    Anterior Cervical Corpectomy with Cage Fixation for Cervical Spondylotic Myelopathy

    Objective:  This study aimed to report the clinical outcome of anterior cervical corpectomy with cage fixation in patients with cervical spondylotic myelopathy. Material and Methods:  This observational retrospective study included 36 patients from the Neurosurgery department of Lady Ready Hospital MTI, Peshawar from 2014 January to 2015 December. After performing surgery, the patients were followed up for six months for neurological outcome and various post-operative complications such as infection, transient recurrent laryngeal palsy, screw displacement and improvements in paresthesias and gait ataxia. Results:  Most of the patients have no post-operative complications. Seventy-five percent (n = 27) of patients reported an immediate improvement in paresthesia and fine hand movements and gait. The major reported complications were implant failure (5.55%) and recurrent laryngeal nerve transient palsy in two patients (5.55%) each. Conclusion:  In patients with cervical spondylotic myelopathy, with anterior compression, cervical corpectomy with cage fixation is less invasive and an effective procedure with acceptable outcomes

    A Features-Based Privacy Preserving Assessment Model for Authentication of Internet of Medical Things (IoMT) Devices in Healthcare

    Internet of Things (IoT) devices have drawn significant attention over the last few years due to their significant contribution to every domain of life, but the major application of these devices has been witnessed in the healthcare sector. IoT devices have changed the complexion of healthcare set-up, however, the major limitation of such devices is susceptibility to many cyberattacks due to the use of embedded operating systems, the nature of communication, insufficient software updates, and the nature of backend resources. Similarly, they transfer a huge amount of sensitive data via sensors and actuators. Therefore, the security of Internet of Health Things (IoHT) devices remains a prime concern as these devices are prone to various cyberattacks, which can lead to compromising and violating the security of IoT devices. Therefore, IoT devices need to be authenticated before they join the network or communicate within a network, and the applied method of authentication must be robust and reliable. This authentication method has to be evaluated before being implemented for the authentication of IoT devices/equipment in a healthcare environment. In this study, an evaluation framework is introduced to provide a reliable and secure authentication mechanism based on authentication features. The proposed framework evaluates and selects the most appropriate authentication scheme/method based on evaluating authentication features using a hybrid multicriteria decision-making approach. It completes this in two steps: in the first step, the analytic hierarchy process (AHP) method is applied for assigning criteria weights; and in the second step, the technique for order preference by similarity to ideal solution (TOPSIS) approach selects the best authentication solution for IoHT devices based upon identified authentication features.     Polymer Stabilized Metal Nanoparticles for Catalytic Degradation of Methylene Blue in Water

    Methylene blue is highly toxic and releases from various industries. It must be transformed into less toxic compounds. The Core-Shell microgels p (Pst core), Pstcore-NIPMamm-MAa and Ag in Pst-p NIPMamm-MAa have been synthesized using the Core-Shell hybrid micro gelling NIPMamm-MAa emulsion polymerization process. The 0.086mM, MB 6.2mM NaBH4 and 0.2916 mg / mL catalysts in the cuvette were measured using UV-visible spectrophotometers. Spectrums were measured at a one-minute interval. The peak at 600 nm steadily decreased over time and was completely eliminated after 11 minutes. Without the catalyst, MB decreases with NaBH4 which showed that the reaction decreases were slow and MB very high within 120 minutes. The Psty core of FT-IR core microgels, pNIpmam-MMAA, and Ag-pNIpmam-MAA core microgels are hybrids. At 2955 and 2845 cm−1 FT-IR spectra, Psty NiPMaM – MaA and Ag-p NiPMaM – MaA were used for core shell microgels, with C-H vibrations expanding the aromatic ring. In this study degradation of Methylene were carried out with Ag- Nanocomposites at different interval of time to check the degradation at minimum time.     Nordic environmental resilience:balancing air quality and energy efficiency by applying artificial neural network

