The great simulator
Cardiac myxoma often simulates multisystem disease and initially defies diagnosis. In this paper, we present three cases of cardiac myxoma in adults. Their clinical features and differential diagnosis will be reviewed, with special emphasis on their insidiousness and suspicion necessary for correct and prompt diagnosis. The role of echocardiography in diagnosis and treatment will be discussed, and finally, the surgical management and prognosis is briefly outlined.peer-reviewe
Pulmonary infarction in healthy young males
It is widely recognised that the elderly immobilised, sick, traumatised or postoperative patient is the most likely candidate to develop pulmonary embolism and infarction, and that the risk increases in direct proportion to the duration of the illness and to the age of the patient. Over the past few years, a number of autopsy studies have stressed the high incidence of deep venous thrombosis and pulmonary embolism in individuals who are otherwise normal. Unfortunately, many clinicians have been slow to appreciate this point and fail to recognise the condition unless well-established predisposing factors are present. This paper reports the findings in four young male patients who developed pulmonary infarction in the absence of obvious precipitating causes.peer-reviewe
From perceived autonomy support to intentional behaviour: Testing an integrated model in three healthy-eating behaviours
A motivational model integrating self-determination theory, the theory of planned behaviour, and the health action process approach was tested in three samples in three behavioural contexts: fruit and vegetable, breakfast, and snack consumption. Perceived support for autonomous (self-determined) forms of motivation from parents and autonomous motivation from self-determination theory were hypothesised to predict intention and behaviour indirectly via the mediation of attitude and perceived behavioural control from the theory of planned behaviour. It was also expected that planning strategies would mediate the effect of intention on behaviour. Relations in the proposed models were expected to be similar across the behaviours. A two-wave prospective design was adopted. Three samples of high-school students (total N = 1041; 59.60% female; M age = 17.13 years ± 1.57) completed measures of perceived autonomy support, autonomous motivation, theory of planned behaviour constructs, planning strategies and behaviour for each of the three behavioural contexts. Three months later, 816 participants (62,24% female; M age: 17.13 years, SD = 1.58) of the initial sample self-reported their behaviour referred to the previous three months. Structural equation models provided support for the key hypothesised effects of the proposed model for the three health-related behaviours. Two direct effects were significantly different across the three behaviours: the effect of perceived autonomy support on perceived behavioural control and the effect of attitude on intention. In addition, planning strategies mediated the effect of intention on behaviour in fruit and vegetable sample only. Findings extend knowledge of the processes by which psychological antecedents from the theories affect energy-balance related behaviours
The early evolution of Globular Clusters: the case of NGC 2808
Enhancement and spread of helium among globular cluster stars have been
recently suggested as a way to explain the horizontal branch blue tails, in
those clusters which show a primordial spread in the abundances of CNO and
other elements involved in advanced CNO burning (D'Antona et al. 2002). In this
paper we examine the implications of the hypothesis that, in many globular
clusters, stars were born in two separate events: an initial burst (first
generation), which gives origin to probably all high and intermediate mass
stars and to a fraction of the cluster stars observed today, and a second,
prolonged star formation phase (second generation) in which stars form directly
from the ejecta of the intermediate mass stars of the first generation. In
particular, we consider in detail the morphology of the horizontal branch in
NGC 2808 and argue that it unveils the early cluster evolution, from the birth
of the first star generation to the end of the second phase of star formation.
This framework provides a feasible interpretation for the still unexplained
dichotomy of NGC 2808 horizontal branch, attributing the lack of stars in the
RR Lyr region to the gap in the helium content between the red clump, whose
stars are considered to belong to the first stellar generation and have
primordial helium, and the blue side of the horizontal branch, whose minimum
helium content reflects the helium abundance in the smallest mass
(~4Msun)contributing to the second stellar generation. This scenario provides
constraints on the required Initial Mass Function, in a way that a great deal

Psychometric properties of the FertiQoL questionnaire in Italian infertile women in different stages of treatment
Background: Reliable Fertility-specific QoL measures can be obtained through the FertiQoL, a questionnaire with six-subscales that consider different core aspects of the personâs wellbeing and way of behaving during treatment. Objective: Examine the psychometric properties of all six-subscales of the Italian FertiQoL in a sample of infertile women and explore the effects of the ART treatment phases. Method: 323 women, in three different treatment stages (Diagnostic, Stimulation, Transfer), completed the FertiQoL. Raw data were subject to Confirmatory Factor Analysis (CFA), and a structural equation modelling (SEM) was used to validate the hypothesised model. Results: CFA shows a good fit of the data to the FertiQoL hierarchical model (chi-square/df = 1.989, CFI = 0.88, RMSEA = 0.055). After the deletion of 2 items, all FertiQoL scales have good internal consistency. SEM showed that the ART treatment phase was positively associated with fertility-related QoL scores both in the Relational (ÎČ = 0.14, p < 0.05) and in the Tolerability (ÎČ = 0.17, p < 0.05) subscales. Conclusion: All scales of the Italian FertiQoL version maintain good psychometric characteristics; Tolerability and Relational subscales are sensitive to the treatment stage and thus providing relevant information for the medical staff
