    A rare case of vena cava leiomyoma: A case report

    El leiomioma intravascular es una enfermedad infrecuente y dependiente de la afectación vascular y el reto anestésico. Presentamos el caso de una mujer de 53 años sometida a resección de un leiomioma de la vena cava en bypass cardiopulmonar mediante parada circulatoria hipotérmica profunda (PCH). Durante el procedimiento se utilizó monitorización hemodinámica y neurológica invasiva, ecocardiografía transesofágica y prueba de coagulación viscoelástica. La resección quirúrgica total se llevó a cabo sin complicaciones y el paciente fue extubado 2 días después de la intervención sin déficit cardiaco ni neurológico. Aunque infrecuente, la cirugía del leiomioma intravascular de nivel IV es un reto porque la resección total requiere DHCA, bypass cardiopulmonar prolongado y tiempos de pinzamiento cruzado aórtico. Estas condiciones exponen al paciente al riesgo de coagulopatía, síndrome de bajo gasto cardíaco y déficit neurológico.Intravascular leiomyoma is an uncommon disease and depending of vascular involvement and anesthetic challenge. We review a case of a 53-year-old woman who underwent vena cava leiomyoma resection under cardiopulmonary bypass using deep hypothermic circulatory arrest (DHCA). Invasive hemodynamic and neurologic monitoring, transesophageal echocardiography, and viscoelastic coagulation test were used during the procedure. Total surgical resection was accomplished with no complications and the patient was extubated 2 days after surgery without cardiac or neurologic deficit. Although uncommon, level IV intravascular leiomyoma surgery is a challenge because the total resection needs DHCA, prolonged cardiopulmonary bypass and aortic cross-clamp times. These conditions expose the patient to the risk of coagulopathy, low cardiac output syndrome, and neurologic deficit

    Insights into the origin of 'shrew-like' oposssums of South America are gained thanks to a new fossil from the Oligocene Salla Beds in Bolivia. The specimen described here consists of a partial rostrum, palate and postcanine teeth, and shows several generalized features (cranial and dental) in the context of the Paucituberculata. On this basis we recognize Evolestes hadrommatos gen. et sp. nov. In order to evaluate the affinities of the new taxon, we performed a phylogenetic analysis including representatives of the Caenolestidae, Pichipilus and allies (not regarded here as caenolestids), Palaeothentidae, and Abderitidae, with three outgroups.     Introduction We aimed to evaluate IDH1 p.R132H mutation and 2-hydroxyglutarate (2HG) in cerebrospinal fluid (CSF) as biomarkers for patients with IDH-mutant gliomas. Methods CSF was collected from patients with infiltrating glioma, and 2HG levels were measured by liquid chromatographymass spectrometry. IDH1 p.R132H mutant allele frequency (MAF) in CSF-ctDNA was measured by digital droplet PCR (ddPCR). Tumor volume was measured from standard-of-care magnetic resonance images. Results The study included 48 patients, 6 with IDH-mutant and 42 with IDH-wildtype gliomas, and 57 samples, 9 from the patients with IDH-mutant and 48 from the patients with IDH-wildtype gliomas. ctDNA was detected in 7 of the 9 samples from patients with IDH-mutant glioma, and IDH1 p.R132H mutation was detected in 5 of the 7 samples. The MAF ranged from 0.3 to 39.95%. Total 2HG level, D-2HG level, and D/L-2HG ratio in CSF were significantly higher in patients with IDH-mutant gliomas than in patients with IDH-wildtype gliomas. D-2HG level and D/L-2HG ratio correlated with total tumor volume in patients with IDH-mutant gliomas but not in patients with IDH-wildtype gliomas. Conclusion Our results suggest that detection of IDH1 p.R132H mutation by ddPCR and increased D-2HG level in CSF may help identify IDH-mutant gliomas. Our results also suggest that D-2HG level and D/L-2HG ratio correlate with tumor volume in patients with IDH-mutant gliomas. Further prospective studies with larger cohorts are needed to validate these findings

    Effect of hazelnut on serum lipid profile and fatty acid composition of erythrocyte phospholipids in children and adolescents with primary hyperlipidemia : a randomized controlled trial

