Osteoma osteoide sobre callo de fractura
Se comunica un caso de osteoma osteoide de localización tibial en un hombre
de 23 años, cuya particularidad ha sido su manifestación clínica después de un largo intervalo
libre y de haberse desarrollado sobre el lugar mismo del foco de una fractura cerrada tibial sucedida
hacía 4 años. El diagnóstico se realizó por la clínica dolorosa de ritmo nocturno, por el
estudio radiológico convencional, por tomografía axial computarizada (TAC) y Resonancia Nuclear
Magnética (IRM). Su confirmación se hizo por estudio anatomopatológico. El tratamiento
consistió en exéresis quirúrgica de la tumoración tras la cual desapareció el dolor no existiendo
residuos después de un año. Aunque numerosas publicaciones han sido consagradas al osteoma
osteoide, nos ha parecido interesante publicar esta observación particular de esta
tumoración en razón de la rareza de su asiento sobre el lugar mismo del callo de una fractura.We report a case of osteoid osteoma in the tibial region in a 23 year old male,
whose interest resides in its clinical appearance following a long period without pain and having
developed in the same location of a closed tibial fracture that occured some 4 years ago. The
diagnosis was performed in addition to the night-time pain experienced, by means of conventional
Radiographic study, and Nuclear Magnetic Resonance Diagnostic. Its confirmation was carried
out by pathological study. Treatment consisted in surgical exeresis of the tumour. Pain
disappeared, after exeresis, not existing tumour ral tissue after one year follow-up. Although
numerous publications have been made concerning the osteoide osteoma, it seems interesting
to publish this particular case, because of the rarity of its occurrence in the same location of
the callus of a fracture
Pharmacokinetics, thrombogenicity and safety of a double viral inactivated factor IX concentrate compared with a prothrombin complex concentrate
Therapeutic options for developing countries have to assure an optimum safety and efficacy and low-cost antihaemophilic concentrates. A single blind randomized crossover study was carried out in 12 previously treated HB patients, comparing the pharmacokinetics (PK), thrombogenicity (TG) and safety of two plasma-derived double-inactivated (solvent/detergent heating at 100?C, 30 min) factor IX (FIX) concentrates, UMAN COMPLEX DI (product A) [plasma-derived prothrombin concentrates (PCC)] and a high purity FIX concentrate AIMAFIX DI (product B, HPFIX). In a non-bleeding state, they received one single intravenous dose 50 IU FIX kg−1 of PCC or HPFIX, and after a wash-out period of 14 days, the other product. We evaluated acute tolerance and determined PK parameters based on FIX levels measured over a 50 h postinfusion period. We studied fibrinogen, platelets, antithrombin, F1 + 2, TAT, D-dimer, over a 360 min postinfusion period. Ten cases remained in on-demand treatment for 6 months, five with PCC and five with HPFIX. PK and anti-FIX inhibitors were repeated at 3 and 6 months. No inhibitors were detected. PK values (PCC vs. HPFIX): clearence (CL; mL h−1 kg−1) 5.2 ? 1.4 vs. 6.5 ? 1.4; the volume of distribution at steady state (mL kg−1) 154.9 ? 54.9 vs. 197.5 ? 72.5; mean residence time (h) 29.7 ? 8.1 vs. 30.7 ? 9.2; T1/2 (h) 22.3 ? 7 vs. 23.5 ? 12.3; incremental recovery (IR; U dL−1 U−1 kg−1) 0.96 ? 0.17 vs. 0.76 ? 0.13. HPFIX showed significant lower IR and higher CL. There were no differences in PK at 3 and 6 months. In TG, significant increments in TAT and F1 + 2 at 30 min and 6 h were found with PCC. Product B PK results agrees with reported results for other HPFIX preparations. Use of PCC product A has to consider its thrombogenic activity
Footprints of fixed-gear fisheries in relation to rising whale entanglements on the U.S. West Coast
On the U.S. West Coast, reports of whales entangled in fishing gear increased dramatically in 2014. In this study, a time series of fishing activity maps was developed from 2009 to 2016 for the four fixed-gear fisheries most commonly implicated in entanglements. Maps were generated using vessel monitoring system (VMS) data linked to port-level landings databases, which were related to entangled whale reports over the same time period and with modelled distributions of humpback whales Megaptera novaeangliae Borowski. Over the full study period, neither marked increases in fishing activity nor changes in fisheries footprints within regions with high whale densities were detected. By contrast, a delayed fishery opening in California due to a harmful algal bloom in spring of 2016 led to ~5–7 times average levels of Dungeness crab Metacarcinus magister (Dana) fishing activity, which was consistent with a high rate of entanglement in that year. These results are consistent with current hypotheses that habitat compression caused by a marine heatwave increased the overlap of whales with fishing activity, despite minimal changes in the fisheries themselves. This study adds to literature on bycatch of protected species in otherwise sustainable fisheries, highlighting the value of using VMS data for reducing human–wildlife conflict in the ocean
