    Efficacy of a Mouthwash Containing CHX and CPC in SARS-CoV-2–Positive Patients: A Randomized Controlled Clinical Trial

    Soon after the outbreak of the coronavirus disease 2019 (COVID-19) pandemic, preprocedural mouthwashes were recommended for temporarily reducing intraoral viral load and infectivity of individuals potentially infected with the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in order to protect medical personnel. Particularly, the antiseptic cetylpyridinium chloride (CPC) has shown virucidal effects against SARS-CoV-2 in vitro. Therefore, the aim of this randomized controlled clinical trial was to investigate the efficacy of a commercially available mouthwash containing CPC and chlorhexidine digluconate (CHX) at 0.05% each in SARS-CoV-2–positive patients as compared to a placebo mouthwash. Sixty-one patients who tested positive for SARS-CoV-2 with onset of symptoms within the last 72 h were included in this study. Oropharyngeal specimens were taken at baseline, whereupon patients had to gargle mouth and throat with 20 mL test or placebo (0.9% NaCl) mouthwash for 60 s. After 30 min, further oropharyngeal specimens were collected. Viral load was analyzed by quantitative reverse transcriptase polymerase chain reaction, and infectivity of oropharyngeal specimens was analyzed by virus rescue in cell culture and quantified via determination of tissue culture infectious doses 50% (TCID50). Data were analyzed nonparametrically (α = 0.05). Viral load slightly but significantly decreased upon gargling in the test group (P = 0.0435) but not in the placebo group. Viral infectivity as measured by TCID50 also significantly decreased in the test group (P = 0.0313), whereas there was no significant effect but a trend in the placebo group. Furthermore, it was found that the specimens from patients with a vaccine booster exhibited significantly lower infectivity at baseline as compared to those without vaccine booster (P = 0.0231). This study indicates that a preprocedural mouthwash containing CPC and CHX could slightly but significantly reduce the viral load and infectivity in SARS-CoV-2–positive patients. Further studies are needed to corroborate these results and investigate whether the observed reductions in viral load and infectivity could translate into clinically useful effects in reducing COVID-19 transmission (German Clinical Trials Register DRKS00027812)

    Normative brain volumetry derived from different reference populations: Impact on single-subject diagnostic assessment in dementia

    Brain imaging data are increasingly made publicly accessible and volumetric imaging measures derived from population-based cohorts may serve as normative data for individual patient diagnostic assessment. Yet, these normative cohorts are usually not a perfect reflection of a patient’s base population, nor are imaging parameters such as field strength or scanner type similar. In this proof of principle study, we assessed differences between reference curves of subcortical structure volumes of normal controls derived from two population-based studies and a case-control study. We assessed the impact of any differences on individual assessment of brain structure volumes. Percentile curves were fitted on the three healthy cohorts. Next, percentile values for these subcortical structures for individual patients from these three cohorts, 91 mild cognitive impairment (MCI) and 95 Alzheimer’s Disease (AD) cases and patients from the Alzheimer Center (AC) were calculated, based on the distributions of each of the three cohorts. Overall we found that the subcortical volume normative data from these cohorts is highly interchangeable, suggesting more flexibility in clinical implementation

    Mechanical weeding enhances ecosystem multifunctionality and profit in industrial oil palm

    A full factorial experiment in a state-owned industrial oil palm plantation in Indonesia evaluates whether reduced management via reduced fertilization rates and mechanical weeding can decrease the negative impacts on ecosystem functions and biodiversity while maintaining profitability of the plantation.Oil palm is the most productive oil crop, but its high productivity is associated with conventional management (that is, high fertilization rates and herbicide application), causing deleterious environmental impacts. Using a 2(2) factorial experiment, we assessed the effects of conventional vs reduced (equal to nutrients removed by fruit harvest) fertilization rates and herbicide vs mechanical weeding on ecosystem functions, biodiversity and profitability. Analysing across multiple ecosystem functions, mechanical weeding exhibited higher multifunctionality than herbicide treatment, although this effect was concealed when evaluating only for individual functions. Biodiversity was also enhanced, driven by 33% more plant species under mechanical weeding. Compared with conventional management, reduced fertilization and mechanical weeding increased profit by 12% and relative gross margin by 11% due to reductions in material costs, while attaining similar yields. Mechanical weeding with reduced, compensatory fertilization in mature oil palm plantations is a tenable management option for enhancing ecosystem multifunctionality and biodiversity and increasing profit, providing win-win situations