    Background & aim: Regular intake of nuts improves lipid profile and thus reduces the cardiovascular (CV) risk associated with hyperlipidemia. The aim of the study was to investigate the effect of a dietary intervention with hazelnuts (HZNs, 15-30 g/day, depending on patient weight) on serum lipid profile, anthropometric parameters and fatty acids (FAs) composition of erythrocyte phospholipids in children and adolescents with primary hyperlipidemia. Methods: Eight-week randomized, single blind, controlled, three-arm, parallel-group study. Sixty-six subjects were enrolled and randomized in 3 groups receiving: 1) hazelnuts with skin (HZN+S); 2) hazelnuts without skin (HZN-S); 3) dietary advices for hyperlipidemia only (controls). Before and after intervention, clinical parameters were measured and blood samples were collected for the evaluation of serum lipid levels and phospholipid FA composition of erythrocytes. Results: Two-way ANOVA showed a significant effect of time on serum low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C)/LDL-C ratio and non-HDL-C (p < 0.001), but not of treatment and time x treatment interaction. In particular, HZN+S and HZN-S significantly reduced the concentrations of LDL-C and increased HDL-C/LDL-C ratio. HZNs also had a favorable impact on FAs composition of erythrocyte phospholipids, as demonstrated by time x treatment interaction, with a significant increase of monounsaturated fatty acids (MUFAs) (p = 0.008) and MUFAs/saturated fatty acids (SFAs) ratio (p = 0.002) with respect to the control group. Conclusions: For the first time, we documented a positive effect of HZN consumption on lipid profile and FA composition of erythrocyte phospholipids in children with primary hyperlipidemia. Further studies are encouraged to better define HZN impact on the markers of CV risk in this population

    Uso de mezclas de azoxistrobina y triazoles para controlar enfermedades de fin de ciclo de la soja

    134-139Widely spread in Argentina, late season diseases (LSD), cause yield losses and seed quality changes. Leaf application of fungicides is an effective procedure to manage LSD under the current cropping conditions (monocrop and no-till system). The aims of the present study were 1) to determine causal agents of LSD, 2) to evaluate yield reduction caused by LSD and 3) to evaluate the efficiency of mixtures containing triazoles and strobirulins, sprayed during R3 or R5 reproductive stages to control LSD. Four experiments were carried out in a randomized complete block design, with four replicates in Armstrong, Santa Fe, Argentina, in a soybean cultivation area in the Pampeana Region. Two assays were performed in 2004/2005, and the remaining ones in 2005/2006 growing seasons. Two azoxystrobin mixtures, one with cyproconazole and the other with difenoconazole, and a mixture of the latter two were tested. The following pathogens were detected: Cercospora kikuchii, Colletotrichum truncatum, Septoria glycines, Glomerella glycines y Phomopsis sojae. All treatments presented higher yield than control, with 5 percent of statistic significance. It can be concluded that, under the present experimental conditions, characterized by frequent rains between R1 and R5.5, the tested mixtures efficiently controlled LSD

    The SWELL1-LRRC8 complex regulates endothelial AKT-eNOS signaling and vascular function

    The endothelium responds to numerous chemical and mechanical factors in regulating vascular tone, blood pressure, and blood flow. The endothelial volume-regulated anion channel (VRAC) has been proposed to be mechanosensitive and thereby sense fluid flow and hydrostatic pressure to regulate vascular function. Here, we show that the leucine-rich repeat-containing protein 8a, LRRC8A (SWELL1), is required for VRAC in human umbilical vein endothelial cells (HUVECs). Endothelial LRRC8A regulates AKT-endothelial nitric oxide synthase (eNOS) signaling under basal, stretch, and shear-flow stimulation, forms a GRB2-Cav1-eNOS signaling complex, and is required for endothelial cell alignment to laminar shear flow.     Random graphs with arbitrary degree distributions and their applications

    Recent work on the structure of social networks and the internet has focussed attention on graphs with distributions of vertex degree that are significantly different from the Poisson degree distributions that have been widely studied in the past. In this paper we develop in detail the theory of random graphs with arbitrary degree distributions. In addition to simple undirected, unipartite graphs, we examine the properties of directed and bipartite graphs. Among other results, we derive exact expressions for the position of the phase transition at which a giant component first forms, the mean component size, the size of the giant component if there is one, the mean number of vertices a certain distance away from a randomly chosen vertex, and the average vertex-vertex distance within a graph. We apply our theory to some real-world graphs, including the world-wide web and collaboration graphs of scientists and Fortune 1000 company directors. We demonstrate that in some cases random graphs with appropriate distributions of vertex degree predict with surprising accuracy the behavior of the real world, while in others there is a measurable discrepancy between theory and reality, perhaps indicating the presence of additional social structure in the network that is not captured by the random graph.Comment: 19 pages, 11 figures, some new material added in this version along with minor updates and correction

    Correlations in Scale-Free Networks: Tomography and Percolation

    We discuss three related models of scale-free networks with the same degree distribution but different correlation properties. Starting from the Barabasi-Albert construction based on growth and preferential attachment we discuss two other networks emerging when randomizing it with respect to links or nodes. We point out that the Barabasi-Albert model displays dissortative behavior with respect to the nodes' degrees, while the node-randomized network shows assortative mixing. These kinds of correlations are visualized by discussig the shell structure of the networks around their arbitrary node. In spite of different correlation behavior, all three constructions exhibit similar percolation properties.Comment: 6 pages, 2 figures; added reference