    Maintaining public health and environmental safety in the Nordic nations calls for a strict plan to define exact benchmarks on air quality and energy efficiency. This study investigates the complicated interaction of decentralized energy production (DEP) with energy efficiency, and air quality index in the Nordic nations from 1990 to 2022 using System GMM and Artificial Neural Network (ANN) approach. Our research explored positive role of decentralized energy production and technological advancement to propel notable increases in energy efficiency, hence lowering pollution expressed as PM2.5 level. Our research indicates, however, that although international trade, GDP and urbanization assist to enhance energy efficiency, they also contribute to pollution by raising PM2.5 Level by higher energy usage. Furthermore damaging to environmental quality is the persistent link shown by economic disparity and the energy price index with increased degrees of pollution and less energy efficiency. Policy frameworks must devised sustainable development policy (decentralized energy production) to significantly improve energy efficiency and lower the amount of pollution. This calls for proper urban planning and a close observation of the possible drawbacks of growing GDP, trade, economic disparity, and energy expenses

    Computational and pharmacological evaluation of stevioside derivatives for antinociceptive and antiinflammatory potential

    Purpose: To carry out computational and pharmacological evaluation of two stevioside derivatives in order to develop more effective candidates for analgesia and inflammation.Methods: Primarily, compounds were docked against targets of nociception and inflammation such as cyclooxygenase-1, cyclooxygenase-2, 5-lypooxygenase 12-lypooxygenase, 15-lypooxygenase, prostaglandin synthase, leukotrienes C4 synthase, mu, kappa, and delta receptors to obtain their possible binding modes. Test compounds were then screened in animal model of nociception and inflammation.Results: The results of docking show that IO possesses good affinity when compared to ID. IO showed two hydrogen bonds against COX-1 and COX-2. IO also demonstrated good binding against 5-LOX, 12- LOX and 15-LOX, exhibited four, one and two hydrogen bonds respectively. Against PG synthase and LTC4, both IO and ID produced moderate binding. IO also showed significant binding against opoid receptors (p < 0.05). IO and ID significantly decrease the number of writhes to 21.20 ± 2.1 and 27.0 ± 2.12 at 10 mg/kg in acetic acid mediated pain test respectively. In hot plate method, IO and ID increase the latency period of mice to 14.14 ± 0.40 and 10.50 ± 0.34 s, respectively. IO and ID significantly reduced the paw edema to 1.69 ± 0.14 and 1.94 ± 0.14 mL, respectively, in acute inflammation (p < 0.05). In chronic inflammatory model, IO and ID decreased paw volume to 3.26 ± 0.38 and 4.20 ± 0.38 mL, respectively.Conclusion: The results show that IO is a promising candidate for further development as analgesic and anti-inflammatory agents. However, their pharmacokinetic and pharmacodynamic profiles need to be investigated. Keywords: Computational, Stevioside, Docking, Analgesic, Anti-inflammator

    Hospital-based ultra-sonographic prevalence and spectrum of thyroid incidentalomas in Pakistani population

    Introduction: Thyroid incidentalomas (TIs) are clinically asymptomatic nodules found accidentally during imaging studies ordered for some other reasons. Being easily accessible, non-invasive, and inexpensive, thyroid ultrasound (US) is a key investigation in the management of thyroid nodules.Methods: This ultrasound-based cross-sectional study was performed in the radiology department of a major tertiary care hospital. Every second patient visiting the emergency department was a potential candidate for a thyroid ultrasound. Patients having ages greater than 20 years were included in the study.Results: A total of 250 patients were included in the study. Out of these, 175 were female and 75 were male. The majority (54.80%) were in the age group 21-30 years. Nodules were found in 65 (26%) patients and in the majority of cases (67.7%) they were multiple in number. Associated lymphadenopathy was seen in only one patient. Thyroid nodules were more common in females as compared to males (75.38% versus 24.62%). According to Thyroid Imaging and Reporting Data System (TI-RADS) classification, the majority of the nodules were falling in TI-RADS 1 (74%) followed by TI-RADS 3 (9.60%) and 4A (8.80%).Conclusion: The thyroid nodules are more commonly seen in females as compared to males. A significant association is seen between the frequency of thyroid nodules and increasing age. The majority of thyroid nodules fall in TI-RADS 1 category followed by TI-RADS 3 and 4A
