malformation, but acquired clitoral enlargement is relatively rare. METHODS: Two acquired clitoromegaly cases treated in Atatürk Training Hospital, Izmir, Turkey are presented. RESULTS: History from both patients revealed clitoromegaly over the last three years. Neither gynecological nor systemic abnormalities were detected in either patient. Karyotype analyses and hormonal tests were normal. Abdominal and gynaecological ultrasound did not show any cystic lesion or other abnormal finding. Computerized tomography scan of the adrenal glands was normal. Clitoroplasty with preservation of neurovascular pedicles was performed for the treatment of clitoromegaly. CONCLUSION: The patients were diagnosed as "idiopathic isolated" clitoromegaly. To the best of our knowledge, there has been no detailed report about idiopathic clitoromegaly in the literature

    CHA2DS2-VASc score predicts atrial fibrillation recurrence after cardioversion: systematic review and individual patient pooled meta-analysis

    BackgroundDespite progresses in the treatment of the thromboembolic risk related to atrial fibrillation (AF), the management of recurrences remains a challenge.HypothesisTo assess if congestive heart failure or left ventricular systolic dysfunction (CHA2DS2‐VASc) score is predictive of early arrhythmia recurrence after AF cardioversion.MethodsSystematic review and individual patient pooled meta‐analysis following Preferred Reporting Items for Systematic reviews and Meta‐Analyses guidelines. Inclusion criteria: observational trials in patients with AF undergoing cardioversion, available data on recurrence of AF and available data on CHA2DS2‐VASc score. Clinical studies of interest were retrieved by PubMed, Cochrane Library, and Biomed Central. Seven authors were contacted for joining the patient level meta‐analysis, and three shared data regarding anthropometric measurements, risk factors, major comorbidities, and CHA2DS2‐VASc score. The primary outcome was the recurrence of AF after cardioversion in patients free from antiarrhythmic prophylaxis. Univariate and multivariate logistic regression was performed.ResultsOverall, we collect data of 2889 patients: 61% were male, 50% with hypertension, 12% with diabetes, and 23% with history of ischemic heart disease. The median CHA2DS2‐VASc score was 2.. At the multivariate analysis, chronic kidney disease (odds ratio [OR] 1.94; 95% confidence interval [CI] 1.12‐3.27; P = 0.01), peripheral artery disease (OR 1.65; 95% CI 1.23‐2.19; P P  2 (OR 1.37; 95% CI 1.1‐1.68; P = 0.002) were independent predictors of early recurrence of AF.ConclusionsCHA2DS2‐VASc score predicts early recurrence of AF in the first 30 days after electrical or pharmacological cardioversion.N
