    WASTE MANAGEMENT OF PERSONAL PROTECTION ELEMENTS IN INDUSTRIES

    El uso de Elementos de protección personal (EPPs) es obligatorio, ya que ayudan en la mitigación de lesiones o daños en la salud de los trabajadores. Sin embargo, la disposición final de los EPPs se desconoce, y se presume que estos elementos son dispuestos directamente en los rellenos sanitaros, o llegan directamente a áreas naturales. El objetivo del estudio fue analizar los aspectos económicos y ambientales de la incorporación de los residuos de EPPs a la economía circular. El estudio se realizó con información a nivel nacional (Marca A) y local (Marca B). Los datos se segmentaron por familias de EPPs, se cuantificaron y caracterizaron según su material de fabricación y peso. However, the final disposition of the PPEs is unknown, and it is presumed that these elements are disposed of directly in sanitary landfills, or arrive directly in natural areas.     Los comerciantes minoristas de Mercaneiva de la ciudad de Neiva (Huila), han tenido una dinámica en la forma administrar de los negocios, que les ha generado un descontrol de sus finanzas. Por lo tanto, de acuerdo con sus necesidades se diseñó un prototipo de software en Visual Basic para Aplicaciones (VBA) (Manual Microsoft Office Excel 2010), que permitió sistematizar el registro y control de sus inventarios y ventas, de manera fácil, ágil y segura. Su desarrollo se realizó mediante la metodología Scrum, con la programación de macros en VBA en Excel, con la facilidad de utilizarse en un computador, tablet o celular, para la mejorar la administración de la información y la toma de decisiones.Mercaneiva retailers in the city of Neiva (Huila), have had a dynamic in the way they manage their businesses, which has generated a lack of control of their finances.     Matriz, fotosEl presente artículo muestra un análisis cualitativo del proceso de la Policía Nacional a través de la puesta en marcha del programa jóvenes a lo bien que se adelanta en términos de prevención y de la importancia de implementar acciones de orden Nacional y local que permitan mejorar la seguridad y la convivencia ciudadana implementando procesos de largo aliento, el programa jóvenes a lo bien que se desarrolla en el territorio, del cual se realizando una observación en la ciudad de Bogotá en la localidad de Kennedy, cuyo proceso ha sido ininterrumpido en los últimos diez años, encontrando alta incidencia del mismo en la población de jóvenes y sus familias, mostrando un rostro diferente a la seguridad territorial y si de los funcionarios de Policía, de la forma como se realiza un abordaje para llevar un mensaje de reducción de delitos alto impacto, como el homicidio, las lesiones personales entre otras, presentan un escenario diferencial para el mejoramiento de la convivencia mediante el fortalecimiento de las habilidades para la vida, celebrando una alianza estratégica con el servicio nacional de aprendizaje SENA, donde a través de recursos propios permiten la inclusión de la comunidad joven y en riesgo, brindando la oportunidad de formarse en carreras tecnológicas, técnicas, especializaciones tecnológicas y técnicas, formación complementaria que les permita certificar sus competencias laborales que los haga productivos socialmente y líderes en sus comunidades, en este sentido el logro más importante es la alianza estratégica de las instituciones del orden nacional que posibilitan el mejoramiento de la convivencia y la seguridad desde la educación, transformando, construyendo tejido social.This article shows a qualitative analysis of the National Police process through the implementation of the youth program to how well it is advancing in terms of prevention and the importance of implementing national and local actions that allow improving security and citizen coexistence by implementing long-term processes, the youth program to how well it develops in the territory, of which an observation is made in the city of Bogotá in the town of Kennedy, whose process has been uninterrupted in the last ten years , finding a high incidence of it in the population of young people and their families, showing a different face to territorial security and yes to Police officials, in the way an approach is carried out to carry a message of reducing high impact crimes, such as homicide, personal injuries among others, present a differential scenario for the improvement of coexistence through the strengthening of life skills, celebrating a strategic alliance with the national learning service SENA, where through their own resources they allow the inclusion of the young and at-risk community, providing the opportunity to train in technological and technical careers, specializations technological and technical, complementary training that allows them to certify their work skills that make them socially productive and leaders in their communities, in this sense the most important achievement is the strategic alliance of the institutions of the national order that enable the improvement of coexistence and security from education, transforming, building social fabric

    Health Care Avoidance Among Rural Populations: Results From a Nationally Representative Survey

    BACKGROUND: Previous research suggests that certain populations, including rural residents, exhibit health care avoidant behaviors more frequently than other groups. Additionally, health care avoidance is related to sociodemographics, attitudes, social expectations, ability to pay for care, and prior experiences with providers. However, previous studies have been limited to specific geographic areas, particular health conditions, or by analytic methods. METHODS: The 2008 Health Information Trends Survey (HINTS) was used to estimate the magnitude of health care avoidance nationally and, while controlling for confounding factors, identify groups of people in the United States who are more likely to avoid health care. Chi-square procedures tested the statistical significance (P \u3c .05) of bivariate relationships. Multivariable analysis was conducted through a weighted multiple logistic regression with backward selection. RESULTS: For 6,714 respondents, bivariate analyses revealed differences (P \u3c .05) in health care avoidance for multiple factors. However, multiple regression reduced the set of significant factors (P \u3c .05) to rural residence (OR = 1.69), male sex (OR = 1.24), younger age (18-34 years OR = 2.34; 35-49 years OR = 2.10), lack of health insurance (OR = 1.43), lack of confidence in personal health care (OR = 2.24), lack of regular provider (OR = 1.49), little trust in physicians (OR = 1.34), and poor provider rapport (OR = 0.94). CONCLUSION: The results of this study will help public health practitioners develop programs and initiatives targeted and tailored to specific groups, particularly rural populations, which seek to address avoidant behavior, thereby reducing the likelihood of adverse health outcomes