GeneCodis: interpreting gene lists through enrichment analysis and integration of diverse biological information
GeneCodis is a web server application for functional analysis of gene lists that integrates different sources of information and finds modular patterns of interrelated annotations. This integrative approach has proved to be useful for the interpretation of high-throughput experiments and therefore a new version of the system has been developed to expand its functionality and scope. GeneCodis now expands the functional information with regulatory patterns and user-defined annotations, offering the possibility of integrating all sources of information in the same analysis. Traditional singular enrichment is now permitted and more organisms and gene identifiers have been added to the database. The application has been re-engineered to improve performance, accessibility and scalability. In addition, GeneCodis can now be accessed through a public SOAP web services interface, enabling users to perform analysis from their own scripts and workflows. A Rare Case of an Intramedullary Metastasis of a Myxopapillary Ependymoma
Background: Myxopapillary ependimoma (MPE) is a benign slow-growing tumor, and it has been designated histologically as a Grade I neoplasm according to the 2016 World Health Organization classification. Despite the benign character, dissemination and metastasis have occasionally been reported. The retrograde dissemination to other levels of the neuraxis is extremely rare, being more frequent to the intracranial compartment.
Case description: We hereby present a case of medullary metastasis of cauda equina MPE, with a history of having undergone a subtotal resection and postoperative adjuvant radiotherapy. The patient presents complaints of night dorsal pain attributable to intradural metastasis twenty-one years after the first surgical intervention.
Conclusion: The case reported highlights the importance of long follow-up in patients with MPE, since the possibility of secondary seeding to distant craniospinal sites or local spinal sites after surgery, and radiotherapy should be considered in metastatic disease.info:eu-repo/semantics/publishedVersio

Roturas agudas de L.C.A. de la rodilla: refuerzo con cordón RD.S
Se presenta la técnica de reconstrucción en las roturas agudas del L.C.A.
mediante reparación con cordón de P.D.S., así como los criterios seguidos por los autores ante
las roturas agudas de dicho ligamento. Expone una casuística de 24 intervenciones, con un
seguimiento de 1.5 años, sin haber encontrado complicaciones destacables y con un 75% de Muy
Buenos Resultados y un 25% de Buenos Resultados.The technique used in the repair of acute rupture of the ACL with a
band of PDS, together with the criteria followed by our department in such cases
both acute are decribed. The describe 24 operations performed by them, with a
follow-up of 1.5 years, with no major complications, 75% excellent results and 25%
good results
The 2-79 keV X-ray Spectrum of the Circinus Galaxy with NuSTAR, XMM-Newton and Chandra: a Fully Compton-Thick AGN
The Circinus galaxy is one of the nearest obscured AGN, making it an ideal
target for detailed study. Combining archival Chandra and XMM-Newton data with
new NuSTAR observations, we model the 2-79 keV spectrum to constrain the
primary AGN continuum and to derive physical parameters for the obscuring
material. Chandra's high angular resolution allows a separation of nuclear and
off-nuclear galactic emission. In the off-nuclear diffuse emission we find
signatures of strong cold reflection, including high equivalent-width neutral
Fe lines. This Compton-scattered off-nuclear emission amounts to 18% of the
nuclear flux in the Fe line region, but becomes comparable to the nuclear
emission above 30 keV. The new analysis no longer supports a prominent
transmitted AGN component in the observed band. We find that the nuclear
spectrum is consistent with Compton-scattering by an optically-thick torus,
where the intrinsic spectrum is a powerlaw of photon index ,
the torus has an equatorial column density of cm and the intrinsic AGN keV luminosity is
erg/s. These values place Circinus along the same
relations as unobscured AGN in accretion rate-vs- and -vs-
phase space. NuSTAR's high sensitivity and low background allow us to study the
short time-scale variability of Circinus at X-ray energies above 10 keV for the
first time. The lack of detected variability favors a Compton-thick absorber,