The implementation and evaluation of a media literacy intervention about PAES use in sport science students
With respect to both competitive and amateur/fitness sports, media may strongly influence young peopleâs opinions and behaviors concerning the use of PAES (Performance and Appearance Enhancing Substances). The present investigation addressed this topic by focusing on sport sciences studentsâ beliefs concerning the possible role of media related to the implementation and evaluation of a PAES-focus media literacy intervention conducted with sport science students. This study relied on a sample of 521 students (attrition rate 10.3%) (45.1% female, mean age = 22.6, SD = 2.20), which provided baseline data on studentsâ levels of media literacy concerning the use of PAES (i.e. âdescriptive sampleâ), and a sample of 248 students, who participated in and provided data on the media literacy intervention. This latter sample included a group of 128 students (44.5% female, mean age = 23.03, SD = 3.76) who actively participated in the intervention (i.e. âintervention groupâ), and a group of 120 students who did not (i.e. âcontrol groupâ, 53.3% female, mean age = 22.25, SD = 2.47). All students filled out media literacy questionnaires targeting studentsâ awareness of media influence, their views about the realism of media content, their sense of confidence in dealing with media messages, and their positive attitudes toward PAES use. Analyses of questionnaire data showed that students are relatively aware of media influence on peopleâs views and behaviors with respect to PAES use. At the same time, students also believed that young people do not consider media as ârealistic sourcesâ of information; nonetheless, they also did not consider themselves entirely capable of dealing effectively with media messages. With respect to the intervention, students overall appreciated and greatly welcomed the educational program on media literacy, and the analyses of intervention data across intervention and control groups showed that key media literacy variables changed over time, attesting to the overall effectiveness of the intervention
The Unique History of the Globular Cluster Omega Centauri
Using current observational data and simple dynamical modeling, we
demonstrate that Omega Cen is not special among the Galactic globular clusters
in its ability to produce and retain the heavy elements dispersed in the AGB
phase of stellar evolution. The multiple stellar populations observed in Omega
Cen cannot be explained if it had formed as an isolated star cluster. The
formation within a progenitor galaxy of the Milky Way is more likely, although
the unique properties of Omega Cen still remain a mystery.Comment: published version with minor change
XO-2b: Transiting Hot Jupiter in a Metal-rich Common Proper Motion Binary
We report on a V=11.2 early K dwarf, XO-2 (GSC 03413-00005), that hosts a
Rp=0.98+0.03/-0.01 Rjup, Mp=0.57+/-0.06 Mjup transiting extrasolar planet,
XO-2b, with an orbital period of 2.615857+/-0.000005 days. XO-2 has high
metallicity, [Fe/H]=0.45+/-0.02, high proper motion, mu_tot=157 mas/yr, and has
a common proper motion stellar companion with 31" separation. The two stars are
nearly identical twins, with very similar spectra and apparent magnitudes. Due
to the high metallicity, these early K dwarf stars have a mass and radius close
to solar, Ms=0.98+/-0.02 Msolar and Rs=0.97+0.02/-0.01 Rsolar. The high proper
motion of XO-2 results from an eccentric orbit (Galactic pericenter, Rper<4
kpc) well confined to the Galactic disk (Zmax~100 pc). In addition, the phase
space position of XO-2 is near the Hercules dynamical stream, which points to
an origin of XO-2 in the metal-rich, inner Thin Disk and subsequent dynamical
scattering into the solar neighborhood. We describe an efficient Markov Chain
Monte Carlo algorithm for calculating the Bayesian posterior probability of the

Homesickness Experience, Distress and Sleep Quality of First-Year University Students Dealing with Academic Environment
The literature shows that homesickness can have a negative impact on different areas of psychological functioning such as cognitive, emotional, behavioral, physical and social ones. The study, conducted on a sample of first-year university students, aimed to test a model hypothesizing that two distinct dimensions of homesickness, attachment to home and disliking university, could have both direct and indirect effects on their psychological distress and sleep difficulties. Two hundred and seventy-seven first-year students (70.4% Female) living away from home (mean age= 21.3, SD= 2.7) were included in the study and filled out questionnaires assessing homesickness, psychological distress and sleep difficulties. Structural Equation Model (SEM) analysis confirmed that the two homesickness dimensions worked differently since only the one relating to new place adjustment difficulties (i.e. disliking the university) resulted having both direct and indirect effects on studentsâ sleep difficulties. Although female students reported higher levels of psychological distress compared to males, the multi-group SEM analysis showed that the pattern of multivariate relationships linking the two dimensions of homesickness, psychological distress and sleep difficulties, was invariant across gender. The results of the present study suggest that university counselling and guidance services should quickly screen students experiencing homesickness and offer effective counselling programs focusing on enhancing their capacities to deal with the new academic environment