    HPV Vaccination in the Virginia Context: Demographic Disparities, Patient-Provider Gender Concordance and the Impact of Changing Recommendations

    Objective: Despite its effectiveness in preventing several cancers, there are marked disparities in HPV vaccination initiation and series completion. The present study sought to understand disparities in HPV vaccinations among patients in northern Virginia (ages 9-26) and the impact of patient and provider gender concordance, in lieu of CDC’s vaccine recommendation changes in 2016, which reduced the recommended doses from three to two, in this population. Design: Analyses of electronic medical records collected from 2012 to 2017. Setting: A large health care system in Northern Virginia. Participants: A total of 37,427 patients, ages 9 to 26, were included in analyses. Main outcome measures: We examined odds of initiating vaccination, completing vaccination at 6 months, completing vaccination at 12 months and clinical completion. We examined if patient and provider characteristics were associated with initiating vaccination and vaccination completion at different durations. Results: Racial minorities had higher odds of getting vaccinated, relative to non-Latino whites. Each additional year between the patient’s first and last visit was associated with higher odds of initiating vaccination, completing vaccination at both 6 and 12 months, and clinical completion. Compared to female patients who were 19 years and older, female patients aged 9-18 years had higher odds of initiating vaccination and clinical completion. Compared to male patients who were 19 years and older, female patients aged 9-18 years had higher odds of clinical completion Female and male patients had better outcomes when seen by female primary care providers than male primary care providers. Conclusions and Relevance: Further research should investigate the observed benefit of female providers and to understand the long-term impact of changes in CDC recommendations

    Diabetes management before and after cancer diagnosis: missed opportunity

    Background Few studies have examined the management of comorbidities in cancer patients. This study used population-based data to estimate the guideline concordance rates for diabetes management before and after cancer diagnosis and examined if diabetes management services among cancer patients was associated with characteristics of the hospital where the patient was treated. Methods We linked 2005-2009 Medicare claims data to information on 2,707 breast and colorectal cancers patients in state cancer registry files. Multivariate logistic regression models examined hospital characteristics associated with receipt of diabetes management care after cancer diagnosis. Results The rates of HbAlc testing, LDL-C testing, and retinal eye exam decreased from 72.7%, 79.6%, and 57.9% before cancer diagnosis to 58.3%, 69.5%, and 55.8% after diagnosis. The pre- and post-diagnosis diabetes management care was not significantly different by hospital characteristics in the bivariate analysis except for that the distance between residence and hospital was negatively related to retinal eye exam after diagnosis (P Conclusions Cancer patients received fewer diabetes management care after diagnosis than prior to diagnosis, even for those who were treated in large comprehensive centers. This may reflect a missed opportunity to connect diabetic cancer patients to diabetes care. This study provides benchmarks to measure improvements in comorbidity management among cancer patients

    Adjuvant therapy use among Appalachian breast cancer survivors

    There is a paucity of literature systemically examining the effects of access to cancer care resources on adjuvant endocrine therapy (AET) use behaviors, especially in underserved regions such as the Appalachian region in the United States, where gaps in healthcare access are well documented. The objectives of this study were to explore AET adherence and persistence in Appalachia, delineate the effects of access to care cancer on adherence/persistence, and evaluate the influences of adherence and persistence on overall survival. A retrospective cohort study from 2006 to 2008 was conducted among female breast cancer survivors living in the Appalachian counties of 4 states (PA, OH, KY, and NC). We linked cancer registries to Medicare claims data and included patients with invasive, nonmetastatic, hormone-receptor-positive breast cancer who received guideline-recommended AET. Medication adherence was defined as corresponding to a Medication Possession Ratio (MPR) ≥0.8 and logistic regression was utilized to assess predictors of adherence. Medication nonpersistence was defined as the discontinuation of drugs after exceeding a 60-day medication gap, and multivariate adjusted estimates of nonpersistence were obtained using the Cox proportional hazards (PH) model. About 31% of the total 428 patients were not adherent to AET, and 30% were not persistent over an average follow-up period of 421 days. Tamoxifen, relative to aromatase inhibitors, was associated with higher odds of adherence (odds ratio = 2.82, P \u3c 0.001) and a lower risk of nonpersistence (hazard ratio = 0.40, P \u3c 0.001). Drug-related side effects like pain may be an important factor leading to nonadherence and early discontinuation. In addition, aromatase inhibitor (AI) adherence and persistence were significantly influenced by out-of-pocket drug costs, dual eligibility status, and coverage gaps. Nonadherence to and nonpersistence with AET were associated with higher risks of all-cause mortality. Our findings of suboptimal AET adherence/persistence in Appalachia as well as positive associations between AET adherence/persistence and overall survival outcomes further underscore the importance of ensuring appropriate AET use in this population to reduce breast cancer mortality disparities. Our findings also suggest that intervention strategies focusing on individualized treatment and medication-related factors may improve adjuvant treatment use
